사전검사를 통한 고립성 폐결절 환자에서의 악성 확률 타당성에 대한 연구

Background: Solitary pulmonary nodules (SPN) are encountered incidentally in 0.2% of patients who undergo chest X-ray or chest CT. Although SPN has malignant potential, it cannot be treated surgically by biopsy in all patients. The first stage is to determine if patients with SPN require periodic observation and biopsy or resection. An important early step in the management of patients with SPN is to estimate the clinical pretest probability of a malignancy. In every patient with SPN, it is recommended that clinicians estimate the pretest probability of a malignancy either qualitatively using clinical judgment or quantitatively using a validated model. This study examined whether Bayesian analysis or multiple logistic regression analysis is more predictive of the probability of a malignancy in SPN. Methods: From January 2005 to December 2008, this study enrolled 63 participants with SPN at the Kangnam Sacred Hospital. The accuracy of Bayesian analysis and Bayesian analysis with a FDG-PET scan, and Multiple logistic regression analysis was compared retrospectively. The accurate probability of a malignancy in a patient was compared by taking the chest CT and pathology of SPN patients with <30 mm at CXR incidentally. Results: From those participated in study, 27 people (42.9%) were classified as having a malignancy, and 36 people were benign. The result of the malignant estimation by Bayesian analysis was 0.779 (95% confidence interval [CI], 0.657 to 0.874). Using Multiple logistic regression analysis, the result was 0.684 (95% CI, 0.555 to 0.796). This suggests that Bayesian analysis provides a more accurate examination than multiple logistic regression analysis. Conclusion: Bayesian analysis is better than multiple logistic regression analysis in predicting the probability of a malignancy in solitary pulmonary nodules but the difference was not statistically significant.
데이터 스트림 환경에서 임의 시간 구간에 대한 효율적 클러스터링 알고리즘

최근에 데이터의 양이 방대하게 늘어남에 따라 이러한 데이터의 처리를 위한 여러 연구들이 진행되어지고 있다. 이 중에 데이터들 간의 군집 관계를 파악하기 위하여 사용되는 클러스터링에 관한 연구가 많이 수행되었는데, 이중 BIRCH는 대용량의 데이터를 처리하는데 있어서 적합한 모델로 제시되고 있다. 하지만 BIRCH는 데이터 스트림 환경에서 클러스터링은 효과적이지 못한 단점을 가지고 있다. 본 논문은 데이터 스트림 환경에서 과거의 임의 시간구간에 대한 클러스터링을 수행하는 방법을 제안한다. 이를 위하여 CF-트리를 일정 시간 마다 생성 및 저장하고 이를 이용하여 사용자가 원하는 시간 구간에 대해 동안의 클러스터링을 수행한다. 본 논문에서는 임의 시간구간에 대한 효율적인 클러스터링을 위해 기존의 CF-트리 노드 구조에 추가 데이터를 사용하는 CFδ-트리를 제안한다. 그리고 δ에 대한 연구를 통해, 근사적 접근법을 제안하였다.

본 논문에서는 절차 중심 소프트웨어를 객체 지향 소프트웨어로 재/역공학기 위한 다단계 절차 중 객체 추출 단계에서 선 클러스터링을 통해 불 필요한 정제 결합단계를 축소하고, 영역 전문가의 선택으로 영역 모델링에 가장 가까운 객체 후보군을 제시하는 알고리즘을 제안하고자 한다. 기존의 연구에서는 영역 모델링과 다중 객체 후보군과의 유사도를 측정하여 영역 전문가에게 최적합 후보를 선택할 수 있는 측정 결과를 제시하였다. 하지만 영역 전문가가 제시하는 영역 모델링이 존재한다면 정제 결합단계이전에 최대한의 선 클러스터링을 통해서 영역 모델링과 가장 유사한 통합 객체를 제시할 수 있고, 정제 결합 단계를 선 클러스터링을 통해서 축소할 수 있으며 이를 통해서 객체 후보군과 영역모델링의 유사도를 향상 시키며 클러스터링에 따른 시간과 공간을 절약할 수 있다. 따라서 본 논문에서는 영역 모델링과 사용자의 함수, 전역변수의 선택을 통해 영역 모델링에 가장 유사한 객체 후보군을 찾는 선 클러스터링 알고리즘 제안 하고자 한다.

Purpose: To report a case of acyclovir-induced acute renal failure (ARF) suspected as acute retinal necrosis syndrome. Case summary: The authors report a 55-year-old male patient who presented with left eye visual disturbance due to suspected acute retinal necrosis syndrome. Non-oliguric ARF developed after the infusion of intravenous acyclovir (850 mg every 8 hours). The patient did not show any uremic symptoms or signs. The crystal was not discovered in the urine. After stopping the acyclovir infusion and hydration, acyclovir-induced ARF was reversed. Conclusions: Although possessing critical nephrotoxicity, acyclovir is a useful antiviral drug. Therefore, when using acyclovir, the importance of hydration and preventing acyclovir-induced ARF should be considered. J Korean Ophthalmol Soc 2010;51(11):1528-1531
Salmonella Gallinarum 세포외막단백질의 프로테옴 분석 및 닭에서의 방어능 효과

Salmonella Gallinarum (SG) is known as an important pathogen that causes fowl typhoid in chickens. To investigate SG outer-membrane proteins (OMPs) as a vaccine candidate, we used proteomic mapping and database analysis techniques with extracted OMPs. Also, extracted OMPs were evaluated in several aspects to their safety, immune response in their host and protective effects. Our research has established a proteomic map and database of immunogenic SG-OMPs used as inactive vaccine against salmonellosis in chickens. A total of 22 spots were detected by 2-dimensional gel electrophoresis and immunogenic protein analysis. Eight spots were identified by Matrix-Assisted Laser Desorption/Ionization-Time of Flight-Mass spectrometry (MALDI-TOF-MS) and peptide mass fingerprinting (PMF) and categorized into four different types of proteins. Among these proteins, OmpA is considered to be an immunogenic protein and involved in the hosts' immune system. To estimate the minimum safety dose in chickens, 35 brown layers were immunized with various concentrations of OMPs, respectively. Consequently, all chickens immunized with more than a $50{\mu}g$ dose were protected against challenges. Moreover, intramuscular administration of OMPs to chickens was more effective compared to subcutaneous administration. These results suggest that the adjuvanted SG-OMP vaccine not only induces both the humoral and cellular immune response in the host but also highly protects the hosts' exposed to virulent SG with $50{\mu}g$ OMPs extracted by our method.

The purpose of this study was that the optimal hydrolysis conditions of endo- and exo-type enzymes were selected to utilize organic cheese byproducts. Optimal substrate concentration and optimum enzyme ratio were measured by using 4 kinds of endo-type enzymes (alcalase, neutrase, protamex, and foodpro alkaline protease) and two exo-type enzymes (flavourzyme and prozyme 2000P) for whey protein hydrolysis were analyzed using liquid chromatography. As a result, the optimal endo-type enzyme through the first enzyme reaction was selected as alcalse, and as a result of the secondary enzyme reaction, flavourzme was selected as the Exo type enzyme. The concentration of whey protein substrate for optimal primary and secondary enzyme reactions was 10%. In addition, the optimum ratio of enzyme was 0.5% of alcalase and 0.2% of flavourzyme, which showed low molecular weight chromatography pattern compared to 2% of alcalase and 1% of flavourzyme hydrolyzate. Therefore, hydrolyzing the endo-type enzyme alcalase at a concentration of 0.5% for 10 hours and then hydrolyzing the exo-type enzyme flavouryme at a concentration of 0.2% for 4 hours was considered to be the optimum condition.

IgA nephropathy and thin basement membrane disease are common glomerular diseases in persistent microscopic hematuria with or without proteinuria. However, these two conditions cannot be easily distinguished on the biochemical or urinary findings alone. Therefore, renal biopsy is required for correct identification of the two conditions in most cases. Recently, it has been reported that thinning of glomerular basement membrane is accompanied with precipitation of electron dense deposits in some patients with IgA nephropathy. We report a case of IgA nephropathy associated with thin basement membrane disease in a 19-year-old male with microscopic hematuria and mild proteinuria. After 2 years` treatment with angiotensin II receptor blocker, the patient exhibited persistent microscopic hematuria but decreased proteinuria. Our finding concurs with the previous reports indicating that patients with both IgA nephropathy and thin basement membrane disease do not have different clinical features compared to those with IgA nephropathy alone. In addition, clinical outcome does not appear to be affected by thin basement membrane disease when these two conditions are combined.
모바일 기기에서 한글키워드 입력을 통한 인터넷 접속 방법

본격적인 휴대인터넷 서비스의 시작으로 모바일 기기에서 유선인터넷과 동일한 수준의 인터넷 서비스를 제공받는 것이 가능하게 되었다. 이러한 변화에 맞춰 모바일 기기에서 인터넷에 접속하는 방법에 대한 연구가 필요하게 되었다. 현재 휴대인터넷을 사용하기 위한 인터넷 접속 방법으로는 숫자도메인 입력을 통한 접속을 지원하는 WING 방법과, URL-SMS를 수신한 뒤 접속하는 방법 모바일 코드를 통해 접속하는 방식 등으로 나누어져 있다. 기존의 방식들은 이해하기 어렵거나 접속을 위해 몇 번의 작업을 거쳐야 하는 문제점을 갖고 있다. 이러한 문제점을 해결하기 위해 본 논문에서는 다른 과정을 거치지 않고 한글키워드 입력을 통하여 인터넷에 직접 접속하는 방법을 제시 및 구현해 보았다.
