        Clinical Features and Causes of Endogenous Hyperinsulinemic Hypoglycemia in Korea

        Background: Endogenous hyperinsulinemic hypoglycemia (EHH) is characterized by an inappropriately high plasma insulin level, despite a low plasma glucose level. Most of the EHH cases are caused by insulinoma, whereas nesidioblastosis and insulin autoimmune syndrome (IAS) are relatively rare. Methods: To evaluate the relative frequencies of various causes of EHH in Korea, we retrospectively analyzed 84 patients who were diagnosed with EHH from 1998 to 2012 in a university hospital. Results: Among the 84 EHH patients, 74 patients (88%), five (6%), and five (6%) were diagnosed with insulinoma, nesidioblastosis or IAS, respectively. The most common clinical manifestation of EHH was neuroglycopenic symptoms. Symptom duration before diagnosis was 14.5 months (range, 1 to 120 months) for insulinoma, 1.0 months (range, 6 days to 7 months) for nesidioblastosis, and 2.0 months (range, 1 to 12 months) for IAS. One patient, who was diagnosed with nesidioblastosis in 2006, underwent distal pancreatectomy but was later determined to be positive for insulin autoantibodies. Except for one patient who was diagnosed in 2007, the remaining three patients with nesidioblastosis demonstrated severe hyperinsulinemia (157 to 2,719 μIU/ mL), which suggests that these patients might have had IAS, rather than nesidioblastosis. Conclusion: The results of this study suggest that the prevalence of IAS may be higher in Korea than previously thought. Therefore, measurement of insulin autoantibody levels is warranted for EHH patients, especially in patients with very high plasma insulin levels.

        Inhibition of Ceramide Accumulation in Podocytes by Myriocin Prevents Diabetic Nephropathy

        Background: Ceramides are associated with metabolic complications including diabetic nephropathy in patients with diabetes. Recent studies have reported that podocytes play a pivotal role in the progression of diabetic nephropathy. Also, mitochondrial dysfunction is known to be an early event in podocyte injury. Thus, we tested the hypothesis that ceramide accumulation in podocytes induces mitochondrial damage through reactive oxygen species (ROS) production in patients with diabetic nephropathy. Methods: We used Otsuka Long Evans Tokushima Fatty (OLETF) rats and high-fat diet (HFD)-fed mice. We fed the animals either a control- or a myriocin-containing diet to evaluate the effects of the ceramide. Also, we assessed the effects of ceramide on intracellular ROS generation and on podocyte autophagy in cultured podocytes. Results: OLETF rats and HFD-fed mice showed albuminuria, histologic features of diabetic nephropathy, and podocyte injury, whereas myriocin treatment effectively treated these abnormalities. Cultured podocytes exposed to agents predicted to be risk factors (high glucose, high free fatty acid, and angiotensin II in combination [GFA]) showed an increase in ceramide accumulation and ROS generation in podocyte mitochondria. Pretreatment with myriocin reversed GFA-induced mitochondrial ROS generation and prevented cell death. Myriocin-pretreated cells were protected from GFA-induced disruption of mitochondrial integrity. Conclusion: We showed that mitochondrial ceramide accumulation may result in podocyte damage through ROS production. Therefore, this signaling pathway could become a pharmacological target to abate the development of diabetic kidney disease.

        Mitochondrial Dysfunction in Adipocytes as a Primary Cause of Adipose Tissue Inflammation

        Adipose tissue inflammation is considered a major contributing factor in the development of obesity-associated insulin resistance and cardiovascular diseases. However, the cause of adipose tissue inflammation is presently unclear. The role of mitochondria in white adipocytes has long been neglected because of their low abundance. However, recent evidence suggests that mitochondria are essential for maintaining metabolic homeostasis in white adipocytes. In a series of recent studies, we found that mitochondrial function in white adipocytes is essential to the synthesis of adiponectin, which is the most abundant adipokine synthesized from adipocytes, with many favorable effects on metabolism, including improvement of insulin sensitivity and reduction of atherosclerotic processes and systemic inflammation. From these results, we propose a new hypothesis that mitochondrial dysfunction in adipocytes is a primary cause of adipose tissue inflammation and compared this hypothesis with a prevailing concept that “adipose tissue hypoxia” may underlie adipose tissue dysfunction in obesity. Recent studies have emphasized the role of the mitochondrial quality control mechanism in maintaining mitochondrial function. Future studies are warranted to test whether an inadequate mitochondrial quality control mechanism is responsible for mitochondrial dysfunction in adipocytes and adipose tissue inflammation.

        지백지황탕(知栢地黃湯)의 골관절염 Rat 모델에 대한 항산화, 항염증 효과

        Objectives This study intended to evaluate antioxidative, anti-inflammatory effects of Jibaekjihwang-tang on monosodium iodoacetate (MIA)-induced osteoarthritic rat model and investigate the potential mechanism. Methods Jibaekjihwang-tang (100 or 200 ㎎/㎏ body weight) was orally administered once daily for 2 weeks days from day 7 after intra-articular MIA injection. And blood analysis, the histologic examinations were performed. Moreover, protein expressions related to anti-oxidant and cartilage degradation and anti-inflammatory cytokines were measured by western blot analysis in cartilaginous tissue. Results Jibaekjihwang-tang reduced serum inflammatory cytokines such as tumor necosis factors-α and interleukin-6. Furthermore, the increase of anti-oxidant enzymes reversed the oxidative stress caused by MIA. Meanwhile, Jibaekjihwang-tang suppressed MIA-induced inflammation and cartilage degradation in cartilaginous tissue. Conclusions Jibaekjihwang-tang alleviated MIA-induced inflammation. Jibaekjihwang-tang was associated with a protective effect on cartilage and by reducing inflammation and cartilage degradation. These findings provide new approaches for understanding osteoarthritis therapy. (J Korean Med Rehabil 2020;30(2):1-18)

        우리는 한때 모두 개인 컴퓨터를 이용했습니다. 지금도 크게 다르진 않지만, 대부분의 시간을 함께하는 건 모바일 폰입니다. 실제로 PC 출하량은 마이너스 성장률을 보이고 있으며 반대로 스마트폰은 계속해서 성장 가도를 달리고 있습니다. 이 추세는 사용자들의 행동 양식에도 큰 변화를 가져왔습니다. PC 기반의 SNS 싸이월드가 몰락하고, 모바일 기반의 SNS들이 기염을 토하고 있습니다. 우리는 이미 이 변화를 예측하지 못하고 또 제때 따라가지 못해 크게 뒤쳐졌던 경험이 있습니다. 현재 우리가 사용하는 SNS 대부분이 우리나라 것이 아니라는 것이 그 증거입니다. 이제 우리는 또 다른 변화를 맞이하고 있습니다. 3G 시대는 이미 오래전에 저물었고, LTE 시대를 넘어 5G 시대를 목전에 두고 있습니다. 압도적인 통신 속도에 힘입어 지금까지 SNS 시장을 선도해 왔던 글과 이미지 기반의 플랫폼은 저물어 가고 있으며, 동시에 영상 기반의 플랫폼이 급격히 성장하고 있었습니다. 이런 흐름에 불을 지핀 것은 다름 아닌 코로나-19의 대유행입니다. 사회적 거리두기의 영향으로 대면교육과 일상적인 친목 활동이 어려워지면서 동영상을 통한 비대면 교육과 소통이 폭발적으로 증가했습니다. 지금 그 영상 기반의 플랫폼을 대표하는 것은 다름 아닌 유튜브입니다. 여전히 이북보다는 종이책이 편한 80년생까지의 세대에게는 가혹한 일입니다. e-Book도 아닌, 영상이라니. 하지만 싫다고 거부만 할 수는 없는 노릇입니다. 당랑거철처럼 이 거대한 흐름은 이미 우리 생활 전반에 영향을 미쳤고, 특히 MZ 세대 (1980년대 초 ~ 2000년대 초 출생한 세대) 에게는 소통의 방식을 변화시켰습니다. 바야흐로 뉴미디어 시대가 열렸습니다. 이 뉴미디어 시대에서 의사 아니, 의사를 넘어 모든 전문가들이 어떻게 살아남을 수 있는가에 대한 고민이 이 강연의 전반적인 이야기입니다. 다시 말해 현재 시대 흐름과 이 흐름에 따라 우리가 어떻게 행동해야 하는 가에 대한 구체적인 경험을 나누고자 합니다.

        자궁적출술 후유증으로 입원치료를 받은 환자 5례에 대한 보고

        Objectives: This paper was aim to report the clinical study for five cases received Korean medicine hospital treatment because of hysterectomy sequelae within 5 days after surgery. Methods: The patients having treated with hospital management, from March 1st in 2014 until March 24th in 2015, was enrolled in this study. We collected their age, chief complaint, vice complaint, diagnosis, visit days after surgery, treatment period, and etc. We treated the patients with acupuncture, moxibustion, and herbal medicine. Results: All of the cases treated with Korean medicine in this study revealed the recovery. The vice complaint also had improved. Conclusions: Korean medicine is effective on patients having variable postoperative symptoms following hysterectomy. And it is necessary to conduct an additional treatment about menopause symptoms.

        청상방풍탕 열수 추출물의 피부재생, 주름개선, 미백, 보습 효과 및 세포독성 평가

        목 적: 본 연구는 한의학에서 면포질환에 빈번히 사용하는 대표적인 복합처방 인 청상방풍탕의 열수 추출물 동결건조물의 피부 노화 개선 효과 평가를 위하여 피부재생, 주름개선, 미백 및 보습 효과를 각각 평가하였고, 기본적인 독성 평가의 일환으로 세포독성 또한 in vitro 방법으로 평가하였다. 방 법: 본 연구에서는 human normal fibroblast(CRL-2076) 세포 및 B16/F10 murine melanoma(CRL-6475) 세포에 대한 청상방풍탕 열수 추출 동결건조물(수율 18.71%)의 세포독성을 MTT(3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-tetrazolium Bromide) 방법을 통해 평가하였고, B16/F10 melanoma cells의 melanin 생성 억제 정도 및 melanin 합성 필수 효소인 tyrosinase 활성 억제와 arbutin을 비교 평가하여 미백효과를 평가 하였다. 또한 피부 재생 및 주름 개선 효과를 transforming growth factor(TGF)-β1와 fibroblast의 collagen type I 합성능을 비교, phosphoramidon disodium salt(PP)와 세포 외 기질의 분해에 관여하는 elastase활성 억제를 비교, oleanolic acid(OA)와 hyaluronidase, collagenase 및 matrix metalloproteinase(MMP)-1 활성 억제를 비교하여 각각 평가 하였고, 마우스 피부 수분함량 변화를 관찰하여 보습효과를 평가하였다. 결 과: 본 실험의 결과, 청상방풍탕은 human normal fibroblast 세포 및 B16/F10 murine melanoma 세포에서 최고 농도인 500 mg/ml까지 유의한 세포독성이 나타 나지 않았고, fibroblast의 collagen type I 합성을 증가시켰다. 또한 세포외기질 파 괴와 연관된 것으로 알려진 hyaluronidase, elastase, collagenase 및 MMP-1 활성을 각각 억제하였고 melanin의 생성에 관여하는 tyrosinase의 활성 및 B16/F10 melanoma 세포의 melanin 생성을 농도 의존적으로 차단함이 관찰되었다. 이와 더불어 정상 매체 대조군에 비해 청상방풍탕 경구 투여군이 투여 용량 의존적으로 마우스 피 부 수분 함량을 유의성 있게 증가시켰다. 결 론: 이상의 결과에서, 청상방풍탕은 세포 독성 없이 피부재생, 주름개선, 미 백 및 보습 효과를 나타남이 관찰되어 차후 피부 개선 소재로서 그 가치가 높을 것으로 판단되나 금후 개별 구성 약재 각각에 대한 효능 및 생리활성을 나타내는 화학성분의 검색과 더불어 다양한 방면으로 기전적인 연구와 피부 보호 효과에 대한 in vivo평가를 체계적으로 수행해야 할 것으로 판단된다. Objectives: The present study is to observe the skin-regeneration, anti-wrinkle, whitening and skin moisturizing effects of Cheongsangbangpung-tang (CSBPT) with cytotoxicity. Methods: In the present study, cytotoxicity of CSBPT lyophilized aqueous extracts (yield=18.71%) was experimented against human normal fibroblast cells and B16F10 murine melanoma cells by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium Bromide (MTT) assay, and skin regeneration and anti-wrinkle effects were also showed through the assay of collagen type I synthesis by an enzyme immunoassay (EIA) kit as comparing with transforming growth factor (TGF)-β1, hyaluronidase, collagenase and matrix metalloproteinase (MMP)-1 inhibitory assays as comparing with oleanolic acid (OA), and elastase inhibitory effects as comparing with phosphoramidon disodium salt (PP). In addition, whitening effects of CSBPT were observed by tyrosinase inhibitory assay and melanin formation test in B16/F10 melanoma cells as comparing with arbutin, and skin moisturizing effects were measured through mouse skin water contents test, respectively. Results: No CSBPT treatment related cytotoxic effects were demonstrated against human normal fibroblast cells and B16/F10 murine melanoma cells. CSBPT concentration-dependent increased collagen type I synthesis at human normal fibroblast cells. It also effectively suspreessed hyaluronidase, collagenase, elastase and MMP-1 activities, which were enzymes that related to declining of ECM and formation of wrinkle. CSBPT supressed B16/F10 melanoma cells`s melanin productions with tyrosinase activity, which was an enzyme connected with melanin formation, and dose-dependent and significant increases of skin water contents were detected in CSBPT treated mouse skin as compared with vehicle control skins. Conclusions: CSBPT showed favorable and enough skin regeneration, anti-wrinkle, whitening and skin moisturizing effects at least in a condition of this experiment. However, more detail mechanism and in vivo skin protective efficacy studies should be conducted in future with the screening of the biological active compounds in individual herbs of Cheongsangbangpung-tang.

        기본원칙의 중요도를 반영한 유니버설디자인 공간환경 평가체계

        The purpose of this study was to develop the evaluation system for the universal design space environment using weighted values asuniversal design principles for everyone. The basic principles for the universal design was drawn to equity, clarity, safety, convenience, andplacement through focused group interview and arrangement of key words of precedent scholars’ theories and the enacted ordinances. Then, aone-sample t-test was conducted for the various user and experts in the relevant fields to verify the suitability of the basic principles andcomponents used as evaluation indicators. An analysis result of the AHP(Analytic Hierarchy Process) to experts, The basic principle is thatequity and safety are important, and the evaluation indicators is shown as accessibility, safe, low physical effort, intuitive cognition,simplicity, crime prevention, continuity of space, compatibility of function, and exchange and participation. Based on the above results, itsuggests the need to rethink the meaning of universal design in urban architectural spaces. In order to create and spread a space where allcitizens can live safely and conveniently, it presents the need for awareness education for experts as well as the participation of various usersin implementing universal design policies. 본 연구의 목적은 유니버설디자인 공간환경 평가체계를 개발하는 것으로, 유니버설디자인 조례와 기존이론의 분석, FGI과정을 통해 기본원칙은 형평성, 명료성, 안전성, 편의성, 장소성으로 도출했다. AHP분석결과, 기본원칙은 형평성과 안전성이 중요하고, 평가지표는 접근가능성, 안전함, 적은 노력, 직관적 인지, 간결함, 범죄의 예방, 연속성, 기능의 호환, 그리고 교류와 참여로 나타났다. 위 결과를 바탕으로, 유니버설디자인 정책시행에 있어 다양한 사용자의 참여뿐 아니라 전문가를 대상으로 한 인식교육의 필요성을 제시한다.
