Objectives : To investigate the relationship in acute lateral tortipelvis and breaks in low back pain patients with acute lateral tortipelvis were discussed. Methods : The subject of this study consisted of 43 patients who were diagnosed as acute lateral tortipelvis by X-ray and 23 patients who were diagnosed as none tortipelvis by X-ray. We investigated acute lateral tortipelvis and breaks on X-ray film. Results and Conclusions : In the distribution of acute lateral tortipelvis, L4 was 17 cases(39.5%), L3 was 14 cases(32.6%), L2 was 9 cases(18.6%), L1 was 4 cases(9.3%) in the sequence. In acute lateral tortipelvis group, the number of breaks was more than none tortipelvis group(p>0.05). The most common parity of breaks was L4-5 in low back pain patient. Between distribution of lumbar level of acute lateral tortipelvis and breaks, there was no significant relationship.

Objective : The objective of this study is to evaluate the treatment effect of Band training in Osteoporotic Compression Fracture. Methods : Band training, Herb medicine and Acupuncture were performed for 56days admission in a patient who had Osteoporotic Compression Fracture. Result : ROM of Lumbar, Milgram test and Dorsiflexion has improved. VAS and ODI score were also decreased.

Cauda equina syndrome(CES) and disc hemiation have been implicated as a potential complication of Chuna Manipulation Treatment. We reported a man who developed sudden complication alter lumbar Chuna Manipulation Treatment. It is supposed that this patient suffered a disc herniation secondarily due to a lumbar Chuna Manipulation Treatment. Therefore, doctors should be aware that neurological complication may occur as a result of lumbar Chuna Manipulation Treatment.
인공 슬관절 전 치환술 시 경골 후 내측부 골 결손에 대해 내고정 없이 시행한 자가골 이식술의 결과

목적: 심한 내반 변형 시 경골의 골 결손에 대하여 내고정이나 주대의 사용 없이 자가골 이식술과 함께 표준형 슬관절 전치환술을 시행하고 임상적 및 방사선적 결과를 보고하고자 한다. 대상 및 방법: 2012년 7월부터 2014년 4월까지 자가골 이식술을 시행한 32명(45예; 남성 4예, 여성 41예; 평균 연령 74.2세)을 대상으로 하였으며 평균 추시 기간은 34.4개월이었다. 골 결손 부위에 전후방 고랑을 만들고 자가골 조각을 45° 사선으로 삽입한 후 표준형 치환술을 시행하였다. 골 결손의 크기를 측정하고 방사선 및 임상적 평가를 시행하였다. 결과: 방사선 사진으로 측정한 골 결손의 크기는 전후방 평균 15.31×30.36 mm였고, 측면 15.46×45.98 mm였다. 수술 시 측정한 골 결손의 평균 깊이는 8.38 mm였다. 대퇴-경골간 각은 수술 전 평균 내반 14.1° (4.0°-26.9°), 최종 추시 시 평균 외반 5.4° (0.5°-10.5°)였고. 치환물의 위치는 최종 추시 시 α=95.7°, β=90.4°, γ=2.1°, δ=89.1°였다. 최종 추시 시 인공관절 삽입물의 해리는 없었으며 평균 골 유합 기간은 4.3개월이었다. Hospital for Special Surgery 점수가 수술 전 평균 50.1점, 최종 추시 시 평균 90.4점이었다. 결론: 경골 골 결손에 대하여 내고정물 없이 자가골 이식을 통하여 표준형 인공 치환물의 사용으로 빠르고 안정적인 골 유합과 좋은 방사선적 및 임상적 결과를 얻었으며 골 이식에 효과적인 수술 방법이라 생각된다. Purpose: To evaluate the radiological and clinical outcomes of the standard total knee arthroplasty without internal fixation or extended long stem in tibial bone defect with severe varus deformity. Materials and Methods: Between July 2012 and April 2014, 32 patients (45 cases; 4 men and 41 women with a mean age of 74.2 years) who underwent total knee arthroplasty with autologous bone grafting were enrolled for analysis. The mean follow-up period was 34.4 months. The cancellous bone defect site was exposed, and a longitudinal sulcus was made. Subsequently, a premolded bone graft was inserted in the sulcus at 45°. The defect size was measured, and the radiological and clinical results were evaluated. Results: The mean defect size according to the radiograph was found to be 15.31×30.36 mm in the frontal view and 15.46×45.98 mm in the sagittal view. The mean defect size of depth during the operation was found to be 8.38 mm. The preoperative mean varus angle was 14.1° (4.0°–26.9°), and the follow-up mean valgus angle was 5.4° (0.5°–10.5°). The implant position was α=95.7°, β=90.4°, γ=2.1°, δ=89.1° on the follow-up. No implant loosening was observed, and the mean bone union period was 4.3 months. The Hospital for Special Surgery score was improved from a preoperative mean of 50.1 to a postoperative mean of 90.4. Conclusion: Standard total knee arthroplasty using autologous structural bone grafting without internal fixation in a tibial bone defect demonstrated a rapid, stable bone healing and excellent radiological and clinical results. Thus the index procedure was considered to be simple, and effective for bone grafting.

국내 도시철도 차량을 위한 FRACAS 기반의 신인성 관리 플랫폼 연구

This paper describes a study on the FRACAS(Failure Reporting Analysis and Corrective Action System)-based dependability control platform for domestic urban railway trains. There are more demands for the verification of the dependability of trains as it becomes a regulation for train manufacturers to verify the dependability recently. Train manufacturers as well as railway operators need a effective FRACAS solution to perform the verification of the dependability. Yet current FRACAS solutions have limitations to support the verification processes effectively. This paper addresses the issues of current FRACAS solutions and suggests a FRACAS framework designed for the domestic urban railway trains. Service failure scenarios are standardized using the proposed availability model to implement a more user-friendly and reliable platform. A new FRACAS-based platform for the dependability control (SCARF??) has been developed to implement the suggested framework. The detail interfaces and functions of the platform are explained. The SCARF?? platform is expected to engage the increasing demands for the dependability control successfully enhancing the reliability, maintainability, availability and safety of domestic urban railway trains eventually.
