Hyperinsulinism caused by a functioning islet cell tumor of the pancreas is an uncommon but well established metabolic entity which can usually be diagnosed accurately. We treated a patient with functioning insulinoma recently. The outline of the particular management given for the course of the anesthesia of the patient was as follows: 1) Control of blood glucose a) pre- and intraoperative hypoglycemia; Glucose was administered through intravenous drip or orally. b) postoperative transient hyperglycemia; Insulin was administered if necessary and a small amount of glucose uptake, occurred. 2) Anesthetics, and supplemental drugs Drugs with no or little effect in increasing blood glucose were used. 3) Beta-adrenergic blockers These drugs were not used in the hypoglycemic state because of potentiating insulin activity.

S-120 : 코일 색전술로 치료된 선천성 간문맥전신단락증 1예

서론: 간문맥전신단락(Intrahepatic portosystemic venous shunt, IPSVS)은 매우 드문 질환으로 고암모니아혈증과 간성혼수를 동반한 경우는 치료가 필요하다. 내과적 치료로 경구 락툴로즈 등을 사용하거나 외과적 수술을 고려할 수 있으며, 중재적 치료로 경도관색전술을 이용해 단락을 차단할 수도 있다. 증례: 77세 여자 환자가 3시간 전부터 발생한 행동장애와 의식변화를 주소로 내원하였다. 환자는 7년 전 같은 증상으로 입원하여 선천성 간문맥전신단락증에 의한 간성혼수로 진단받고 경구 락툴로즈 투약치료 중이었다. 내원시 의식은 기면상태였으며 활력징후는 혈압 134/83 mmHg, 맥박 93회/분, 호흡 16회/분, 체온 36.7℃이었다. 말초혈액검사상 암모니아가 179 umol/L로 증가되었고 이학적 검사상날개치기 진전(flapping tremor)을 보였다. 심전도와 흉부 및 복부단순촬영은 정상이었고 뇌 전산화단층촬영에서도 특이 소견 없었다. 복부전산화단층촬영상 간의 전체적인 모양은 정상 범주이나 좌 내측 간문맥지와 중간 간정맥이 확장되어 있고 그 사이에 굵고 구불구불한 간문맥전신단락이 있었다. 환자는 락툴로즈 관장 후 의식은 호전되었지만 선천성 간문맥전신단락에 의한 고암모니아혈증에 대해 내과적 치료중 재발한 간성혼수로 판단하여 입원 4일째 중재적 코일 색전술을 시행하였다. 우측 총대퇴정맥을 천자한 후 주간문맥으로 접근하여 총 19개의 detachable microcoil (Interlock, Boston scientific, Natick, MA)을 사용하여 shunt embolization을 하였으며, 시술 후 shunt를 통한 혈류가 차단됨을 확인할 수 있었다. 치료 후 지속적으로 암모니아 수치가 감소하여 시술 4일 후 34 umol/L로 회복되고 퇴원하였다. 2주 후 외래 경과관찰에서 암모니아를 비롯한 간기능 수치는 모두 정상이었고 1개월 후 시행한 복부 전산화단층촬영에서 효과적으로 단락이 차단되고 양측 간문맥 분지들이 시술 전에 비해 굵어진 모습이 확인되었다. 고찰: 간문맥전신단락은 간 조직검사나 외상 또는 문맥고혈압에 의해 이차적으로 발생할 수 있으며 발생학적 변이로 인한 선천적 원인으로 발생하는 경우는 매우 드물다. 본 증례는 선천적 원인으로 인한 간문맥전신단락증으로 본 증례와 같이 고암모니아혈증과 간성혼수를 동반하는 경우 치료가 필요하며 비수술적인 치료로 경도관색전술을 이용해 단락을 차단할 수 있었다. 우리나라에서는 2010년과 2012년 단 두 차례 색전술을 통해 치료한 선천성 간문맥전신단락증 증례가 보고된 바 있다. 저자들은 코일을 이용한 색전술을 통해 비수술적인 방법으로 치료된 선천성 간문맥전신단락증 1예를 경험하여 문헌 고찰과 함께 보고하는 바이다.

Understanding Seon in Go Eun’s 108 Poems in What (뭐냐Mwonya)?

The selection of the creative writings of Go Eun What?:108 Seon Poems is specified as the spirit of Seon. It is true that the Seon is inherent in all his works, but it is considered that in this selection this spirit of Seon accomplishes Realism, especially. First of all, Go Eun pays attention to the history which is duly regarded as the whole reality of the past because he saw the origin of Seon as a historic event. In his opinion, Seon as the sphere of practice is generated from “the protests of people” against authority of tradition. So, in the theme of historicity, it represented the old scriptures as the Lotus Sutra, included the episode of burning the Blue Cliff Records, and also parodied the linage of patriarchs of Seon. In this way, Go Eun insists that old scriptures and teaching of patriarchs are only a hindrance to awakening, so, if a practitioner depends on such things, it is impossible to attain the goal of liberation. Along the same lines, it is strongly emphasised in the theme of awakening that while practicing Seon meditator must go his own way because going one’s own way is the only way to find the true self and attain the enlightening. Meanwhile, the world of mindfulness is the ultimate goal of Seon practice, and to achieve this aim one should be mindful at every time and every place, referred to as ordinary mind. Therefore, in Go Eun’s poems it is emphasised that everyday life is very important. The scenes of ordinary life in which we live come to the fore for Seon practice in the theme of “everyday life.” At last, recognizing that whole cosmos is united each other, the poet represents this sense of unity as insight into nature and in the theme of “nature” every creature is being in the very state of existence in whole reality —“here and now.”

주요 약용작물로 사용하고 있는 천궁의 경정을 기내배양 하여 건전한 종경을 대량으로 증식하는데 적함한 배양조건을 구명하기 위하여 실험한 결과를 요약하면 7월에 채취한 재료를 1%의 NaOCI에 20분간 표면살균한 후, carbenicillin 500 mg/L, BA 1.0 mg/L 와 $GA_3$ 1.0 mg/L가 첨 가된 1/2 MS배지가 오염율 감소와 신초 재분화율을 높이는데 효과적이었고, 재분화한 신초의 기내증식은 BA 0.5 mg/L와 당 60 g/L가 함유된 1/2 MS가 양호하였으며 이들 유묘는 1/2 MS에 NAA 3.0 mg/L 단용배지에서 배양하였을때 발근하였다. This experiment was conducted to identify the optimal in vitro propagation condition of Cnidii rhizoma (Cnidium officinale Makino). It was effective to reduce contamination and improve regeneration of shoot when shoot tips taken in July were cultured in 1/2 strength Murashige and Skoog medium supplemented with 500 mg/L carbenicillin disodium 1.0 mg/L BA and 1.0mg/L $GA_3$followed by surface sterilization of explant source in solution of 1% sodium hypochlorite for 20 minutes. When shoot tips were 쳐cultured in 1/2 strength MS medium with 0.5 mg/L BA and 60 g/L sucrose, shoot elogation and subsequent multiplication of the formed shoot were favorable than in other media. Regenerants were well rooted in 1/2 strength MS medium containing 3.0 mg/L NAA.
