        Dental Management in a Patient with Turner Syndrome with Dental Anomalies : A Case Report

        터너 증후군이란 한 개의 X 염색체와 전부 혹은 부분이 결여된 X 염색체를 나타내는 염색체 이상 질환이다. 치근 형태 이상이 터너 증후군 환자에서 정상에서보다 호발한다고 보고되었는데, 치근의 이러한 특징은 근관치료를 까다롭게 만들고, 특수한 처치를 요하기도 한다. 또한 터너 증후군 환자들은 종종 심장 또는 신장의 기형과 같은 전신적인 문제를 동반하기도 하므로, 임상의는 터너 증후군의 특징적인 구내 소견을 인지하고, 사전에 구내 합병증을 예방하기 위해 환자들이 정기적으로 치과 검진을 받을 수 있게 해야 한다. 10세 때에 터너증후군 진단을 받은 12세 여자 환자가 하악 양측 소구치 부위의 불편감을 주소로 내원했다. 특징적으로 하악의 모든소구치에서 우상치와 치외치가 관찰되었다. 하악 양측 제1소구치는 세 개의 치근을, 근단부 병소를 보이는 하악 양측 제2소구치는 두 개의 치근을 가지고 있었다. 근관의 복잡한 해부학적 특징 때문에 치과 현미경을 이용한 적절한 시야를 확보하여 근관치료를 마무리 했다. 2년간의 정기적인 관찰 결과 해당치아와 관련된 임상적 징후나 증상, 치근단 병소는 관찰되지 않았다. 본 증례 보고를 통해 터너 증후군 환자의 구강내 특징과 이의 치과적 관리에 대해 보고하고자 하였다. Turner syndrome (TS) is a chromosomal disorder caused by monosomy of the X chromosome, with complete or partial absence of the second sex chromosome. Anomalies of root morphology have been found to occur more often in patients with TS, which make endodontic treatment challenging and requires special handling. The patients with TS may also have systematic problems such as cardiac or renal malformations, so in treating these patients it is important for clinicians not only to be aware of the characteristic intraoral findings, but also to make the patients have regular dental check-ups to prevent oral complications in advance. An 12-year-old girl who had been diagnosed with TS at the age of 10 years was referred due to discomfort in the bilateral mandibular premolar regions. Dens evaginatus and taurodontism were detected in all the mandibular premolars characteristically. The bilateral mandibular first premolars had three roots and the bilateral mandibular second premolars had periapical lesion with two roots. Due to the complexity of the root canal anatomy, root canal treatment were completed with a dental microscope to ensure adequate visualization. After 2 years of regular follow-up examinations, there were no clinical sign or symptom associated with the teeth, and no periapical lesion, was found. This case report describes the characteristic oral features and dental management of TS patients.

        운동 스트레스와 면역반응

        Immune functions are claimed to be affected by psychological stressors and physiological mechanism, which involves the close interaction between nervous system, endocrine system, and immune system. In particular, acute intensive exercise causes much larger changes in the concentration of circulating immune cells mid hormones than does psychological stress. Continuous exercises as physical stress are known to affect immune function through three specific pathways. In the first pathway, the secretion of ACTH is promoted by stimulating the HPA axis, and catecholamines secreted in adrenal medulla and glucocorticoirds secreted in adrenal cortex affect immune functions. In the second pathway, immune function are affected by increasing the level of ACTH, β-endorphin, growth hormone (GH), and prolactin through the interaction of nervous system and endocrine system. In the third pathway, immune functions are affected through direct influence of lymphoid organ or sympathetic nervous system Besides, immune functions may be affected by the immune parameters like cytoldnes and external environment. Previous researchers reported that continuous exercise with moderate intensity is an important factor in the change of immune response. Although recently a great number of researches have been carried out about the correlation of nervous system, endocrine system and immune system, their exact mechanism is yet to be worked out, requiring further researches in the future. It may not be an exaggeration to say that life in the modern world is life under stress. It is generally approved that one of the most efficient ways to resolve stress is exercise. However, since exercise itself can act as distress weakening immune functions, it is very important to carry out continuous researches to find out the influence of exercise on immune functions in the light of exercise immunology.

        올레산으로 유도된 비알코올성 지방간 세포 모델에서의 미세전류 자극의 지질 대사 조절 효능 평가

        Non-alcoholic fatty liver disease(NAFLD) is excessive hepatic lipid accumulation mainly caused by obesity. This study aimed to evaluate whether micro-current stimulation(MCS) could modulate lipid metabolism regarding the Sirt1/AMPK pathway, fatty acid β-oxidation pathway, and lipolysis and lipogenesis-related factors in FL83B cells. For the NAFLD cell model, FL83B cells were treated with oleic acid for lipid accumulation. MCS were stimulated for 1 hr and used frequency 10 Hz, duty cycle 50%, and biphasic rectangular current pulse. The intensity of MCS was divided into 50, 100, 200, and 400 μA. Through the results of Oil red O staining, it was confirmed that MCSs with the intensity of 200 μA and 400 μA significantly reduced the degree of lipid droplet formation. Thus, these MCS intensities were applied to western blot analysis. Western blot analysis was performed to analyze the effects of MCS on lipid metabolism. MCS with the intensity of 400 μA showed that significantly activated the Sirt1/AMPK pathway, a key pathway for regulating lipid metabolism in hepatocytes, and fatty acid β-oxidation-related transcription factors. Moreover, it activated the lipolysis pathway and suppressed lipogenesis-related transcription factors such as SREBP- 1c, FAS, and PPARγ. In the case of MCS with the intensity of 200 μA, only PGC1α and SREBP-1c showed significant differences compared to cells treated only with oleic acid. Taken together, these results suggested that MCS with the intensity of 400 μA could alleviate hepatic lipid accumulation by modulating lipid metabolism in hepatocytes.

        Miniscrew implant를 이용한 상악 구치부 압하와 개방교합 치료의 효과 및 안정성

        최근 절대적인 고정원의 등장으로 다른 부작용 없이 구치부 압하 치료를 가능하게 하였고, 비수술적으로 개방교합의 심미적인 치료를 가능하게 하였다. 그러나, 이러한 치료법의 장기적인 안정성에 대한 연구는 많이 부족한 실정이다. 이에 본 연구에서는 miniscrew implant를 이용하여 상악 구치를 압하시켜 치료한 성인 개방교합 환자 11명 (남자 1명, 여자 10명)을 대상으로 치료 전후, 그리고 유지기의 골격성, 치아 치조성 변화를 측모 두부 방사선사진을 이용하여 평가하였으며 계측치의 치료 전후 변화량간의 상관관계와 치료에 의한 변화량과 재발량과의 상관관계를 규명하여 상악구치의 압하를 통해서 개방교합을 치료할 때의 안정성을 평가하였다. 그 결과로 상악 구치는 2.22mm 압하(P < 0.001)가 일어났고, 평균 17.4개월 유지 후 0.23 mm의 정출(P = 0.359)이 나타났으며, 재발율은 10.36%로 나타났다. 전치부의 수직피개는 평균 5.47 mm의 증가(P < 0.01)가 나타났고 평균 17.4개월 유지 후 0.99 mm의 감소(P < 0.05)가 나타나서, 재발량은 18.10%로 계산되었다. 치료 전후 수치 변화량의 상관분석을 통해 상악구치 압하량과 하악 평면각 변화량이 상관관계(P < 0.05)가 있었다. 치료 전의 개방교합량, 하악 평면각, 전하안면고경과 치료 후의 수직피개 재발량과는 상관관계가 없었다. 상악 구치 압하량과 수직피개 개선량에서 치료에 의한 변화량과 재발량이 통계적으로 유의한 상관관계를 보였고, 이를 토대로 재발량을 예측할 수 있었다. 이상의 결과를 통하여 성인 개방교합 환자에서 miniscrew implant를 이용한 상악구치부의 압하는 비수술, 심미적 접근방법으로 효율적이고 안정적인 치료방법으로 유용하게 사용할 수 있을 것으로 생각된다. Objective: This study investigated the skeletal and dentoalveolar changes after intrusion of maxillary posterior teeth using miniscrew implants during the treatment and posttreatment period. Method: The subjects consisted of 11 adults (1 male, 10 females) whose open bites were treated by posterior teeth intrusion with miniscrew. The lateral cephalometric x-rays of pre-treatment, post-treatment, and the retention period were evaluated. Results: The average intrusion of maxillary posterior teeth at the end of the treatment period was 2.22 mm (P < 0.001) and the average extrusion of maxillary posterior teeth at the end of the 17.4 month retention period was 0.23 mm (P = 0.359). The relapse rate was calculated at 10.36%. A decrease in overbite after the 17.4 month retention period was 0.99 mm (p < 0.05). The relapse rate was calculated at 18.10%. The skeletal changes and vertical height change of maxillary posterior teeth during the retention period was statistically insignificant, but there was a significant decrease in overbite (mean 0.99 mm, P < 0.05). The amount of intrusion and the overbite improvement from treatment significantly correlates to the degree of relapse. Conclusions: The results indicate that intrusion of the maxillary posterior teeth using miniscrews is an effective, non-surgical treatment modality which can reasonably be used to address adult open bite.

        A Survey of Satisfaction of Physical Therapy Course according to Teaching Ways after COVID-19

        Purpose: We investigated the satisfaction of physical therapy course according to teaching ways after COVID-19. Methods: 336 students in major of physical therapy were recruited in this study. Based on the classification of subjects in the national examination, the questionnaire was divided into 6 subjects in the basic field of physical therapy, 2 subjects in the field of physical therapy diagnostic evaluation, 8 subjects in the field of physical therapy intervention, and 3 subjects in other fields. The Likert scale was used. Results: In the basic field of physical therapy, all subjects were shown the high score of the satisfactory in face-to-face classes except for the public health and medical law compared to the non-face-to-face classes and mixed classes. Regarding the field of physical therapy diagnostic evaluation, the principle of diagnostic evaluation was shown the high score of the satisfactory in face-to-face classes compared to the non-face-to-face classes and mixed classes. In the field of physical therapy intervention, all subjects were shown the high score of the satisfactory in face-to-face classes compared to the non-face-to-face classes and mixed classes. Conclusion: We found that the face-to-face classes in most of subjects was shown the high score of satisfactory. We believed that our results can be used as basic data for physical therapy major learning methods.

        함수율(含水率)이 옥수수립(粒)의 압축특성(壓縮特性)에 미치는 영향(影響)

        옥수수 수확(收穫)의 기계화(機械化) 사업(事業)을 촉진(促進)시키기 위(爲)하여, 옥수수립(粒)의 제반압축특성(諸般壓縮特性)을 규명(糾明)하며 모든 기계화(機械化) 작업도정(作業過程)에서 손상율(損傷率)이 최소(最小)로 되는 기계(機械)의 설계자료(設計資料)를 얻고자 함수율(含水率)이 옥수수립(粒)의 압축특성(壓縮特性)에 미치는 영향(影響)을 구명(究明)하기 위(爲)하여 국내재배(國內栽培)한 3품종(品種)의 옥수수를 공시재료(供試材料)로 압축방향(壓縮方向)은 평면(平面), 측면(側面), 직립(直立)의 3 수준(水準), 함수율(含水率)은 약(約) 13, 17, 21, 25%(w.b)의 4수준(水準)으로 하고 재하속도(載荷速度) 1.125mm/min의 준(準) 정하량(靜荷重)에서 Straingage system을 이용(利用), 평판축시험(平板縮試驗)을 실시(實施)하여 옥수수의 강복점(降伏點) 및 파괴점(破壞點)에 대(對)한 압축응력(壓縮應力), 변형(變形), 에너지 및 강성계수(剛性係數)를 측정(測定), 분석(分析)하였으며 그 결과(結果)를 요약(要約)하면 다음과 같다. 1. 옥수수의 함수율(含水率)이 약(約)12.5~24.5%(w.b)의 범위(範圍)일 때 평면위치(平面位置)의 강복하중(降伏荷重)은 13.63~26.73kg, 최대압축강도(最大壓縮强度)는 21.55~47.65kg 으로 함수율(含水率)이 약(約) 17% 일 때 최소(最小), 약(約) 21% 일 때 최대(最大)였으며, 측면(側面) 강복하중(降伏重荷)은 13.58~6.70kg 이었고, 측면(側面)의 최대압축강도(最大壓縮强度) 16.42~7.82kg은 직립(直立)의 최대압축강도(最大壓縮强度)인 18.55~9.05kg 보다 약간 작게 나타났다. 2. 함수율(含水率)이 약(約) 12.5~24.5%의 범위(範圍)에서, 강복변형(降伏變形)은 0.43~1.37mm, 파괴변형(破壞變形)은 0.70~2.66mm로 함수율(含水率)에 비례(比例) 변형(變形)하였으며 함수율(含水率)이 높을수록 변형율(變形率)도 증가(增加)하였다. 3. 옥수수의 함수율(含水率)이 약(約) 12.5~24.5%일 때, 탄성(彈性)에너지는 $2.60{\sim}8.57kg{\cdot}mm$, 인성에너지는 $6.41{\sim}34.36.kg{\cdot}mm$로 함수율(含水率)이 증가(增加)할수록 측면(側面)의 인성에너지는 증가(增加)하였고, 직립(直立)의 경우에 감소(減少)하여 22~23%의 함수율(含水率) 구간(區間)에서 측면(側面)과 직립(直立)에 압축(壓縮)에 관(關)한 인성에너지의 값은 동일(同一)하였다. 4. 강성계수(剛性系數)는 함수율(含水率)의 증가(增加)에 따라 감소(減少)하였고, 측면(側面)의 32.07~5.86kg/mm 보다 평면압축(平面壓縮)에서 42.12~18.68kg/mm로 컸으며 수원(水原)19호(號)가 부여계통(扶餘系統)보다 크게 나타났다. 5. 옥수수는 평면(平面)에 관(關)한 압축특성(壓縮特性)이 각종(各種) 기계(機械) 설계(設計)의 자료(資料)로 가장 중요(重要)하며, 함수율(含水率)이 약(約) 12.5~17%(w.b)일 때 최소(最少)의 분쇄(粉碎)에너지가 소요(所要)되며, 약(約) 19~24%(w.b)의 함수율(含水率)에서 옥수수를 조작(操作)할 때 곡립손상(穀粒損傷)을 감소(減少)시킬 수 있을 것으로 사료(思料)된다. In order to promote mechanization of corn harvesting in Korea, this study was conducted to find out the effect of moisture content on compressive properties such as force, deformation, energy and modulus of stiffness to the bioyield and the rupture point for Korean corn kernel. In this study, the loading positions of corn were flat, edge, longitude and the moisture contents were about 13, 17, 21, 25% in wet basis. The compression test was carreied out with flat plate by use of dynamic straingage for three varieties of Korean corn under quasi-static force when the loading rate was 1.125mm/min. The results of this study are summarized as follows; 1. When the moisture content of corn ranged from 12.5 to 24.5 percent, at flat position, the bioyied force was in the range of 13.63-26.73 kg and the maximum compressive strength was in the range of 21.55-47.65kg. Their values were reached minimum at about 17% and maximum at about 21% moisture content. The bioyield force was in the range of 13.58-6.70kg at edge position and the maximum compressive strength which was 16.42 to 7.82kg at edge position was lower than that which was 18.55-9.05kg at longitudinal position. 2. Deformation of corn varied from 0.43 to 1.37 mm at bioyield point and from 0.70 to 2.66mm at rupture point between 12.5 to 24.5% moisture content. As the moisture content increased, deformation was increased. 3. The moduli of resilience and toughness of corn ranged from 2.60 to 8.57kg. mm and from 6.41 to 34.36kg. mm when the moisture content ranged from 12.5 to 24.5 percent, respectively. As the moisture content increased, the modulus of toughness was increased at edge position and decreased at longitudinal position. And their values were equal each other at 22-23% moisture content. 4. The modulus of stiffness was decreased with increase in the moisture content. Its values ranged from 32.07 to 5.86 kg/mm at edge position and from 42.12 to 18.68kg/mm at flat position, respectively. Also, the values of Suweon 19 were higher than those of Buyeo. 5. It was considered that the compressive properties of corn at flat position were more important on the design data for corn harvesting and processing machinery than those of edge or longitudinal position. Also, grinding energy would be minimized when a corn was processed between about 12.5 to 17% moisture content and corn damage would be reduced when a corn was handled between about 19 to 24% moisture content in wet basis.
