        슬개대퇴통증증후군 환자의 체중부하운동 유형별 하지 근활성량의 차이

        [PURPOSE] The purpose of this study was to investigate analysis of difference of electromyographic activity on rectus femoris(RF), vastus medialis oblique(VMO), vastus lateralis(VL) and gluteus medius(GM) on type of weight-bearing in patients with patellofemoral pain syndorme. [METHODS] The subjects of this study were 13 female patients with patellofemoral pain syndorme 16 case(Right 8 case, Left 8 case) who are 20 - 30 years. The subjects of the study were rectus femoris, vastus medialis oblique, vastus lateralis and gluteus medius attached EMG electrodes to measure the electromyographic activity during weight-bearing exercise. Weight-bearing exercise was conducted half squat(HS), wall half squat(WHS), pillow wall half squat(PWHS) and single lim wall squat(SLWS). The investigator measured and analyzed the factors related to iEMG of muscle and electromyographic activity ratio during four type weight-bearing exercise the experiment collected data through measures of one-way ANOVA with the statistical significance level set at α=.05. [RESULTS] Weight-bearing exercise effective way to increase muscle activation of the VMO during weight-bearing eccentric contraction was shown to SLWS, weight-bearing exercise effective way to increase muscle activation of the GM were to PWHS. Second, weight-bearing exercise effective way to increase muscle activation of the GM during weight-bearing isometric contraction was shown to WHS and PWHS. Third, the weight-bearing concentric contraction of muscles during four kinds of effective weight-bearing exercise to increase muscle activation of specific muscles were not. [CONCLUSIONS] This study the results of the four type weight-bearing exercise according to the class of muscle contraction in the muscle activity with PFPS patients through each muscle was confirmed that the most efficient forms of exercise.

        남성의 연령에 따른 최대산소섭취량 예측을 위한 사이클 에르고미터 검사와 20m 셔틀런 검사의 준거타당도

        [INTRODUCTION] Despite the various cardiorespiratory fitness assessments, VO2max decreases progressively with age. Therefore, there were reports in that the error of measurement was increased. This investigation compared the results of direct and indirect measurements of VO2max of men according age. [METHOD] Healthy 125 men were recruited for this study. Direct estimation of cardiorespiratory endurance (VO2max) comprised graded treadmill exercise test (GXT) followed by expired gas analysis. VO2max was indirectly predicted by Astrand submaximal cycle ergometry test (AST) and 20m multistage shuttle run test (20m-MST). To evaluate differences according age, One-way ANOVA was used. Two-way repeated ANOVA was applied to detect significant differences among the three measurements. Associations among the parameters investigated were analyzed with Pearson correlation and linear regression was used for analyses of relationships among variables. The accuracy of measurements was assessed by Bland-Altman and interclass correlation coefficient analysis. [RESULT] VO2max, which there was a differences in age, in 20s was higher than in other age groups, and 30s or 40s were higher than in 60s. The measurement of GXT showed a significant difference in ages, except in 60s, from the estimates of AST and 20m-MST. Both estimated VO2max of AST and 20m-MST were found underestimated. The measurements of GXT had a low correlation with the estimates of AST and 20m-MST. The error between estimated of 20-MST and measured of GXT values was affected by age (p<.01), weight (p<.01), body mass index (BMI) (p<.001), and percentage of body fat (p<.001) respectively. Moreover, the reliability of 20m-MST was higher than AST. [CONCLUSION] Therefore, 20m-MST is more valid method to evaluate VO2max than AST for men. [서론] 최대산소섭취량을 측정하는 방법은 다양하지만 연령에 따라 최대산소섭취량은 감소하므로 연령변화에 따른 측정의 오차가 발생하게 된다. 이 연구는 연령에 따른 최대산소섭취량의 변화를 살펴보고, 최대산소섭취량 예측을 위한 최대하부하검사인 사이클 에르고미터 검사와 필드검사인 20m 멀티 셔틀런 검사의 추정방법 타당성과 최대산소섭취량과의 상관관계를 알아보고 오차에 영향을 주는 요인을 분석하고자 하였다. [방법] 이 연구는 지역광고를 통하여 자발적으로 참여에 동의한 지원자 남자 125명을 대상으로 측정하였다. 최대산소섭취량의 직접측정은 점진적인 트레드밀 운동부하검사(GXT)를 통해 측정하였으며, 최대하 사이클에르고미터 검사(AST)와 20m 멀티 셔틀런 검사(20m-MST)를 통해 최대산소섭취량을 산출하여 추정하였다. 연령에 따른 차이는 일원변량분석을 이용하여 분석하였으며, 측정 간 차이는 반복측정 이원변량분석으로 분석하였다. 측정값의 상관관계는 pearson correlation을 이용하였고, 회귀분석을 실시하였다. 또한 검사의 일치도는 Bland-Altman분석과 급내상관계수로 분석하였다. [결과] 최대산소섭취량은 연령에 따른 차이를 나타냈으며, 20대가 다른 연령보다 가장 높고, 30대와 40대가 60대보다 높은 것으로 나타났다. 직접 측정한 최대산소섭취량과 간접 측정을 통하여 추정된 최대산소섭취량은 60대를 제외한 모든 연령에서 차이를 나타내었다. GXT 측정값에 비하여 AST 추정값과 20m-MST 추정값은 과소 추정하는 것으로 나타났으며, GXT 측정값과 20m-MST 추정값의 차이는 연령(p<.01), 체중(p<.01), 체질량지수(p<.001) 및 체지방율(p<.001)에 영향을 받는 것으로 나타났다. 또한, 20m-MST 추정값의 차이값은 AST 추정값의 차이값보다 높은 내적 신뢰도를 나타내었다. [결론] 따라서, 남성의 연령에 따른 최대산소섭취량 예측을 위한 검사방법으로 20m-MST 검사방법이 AST 검사방법보다 더 신뢰도가 높은 것으로 나타났다.

        Sports Injury Prevention and Functional Training: A Literature Review

        [OBJECTIVES] The aim of this review study was to proposes a new paradigm of functional injury prevention(FIP) based on analysis of the effects of sports injury prevention(SIP) and functional training(FT). [METHODS] The literature used in the study is based on the electronic database and international papers published from 2014 to 2021 related to functional training were searched, screened and evaluated by using the follow keywords: functional training, sports injury, rehabilitation, and prevention. [RESULTS] FT was divided into the concept of training both improving the sports performance and restoring physical function in the field of rehabilitation. In two areas, FT and SIP are somewhat different in approach method, object and environment, but they are highly correlated that both the activation of proprioceptive sensation and integrated neuromuscular function are reflected in training program. Especially, intrinsic injury factors such as reduced physical ability, skills and conditioning are associated with reduction of sports performance and increased sports injury. In addition, decreased proprioceptive sensation and neuromuscular function is a typical symptom of reduction of sports performance and injury. Therefore, FIP is needed as a training program that not only to prevent sports injury but also to improve sports performance. [CONCLUSIONS] In this review study, FIP is propose as a new paradigm combining SIP and FT. The follow-up study of FIP should be reviewed on athletes, club members and the general public for the development of programs to prevent sports injury and improve sports performance and the effects by sports events, body parts and age.

        척추분절안정화운동을 통한 국소근육 활성화가 골프 수행능력에 미치는 영향

        [INTRODUCTION] This study aimed to investigate changes in thickness of multifidus and transversus abdominis, and analyze swing mechanism and golf performance after applying spinal segmental stabilization exercise for prevention and treatment of backache targeting professional golfers. [METHOD] Subjects of this study include 6 female professional golfers belonging to KLPGA as regular members and this study conducted measurement of dependent variables before and 8 weeks after application of SSE respectively. [RESULT] As results of the foregoing, rate of change in thickness of multifidus and transversus abdominis were increased all on average. With respect to the characteristics of golf shot, there were no considerable changes in total distance and club head speed, whereas grouping radius, ball speed and smash factor were decreased on average. In the measurement this study carried out for the purpose of investigating features of the motion of golf performance, phased change of time was not shown and ground reaction force also remained without a great change. [CONCLUSION] In conclusion according to the application of SSE, First, the degree of activation of multifidus and transversus abdominis differed by individual golfers with normal muscular functions. Second, with respect to the changes in characteristics of golf shot, a positive result came out in the aspect of accuracy as grouping radius was decreased. Third, there was no significant difference in the time required for each phase of golf shot. Fourth, of the golf shot, weight movement showed no strong correlation as no great changes were shown in ground reaction force. From all these considerations, various physical factors as well as stability of spine affect golf performance ability. Further studies need to determine the relationship between local muscle activity and improved swing accuracy and statistical verification procedure targeting more golfers is considered necessary. [서론] 이 연구의 목적은 프로 골퍼를 대상으로 요통의 예방과 치료를 위한 척추분절안정화운동(SSE; Spinal Segmental Stabilization Exercise)을 적용한 후 뭇갈래근과 배가로근의 두께변화, 스윙 기전, 골프 수행능력을 분석하고자 하였다. [방법] 이 연구의 대상은 한국여자프로골프협회 정회원 프로 골퍼 6명으로 종속변인의 측정은 SSE 적용 전, SSE 적용 8주후에 각각 실시하였다. [결과] 뭇갈래근과 배가로근 모두 전체 평균적으로 두께 변화율이 증가하였다. 골프샷의 특성 중 총거리와 클럽헤드속도는 골퍼 전체 평균적으로 큰 변화는 없었으며, 탄착군 반지름, 볼 속도, 스매시 팩터는 평균적으로 감소하였다. 골프 수행동작 특성을 알아보기 위해 살펴본 구간별 시간의 변화는 나타나지 않았고, 지면반력도 큰 변화가 없었다. [결론] SSE 적용에 따른 이 연구의 결론은 첫째, 근육의 기능이 정상적인 골퍼의 경우 뭇갈래근과 배가로근의 활성도는 골퍼 개인에 따라 차이가 있었다. 둘째, 골프샷의 특성 변화 중 탄착군의 반지름이 줄어들어 정확성 측면에서 긍정적인 결과를 나타냈다. 셋째, 골프샷의 구간별 소요시간은 큰 차이가 없어 스윙 템포에는 영향을 주지 않았다. 넷째, 골프샷 중 지면반력의 변화가 크게 나타나지 않아 체중이동 양상에는 관련성이 없었다. 이상의 결과를 종합해 볼 때 골프수행능력에 영향을 미치는 요인으로는 척추의 안정성 뿐만 아니라 다양한 많은 체력적 요인들이 작용함을 알 수 있었다. 추후 연구에서는 척추분절안정화운동을 통한 국소근육의 활성도와 스윙의 정확도를 향상시키게 된 요인과의 인과관계를 밝히는 것이 필요하며, 이를 위해 좀 더 많은 골퍼를 대상으로 통계적 검증 절차가 필요 하다고 판단된다.
