기관을 침범한 유두상 갑상선 암 환자에서 흉설골근을 통한 기관재건술 1예

Papillary thyroid carcinoma is known as its relatively high cure rate after surgical treatment. But invasion of the trachea by thyroid carcinoma is poor prognostic factor and the best management is en bloc surgical resection of the tumor invading the trachea. A 55-year-old man was diagnosed as papillary thyroid cancer with tracheal in-vasion. We treated the patient by total thyroidectomy with window resection of invading trachea followed by im- mediate reconstruction with sternohyoid muscle flap and tracheostomy. At 48 days after surgery, tracheostoma was closed and the patient had no functional complication by the surgical process. Until 10 months after surgery, there was no sign of recurrence and the patient led social life without any discomfort. We present this case with a review of the related literatures.

오늘날 국제정치학계의 E. H. 카 연구는 그의 가장 중요한 연구업적이라 할 러시아 혁명 및 소비에트사 관련 저술들이 다뤄지지 않은 채 진행되고 있다. 이 논문은 이러한 학계의 연구 현황에 문제를 제기하고, 카라는 한 인물이 저술한 다양한 주제의 저서들을 학문 분과에 구애되거나 편향되지 않고 읽음으로써 카에 대한 이해를 진척하려는 의도 아래 작성된 것이다. 즉, 그의 학문 연구 활동이 비롯된 가장 원초적인 문제의식에 주목하면서 동시에 그러한 문제의식이 다양한 주제의 저서들로 결실을 이루어 가는 모습에 주목한다. 카가 지녔던 문제의식은 좁게는 역사적 사건으로서 러시아 혁명과 이후 소비에트 러시아의 역사, 넓게는 20세기 서유럽에서 발생하고 있던 역사적 전환이라고 할 수 있다. 이 중에서도 특히 광의의 차원에서 이해한 카의 문제의식은 그의 저서 대부분을 관통하는 가장 중요한 지점이기도 하다. 이 논문은 광의의 차원에서 카가 보여준 문제의식에 주목하고 이에 대한 학문적 관심이 어떻게 카의 여러 저서에 직간접적으로 반영되고 있는지를 밝히고자 한다. 이러한 작업을 통해 카 본인이나 그의 저서가 지닌 가장 근원적인 문제의식에 접근하는 것은 물론, 그의 다양한 저서들이 공통된 주제의식 위에 어떻게 상호연관성을 가지는지 확인할 수 있다. 그 결과 카라는 인물에 대한 이해를 진전시키는 성과는 물론, 그를 통해 궁극적으로는 20세기 서유럽이 처해 있던 역사적 맥락과 시대적 배경에 접근하는 기회 또한 획득할 수 있다. S tudies on E. H. Carr within international relations department these days lack his most important research achievement in the Russian Revolution and the Soviet history. This study tries to bring his studies on revolution and international relations altogether without discriminating certain books of his so as to further our understanding on Carr. That means we focus on his most nascent and original theme of research that occupies most of his oeuvre and on how it was developed into his actual writings. Narrowly, it could be the Russian Revolution as a critical event in history and the subsequent course of the Soviet history while, in a much broader perspective, it must be a transition, or revolution, which was in progress in the context of the 20th century Western Europe. The latter is the actual leitmotif of Carr’s which his writings mostly share and repeat. To supplement and reinvigorate the hitherto incomplete studies on Carr, this study will pay attention to the latter, the revolution in the 20th century transitional Europe, to which his thoughts and writings, whether directly or indirectly, are connected. Through this work, it can be possible to catch his leitmotif and to see how his diverse books are related to it, as well as to each other. Finally, not only can we more thoroughly understand Carr, one of the most important scholars in International Relations, but also, from his writings, we can finally face the 20th century Europe where international relations studies were first initiated.

Mucoceles are expansile, encapsulated, benign cystic lesions with the potential for adjacent bony remodeling and resorption. Paranasal mucoceles predominantly develop in the frontal sinuses (60%), followed in frequency by the ethmoidal (20-30%), maxillary (10%), and sphenoid (2-3%) sinuses. Nasal septal mucoceles develop in only very seldom cases. Septal mucoceles might develop from infected nasal septum or trauma. In the differential diagnosis of a midline septal mass, dermoid cyst, encephalocele, meningocele or intraseptal abscess should be considered. Persistent Dysphonia after Laryngomicrosurgery for Benign Vocal Fold Disease

Objectives. Laryngomicrosurgery (LMS) is used to manage most vocal fold lesions. However, the functional voice outcome of the LMS might be diverse due to the influence of various factors. We intend to evaluate the incidence and etiologic factors of persistent dysphonia after LMS for benign vocal fold disease (BVFD). Methods. We performed a retrospective review of 755 patients who underwent LMS for BVFD. We analyzed the clinical characteristics, preoperative and postoperative two months voice studies. Postsurgical dysphonia was defined as grade 1 or above in GRBAS (grade, roughness, breathiness, asthenia, and strain) scale. Thirty nine patients (5.2%; 25 males and 14 females; average, 42.9 years; range, 21 to 70 years) were diagnosed with postsurgical dysphonia. Results. There was no correlation between the diagnosis, coexistence with laryngopharyngeal reflux disease, habit of smoking,or occupational voice abuse and voice outcome. The patients with a worse preoperative acoustic parameter had a worse voice outcome. Stroboscopic findings showed excessive scarring or bowing in 21 cases, presence of lesion remnant in eight cases, prolonged laryngeal edema in five and no abnormal findings in three. Conclusion. Great care should be taken in patients with worse preoperative jitter. With a few exceptions, postoperative dysphonia can be avoided by the use of an appropriate surgical technique.
