A case report of a surgical guide fabricated via intraoral scanning-based implant planning and wax-based rapid prototyping

With the recent progress of digital technology, the computer guided surgery utilizing a guide template in the placement of implant has been actively performed, and the method employing the intraoral scanner at the implant prosthesis introduced. Fabrication method of the guide template can be largely classified into design-related rapid prototyping (RP) system and vector milling system, and each of the method has its own weakness in the clinical application despite of excellent accuracy. Thus, in this case study, a working model was fabricated by the wax RP technology using images acquired by CBCT and an intraoral scanner, and the metal bushing was picked up with orthodontic resin cast upon the wax model. Using this method, a surgical guide template was fabricated and used in surgery. From this, we could obtain a satisfactory outcome clinically in the implant placement and the fabrication of the final prostheses and thus report this case herein. 최근 디지털 기술이 발달하면서 임플란트 식립 시 가이드 템플릿을 이용하는 컴퓨터 가이드 수술이 활발하게 이루어지고 있으며, 임플란트 보철시 구강스캐너를 이용하는 방법이 소개되고 있다. 가이드 템플릿의 제작방법은 크게 RP 제작 시스템과 벡터 밀링 시스템으로 나눌 수 있는데, 각각은 우수한 정확성에도 불구하고 임상적 적용에 있어 약간의 아쉬운 점을 보이고 있다. 이에 본 증례에서는 CBCT 및 구강스캐너로 획득한 디지털 영상을 이용하여 쾌속조형법으로 왁스재료의 작업모형을 제작하고 그 위에 교정용 레진으로 금속애관을 픽업해 내는 방법을 사용하여 임플란트 수술용 가이드 템플릿을 제작하여 수술에 사용하였으며, 임상적으로 임플란트 식립 및 최종보철물 제작 시 만족스러운 결과를 얻을 수 있어서 이를 보고하고자 한다.

Purpose: To investigate the biologic effect of mitomycin C, dexamethasone and cyclosporine A 0.05% on cultured human keratocytes in vitro. Methods: Human corneal keratocytes were exposed to a concentration of mitomycin C (0.05%), dexamethasone (0.05%) and cyclosporine A (0.05%) for a period of 3, 5, and 10 minutes. MTT-based colorimetric assay was performed to assess the metabolic activity of cellular proliferation and the concentration of type I procollagen COOH-terminal peptide (PIP) and laminin were measured. Cell damage was determined by using the lactate dehydrogenase (LDH) assay. Apoptotic response was evaluated utilizing flow cytometric analysis with Annexin V and propiodium iodide. Results: The inhibitory effect of cellular proliferation and cytotoxicity in cultured human keratocytes showed a time-dependent response in all drugs. The production of PIP and laminin showed a time-dependent response in cultured cells. Apoptosis was observed in flow cytometry after being treated with mitomycin C, dexamethasone and cyclosporine A. Cyclosporin A resulted in less apoptosis of keratocytes than mitomycin C and dexamethasone. Conclusions: The apoptotic response of mitomycin C, dexamethasone and cyclosporine A is associated with the inhibitory effect of human corneal keratocyte proliferation. To decrease corneal opacity, mitomycin C and dexamethasone were more effective than cyclosporine A in the present study. Additionally, a high concentration of cyclosporine A greater than 0.05% is necessary to lower corneal opacity. J Korean Ophthalmol Soc 2011;52(10):1215-1221

Purpose: Orbital myxoma is an extremely rare tumor. To date, only a few cases have been reported in the literature. The present study reports a case of an orbital myxoma and demonstrates the clinical and histological features. Case summary: A 37-year-old woman presented with a painless movable mass in the orbital margin of the left eye. Computed tomography of the orbit revealed a sharply outlined solid mass, and an excisional biopsy was performed. The excised specimen consisted of a 17x9x10 mm3 brownish mass of scattered spindle cells and small vascular spaces in a fibromyxoid background. Immunohistochemically, the tumor cells were not immunoreactive for SMA, S-100 protein or CD34. Histopathologic examination revealed the tumor to be a myxoma. There was no mass lesion in computed tomography of the orbit, and no recurrence or complications were found one month after removal. Conclusions: When a patient presents with a painless and movable mass in the orbital area, orbital myxoma should be considered as a possible diagnosis. The orbital myxoma with bone invasion should be completely removed to prevent the lesion from recurring. J Korean Ophthalmol Soc 2010;51(8):1142-1145

Pseudoxanthoma elasticum is a rare, heritable, systemic disease of connective tissue characterized by degeneration of elastic fiber and mainly affects the skin, eye and blood vessels. We report a case of pseudoxanthoma elasticum mimicking cutis laxa. She had a generalized laxity of the skin for about 30 years. There were similar skin lesions in her sister. No other systemic involvement was found. Histopathologic examination revealed clumping, fragmentation and calcification of elastic fibers in the dermis. (Korean J Dermatol 2000;38(2):227~230)

To answer what the European identity exactly means is not simple. Apart from the fact that the term ``Europeaness`` is as such a controversial concept, we can start from the assumption that it is a constructed term which has gradually evolved in the history of Europe. This article is engaged in a study of this awkward question of European identity, by which the process of the enlargement of the EU presents an analytical frame. In this article were firstly the whole process of the enlargement of the EU up to now, the legal basis of the enlargement, and the meaning of the enlargement in the politics of the EU etc. examined. Then, I have attempted to explain the relationship between the EU and Turkey in which the joining of Turkey to the EU was a central theme. It seems that the ``Europeans`` have until now considered the ``Muslims`` not to have association to Europe. Whether the EU of the Christian states will accept or reject the Islamic Turkey as a member of her community is the question that shall be answered in the near future. The decision on it will show whether an important part of the future identity of Europe shall be newly formed or not.

본 논문은 유럽통합과 냉전의 상관관계를 살펴보려는 목적을 가지고 있다. 구체적으로는 유럽방위공동체 창설 노력을 통하여 한국전쟁이 유럽통합에 미친 영향을 분석한다. 한국전쟁은 냉전의 대립이 첨예화되었던 유럽에서도 유사한 전쟁이 발발할 수 있다는 공포를 심어줌으로써 독일의 재무장이 서방세계 안보의 중요한 이슈로 등장했다. 유럽방위공동체 창설 구상은 독일 재무장을 유럽적 차원에서 해결하기 위한 방안으로서 프랑스에 의해서 제안되었다. 그러나 프랑스가 유럽방위공동체 조약의 비준을 거부함으로써 유럽방위공동체는 실패한 드라마로 낙인 찍히게 되었다. 유럽방위공동체 창설의 드라마는 냉전의 글로벌한 성격을 보여주는 의미 있는 사건으로 이해될 수 있을 것이다. This paper aims to examine the correlation between European integration and the Cold War. Specifically, it analyzes the impact of the Korean War on European integration through efforts to create a European Defense Community. German rearmament has emerged as an important issue of Western security as the Korean War instilled fears that a similar war could break out in Europe, where the Cold War confrontation was acute. The creation of the European Defence Community was proposed by France as a way to resolve the German rearmament on a European level. But France’s refusal to ratify the Treaty of the European Defence Community has branded it a failed drama. The drama of the creation of the European Defence Community could be understood as a meaningful event that shows the global nature of the Cold War.

경제통화동맹 창설과 독일통일의 동시성을 중심으로 고찰한 유럽통합과 독일문제

이 글에서는 독일문제의 성격을 유럽사의 맥락에서 고찰하였다. 특히 역사적으로 독일문제에 대한 하나의 해결책으로 독일통일을 거론하는 관점을 소개하면서, 이 시기 논의되었던 유럽 경제통화동맹 창설에 대해 설명하였다. 독일문제에 대한 해결책으로서 독일통일이 독일을 제외한 유럽국가들에게 우려를 낳게 한다는 점과 이러한 이웃국가들의 염려를 불식시키면서 독일통일을 달성하는 과제를 성공적으로 완수하였다는 점을 설명하였다. 또한 유럽경제통화동맹 창설과 관련하여 독일 내에서는 유로화 도입을 위한 마르크화의 포기가 독일통일을 위한 길을 열기 위한 대가로 지급되었다는 견해와 독일 통일을 위해 마르크화를 포기한 거래는 없었고, 통화동맹 문제는 독일통일 결정 과정에서 작은 역할을 했다는 시각이 대치하고 있었다는 내용을 소개하였다. 결론적으로 경제통화동맹 창설과 독일통일의 문제는 동시적으로 프랑스와 서독 양국 이해관계에 따라 전개되었고, 서독은 독일통일을, 프랑스는 유럽통화를 얻게 되는 역사적 거래가 성사되었던 것이다. 따라서 마르크화가 독일통일을 위해 유럽통합의 제단에 희생되었다는 견해도 과장된 미사여구로만 흘려들을 수는 없을 것이다. This article examines the character of German unification within the context of European history. It introduces German unification as one solution to the German issue in history, and considers how it impacted discussions of European Economic and Monetary Union (EMU). The prospect of German unification created much unease in Europe, and EMU was designed to allay such feelings, which it succeeded in doing. Further, the view that Germany agreed to give up the Deutsche Mark and introduce the Euro in order to open the way for German unification is contrasted with the view that there was no deal of this kind, and that EMU played a minor role in decision making surrounding German unification. Overall, this article argues that EMU and German unification proceeded in accordance with the interests of France and West Germany. A historic deal that allowed West Germany to secure German unification and France a European currency was achieved. Hence, the argument that the Deutsche Mark was sacrificed atop the alter of European integration to achieve German unification cannot merely be written off as mere exaggeration.
