        여성노인에게 적용한 낙상예방 작업치료가 균형 능력에 미치는 영향

        지역사회 여성노인에게 적용한 작업치료가 균형 능력에 미치는 영향을 규명하여 노인들의 낙상 예방을 위한 작업치료의 효용성을 알아보고자 하였다. 자립보행이 가능하고 균형에 영향을 주는 약물 복용을 하지 않는 65세 이상 여성 노인들을 대상으로 실험군 13명, 대조군 13명으로 분류하여 낙상 예방을 위한 교육과 운동을 병행한 6주 간의 작업치료프로그램을 적용하였다. 균형 능력 평가는 Berg 균형척도와 Tetrax 낙상지수가 사용되었다. 실험 결과, 실험군의 Berg 균형척도는 6주간의 프로그램 적용을 통하여 48.38에서 50.69로 유의한 수준의 변화를 나타내었으며(p=.017). 실험군의 Tetrax 낙상지수 또한 6주간의 프로그램 적용을 통하여 46.77에서 35.54으로 유의한 수준의 변화를 나타내었다(p=.000).대조군의 Berg 균형척도와 Tetrax 낙상지수는 모두에서 유의한 수준의 변화를 보이지 않았다. 본 연구의 결과로 볼 때 낙상예방 교육과 운동을 병행한 낙상예방 작업치료 프로그램의 6주 간의 적용이 노인의 균형 능력증진에 효과적이라는 것을 알았으며, 추후 낙상 예방 교육의 효과만을 다룬 연구들이 시행되어야 할 것이다. This study is to look into the effectiveness of fall-preventive occupational therapy of elderly by clarifying the influence of occupational therapy applied to female elderly in the community upon balance ability. A 6-week occupational therapy program including both training and exercise for prevention of fall down was applied by dividing into 13 persons of experimental group and 13 persons of control group for female elderly aged more than 65 years who were capable of independence gait and have not taken any drug to adversely affect balance. For balance ability assessment, the Berg balance scale and the Tetrax fall index were used. The Berg balance scale in experimental group was shown to be 48.38 to 50.69 through the 6-week program, indicating changes in significance level(p=.017). The Tetrax fall index in experimental group was shown to be 46.77 to 35.54 through the 6-week program(p=.000). Changes in significance level were not found for both the Berg balance scale and the Tetrax fall index in control group. As seen from the results of this study, it is found that 6-week application of the fall-preventive occupational therapy program including fall prevention training and exercise was effective for promotion of elderly's balance ability, and further studies just on the effect of fall prevention training are required to be made.

        Heat pumps are used for air-conditioning systems in commercial buildings, schools, and factories because of low operating and maintenance costs. These systems use the earth as a heat source in heating mode and a heat sink in cooling mode. Ground heat exchangers are classified by a horizontal type and vertical type according to the installation method. A horizontal type means that a heat exchanger is laid in the trench bored in 1.2 to 1.8 m depth. And a vertical type is usually constructed by placing small diameter high density polyethylene tube in a vertical borehole. Vertical tube sizes range from 20 to 40 mm nominal diameter. Borehole depth range between 100 and 200 m depending on local drilling conditions and available equipment. In this study, to evaluate the performance of single u-tube with bentonite grouting, single u-tube with broken stone grouting and double n-tube bentonite grouting of vertical ground heat exchangers, test sections are buried on the earth and experimental apparatus is installed. Therefore the heat transfer performance and pressure loss of these are estimated.

        작업기반 중재 수행 경험을 통한 작업치료사 직업정체성 찾기 과정연구 : 근거이론을 중심으로

        본 연구는 작업기반 중재를 수행하고 있는 작업치료사의 경험과 인식을 알아보고, 작업기반 중재경험을 통한 직업정체성 형성과정을 확인하기 위한 목적을 가지고 시행하였다. 본 연구에서는 다양한경험과 배경의 작업치료사를 11명 선정하고, 심층면담을 실시하였다. Strauss와 Cobin의 근거이론방법을 사용하여 수집된 자료를 분석한 결과 총 114개의 개념과 24개의 하위범주, 10개의 범주를 도출하였다. 과정분석 시행을 통해 시간의 흐름에 따른 상황의 변화를 확인한 결과, 작업치료사들의 작업기반중재를 통한 직업정체성 형성과정은 정체성 저하와 위축의 위기단계에서 변화동기 외현화 단계, 중재변화 시도와 전략적 대처단계, 정체성획득과 안정화단계로 변화하고 있는 것을 알 수 있었다. 이상의결과를 바탕으로 작업기반 중재를 수행하면서 내외부의 갈등과 제약을 극복하고 전략수립을 통해 작업치료사로서 다시 정체성을 찾아가는 과정을 종합적으로 논의하고 연구의 시사점을 제시하였다. 향후 연구결과가 이들을 지원하기 위한 연구와 정책 수립에 기초연구로 사용되기를 바란다. The purpose of this study was to identifying the experiences and awareness of occupational therapists performing occupation-based interventions, and explore and understanding the process of forming job identity through experiences of occupation-based intervention. In this study, in-depth interviews were conducted with 11 occupational therapists who have various experience and background. The collected data were analyzed and interpreted according to the data analysis procedures of the ground theory method presented by Stauss and Corbin(Strauss & Corbin, 1990). The results are as follows: Through open coding analysis, 114 concepts, 24 subcategories, and 10 categories were derived. As a result of the process analysis through axial coding, the occupational therapists’ job identity development process 4 stages including ‘crisis stage of decreased identity and lose confidence’, ‘stage of motivation for change and externalization’, ‘stage of attempt to change intervention and strategic action’ and ‘stage of identify acquisition and stabilization’. Based on the results above, the process of overcoming internal and external conflicts and constraints while performing occupation-based intervention, the process of finding identity again as an occupational therapist through strategy establishment was comprehensively discussed and suggested implications for the study. In the future, we hope that the current study results would be useful as basic research to contribute to research and policy establishment to support the occupational therapist.

        집중 소자를 이용한 광대역 평판형 마이크로파 바이어스-티의 설계

        본 논문에서는 집중 소자를 이용한 광대역 평판형 마이크로파 바이어스-티의 설계를 보였다. 설계된 바이어 스-티는 DC 블록과 RF 초크로 구성된다. DC 블록용 커패시터는 광대역으로 동작하는 커패시터를 사용하였고, DC 공급 및 RF 초크용 인덕터는 서로 다른 자기 공진 주파수(SRF: Self Resonance Frequency)를 가지는 인덕터들을 직렬로 연결하였다. RF 초크에서 집중 소자들의 직렬 공진에 의하여 발생하는 신호의 손실을 병렬의 저항과 커패시터를 연결하여 해결하였다. 설계된 바이어스-티는 1608 칩 형태의 집중 소자들을 이용 조립하여 제작하였다. 측정은 커넥터의 손실과 영향을 제거하기 위하여 Anritsu 3680K jig에 연결하여 측정하였다. 제작된 바이어스-티는 10 ㎒~18 ㎓의 광대역에서 동작하고, 측정된 반사 손실이 ?15 ㏈ 이하를 가지며, 삽입 손실은 ?1.5 ㏈ 이하인 것을 확인하였다. In this paper, a design of planar microwave bias-tee using lumped elements was presented. The bias-tee is composed of 2 blocks; DC block and RF choke. For this design of the bias-tee, a wideband capacitor was used for DC block. For a RF choke, a series connection of inductors which have different SRFs is used for a RF choke. In the RF choke, a series connection of resistor and capacitor was added in shunt to eliminate a loss from a series resonance. The designed bias-tee was implemented by using 1608 SMT chip components. The fabricated bias-tee was measured using Anritsu 3680K fixture which enables to remove an effect of a connector. The fabricated bias-tee presented ?15 ㏈ of return loss and ?1.5 ㏈ insertion loss at 10 ㎒~18 ㎓.

        에비안스 함베르크 스트레칭 방법이 고관절의 능동 가동범위와 우모각에 미치는 영향

        The purpose of this study was to investigate the effects of Evjenth-Hamberg stretching on the active range of motion (ROM) of the hip joint and the pennation angle of the semitendinosus muscle. Eighty healthy adults articipated in this study. The active ROM of the hip joint was measured by a goniometer and the pennation angle of the semitendinosus muscle was measured by ultrasonographic imaging (USI). Both ROM and pennation angle were recorded before and after the static stretching and the Evjenth-Hamberg stretching, respectively. Data were analyzed using paired t-test and independent t-test at p<.05. The results were as follows: 1) The active ROM of the hip joint increased significantly after both stretching interventions compared with the baseline (p<.001). However, the active ROM of the hip joint increased significantly in Evjenth-Hamberg stretching compared with static stretching. 2) The pennation angle decreased significantly after both stretching interventions compared with the baseline (p<.001). However, the pennation angle decreased significantly in Evjenth-Hamberg stretching compared with static stretching. 3) Reliability data showed that there was a high consistency in USI measurements ICC=.978. Our findings suggest that the Evjenth-Hamberg stretching was more effective than static stretching in increasing the active ROM of the hip joint and decreasing the pelmation angle of the semitendinosus muscle.

        길항근 수축이 결합된 등척성 수축 후 신장 방법이 근력 및 유연성에 미치는 영향

        The purpose of this study was to investigate the effects of post-isometric contraction stretching combined with antagonist muscle contraction on muscle strength and flexibility using electromyography (EMG) and ultrasound imaging (USI). Healthy adults were recruited. The static stretching and post-isometric contraction stretching combined with antagonist muscle contraction method was applied to each groups, respectively. Muscle activation using surface EMG, range of motion (ROM) using goniometer, and pennation angle (PA) using USI were measured in the hamstring muscle in pre- /post-tests. Data was presented descriptive statistics, paired t-test, and independent t-test through SPSS 12.0. The paired t-test showed that the amplitude of muscle activation was significantly improved at post-isometric contraction stretching combined with antagonist muscle contraction (p=0.00), whereas was not significant at static stretching (p=0.44), the ROM and the PA were significantly improved both groups, respectively (p=0.00). The independent t-test was significant improved at post-test between two groups (p=0.05). Our findings suggest that the post-isometric contraction stretching combined with antagonist muscle contraction was effective in management of muscle strength and flexibility compared to the static stretching. Additionally, the PA using USI was useful of measuring the flexibility.

        보상작용 억제와 피드백을 제공한 가상현실 치료가 만성 뇌졸중 편마비 환자의 상지기능에 미치는 영향

        The purpose of this study was to investigate the effects of virtual reality (VR) therapy with compensation inhibition and feedback (CIF) on upper extremity function in chronic stroke patients. Seven chronic stroke patients participated in this study, which was a randomized controlled trial with a crossover design. Self upper extremity exercise, conservative VR therapy, and VR therapy with CIF were performed for one hour per session, 5 times per week, over a 3 week period. The main outcome measures involved range of motion (ROM) including shoulder, elbow, and wrist joints, a Manual Function Test (MFT), and a Motor Activity Log (MAL). Data were calculated as posttest and pretest changes in every session and were analyzed using Friedman and Wilcoxon signed-rank tests at p<.05. The results were as follows: 1) Statistically significant increase in ROM measurements of shoulder and elbow joints were seen with VR therapy with CIF compared to VR therapy and self upper extremity exercise (p<.05), whereas no significant increasing was noted for the wrist joint (p>.05). 2) Statistically significant increase in the MFT was seen with VR therapy with CIF compared with VR therapy and self upper extremity exercise (p<.05). 3) VR therapy with CIF also resulted in statistically significant increase in both activity of use (AOU) (p<.05) and quality of movement (QOM) (p<.05) on the MAL test when compared with VR therapy and self upper extremity exercise, respectively. In conclusion, VR therapy with CIF was more effective than conservative VR therapy and self upper extremity exercise in improving the upper extremity function in hemiplegic patients with chronic stroke.

        저항성 턱 열기 운동과 저항성 턱 당기기 운동 수행 시 목뿔위근과 목뿔아래근의 근 활성도 차이 비교

        본 연구는 저항성 턱 열기 운동과 저항성 턱 당기기 운동 수행에 따른 목뿔위근과 목뿔아래근의 근 활성도를 확인하고 그 차이를 비교하기위해 시행되었다. D광역시에 거주 중인 건강한 성인 48명을 대상으로 두 운동을 적용하였으며 운동 수행에 따른 목뿔위근과 목뿔아래근의 근 활성도는 표면 근전도를 사용하여 측정하였다. 운동 수행 간 측정된 목뿔위근과 목뿔아래근의 근 활성도는 대응표본 T 검정을 통해 차이를 비교하였다. 연구결과 두 운동 간 목뿔위근의 근 활성도에 있어서 통계학적 차이를 보였으며(p<.01), 목뿔아래근의 근 활성도에서는 통계학적인 차이를 보이지 않았다(p=1.00). 본 연구결과 저항성 턱 열기 운동과 저항성 턱 당기기 운동 모두 목뿔위근의 근 수축을 유도할 수 있으며 목뿔위근의 보다 많은 근 수축을 유도할 수 있는 운동은 저항성 턱 열기 운동인 것으로 확인되었다. 목뿔아래근의 근 수축은 통계학적인 차이는 보이지 않았으나, 이러한 결과는 두 운동간 목뿔아래근의 유사한 근 수축을 유도할 수 있을 것으로 생각할 수 있겠다. 추후 목뿔위근과 목뿔아래근의 근약화를 가지고 있는 환자를 대상으로 다음의 두 운동의 효과를 비교한다면 명확한 이론적 근거가 될 것이다 This study was conducted to review the muscle activity of the supra-hyoid muscle and infra-hyoid muscle during Jaw Opening Agasinst Resistance Exercise and Chin-Tuck Against Resistance Exercise. The two exercises were applied to 48 adults living in City D. The surface electromyogram of the supra-hyoid muscle and infra-hyoid muscle were measured. The muscle activity of the the supra-hyoid muscle and infra-hyoid muscle in accordance with the exercises were compared using a corresponding sample t test. The study showed a statistically significant difference in the muscle activity of the supra-hyoid muscle (p<.01), while there was no such difference in the muscle activity of the infra-hyoid muscle (p=1.00). The findings show that both resistant lower Jaw Opening Agasinst Resistance Exercise and Chin-Tuck Against Resistance Exercise can lead to muscle activation in the supra-hyoid muscle and the infra-hyoid muscle. Jaw Opening Agasinst Resistance Exercise was found to encourage more muscle activation in the supra-hyoid muscle. While there was no statistcial difference in the muscle activation of the infra-hyoid muscle, the findings indicate that the two exercise can encourage similar muscle activation in the infra-hyoid muscle. A more clear theoretical basis can be provided if the the effects of the two exercises are compared in patients with muscular weaknesses in the supra-hyoid muslce and infra-hyoid muscle.
