        시신경을 침범한 밀러피셔증후군

        Purpose: To report an extremely rare case of optic nerve involvement in Miller-Fisher syndrome. Case Summary: A 74-year-old woman presented to our clinic with decreased visual acuity in both eyes. Such symptoms started 1 month prior to her visit, after sudden onset of left-side motor power weakness and dysphasia. Following the initial symptoms, our patient consecutively experienced worsening dysphagia, ptosis, and lateral gaze limitation. We confirmed the diagnosis as Miller Fischer syndrome with a positive anti-GQ1b antibody serology. She received immunoglobulin treatment for 5 days in other departement. She visited our clinic due to continuing diplopia, decrease of visual acuity and gait disturbance. On initial examination, the corrected visual acuity was 0.5 in both eyes. Ptosis of both eyelids, esodeviation, limitation in ocular movement was noted. The fundoscopic examinations revealed relatively pale optic disc. There was decrease in color vision in both eyes. The visual evoked potential test showed low amplitude and delayed latency in P100 wave in both eyes. Two months after her initial visit to our department her symptoms started to improve, and after 4 months all the initial problems resolved completely and her corrected visual acuity checked out to be 0.8 in both eyes and VEP abnormality was recovered. Conclusions: It is extremely uncommon for Miller Fisher syndrome to involve the optic nerve. We present such a case of a 74-year-old woman whose symptoms improved 4 months after its first attack with treatment. J Korean Ophthalmol Soc 2010;51(12):1676-1680

        歸脾湯 및 歸脾湯加味方의 止血效果에 관한 實驗的 硏究

        This Experiment was conducted to investigate the hemostatic effects of Guipitang and Guipitangjiaweifang. For this purpose, the effects of the crude extracts on the platelet count. prothrombin time, fibrinogen level and partial prothromboplastin time were estimated. The rats were divided into normal group, control group and experimental group. Control group was administrated normal saline and experimental group was divided into Sample 1 (Guipitang) and Sample 2 (Guipitangjiaweifang). The results obtained were as follows : 1. The platelet count was significantly increased in both of two samples as compared with the control group. 2. The prothrombin time was significantly shortened in case of Sample 2. however there were no significant changes in case of Sample 1 as compared with the control group. 3. The fibrinogen level was significantly increased in case of Sample 1. however there were no meaningful changes in case of Sample 2 as compared with the control group. 4. The partial prothromboplastin time was significantly shortened in both of two samples as compared with the control group. According to above results. it is supposed that Guipitang and Guipitangjiaweifang have the hemostatic effects in experimental animals.

        A-TIG 용접에서 용입 형상비에 미치는 아크길이와 실드가스의 영향

        TIG welding enables to produce high quality weldment. However it has some problems such as shallow penetration and large distortion due to low penetration aspect ratio after welding. In order to overcome those problems, there are many ongoing studies on A-TIG welding, which use active flux. In this study, the effect of arc length and shield gas on penetration aspect ratio with melt-run welding on STS 304 6t, on which active flux was spreaded, was investigated. Arc length was changed from 1mm to 3mm, and aspect ratio became higher as arc length was decreased in this range. 100% Ar gas, Ar-$H_2$ mixed gas, Ar-He mixed gas, and 100% He gas were used as shield gas in this study. When Ar-$H_2$ mixed gas, Ar-He mixed gas, and 100% He gas were applied, penetration and melting efficiency were both increased as compared with 100% Ar gas. Aspect ratio was the highest with Ar-2.5% $H_2$ mixed gas.

        티트리오일을 이용한 유아 눈꺼풀 사면발이증 치료 1예

        Purpose: The authors of the present study report a case of phthiriasis palpebrarum treated with petrolatum ointment and tea tree oil in a 12-month-old boy. Case summary: A bluish-gray macule and blood clots were found on the central portion of the upper eyelid of a boy who had rubbed his eyelid vigorously 1 week prior. Several 0.5 mm sized brownish-gray nits and 1.6 mm sized lice on the eyelashes were visible and was diagnosed with phthiriasis palpebrarum after light microscopic examination. The nits and lice were gently removed mechanically with the help of a white petrolatum ointment (Vaseline) and the eyelashes were cleaned with 50% tea tree oil. Nits and lice were successfully eradicated without recurrence 10 days after daily treatment with petrolatum ointment and 10% tea tree oil eyelash cleansing.         아메바 모양의 침착물을 동반한 독성 전방 증후군에서 야그레이저를 이용한 치료

        Purpose: To report 3 unusual cases of toxic anterior segment syndrome resembling transparent amoeboid deposits following cataract surgery and to determine effective treatment methods. Case summary: Three patients underwent uneventful phacoemulsification with implantation of a hydrophobic acrylic intraocular lens (IOL) and patching with antibiotic/steroid ointment at the end of surgery. At a mean of 15 days postoperatively, a significant number of anterior chamber inflammatory cells and multiple transparent amoeboid deposits with central, elevated, gray-white opaque material on the anterior surface of the IOL were observed. In two cases, greasy material coated the anterior surface of the IOL in a wavy pattern. All laboratory tests to exclude infectious endophthalmitis were negative. Presuming postoperative inflammation, intensive topical steroid was administered. After treatment, anterior chamber reactions decreased; however, the multiple whitish deposits remained. Subsequently, a Nd:YAG laser (0.8 mJ) was used to disrupt and remove the precipitates. The masses were easily and obviously removed by laser therapy, and the mild residual inflammation resolved satisfactorily with mild topical steroid treatment. No recurrence or related complications developed during the two-year follow-up period in any patients. Conclusions: The delayed postoperative inflammation onset resembling amoeboid deposits is thought to be caused by ingress of ointment into the eye. The specific lumpy deposits cannot be controlled with topical steroids, although Nd:YAG laser treatment can effectively remove inflammatory cell deposits mingled with greasy oil material.         선형펌프 방식 수중발사 시스템의 동특성 해석

        In this paper, the mathematical model of the underwater discharge system using a linear pump is derived and the suitable opening pattern of a flow control valve which satisfies the discharge performance requirements is obtained through the underwater discharge simulations. The simulation results show that the dynamic characteristics of a projectile are greatly affected by the opening pattern of the flow control valve, however, hardly by the diving depth. It is anticipated that the simulation model can be used to derive the design parameters and analyze the performance of the designed underwater discharge system using a linear pump.

        압축공기 방식 수중발사 시스템의 동특성 시뮬레이션

        In this paper, simulations of the underwater discharge system with compressed air are performed to predict dynamic characteristics of the system and to find optimal opening trajectories of the expulsion valve. Major components of the system are defined and their governing equations are derived to make up the mathematical model. The compressed air discharge method is affected largely by the discharge depth, and therefore the opening trajectories according to the discharge depth should be found to satisfy the demands of discharge performances. Simulation results are compared with experimental data to confirm the validity of the system model.

      • 유아의 지역사회 참여 교육에 관한 탐색적 연구

        As the importance of child education is becoming more emphasized and its social demands are getting various in many ways, not only the importance of the education service of children but also the needs of inclusive education service that includes their family and community has been increased. A community is an important place for children to explore and interplay and also it's a good place to develop their physique and emotion to form their experience. When they are born, they among themselves in family and community and they form their ego in these environments and as they become socialized they develop themselves. Children's growth and experience is mostly effected by members of the family and community and it is very important to make the children to contribute to society. This essay is aimed to closely examine effectiveness of children's educational activity, 'becoming a member of a community' which understands the meaning of becoming a member of a community and participate progressively by applying experimental curriculum to approaches of Reggio Emilia to solve social problem. Under this aim, the problems raised in this study are: Firstly, I examined how children's educational institution and community linking program's activation effects children's cooperative learning. Secondly, I examined the changes in minds of welfare of local residents and awareness when children's are involved in a community. In this study, Reggio Emilia program is executed parallel to basic kindergarten's daily lesson in total of 17 times in 6 weeks. The researchers have involved in the education fields and anticipated every 5 year old target classes and put 6 of those who are interested (4 boys and 2 girls) into a small group and executed the procedure through conference with the researchers, main teachers and secondary teacher. Children's participation, observation and symbolic representation results are collected and results from digital camera, video capture and recorded tapes are recorded by description of scene, situation and movement, and verbal interactions are transcribed for analysis data. Drawings and presentations from children are analyzed for their activity conditions and verbal explanation as reference. 최근 유아교육에 대한 중요성이 강조되고 이에 대한 사회적 욕구가 다양화되면서 유아에 대한 교육서비스의 필요성뿐만 아니라 그 가족과 지역사회를 포괄하는 포괄적 교육서비스의 필요성이 증가되었다. 지역사회는 유아에게 탐구와 상호작용의 중요한 장이며 유아의 신체적 정서적 발달을 위한 장으로 유아의 경험을 형성하는데 매우 중요한 역할을 한다. 유아는 태어나면서 가정과 다양한 지역사회에 속하게 되며 이러한 환경 속에서 자아를 형성해 가고 사회화 되면서 자기 자신을 발전 시켜 나간다. 영유아 성장과 경험에 중요한 영향을 미치는 요인은 가족과 지역사회 구성원이며 성장하여 사회에 소속되어 사회에 기여 할 수 있는 인간으로 기르기 위해서 지역사회의 역할이 매우 중요하다. 본 논문에서는 레지오 에밀리아 접근법을 사회문제를 해결하는 시험적 교육과정에 적용하여, 지역사회 구성원으로서 의미를 이해하고 지역사회에 적극적으로 참여 할 수 있는 유아의 '지역사회 구성원 되기' 교육활동에 대한 효과성을 규명하는데 목적을 두고 있다. 이러한 목적 하에 본 연구에서 제기하고자 한 문제는 다음과 같다. 첫째 유아 교육기관과 지역사회 연계 프로그램의 활성화가 유아의 협력학습에 어떻게 영향을 미치는지 알아보았다. 둘째 유아의 지역사회 참여가 지역협력을 통한 지역주민들의 복지마인드 변화와 인식의 변화를 알아보았다.

        안내렌즈삽입술 후 안압과 전방각의 변화

        Purpose: To evaluate the effects of ICL implantation on angle structures, intraocular pressure (IOP) and pupil diameter. Methods: A Staar implantable contact lens (ICL) was implanted in 81 eyes of 43 patients with spherical equivalent over -6D. Gonioscopy was done 1 day after laser iridotomy and 6 months after ICL implantation and postoperative changes in gonioscopic findings were evaluated. Also IOP and pupil diameter were examined. Results: There were occlusion or narrowing of the LI opening site in 10 eyes (12.3%). Width of angle was wide over 30° in all cases but that was narrowed under 20° in 16 eyes (19.8%) at postoperative 6 months. The mean pigmentation was 2.11 in inferior, 0.47 in nasal, 0.22 in temporal and 0.18 in superior angle at 6 months postoperatively by semiquantitative method (Grade 0~4). There was decrease of pigmentation in nasal and temporal angle but no change in inferior and superior angle after ICL implantation. There was temporary increase of IOP at 1 week and 1 month postoperatively due to steroid eye drops but returned to preoperative level and maintained until the 6 months postoperatively. There was significant decrease of pupil diameter at postoperative 1 and 3 months but returned to the preoperative level at postoperative 6 months. There was no evidence of pigment dispersion syndrome and pigmentary glaucoma. Conclusions: ICL implantation caused the narrowing of width of angle but did not increase trabecular pigmentation. We expect that ICL implantation is safe about pigment dispersion syndrome and pigmentary glaucoma.

        오노디 세포내 곰팡이 덩이에 의한 시신경병증 1예

        Purpose: To report a rare case of optic neuropathy caused by a fungal ball in an Onodi cell. Case summary: A 63-year-old female was referred to our clinic with relapsed visual loss and ocular pain in the right eye. She had been diagnosed as optic neuritis 14 days before and given pulse steroid therapy. She recovered to normal but relapsed 7 days before. In medical referral there was no suspected multiple sclerosis but only a few inflammation in the paranasal sinuses. On our initial examination, best corrected visual acuity was counting finger at 10 cm in the right eye, and 1.0 in the left eye, along with relative afferent pupillary defect in the right eye. The fundoscopic examinations disclosed disc swelling; nearly total visual field defect was observed on visual field examination and visual evoked potential test revealed decreased amplitude at P100 wave in the right eye. Clinical impression was relapsed optic neuritis. After the administration of pulse steroid therapy, her disc swelling was decreased and visual acuity was recovered to 0.6, however, visual acuity was exacerbated to 0.4 in 2 weeks. We checked outside brain magnetic resonance imaging (MRI) and the result showed optic neuropathy caused by a fungal ball in an Onodi cell. The patient was referred to otorhinolaryngologist and fungal ball was removed by endoscopic sinus surgery. 3 weeks after surgery the patient's visual acuity was 0.9, no disc swelling was found and visual evoked potential was recovered to normal. Conclusions: An Onodi cell lesion should be considered in the differential diagnosis of optic neuritis, identified by imaging studies and promptly removed by surgery for visual recovery.
