Angiodysplasia, an important vascular lesion of the gut, defined by the presence of ectasia of normal preexisting intestinal submucosal veins and overlying mucosal capillaries, is a source of major morbidity from bleeding. With the development of diagnostic angiography, angiodysplasia of the bowel has been recognized as a frequent cause of significant intestinal bleeding, particularly in elderly patients. Its cause is unknown but considered mostly to be acquired as a result of a degenerative process associated with aging. We experienced one case of jejunal angiodysplasia in a 64-year old man, diagnosed by angiography, and selective jejunal resection was conducted with the aid of intraoperative mesenteric arterial methylene blue injection, administered via a catheter superselectively positioned in the appropriate jejunal arterial branch prior to operation. This patient has been followed up for 6 months without rebleeding. (Korean J Gastroenterol 1995;27:588- 594)
세침흡인생검으로 진단한 결핵성 Addison 씨병 1 예

We reported a case of Addison's disease due to tuberculosis, which was pathologically confirmed by fine-needle aspiration biopsy. In a 39-year-old man with fatigue, weakness, and generalized cutaneous pigmentation, the diagnosis of Addison's disease was made by the finding of elevated plasma ACTH level and subnormal response to rapid ACTH test. Computed tomographic scan revealed bilateral adrenal mass with heterogenous uptake and peripheral rim enhancement, and calcification. Ultrasoundguided fine-deedle aspiration biopsy of the left adrenal mass disclosed granulomatous inflammation with caseation necrosis. The patient also had active tuberculosis in the right inguinal lymph node(J Kor Soc Endocrinol 10: 296-310, 1995).

Kawasaki disease is an acute febrile vasculitis of the unknown etiology, affecting mainly infant and young children. During the acute stage of the Kawasaki disease, coronary aneurysm develops in about 20 % of the patients, which may cause sudden death or ischemic heart disease as a result of the thrombotic or stenotic occlusion of the coronary artery. Coronary artery sequala of the Kawasaki disease may become an important cause of the ischemic heart disease in young adult. A 20-year old man was admitted because of the exertional chest pair, who had suffered from persistent fever of one month's duration associated with oropharyngeal mucosal changes and cervical lymphadenopathy at infancy. He did not have the coronary risk factors except Kawasaki disease. The exercise electrocardiography test was positive. The left ventriculography showed hypokinesia of the anterolateral, the diaphragmatic segments, and the inferolateral portion of the posterolateral segment. The coronary arteriography showed oval calcified aneruysms with luminal stenosis at the proximal right coronary artery, distal circumflex artery, and proximal left anterior descending artery. Abundant intra and intercoronary collateral vessels were found. We report a case of the adult coronary artery disease due to infantile Kawasaki disease with the review of the literatures.
