Bevacizumab (Avastin(R)) is a monoclonal antibody against the vascular endothelial growth factor (VEGF) receptor that increases the overall survival rate when added to standard chemotherapy regimens in patients with metastatic colorectal cancer. The known toxicities of bevacizumab are hypertension, proteinuria, wound healing complications, arterial thrombosis, bleeding, and gastrointestinal complications. Especially ischemic colitis can rapidly develop into bowel perforation, so an emergency operation often is needed. Recently, a 65-year-old male patient developed ischemic pancolitis after FOLFOX (85 mg/m2 Oxaliplatin, d1; 200 mg/m2 Leucovorin, d1; 400 mg/m2 5-FU iv bolus, d1-2; and 600 mg/m2 5-FU, d1-2, every two wk) and Bevacizumab combination chemotherapy was administered. However, he recovered after early conservative care without surgery. We report this case with a review of literature. (Korean J Gastroenterol 2011;58:42-46)
프레임 율 변환을 위한 새로운 비용함수를 사용한 움직임 추정 기법

본 논문에서는 새로운 움직임 추정(motion estimation, ME) 방식을 사용한 프레임 비율 변환(frame rate conversion, FRC) 기법에 대해 제안한다. 기존의 프레임 비율 변환을 위한 움직임 추정 방식은 영상 압축에서 사용되고 있는 SAD를 사용하여 블록(block) 단위로 움직임 벡터를 추정하는 방식에 기초를 두고 있다. 그러나 잔여 신호(residual signal)를 저장하는 영상 압축과 달리, 잘못된 움직임 추정은 합성된 출력 영상에서 심각한 품질 저하를 가져올 수 있다. 이를 보완하기 위해 움직임 개선(motion refinement, MR)이 사용되고 있지만, 근본적인 해결을 위해서는 정확한 움직임 추정 알고리즘 사용이 필요하다. 특히 SAD를 통한 움직임 추정은 고르지 못한 움직임 벡터장(motion vector field, MVF) 을 형성할 수 있으며, 종래의 연구에서 이를 해결하기 위해 SAD(sum of absolute difference)에 벡터의 공간제약(spatial constraint) 항목을 추가하여 비교적 고른 움직임 벡터장을 형성하는 방식이 제시되었다. SAD와 공간 제약 항목의 반영 비율에 따라 움직임 벡터의 중요성과 움직임 벡터장의 일관성이 서로 상충하는데, 기존의 방식은 이 비율을 일정한 상수(constant)값을 사용하고 있으며, 이러한 방식은 이미지의 특성에 따라 결과가 달라진다. 본 논문에서는 SAD와 공간 제약 항목 사이의 반영 비율을 이미지의 특성에 적응하는 방식을 사용하는 움직임 예측을 제시하고, 수행한 결과를 기존의 방식에 의한 결과와 비교하였다.

Pseudomembranous colitis is mainly caused by antibiotics and Clostridium difficile infection. But conditions such as gastrointestinal surgery, antacid medication, anti-neoplastic agent or immunosuppressive agent which influences the normal flora of colon can induce colitis without the administration of any antibiotics. We experienced a 13 year-old male who was taking low-dose methotrexate for juvenile rheumatoid arthritis complained diarrhea and abdominal pain for 3 weeks. Sigmoidoscopic findings revealed diffuse patch yellowish pseudomembranes on the rectum. Histologic finding was compatible to pseudomembranous colitis. His symptom was improved after stop taking methotrexate and the administration of metronidazole. If a patient treated with immunosuppressive agents or antineoplastic agents complains diarrhea, fever or abdominal pain and has not improved with conservative care, pseudomembranous colitis should be taken into account as a differential diagnosis and prompt treatment is required for better prognosis. 기존 건물 에너지 데이터 분석을 통한 원격 에너지진단 알고리즘 성능 검증 연구

Energy reduction and efficiency in existing buildings are an essential part of energy saving in building sector. Measurement of energy consumption by end-use and analysing data to find opportunities in improvement of energy performance are the key steps and the most important part of energy saving activities for buildings. This paper introduces and studies a recently-developed remote energy diagnosis program which has a new approach in building energy audit and analysis processes. The program consists of two algorithms - disaggregation and prediction. The first one is an algorithm for disaggregation of electricity consumption by end-use which divides the actual whole electricity consumption data of a building into three different end-use loads - heating & cooling load, lighting & other power supply load, and base load. Building characteristic factors in both heating and cooling periods are calculated for each building in this algorithm. The second algorithm provides the prediction of weather-corrected electricity consumption of a whole building. The output of the first algorithm and present outdoor temperature are combined for the prediction of present normal electricity consumption. This program does not require any difficult-to-get information or any hardwares for measurement. The analysis procedure is a lot quicker than the traditional on-site diagnosis approach. The validation of this remote energy diagnosis program is made based on comparison with actual building electricity consumption data. The result shows that the mean bias error(MBE) is -0.12% and coefficient of variation of the root mean square error(CV) is 5.7%, which are much better results than ASHRAE standards of acceptable calibration tolerances of ±5% and 30% respectively.
