        정형외과 영역에서 근골격계 초음파의 활용 범위가 넓어짐에 따라 슬와부 신경 차단(좌골 신경 차단)이나 대퇴 신경 차단(또는 복재 신경 차단)이 초음파를 이용하여 신경 손상이나 불완전 마취에 대한 걱정없이 쉽고, 안전하게 시행될 수 있게 되었다. 그로 인하여 족부 수술 뿐만 아니라 족관절 및 하퇴부의 정형외과적 수술이 전신 마취나 척추 마취의 부담없이 시행될 수 있게 되었다. 이러한 초음파 유도 대퇴좌골 신경 차단에 대한 자세한 술기를 120예 이상의 경험을 바탕으로 보고하고자 한다. Since the extent of use of musculoskeletal ultrasound in orthopaedic surgery is expanding, popliteal block(sciatic nerve block) and femoral nerve block(saphenous nerve block) are easily and safely performed without complications such as nerve injury or incomplete block. Also, due to the expanding use of ultrasound, orthopaedic surgery of not only foot but also ankle and lower leg could be done without general anesthesia or spinal anesthesia. We describe a detailed technique for ultrasound-guided femorosciatic nerve block based on the experience over 120 cases.

      • 케나프 사일리지 품질특성 분석 및 최적 조성조건 설정

        The proper harvester(Camp champion C 3000) equip condition to get under 5mm Kenaf(Hibiscus cannabinus L.) harvesting particle size were set the number of blade more than 4 and rotate over 100 rpm. The small size of harvesting particle could promote the fermentation of lactobacillus and it improved the exterior and chemical evaluation of kenaf silage. The fittest fermentation periods to sustain basic levels of kenaf silage quality was more than 40 days. To secure the most suitable stage for harvest, we analyzed yields, crude protein and total digestible nutrients contents along culture periods. At 120 days after seeding, kenaf yields approach 9,600 kg/10a and crude protein contents was 8.2% which belonged most preferable grade. also the total digestible nutrients reached 52.2%. In the process of kenaf fermentation, add molasses 0.5∼2.0% could strengthen the degradation of cellulose and elevate the preference of flavor. To select the best microbe for making kenaf silage, we applied the 4 kinds of lactobacillus which originated from Kimch and make an patent application by Jeonbuk ARES. As a result L. mesenteroides M-17 was sellected for enhancing the preference of flavor and sustaining the good grade silage quality. Adding L. mesenteroides M-17 in the process of fermentation could elevated the crude protein contents and get good grade of relative feed value.

        Intravenous Patient Controlled Analgesia (IV PCA0 after general or spinal anesthesia may be a method of postoperative pain control, but side effects such as nausea, vomiting, and sedation occurs in most patients. The following research is based on the ultrasound guided femorosciatic nerve block held on parts below the knee joint operation. Because this anesthesia is held locally on the sciatic nerve with continuous anesthesia performed through perineural catheterization, the complications of nausea, vomiting, and sedation may be reduced while postoperative pain caused by the sciatic nerve is controlled. The following report is held on this experience.

        99mTc-HMPAO의 광학이성체 d-, l-Form의 합성과 뇌섭취율 비교

        N/A Purpose: 99mTc-HMPAO is a radiopharmaceutical for imaging cerebral blood flow. HMPAO (RR, SS)-4,8- diaza-3,6,6,9-tetramethylundecan-2,10- dione bisoxime) has three stereoismers such as, meso-, d-, and l-HMPAO. Techentium complexes of meso-HMPAO and d,l-HMPAO are known to have different in vivo brain uptakes. In this study, enantiomers of HMPAO (d-HMPAO and l-HMPAO) were separated from d,l-HMPAO. These enantiomers were labeled with 99mTc and the biodistribution studies were performed in mice. Materials and Methods: An intermediate imine product was produced from 2,3-butanedione monooxime and 2,2-dimethyl- 1,3-propanediamine (54% yield) and was reduced into a mixture of three isomers (35% yield). The meso-isomer was separated from d,l-mixture by repeated fractional crystallization (11% yield). The d- and l-enantiomers were subsequently separated by co-crystallization with optical isomers of tartaric acid (25% and 5% yield, respectively). Each enantiomeric HMPAO was labeled with 99mTc by reacting with SnCl2·2H2O and 99mTc-pertechnetate. Biodistribution study was performed 1 hr after tail vein injection to ICR mice. Results: Radiochemical purities of each compound were over 80%. In biodistribution study, the brain uptakes of d,l- d- and l-form were 1.34, 1.12 and 1.67% ID/g, respectively. In case of l-Isomer the brain uptake was higher (1.5 fold) than d-isomer. Conclusion: We successfully purified each enantiomeric HMPAO. In biodistribution study of stereoismers of 99mTc-HMPAO in mice, l-HMPAO may show better brain image than d,l-HMPAO which was supplied in a commercial kit. (Korean J Nucl Med 2001;35:69-74)

      • Long-baseline single-layer 2nd-order $high-T_c$ SQUID gradiometer

        We have studied feasibility of single-layer second-order $high-T_c$ SQUID gradiometers in magnetocardiography. We have measured human cardiomagnetic signals using a short-baseline (5.8 mm) single-layer second-order YBCO gradiometer in partially shielded environments. The gradiometer has an overall size of $17.6\;mm{\times}6\;mm$ and contains three parallel-connected pickup coils which are directly coupled to a step-edge junction SQUID. The gradiometer showed an unshielded gradient noise of $0.84\;pT/cm^2/Hz^{1/2}$ at 1 Hz, which corresponds to an equivalent field noise of $280\;fT/Hz^{1/2}$. The balancing factor was $10^3$. Based on the same design rules as the short-baseline devices, we have studied fabrication of 30 mm-long baseline gradiometers. The devices had an overall size of $70.2\;mm{\times}10.6\;mm$ with each pickup coil of $10\;mm{\times}10\;mm$ in outer size. As Josephson elements we made two types of submicron bridges, which are variable thickness bridge (VTB) and constant thickness bridge (CTB), from $3\;{\mu}m-wide$ and 300 nm-thick YBCO lines with a thin layer of Au on top by using a focused ion beam (FIB) patterning method. VTB was 300 nm wide, 200 nm thick, 30 nm long with Au removed and CTB 100 nm wide and 30 nm long. In temperature-dependent critical currents, $I_c(T)$, VTB showed an nonmetallic barrier-type behavior and CTB an SNS behavior. We believe that those characteristics are ascribed to naturally formed grain boundaries crossing the bridges.

        커피전문점을 이용하는 고객 라이프스타일에 따른 서비스품질 및 고객성과 차이분석

        본 연구는 커피전문점 이용고객들의 라이프스타일 유형에 따른 서비스품질과 고객성과의 차이를 검증하는데 목적이 있다. 연구결과를 살펴보면, 커피전문점 이용고객의 라이프스타일은 군집간 통계적으로 유의한 차이가 있는 것으로 검증되었으나 서비스품질과 고객성과에서의 군집간 차이는 부분 존재하 였다. 특히, ‘친화교류군’이 서비스품질과 고객성과에 대한 평균이 높게 나타났으며, 서비스품 질의 ‘분위기’와 ‘교통’ 차원은 물론, 고객성과의 ‘만족’과 ‘충성’ 차원에서 ‘친화교류군’이 ‘운동 관리 및 음식조절군’에 비해 통계적으로 유의한 차이가 있으며 평균이 높다는 것을 알 수 있다. 시사점은 다음과 같다. 첫째, 앞으로 커피전문점의 추세는 교통이 편리하고 전반적인 커피 전문점의 분위기를 단순히 커피를 마시거나 약속장소로서의 공간 활용이 아닌 휴식 공간 또는 문화적인 공간으로써 활용이 가능하도록 분위기를 만들어 마케팅에 활용한다면 소비자들이 보다 더 쉽게 이용하여 매출증진에 크게 기여할 것으로 생각된다. 둘째, 본 연구에서 운동관리 및 음식조절군 의 서비스품질 및 고객성과 점수가 낮게 나타났다. 이들을 보완하기 위해서는 기존의 풍부한 크림과 달콤한 메뉴도 중요하지만 건강을 중요시하는 추세에 맞추어 웰빙 음료나 다이어트 음료 등 보다 더 다양한 건강식 메뉴를 개발하여 마케팅 전략을 잘 계획한다면 실무적으로 매우 유용한 정보가 될 뿐만 아니라 매출 증진에도 크게 기여할 것으로 생각된다. The purpose of this study is to verify the differences of service quality and customer performance depending on the lifestyle of coffeehouse chain customers. The study result shows that the lifestyle of coffeehouse chain customers is verified to have statistical significance among groups, but partial difference exists in service quality and customer performance among different groups. In particular, networking group shows high average score in service quality and customer performance and “networking group” shows more statistical significance and higher average score than “excercise and food control group” in “atmosphere” and “transportation” as service quality and “satisfaction” and “loyalty” as customer performance. The implication of this study is as follows. First, the trend of coffeehouse chain is not only to use the place for enjoying coffee or meeting with people but also to use the place for refresh and culture. If the atmosphere is created in this direction and promoted, consumers can easily visit coffeehouses contributing significantly to sales growth. Second, this study shows that the score of service quality and customer performance of “excercise and food control group” is low. To improve this, it is better to develop a healthier menu such as well-being beverage or diet beverage in line with health-conscious trend and plan a marketing strategy better based on the healthier menu even though existing sweet menu with rich cream is also important. Then, the healthier menu is considered to be very useful information and contribute significantly to sales growth.

      • 자기부상열차 RAM DATA 관리방안

        This paper is reviewed RAM(Reliability, Availability and Maintainability) data table utilized for RAM data management to Urban Maglev Transit. As railway systems become more complex, the RAM requirements are reinforced to ensure that a design meets Reliability, Availability, Maintainability criteria. Therefore, it needs the efficient management for RAM data of railway system to meet RAM target. At this study, RAM data management format is suggested to ensure reliability and maintainability based on acquired experience for overseas rolling stock. This RAM data table and FMECA(Failure Mode Effect Criticality Analysis) table are useful to the calculation of MTBF(Mean Time Between Failure), MTBSF(Mean Time Between Service Failure) and Maintainability. Also, this RAM management table will be efficient to improve the RAM evaluation to Urban Maglev Transit.
