        성문하에서 발생한 선양낭성암종 1예

        Adenoid cystic carcinoma (ACC) is a rare malignant tumor. Its incidence is les than 1 percent of all head and neck malig-combination of surgery and postoperative radiotherapy has been usually recommended. Subglotic cancer has a lower incidence than does laryngeal cancer, whose incidence is in the range of 4 to 6 percent of all cancers of the larynx. Because of the rarity of this tumor in the larynx, no series has been large enough to permit statistical evaluation of treatment. We report a case of adenoid e is briefly reviewed.         Maskless lithography 응용을 위한 마이크로렌즈 어레이 개발

        마스크리스 리소그래피(maskless lithography)에 응용하기 위한 마이크로렌즈 어레이(microlens array, MLA)가 석영의 습식 식각과 UV 접착제(UV adhesive)의 코팅을 바탕으로 개발되었다. 제작된 MLA의 초점거리는 ${\sim}45\;{\mu}m$ 정도였으며, 집광되는 광선의 초점은 ${\sim}1\;{\mu}m$로 측정되었다. MLA를 통과하며 초점을 맺은 빔(beam)의 크기 및 세기가 charge coupled device (CCD) 카메라와 빔 프로파일러(beam profiler)를 이용하여 각각 측정되었으며, 일정한 세기의 점들이 초점면에서 고르게 관찰되었다. 초점거리는 코팅된 UV 접착제의 두께에 따라 변화하였으며, UV 접착제의 두께가 두꺼울수록 짧아지는 경향을 보였다. 일반적인 마스크 얼라이너(mask aligner)를 이용한 MLA의 UV 포커싱(UV focusing)이 감광막(photoresist, PR) 상에서 실시되었으며, MLA를 통과한 빛이 감광막 위에 일정하게 집광되었다. 마스크 얼라이너와 MLA 사이의 거리 변화에 따라 감광막에 구현된 패턴 사이즈가 조절 되었다. 고온에서 오랜 시간이 지난 후에도 소자의 특성은 전혀 변함이 없었다. A microlens array (MLA) was developed based on the wet-etched quartz substrate and coating of UV adhesive on the substrate for maskless lithography application. The developed MLA has the focal length of ${\sim}45\;{\mu}m$ and the spot size of ${\sim}1\;{\mu}m$. The spot size of the focused beam passing through the MLA was detected by CCD camera, and its intensity was monitored by beam profiler. Uniform spots with nearly identical intensities were observed on the focal plane when a beam passes through the fabricated MLA. The focal length was varied depending on thickness of the coated UV adhesive. The thicker the thickness of the UV adhesive was, the shorter the focal length of the MLA was. With a general mask aligner, UV beam focusing was tested onto photoresist (PR). The beams were well focused onto PR when UV passes through the MLA. Depending on the variable distances from the MLA, beam sizes onto PR were controlled. Even at high temperature for a long time, the performances of the MLA were not changed.

        Salivary duct carcinoma is a rare primary tumor of the salivary glands arising most frequently in the parotid gland. It has a male preponderance and occurs most often in patient over the age of 50 years. Its distinctive histological features include dilated ducts containing cells arranged in cribriform, papillary of solid patterns often with central necrosis and reminiscent of intraductal carcinoma of the breast. These features are associated with an obvious invasive component. It is an aggressive neoplasm and may metastasize widely, causing death in a high proportion of cases. We experienced one rare case of salivary duct carcinoma in submandibular gland which has been treated with submandibular gland wide excision and functional neck dissection and postopeartive concurrent chemo- therapy and radiation therapy. We report a salivary duct carcinoma in submandibular gland with a review of literature.

        세침흡인검사 후 발생한 갑상선 피막내 범발성 출혈 1예

        Fine-nedle aspiration biopsy (FNAB) is a safe, simple, repeatable and cost-efective method, and has excelent sensitivity and h node in the head and neck area. FNAB has a small number of contraindications and complications, but most of the complications are minor complications. However, uncommon complications had been reported in the literature. In this report, we describe a 63 year-old woman who experienced dyspnea caused by diffuse intracapsular hemorhage of thyroid gland after FNAB. (Korean J Otolaryngol 2007 ;50 :178-81)

        촉지되지 않고 우연히 발견되는 갑상선 결절에서초음파 유도하 세침흡인검사의 의의

        Background and Objectives:With the availability of more sensitive ultrasonography, a larger number of nonpalpable thyroid d nodule. The purpose of this study is to investigate the clinical significance of thyroid incidentaloma and the usefulnes of the USG-FNA in the management of thyroid incidentaloma. Subjects and Method:A retrospective study was performed on 208 pa-tients who underwent USG-FNA for thyroid incidentaloma smaller than 1.5 cm from 2001 to 2005. The cytologic findings were compared with the ultrasonographic findings and the surgical pathology. Results:Suspicious malignant sonographic findings cytologicaly diagnosed as benign lesion (76.4%), 7 cases as follicular neoplasm (3.3%), 33 cases as malignancy (15.8%), and 9 cases as inadequate specimen (4.4%). Of the 40 cases with folicular neoplasm and malignant cytology, 37 cases underwent sur-gery and 30 cases were confirmed to malignancy on the postoperative pathologic diagnosis. In the cases diagnosed to malig-nancy on pathology, extracapsular extension was present in 9 cases (30% ) (26.7%). The sensitivity, specificity, and accuracy of the USG-FNA for malignancy were 93.3%, 71.4%, and 89.1%, respectively. Conclusion:USG-FNA is a useful diagnostic tol in the management of thyroid incidentaloma especially when more than two suspicious malig-nant sonographic findings need to be detected. The clinical characteristics is not significantly diferent between palpable and nonpalpable nodules. (Korean J Otorhinolaryngol-Head Neck Surg 2007 ;50 :622-6)

        전정신경에 대한 혈관성 신경압박증후군에서 미세혈관감압술의 효과

        Disabling positional vertigo (DPV) refers to the cross-compression of the eighth cranial nerve from vessels in the posterior fossa which causes symptoms of vestibular disturbances. The most common symptom is a whirling sensation that is worse when the head is in a specific position and is abated with total bed rest. Many patients with DPV also have symptoms of auditory nerve and/or adjacent cranial nerve involvement, such as tinnitus, hearing disturbance, geniculate neuralgia, facial twitching. To analyze DPV, the authors reviewed patients with DPV who underwent microvascular decompression (MVD) in our institution. Subjects and Method:A retrospective study was performed on 11 patients with DPV who underwent MVD from January 2002 to September 2007. Information was collected on sex, age, symptoms, offending vessels, the success rate, surgical complications. Results:Three patients were male and eight were female. Their mean age was 55.1 years (range 37- 70). The most common offending vessel was AICA in 7 cases (64%) and second vessel was PICA in 3 cases (27%). Only one case (9%) had a vein. Patterns of improvement after MVD could be divided into 3 clinical types. There was complete recovery after operation in 8 cases, partial recovery in 2 cases, and recovery failure in 1 case. There were no major complications except hearing impairment in 1 case. Conclusion:This study shows that MVD provides a high rate of success with low morbidity, and can be regarded as one effective procedure for DPV.         Aperture를 이용한 MLA의 효율 개선

        This paper presents light transmission efficiency by optical adhesive thickness between MLA and aperture layer and by aperture hole size. The gap between MLA and Aperture layer is adjusted by the shim. The more optical adhesive thickness increases, the better light transmission efficiency increases up to a point. After that, the light transmission efficiency decreases because stray lights cannot transmit through the aperture layer owing to cut-off by aperture layer. And as a result of light transmission efficiency with changing aperture hole size, the light transmission efficiency is proportional to area of aperture hole. The more specified process is made, the better data and sample will be got.
