하반신 마비환자가 스키를 탈 수 있도록 고안된 모노스키는 스키 플레이트 위에 좌석이 설치된 형태의 썰매이다. 스키를 탈 때 설면으로부터 전달되는 충격은 정상인의 경우 다리관절의 거동에 의하여 흡수되지만 모노스키의 경우에는 좌석에 탑승한 인체에 그대로 전달된다. 상용화된 대부분의 모노스키에는 이를 보완하기 위하여 스키 마운트와 시트 간에 링크 메커니즘과 충격완충장치가 설치되어 있다. 본 연구에서는 유압 실린더가 장착된 모노스키의 주행 시뮬레이션을 수행하고 불규칙한 표면 주행 중 발생하는 진동 충격 가속도를 분석하여 탑승자의 안락감을 평가할 수 있는 모델을 개발하였다. 안락감 평가방법은 국제 규격인 BS6841 을 사용하였다. 개발 모델을 이용하여 유압 완충장치의 노즐조절에 따른 안락감을 평가하였다. The mono-ski for the paraplegia designed to skiing is formed as seat bucket on the sled. The impact force transferred by snow surface during skiing is absorbed by the leg joints of normal human, but it is transferred to the human body on the seat when using mono-ski. Most of commercially available mono-ski have absorbing device and link mechanism between seat and ski mount in order to complement it. In this study we developed the comfort evaluation model that could provide skiing simulation of mono-ski with hydraulic damper and analyzed vibrational acceleration occurred during skiing uneven surface. The evaluation method used in this study is the international standard BS6841. We evaluated comfort performance of mono-ski in accordance with nozzle adjustment of hydraulic damper.

Giant congenital melanocytic nevi are very rare skin lesions with an estimated prevalence of 1 in 20,000 live births, and have high risk of malignant melanoma development and leptomeningeal melanocytosis. Hence, its early and exact diagnosis in the neonatal period is important and essential. Only seven cases of giant congenital melanocytic nevi have been reported in Korea, of which none of the cases were associated with hemangioma. Herein, the authors describe a case of giant congenital melanocytic nevi with huge hemangioma with the patho logic findings that exhibited benign and provide a literature review.
2009 Influenza A H1N1 감염 중 소아 중증 환자에 대한 고용량 Oseltamivir 투여 효과

Purpose: We aimed to evaluate the use and safety of double dose oseltamivir for patients manifesting severe respiratory symptoms or showing no improvement of clinical symptoms after 72 hours’ treatment with the usual oseltamivir dosage. Methods: We analyzed the clinical features of 2009 influenza A H1N1 inpatients who had been admitted to a university hospital’s Department of Pediatrics between August 2009 and January 2010. The Influenza A H1N1 diagnoses were confirmed by real-time reverse transcriptase polymerase chain reaction. Results: The study participants numbered 157 (mean age, 5.0 years; male-to-female ratio, 1.1:1). Among them, twenty (mean age, 5.2 years) were administered double dose oseltamivir. This double dose group showed higher peak body temperatures and more abnormal radiologic results than the other, usual-dose group. The mean time duration between high fever and afebrile status after initiation of double-dose oseltamivir administration was 2.1 days, whereas that within the usual-dose group was 1.7 days. There were no adverse effects in the patients treated with double-dose oseltamivir. Conclusion: Double-dose oseltamivir was well tolerated in patients with severe 2009 influenza A H1N1 infection. This study validated the musculoskeletal model of the human lower extremity by comparative study<br/> between calculated muscle parameters through simulation using modified hill-type model and measured<br/> them through isokinetic exercise. And the relationship between muscle forces and moments participated<br/> in motion was quantified from the results of simulation. For simulation of isokinetic motion, a<br/> three-dimensional anatomical knee model was constructed using trials of gait analysis and the<br/> EMG-force model was used to determine muscle activation level exciting muscles. The modified<br/> Hill-type model was used to calculate individual muscle forces and moments in dynmaic analysis and<br/> the results were validated by comparing them of experiments on BIODEX. The results showed that<br/> there was a high correlation between calculated torques from simulation and measured them from<br/> experiments for isokinetic motion(R=0.97). Therefore we concluded that the simulation by using<br/> musculoskeletal model was so useful means to predict and convalesce musculoskeletal-related diseases,<br/> and analyze unrealizable experiment such as clash condition.

More than 50% of the countrys annual rainfall occurs in July and October, and local downpours have recently occurred frequently due to abnormal weather conditions. If such heavy rains occur near the entrance and exit of a road tunnel, falling raindrops coexist with the atmosphere. This phenomenon is expected to reduce ventilation in road tunnels in ordinary days due to flow resistance, but factors that take into account heavy rain are not applied to the ventilation system of road tunnels in Korea. Therefore, in this study, we used a three-dimensional numerical analysis program, Fluent, to determine the effect of heavy rain on ventilation in road tunnels, and compared and analyzed the changes in ventilation in road tunnels according to raindrop size during heavy rain.

Objectives : The main purpose of this study is to evaluate effects of acupuncture at Fengchi($GB_{20}$) and Houxi($SI_3$) for acute headache due to whiplash injury. Methods : This study was observed among 50 patients with acute headache due to whiplash injury who admitted to Bundang oriental medicine hospital of Dongguk University from June 16, 2009 to January 31, 2010. These patients were divided into two groups; One was control group that was acupunctured at general acupuncture points except Fengchi($GB_{20}$), Houxi($SI_3$)(Control group) and the other was acupunctured at Fengchi($GB_{20}$), Houxi($SI_3$) with general acupuncture points(Experimental group). Each patient was acupunctured once a day. The effect was assessed through self-rating headache index. Results & Conclusions : As a result of evaluation by using self-rating headache index, pain score at 8th day declined in both groups. But they were not difference between two groups at 8th day.
