한국 중년 및 노년 여성에서 복압성 요실금과 대사증후군 (Metabolic syndrome)과의 상관관계에 대한 연구

목적: 본 연구에서는 복압성 요실금과 대사증후군의 지표인자와의 상관관계를 분석함으로써 복압성 요실금과 대사증후군과의 관련성을 알아보고자 하였다. 연구방법: 2004년부터 2006년까지 복압성 요실금을 진단받고 수술 및 약물치료를 시행한 208명의 환자와 같은 기간 부인과 양성질환을 본원 산부인과에 내원한 대조군 154명을 대상으로 하여 대사증후군과 복압성 요실금의 상관관계를 분석하였다. 대사증후군의 지표인자는 NCEP ; ATP III 진단지침에 따른 중성지방, 고밀도 콜레스테롤, 고혈압 그리고 인슐린 저항성의 기준으로 하였으며 단 복부비만에 대한 지표를 허리둘레가 아닌 BMI로 대체하였다. 따라서 지표인자 중 3가지 이상 해당되는 환자를 대사증후군의 위험군으로 분류하였다. 결과: 대사증후군의 위험군은 복압성 요실금과 유의한 상관관계를 보였으며, 대사증후군의 위험군에서 복압성 요실금의 위험도는 2.503 (95% CI 1.28-4.87, p=0.004)이었다. 다변량 분석을 통해 인슐린 저항성 (OR 2.563; 95% CI 0.04-0.31, p<0.001) 및 BMI (OR 3.574; 95% CI 0.09-0.31, p=0.011)는 복압성 요실금에 대한 독립적인 위험인자임을 알 수 있었다. 결론: 인슐린 저항성을 포함하는 대사증후군은 요실금과 상관관계가 있을 것으로 생각된다. Objective: The purpose of this study was to identify the relationship between metabolic syndrome and stress urinary incontinence. Methods: Based form 362 case, the risk factors including the parameters of metabolic syndrome for stress urinary incontinence (SUI) were analyzed retrospectively: body mass index (BMI), hypertension, insulin resistance, triglyceride, HDL-cholesterol. Results: The risk for metabolic syndrome were associated positively with SUI. Women with the risks for metabolic syndrome had 2.503 times of the odds for SUI compared without the risks for metabolic syndrome (95% CI 1.28-4.87, p<0.001). The independent risk factors for SUI were BMI (OR 3.574 ; 95% CI 0.09-0.31, p<0.001) and insulin resistance (OR 2.563 ; 95% CI 0.04-0.31, p=0.011) by multivariate analysis. Conclusion: Our results suggested that metabolic syndrome was the important factor of the stress urnary incontinence. And we confirmed the need for the treatment of metabolic disturbance and the prevention of metabolic syndrome aimed at correcting hormonal-metabolic disturbance in middle-aged and older women with SUI.

Small cell carcinoma of ovary is extremely rare. This neoplasms compose predominantly or exclusively of small round cells with scant cytoplasm. There are two types of primary small cell carcinoma of ovary described in the literature : the hypercalcemic type and the pulmonary type. The first 11 patients with a primary ovarian small cell carcinoma of the pulmonary type were described by Eichorn et al. in 1992. The prognosis of this tumor is very poor and only limited data on the treatment of this tumor type are available. In the present case report, we describe a patient with the pulmonary type of small cell carcinoma associated with huge mucinous cystadenocarcinoma with a brief review of the concerned literatures.
연구보문 : 형질전환체에서 도입유전자 구조와 메틸화가 유전자 발현에 미치는 영향

35S::hpt-35S::gus-mUbi1::bar 유전자로 형질전환된 벼에서 세대진전을 통한 유전자 구조 및 발현양상을 분석하였다. 1. 하이그로마이신 저항성을 나타낸 형질전환 벼(T0)에서 제초제 감수성을 나타낸 개체는 bar 유전자가 존재하지 않았거나 mUbi1 프로모터의 불완전한 삽입으로 인한 것을 알 수 있었다. GUS 발현은 도입 유전자의 copy 수에 따라 상당한 변이가 있었으며 3 copy 이상 도입된 형질전환체에서는 발현이 약하거나 silencing 현상이 일어났다. 2. 도입 유전자의 구조 및 상태를 알아보기 위하여 Hpa II와 Msp I 으로 genomic DNA를 절단 한 후 Southern 분석을 한 결과 여러 copy가 도입된 형질전환체에서 gus 및 35S 프로모터의 재배열 현상이 뚜렷하였다. 또한 일부 계통에서는 methylation 된 유전자를 확인하였고 demethylation 물질인 5-azaC 처리 후에 GUS 발현이 회복되었다. 3. T0에서 여러 copy의 유전자가 도입되어 발현이 없었던 형질전환체의 경우 후대 개체들에서 1 copy를 포함하고 유전자 발현이 정상적인 개체를 선발할 수 있었다. Transgene expression was analyzed in transgenic rice plants carrying multiple copies of TDNA(RB-35S::hpt-35S::gus-mUbi1::bar-LB) insertions. Only transgenic plants survived on the medium containing hygromycin were used for gus and bar gene expression. Herbicide susceptibility of transgenic lines was caused by incomplete integration of mUbi1::bar gene. Transgenic lines with more than three copies of T-DNA showed partial and complete silencing of gus transgene. In order to investigate the organization and methylation status of T-DNA, Southern blot analysis was carried out using two methylation sensitive enzymes, Hpa II and Msp I. Transgenic lines with multiple copies of T-DNA appeared to have some rearranged and methylated fragments of gus and 35S promoter, but not in transgenic lines with single copy. These results suggested that the methylation derived from these complicated structures of T-DNA induced high variable transgene expression among T2 and T3 progeny sublines. Therefore, in order to produce the GM crops with stable expression of target genes, the molecular characterization of T-DNA insertions should be carefully analyzed across the generation.

N/A Objectives: Apolipoprotein E genetic polymorphism was screened in subjects with FH to determine the genetic effects of Apo E polymorphism on basal cholesterol levels, severity of coronary atherosclerosis and response to lipid lowering therapy using HMG CoA reductase inhibitor. Methods: Total C unrelated patients with FH(M:F=24:21, 48.0/11.5yr.) were included in this study. Apolipoprotein E genetic polymorphism was screened. Clinical parameters were checked. Change of lipid profile to lovastatin; 20mg/d(n=19), 4)mg/d(n=12), and of Achilles tendon thickness were analysed. Results: 1) Genotype frequencies of E2/3, 3/3, 4/3 were 8.9, 60.0, 31.1% respectively, and allele frequencies of ε 2, ε 3, and ε 4 were 0.044, 0.800, and 0.155 respectively. 2) Presence and degree of coronary atherosclerosis, the thickness of Achilles tendon and lipid levels were not significantly different by apolipoprotein E genotype, 3) On multivariate study, age, triglyceride and cholesterol /high density lipoprotein were significantly related to presence of coroanry atherosclerosis. 4) Percent reduction of LDL-cholesterol by lovastatin was significantly low in subjects having E4/3 genotype than those having E3/3 genotype (p=0.05; 40mg/d), and the percent reduction of Achilles tendon thickness was significantly low in subjects having E4/3 genotype than those having E3/3 genotype (p=0.037; 20mg/d). Conclusion: The distribution of apolipoprotein E genotype in patients with familial hypercholesterolemia was not significantly different with that in normal subjects. But apolipoprotein E polymorphism may affect the reduction of LDL-cholesterol and Achilles tendon xanthoma with medication of HMG CoA reductase inhibitor in patients with familial hypercholesterolemia.

생발음(生髮飮)-2 피부도포가 C57BL/6 마우스의 육모촉진에 미치는 효과

Objectives: Saengbal-eum-2(Shengfa-yin-2) is a hair care product which is composed of ten plant extracts used in oriental medicine. This study was carried out to investigate the effects of Saengbal-eum-2(Shengfa-yin-2) on hair regrowth and cytokine changes in a shaving model of C57BL/6 mice. Materials and Methods: Five-week-old mice were acclimated for 1 week at a temperature between 21-23℃, 40-60% relative humidity, and 12h of a light/dark cycle before beginning of the experiment. There were three experimental groups including 50% ethanol (EtOH, control), a positive control of 3% Minoxidil, and 30% Saengbal-eum-2(Shengfa-yin-2) in 50% ethanol in 18 female mice. The test compounds were topically treated once a day over 12 days. The hair regrowth was photographically and histologically determined during the experimental period of 12 days. Revelation of EGF, TGF-β1 and IL-6 in hair follicle were also determined using immunohistochemistry. In addition to that, IL-6, TNF-α, and IL-1β in skin tissue were determined using enzyme-linked immunosorbent assay(ELISA). Results: Hair regrowth in 3% Minoxidil and Saengbal-eum-2(Shengfa-yin-2) groups was promoted earlier and faster than the control group. Concentrations of hairs and thick-hair ratio in 3% Minoxidil and Saengbal-eum-2(Shengfa-yin-2) groups were promoted than the control group. EGF was moderately positive in hair follicle of 3% Minoxidil and Saengbal-eum-2(Shengfa-yin-2) groups, but negative in the control group. TGF-β1 was not significantly difference between the groups. IL-6 in hair follicle of Saengbal-eum-2(Shengfa-yin-2) group was negative, but weakly positive in 3% Minoxidil and control group. IL-6 and IL-1β in skin tissue were significantly decreased in Saengbal-eum-2(Shengfa-yin-2) group, but there was not significantly decreased in 3% Minoxidil and control group. TNF-α in skin tissue was significantly decreased in 3% Minoxidil and Saengbal-eum-2(Shengfa-yin-2) groups. Conclusions: These results suggest that Saengbal-eum-2(Shengfa-yin-2) has hair growth promoting activity and it can be used for treatment of alopecia. And these effects relate to EGF revelation of hair follicle and a decrease IL-6, TNF-α, and IL-1β in skin tissue.
