      • 게이트심장혈액풀 스캔에서 Half-Time 획득 인자 적용에 따른 임상적 유용성 평가

        Purpose: The scan time reduction helps to yield more accurate results and induce the minimization of patient's motion. Also we can expect that satisfaction of examination will increase. Nowdays medical equipment companies have developed various programs to reduce scan time. We used Onco. Flash (Pixon method, SIEMENS) that is an image processing technique gated cardiac blood pool scan and going to evaluate its clinical usefullness. Materials and Method: We analyzed the 50 patients who were examined by gated blood pool scan in nuclear medicine department of Asan Mediacal Center from June $20^{th}$ 2008 to August $14^{th}$ 2008. We acquired the Full-time (6000 Kcounts) and Half-time (3000 Kcounts) LAO image in same position. And we acquired LVEF values ten times from Full-time, Half-time images acquired by the image processing technique and analyzed its mean and standard deviation values. To estimate LVEF in same conditions, we set automatic location of the LV ROI and background ROI based on same X and Y-axis. Also we performed blinding tests to physician. Results: After making a quantitative analysis of the 50 patients EF values, each mean${\pm}$standard deviation is shown at Full-time image $68.12{\pm}7.84%$, Half- time (acquired by imaging processing technique) $68.49{\pm}8.73%$. In the 95% confidence limit, there was no statistically significant difference (p>0.05). After blinding test with a physician for making a qualitative analysis, there was no difference between Full-time image and Half-time image acquired by the image processing technique for observing LV myocardial wall motion. Conclusion: Gated cardiac blood pool scan has been reported its relatively exact EF measured results than ultrasound or CT. But gated cardiac blood pool scan takes relatively longer time than other exams and now it needs to improve time competitive power. If we adapt Half-time technique to gated cardiac blood pool scintigraphy based on this study, we expect to reduce possible artifacts and improve accessibility as well as flexibility to exam. Also we expect patient's satisfaction.

        하악골 부분절제술 시행한 부분 무치악 환자에서 보철 수복 증례

        하악골 연속성의 상실은 하악 기능의 균형과 대칭에 영향을 주며, 하악의 움직임에 변화를 유발하고 절제된 부위로 잔존하악골을 편위시킨다. 또한 연하, 발음, 하악의 운동, 저작, 호흡과 같은 다른 부분에서도 기능이상을 나타낸다. 하악골의 일부가 결손된 환자의 경우, 일반적으로 외과적 재건에 이어 보철적 수복이 이루어진다. 하지만 BRONJ (Bisphosphonate related osteonecrosis of the Jaw)와 같은 전신적 질환을 가진 환자의 경우, 외과적 재건의 적용에 제한이 있게 되고 전적으로 보철적 수복에 의지하게 된다. 다양한 보철적 방법이 하악의 변위를 최소화하고 저작 효율과 기능 및 심미를 개선하기 위하여 제시되었다. 가철성 국소의치로 치료 시, 입술, 뺨, 혀의 운동에 의해 측방 및 수평력이 발생하며 기능 인상을 통해 이를 최대한 줄여 주어야 한다. 또한, Twin occlusion을 사용하면 적절한 구순 및 협측의 지지를 얻으면서 교합관계의 형성을 가능하게 할 수 있다. 이 증례보고에서는 하악골 결손 증례에서 중립대 인상 방법과 twin occlusion을 통하여 기능적 심미적으로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다. Loss of continuity of the mandible destroys the balance and symmetry of mandibular function, leading to altered mandibular movements and deviation of the residual fragment towards the resected side. Apart from deviation, other dysfunctions include difficulty in swallowing, speech, mandibular movements, mastication, and respiration are accompanied. In general, surgical reconstruction is considered first then proceeds to the prosthetic restorations. However, patients with systemic disease such as BRONJ (Bisphosphonate related osteonecrosis of the Jaw), surgical reconstruction may be limited. Thus, the prosthetic restoration remains as the only resort. Numerous prosthetic methods are employed to minimize deviation and to improve masticatory efficiency, function and esthetics. If a removable partial denture is the selected treatment modality, maximum stability of the partial denture base may be accomplished with a functional impression procedure by means of eliminating lateral and horizontal forces caused by the functional movements of the lips, cheeks and tongue. Also, Twin occlusion is used to obtain a favorable occlusal relationship and check support for esthetics. The purpose of this case report is to demonstrate how neutral zone impression technique and twin occlusion scheme were applied to restore a hemi-mandiblectomy patient with BRONJ syndrome to achieve satisfactory results in functional and esthetic aspects.

        신제품 개발을 위한 GAN 기반 생성모델 성능 비교

        Amid the recent rapid trend change, the change in design has a great impact on the sales of fashion companies, so it is inevitable to be careful in choosing new designs. With the recent development of the artificial intelligence field, various machine learning is being used a lot in the fashion market to increase consumers' preferences. To contribute to increasing reliability in the development of new products by quantifying abstract concepts such as preferences, we generate new images that do not exist through three adversarial generative neural networks (GANs) and numerically compare abstract concepts of preferences using pre-trained convolution neural networks (CNNs). Deep convolutional generative adversarial networks (DCGAN), Progressive growing adversarial networks (PGGAN), and Dual Discriminator generative adversarial networks (DANs), which were trained to produce comparative, high-level, and high-level images. The degree of similarity measured was considered as a preference, and the experimental results showed that D2GAN showed a relatively high similarity compared to DCGAN and PGGAN.

        다공성 흑연 소재를 이용한 바나듐 도핑된 반절연 SiC 단결정 성장의 특성 연구

        본 연구에서는 다공성 흑연 캡슐에 Vanadium carbide(VC) 분말을 채워 성장시킨 방법과 SiC 분말과 VC 분말을 혼합하여 다공성 흑연판을 그 위에 덮은 후 성장시키는 방법으로 진행하였으며, 성장된 결정들은 여러 분석방법을 사용하여 각각의 특성들을 관찰하였다. 반절연 SiC 성장은 6H-SiC 종자 결정을 사용하여 PVT(Physical Vapor Transport)법으로 성장을 진행하였다. 반절연으로 성장된 SiC 결정은 XRD를 이용하여 6H-SiC인 것을 확인하였으며, SIMS 분석결과 바나듐 도핑 농도가 바나듐 용해의 한계값 보다 높을 경우 석출물이 발생되며, 결정 품질 저하의 원인이 됨을 확인할 수 있었다. Vanadium-doped SiC crystals have been grown by using a porous graphite inner crucible filled with vanadium carbide (VC) and by using a porous graphite plate and SiC + VC powders, respectively. Semi-insulating SiC crystals were grown onto the 6H-SiC seed crystals by PVT (Physical Vapor Transport) method. The grown crystals were indicated to be 6H-SiC polytype by XRD. As result of SIMS analysis, vanadium-rich precipitates were observed when the vanadium concentration was relatively higher than the maximum solubility of vanadium ($3-5{\times}10^{17}cm^{-3}$) in vanadium-doped SiC crystals, which resulted in degradation of crystal quality.

        야생조류에 대한 조류인플루엔자 예찰의 중요성과 연구 동향

        Avian influenza viruses (AIV) have been isolated from a wide range of domestic and wild birds. Wild birds, predominantly ducks, geese and gulls form the reservoir of AIV in nature. The viruses in wild bird populations are a potential source of widespread infections in poultry. Active surveillance for AIV infection provides information regarding AIV distribution, and global AIV surveillance can play a key role in the early recognition of highly pathogenic avian influenza (HPAI). Since 2003 in Korea, there have been four H5N1 HPAI outbreaks caused by clade 2.5, 2.2 and 2.3.2. Therefore, improvement of AIV surveillance strategy is required to detect HPAI viruses effectively. This article deals with the major events establishing the role of wild birds in the natural history of influenza in Korea. We highlighted the need for continuous surveillance in wild birds and characterization of these viruses to understand AIV epidemiology and host ecology in Korea.

        한국적 비상대비 동원업무 발전방안

        Developments toward a 'the end of the Cold War' are the main factor determining the situation in the Korean Peninsula. While all the combat resources of North Korea are systematically controlled and kept available for instant mobilization even in peacetime. South Korea is inferior to North Korea in terms of mobilization system and efficiency, exercise intensity and equipments. In sum, the South faces far greater restraints in wartime conversion capability than does the North. Mobilization refers to a nation's compulsory control, management and operation of its human and material resources for the purpose of national security. The total capability for safeguarding national security must be pursued by the continuous examination and complementation of our mobilization system and capability. This paper proposes an efficient management of national mobilization system that can be instantly operated on the whole in time of national emergency. Therefore, the actual operation of National Security Council is suggested to prepare operations against the emergencies and establish the foundation for national security.

      • 심장기능상태의 분류에 따른 게이트심장혈액풀 검사와 심장초음파의 심박출계수 상관관계에 관한 고찰

        게이트심장혈액풀 검사와 심장초음파로 산출된 좌심실 심박출계수(Ejection Fraction : EF)는 높은 상관성을 가진다는 연구가 보고되고 있지만 대부분의 연구는 심장기능의 상태를 고려하지 않은 채 모든 환자들을 대상으로 비교 분석하였다. 이에 본 연구에서는 심장의 기능상태에 따라 분류된 대상에 대하여 게이트심장혈액풀 검사와 심장초음파에서 평가된 심박출계수의 상관성에 관하여 고찰하고자 한다. 본원에서 게이트심장혈액풀 검사와 심장초음파를 모두 시행한 환자 중 유방암 진단을 받은 여자 60명을 대상으로 하였다. 감마카메라는 Infinia (GE healthcare, Milwaukee, USA)와 Ventri (GE healthcare, USA)를 사용하였으며, 체내 표지법으로 좌전사위상(Left Anterior Oblique, LAO)영상을 획득한 후, 전면상(Anterior View)과 좌측면상(Left Lateral View)을 각각 획득하였다. 영상분석은 좌전사위상에서 심박출계수가 50% 이상 70% 미만으로 평가된 환자 30명(연령: $58.27{\pm}13.48$)과 50% 미만의 환자 30명(연령: $53.70{\pm}8.45$)을 각각 정상군(Normal)과 비 정상군(Mild LV Dysfunction)으로 분류하여 SPSS ver. 18.0로 통계 분석하였다. 그룹 간 판독결과의 차이를 확인하기 위하여 전문의에 의해 판독된 결과를 참고하였다. 정상군의 게이트심장혈액풀 검사와 심초음파의 심박출계수의 평균값은 각각 $66.43{\pm}5.80$, $60.50{\pm}4.93$로 심초음파의 값이 다소 낮게 측정되었으며, 통계적으로 유의한 차이가 있었다(P<0.001). 비 정상군의 게이트심장혈액풀 검사와 심초음파의 심박출계수는 평균값이 $41.93{\pm}7.58$, $41.70{\pm}11.49$로 통계적으로 유의한 차이가 없었다(P>0.001). 정상군간의 판독결과에서 30건 모두 동일한 판독결과를 보였으나 비 정상군간에서는 30건 중 8건에서 판독의 불일치를 보였다. 본 연구를 통하여 게이트심장혈액풀 검사와 심장초음파의 심박출계수 상관관계를 통계적으로 확인할 수 있었으며, 비 정상군간의 결과에 주관적인 차이가 발생하는 것을 확인 할 수 있었다. 심장기능상태 분류에 따른 결과를 인식하고 게이트심장혈액풀 스캔과 심장초음파를 상호대체 검사로 임상에서 활용한다면 보다 정확한 결과 값을 산출하여 진단에 도움이 될 것으로 사료된다. Purpose We considered the correlation of Ejection Fraction (EF) which was analyzed by Multi Gated Blood Pool Scan (MUGA) and Echocardiography (ECHO) for the patients who were classified according to the condition of cardiac function. Materials and Methods We analyzed the patients (female 60) who were diagnosed with breast cancer and were examined by both MUGA and ECHO. The 30 patients (age: $58.27{\pm}13.48$) who were analyzed into less than 50% to 70% of EF were categorized as normal group and the other 30 patients (age: $53.70{\pm}8.45$) who were analyzed into less than 50% of EF were categorized as abnormal group. Statistical analysis with SPSS ver. 18 was applied. Results Each of the value of mean and standard deviation of normal group was $66.43{\pm}5.80$ (MUGA), $60.50{\pm}4.93$ (ECHO). There was a significant difference (p<0.001). Each of the value of mean and standard deviation of abnormal group was $41.93{\pm}7.58$ (MUGA), $41.70{\pm}11.49$ (ECHO). There was no significant difference (p>0.001). In the result, all 30 cases of normal group showed the same reading. 8 out of 30 cases in abnormal group showed inconsistency of the reading. Conclusion We could confirm the correlation of the EF in MUGA and ECHO statistically. There was difference between abnormal groups from the result of reading. If we are aware of the result according to the different cardiac function categorization, MUGA and ECHO can be used as even more accurate interchangeable test.
