N/A Objectives: EGFR is a transmembrane molecule and is characterized by tyrosine protein kinase activity and ligand-stimulated autophosphorylation. It was found in various cell types including fibroblast and keratinocytes. EGFR appears to have an important role in the development and proliferation of certain human malignancies, including those of neuroglia, bladder, and breast. The heterogeneity of EGFR expression is known to be related with proliferating fraction, histological grade, and susceptibility of anti- cancer drugs, etc. We investigated the biologic significance of the expression of EGFR in lung cancer tissues. Methods: The expression of EGFR was assessed immunohistochemically on paraffin imbedded lung cancer tissues including 28 primary sites with 16 lymph nodes using monoclonal antibody and indirect peroxidase technique. Results: 1) EGFR expression of primary tumor in squamous cell carcinoma was higher than in the others. 2) EGFR expression of primary tumor in stage III was higher than in stage I and stage II. 3) EGFR expression rates of primary and metastatic sites were 60.7% and 61.1%, respectively. No significant difference between primary site and metastatic one was noticed. 4) EGFR expression rates in primary tumor with metastasis was 64.7% and without metastasis was 54.5%. 5) EGFR expression rates of lung cancer by nodal stage were 50% in N1 and 60% in N2. But significant difference among nodal stages was not noticed. 6) EGFR expression rates of lymph nodes according to EGFR positivity of primary tumor were 50% in lymph nodes that have negative steins in lungs and 60% in lymph nodes that have positive stains in lungs. 7) No significant difference in 12-month survival between positive group and negative group was noticed. Conclusion: EGFR expression of routinely processed tissue is a simple technique for the assessment of cell growth and terminal differentiation in non small cell lung cancer. Whether EGFR expression has an independent prognostic value and deserves special attention in pathobiological evaluation in lung cancer remains to be investigated from large series with longer follow - up and to be correlated with multiple biological markers.
전층각막이식술 후 이식편 실패가 온 환자에서의 데스메막내피각막이식술

목적: 전층각막이식술(penetrating keratoplasty) 후 이식편 실패가 온 환자에게 이차적으로 데스메막내피각막이식술(Descemet membrane endothelial keratoplasty, DMEK)을 성공적으로 시행한 1례를 보고하고자 한다. 증례요약: 46세 남자 환자는 양안 원추각막으로 30년 전 우안 전층각막이식술, 14년 전 좌안 전층각막이식술을 받고 경과 관찰하던 중 1년 6개월 전부터 좌안 이식편 실패로 좌안 시력이 서서히 감소하였다. 좌안 이식편 실패에 대해 이차 각막이식술로 데스메막내피각막이식술을 시행하였다. 수술 후 13일째 각막 비측 1/4의 이식편이 분리되어 있는 것이 발견되어 1차례 추가적인 공기주입술을 시행하였다. 수술 후 8달째 환자는 나안시력 0.5까지 확인되었으며 각막은 투명한 상태로 이식편 거부반응 없이 잘 유지되고 있다. 결론: 전층각막이식술 후 이식편 실패가 온 환자에서 이차적인 각막이식술로 데스메막내피각막이식술을 성공적으로 시행하였고 결과도 양호하였다. Purpose: To report a case of a successful secondary Descemet membrane endothelial keratoplasty in failed penetrating keratoplasty. Case summary: A 46-year-old male with keratoconus in both of his eyes underwent penetrating keratoplasty in his right eye 30 years ago and in his left eye 14 years ago. From one and a half year ago, the patient’s visual acuity decreased in his left eye due to graft failure. For treatment, secondary Descemet membrane endothelial keratoplasty was performed. Partial detachment of Descemet membrane was observed at 13 days after the operation, and an additional air injection was performed. At 8 months after the operation, the patient’s uncorrected visual acuity improved to 0.5 and the cornea maintained its clearance without rejection. Conclusions: Secondary Descemet membrane endothelial keratoplasty was successfully performed in a patient with failed penetrating keratoplasty.
진행성 비소세포폐암에 대한 Cisplatin , Etoposide 치료 효과

N/A Thirty patients with stage III non-small cell lung cancer were treated with a combination chemotherapy consisting of Etoposide, 100 to 120 mg/m² i. V. on days 1 -3; and Cisplatin, 60 mg/m²i.v. on day 1, repeated at 21-day intervals. Nineteen patients with stage III non-small cell lung cancer who refused treatment were followed via telephone and letters. The initial symptoma of the patients were cough (39%), chest pain (14%), dyspnea (12%), hemoptysis (8%), hoarseness (6%), palpable mass (6%) and others. Survival rates of 3 months, 6 months, and 1 year were 53%, 26%, and 11%, respectively in the untreated group and 97%, 87%, 45% in the treated group, respectively. Of 16 patients treated with chemotherapy alone, 1 (16 %) achieved complete remission and 4 (25%) achieved partial remission. Of 14 patients treated with chemotherapy and radiotherapy, 3 (21%) achieved complete remission and 5 (36%)achieved partial remission. The overall response rate in treated patients was 43% (complete remission 13%, partial remission 30%). Analysis of the various prognostic factors disclosed that good performance status and non-squamous cell type showed good response rates. The major toxicities were nausea and vomiting (62%), alopecia (100%), anemia (25%), leukopenia (23%), and thrombocytopenia (2%).
Aromatic Toxicants에 의해 유도된 간기능 장해에 미치는 쑥의 영향

간장해 물질인 phenylbutazone, aniline, benzo(a)pyrene을 이용하여 유도한 rats의 간독성발현에 미치는 쑥 추출액의 효과와 간장해 예방 효과를 관찰하여 다음과 같은 결과를 얻었다. 1. 성분 분석 결과 8종의 무기성분이 검출되었으떠 특히 potassium의 함량이 가장 높았고magnessium과 sodium의 함량이 높았다. 2. 총 14증의 지방산이 확인되었으며 그중 필수지방산인 linoleic acid와 linolenic acid의 함량이높았다. 3. 아피노산 분석 결과 총 16종이 검출되었으며 특히 aspartic acid, glutamic acid와 같은 산성아미노산의 함량이 높았다. 4. 쑥 추출액의 전처리는 phenylbutazone의 투여로 증가한 혈청과 조직중의 AST, ALT, ALP함량 증가를 현저히 회복신켰으나 혈청 LDH의 활성과 bilirubin 함량에는 효과를 나타내지않았다. 5. Aniline의 투여로 증가한 혈청과 간장중의 AST, ALT, LDH, ALP 활성을 쑥 추출액의 투여로크게 감소시킬 수 있었다. 그러나, bilirubin 함량에는 큰 변화가 없었다. 6. 쑥 추출액을 투여함으로씨 benzo(a)pyrene의 독성으로 증가한 혈청과 조직중의 AST, ALT,LDH, ALP 활성을 크게 감소시킬 수 있었으며 특히 LDH의 활성 회복 효과가 현저했다. In search for new drugs and methods of pharmacotherapy of liverdamage, we measured the chemical compositions of mugwort. And the mugwortextract was administrated to the experimental animal and investigated its bioche-mical effects in the serum and liver when aromatic hepatotoxicants - phenylbuta-zone, aniline and benzo(a)pyrene- was administrated. As the results are as follows : 1. 14 kinds of fatty acids,16 amino acids and 8 kinds of minerals were quantita-tively analyzed at mugwort. And major components of them were linoleic acid,aspartic acid, potassium and calcium. 2. The increase of serum and liver aminotransferase activity and bilirubin con-tent in hepatotoxicants treated group was markedly decreased by mugwortpretreatment.

Various tumor markers such as CEA., ACTH., Calcitonin and PTH in patient with lung cancer are clinically useful in number of ways: to aid in differential diagnosis, disease staging, to provide an indicator of response to therapy and disease recurrence. In order to evaluate clinical significance of serum PTH in patient with lung cancer, serum PTH level are measured and analyzed in 171 patients who were admitted to Chung-Nam National University Hospital from January 1984 to June 1986. The results were as follows; 1) The value of serum PTH was significantly higher in the patient with lung cancer (mean value; 837.95pg/ ml) compared with normal controls (mean; 524pg/ml) p$lt;0.02. 2) (1) The mean value of serum PTH in patient with squamous cell cancer (103 patients) was 835.95pg/ml and 92 patients (60%) among them were higher than normal controls. (2) The mean value of serum PTH with small cell cancer (37 patients) was 772.86pg/ml and 22 patients(59%) were higher than normal controls. (3) The mean value of serum PTH. in patient with adenocarcinoma (19 patients), Large cell cancer (8 patient), undifferentiate carcinoma (4 patient) were 814.17pg/ml, 520pg/ml, 375pg/ml and 12 patients (62%)., 2 patients (25%), 2 patients (50%) were higher than normal controls, respectively. 3) The mean vlaue of serum PTH was 1037.85pg/ml in metastasis to bone that is higher than 694.12pg/ml in metastasis to liver or 770.3pg/ml in without metastasis, but statistically not significant. 4) In the 37 patients who improved following chemotherphy, mean value of serum PTH is 832.43pg/ml at diagnosis. It is gradually decreased to 681.89pg/ml, 632. 14pg/ml according to follow up check after chemotherphy but statistically not significant. 5) No significant hypercalcemia is developed.
