수용액상에서 키토산의 구조변화를 UV spectra 및 CD spectra를 이용하여 연구하였다. 요오드와 키토산의 상호작용을 UV spectra로 관찰한 결과 pH 감소에 따라 246㎚에서의 새로운 상호작용 peak가 증가되었으며, 이는 NaCl의 농도가 증가함에 따라 나타나는 246㎚의 상호작용 peak와 일치하였다. 그러나 urea의 농도를 변화시킨 경우에는 246㎚의 상호작용 peak가 관찰되지 않았다. 또한 CD spectra에서는 키토산용액의 경우 197㎚에서 강한 양의 Cotton효과를 나타내었으며, 208㎚ 및 222㎚에서 강한 음의 Cotton 효과를 보였다. 이러한 peak들은 NaCl의 농도를 증가시켜 줌에따라 그 세기가 감소하는 경향을 나타내었다. For the study of structural change of chitosan which is dissolved in aqueous solution, UV spectra and CD spectra were used. The interaction between chitosan and iodine observed in UV spectra shows that new interaction peak at 246㎚ increases as the pH decreases, which is also observed as NaCl concentration increases. But there is no observable peak at 246㎚ when the concentration of urea is varied. CD spectra of chitosan solution reveals the strong positive Cotton effect at 197㎚, and strong negative Cotton effect at 208㎚, 222㎚. As the concentration of NaCl increases, the intensity of these peaks decreases.

N/A Dialyzer reuse is an increasingly common practice in the management of end stage renal disease patients on maintenance hemodialysis, but the validity of this form of treatment still remains in question. The performance of dialyzer reuse was studied prospectively in 30 chronic hemodialysis patients using new(n=14) and reused(n=16) membranes. Clearance of solutes, hypoxemia and leukopenia were examined before, during and after hemodialysis. The results were summarized as follows: 1) Membrane clearance of small solutes (urea, creatinine) was maintained over dialyzer reuse. 2) Arterial oxygen tension was significantly lower at 30 minutes after hemodialysis in both new and reused cuprammonium dialyzer, but there was no significant difference between two groups. 3) Leukopenia was noted at 30 minutes after hemodialysis in both new and reused cuprammonium dialyzer group but change in WBC count was significantly lower in reused dialyzer group, 4) Transdialyzer changes in pH, hematocrit and bicarbonate at completion of hemodialysis were similar in both groups.

We report a case of a 25-year old man with chronic kidney disease with secondary hyperparathyroidism who had persistent elevation of serum parathyroid hormone level after the immediate total parathyroidectomy and autotransplantation. To localize supernumerary (ectopic) parathyroid gland, we checked Tc-99m MIBI scintigraphy, MDCT and PET-CT. Contrast-enhanced MDCT showed a small strong enhancing lesion over left bracheocephalic vein, and PET-CT showed multiple brown tumors. We removed the supernumerary parathyroid gland and got a rapid drop of parathyroid hormone level.
갑상선종양환자에서의 혈청 Thyroglobulin 치에 관한 임상적 고찰

Serum thyroglobulin (Tg) was measured by radioimmunoassay in 81 patients with thyroid tumor who were treated in the department of Internal medicine of National Medical Center from January, 1981 to June, 1982. The results were as follows: 1) The mean serum thyroglobulin level in thyroid tumor was significantly higher than normal subjects (p〈0.001). 2) The mean serum thyroglobulin level in benign tumor was lower than malignant tumor (p〈0.05). 3) The thyroid carcinoma with metastasis had significant high level of serum thyroglobulin than without metastasis (p〈0.001). 4) The mean postoperative serum thyroglobulin level was significantly lower than preoperative level (p〈0.001). Data from our study show that serum thyroglobulin determination is useful for assessing the presence of malignant thyroid tumor, metastasis and the extent of residual or recurrent thyroid cancer after surgery.
