        절반용량의 광역학치료를 받은 만성 중심장액맥락망막병증에서 스펙트럼영역 빛간섭단층촬영 소견과 시력과의 상관관계

        Purpose: To evaluate the results of half-dose photodynamic therapy (PDT) and determine the correlation between morphological changes measured by spectral-domain optical coherence tomography (SD-OCT) and visual acuity in patients with chronic central serous chorioretinopathy (CSC). Methods: Twenty-five eyes of 25 patients with chronic CSC who had received half-dose verteporfin PDT were enrolled in the present study. The best-corrected visual acuity (BCVA), outer nuclear layer (ONL) thickness, and the integrity of the photoreceptor inner and outer segment junction (IS/OS) using SD-OCT were evaluated at baseline and at 3 and 6 months after treatment. Results: The neurosensory retinal detachment disappeared in all eyes 6 months after treatment. The BCVA improved significantly from 0.50 ± 0.32 to 0.31 ± 0.29 log MAR at 6 months (p < 0.001). The average ONL thickness at the central fovea was 88.76 ± 19.95 μm at 6 months and the ONL thickness was well correlated with the BCVA (γ = -0.64; p = 0.001). There was no significant correlation between the status of IS/OS and the BCVA. Conclusions: Half-dose PDT is effective in treating chronic CSC resulting in visual improvement and complete resolution of neurosensory retinal detachment. The ONL thickness which was positively correlated with the BCVA could be an indicator for visual prognosis of chronic CSC. J Korean Ophthalmol Soc 2013;54(11):1715-1722

        소규모 주거공간의 효율적 활용을 위한 공간계획 사례연구

        In the domestic housing market, uncompleted lot-solid apartments and unoccupied residences are increasing in the medium-large scale area whose rents and transactions have been unstable, as going through the financial crisis and economical recession. Demands for small-scale housings are steadily increasing by practical consumer group who don't need large places, which are often not needed for the current people who are spending most of their time outside of their houses. Therefore, this study proposes the useful methods for spatial planning for small-scale houses for multiple households, including variable form, levels form, multi-level form, and smart-sizing. For efficient spatial planning for that purpose, here, small-scale housings are not limited to places for families with 1-2 members, but considered as one of broader approaches to graft various households and families. In addition, we examined the existed trends of researches about spatial utility of small-scale places, and did the research about the status of small-scale housing developments, and then proposed the direction for future small-scale housings by analyzing the advanced examples.

        혈액학적 이상과 동반된 급성 구후시신경염

        Purpose: We present a case of a patient with optic neuritis who had underlying suspicious idiopathic thrombocytopenic purpura.Case summary: A 35-year-old female with no other systemic disease visited our clinic due to acutely decreased visual acuity in her left eye 10 days in duration. Relative afferent pupillary defect was observed, but without definite papilledema. Based on brain magnetic resonance imaging (MRI), optic neuritis was suspected. Laboratory tests showed increased red blood cells, hemoglobin and, hematocrit levels and decreased platelets. Peripheral blood smear test showed decreased platelets, relative lymphocytosis and atypical lymphocytes. Specific antibodies for autoimmune disease were not present. High-dose steroid pulse therapy (methyl prednisolone 1.0 g/d, 3 days) was started. One month after treatment her visual acuity and platelet count recovered and her visual field defect improved.         자동사 구문의 억양

        This study aims to investigate the intonational realization of sentences including the two types of intransitive verbs. It has been accepted that intransitive verbs fall into two subclasses-unergative verbs and unaccusative verbs, each associated with a particular argument structure. The unergative-unaccusative dichotomy has also been linked to differences in the phonetic constructions in which they appear. Previous linguistic analyses claim that the placement of pitch accents is determined by the verb-argument structure of the intransitive sentences in a broad focus context. To challenge this claim, we replicated Hoskins` experimental samples and further tested with new phonetically and semantically controlled sentences. We recorded speech of native speakers of English and Korean learners of English and performed prosodic and acoustic analyses. Our results showed that, when the argument structure was controlled, English native speakers produced similar intonation patterns to previous studies. Meanwhile, when the sentences were both semantically and phonetically matched, the patterns were contrary to the previous studies. Therefore, the argument structure approach is insufficient for the explanation of accentuation of intransitive sentences in broad contexts. (Changwon National University)

        공공 도서관의 실내 공간 구성요소 특성에 관한 연구 -핀란드 공공도서관을 중심으로-

        정보화 시대의 도래와 함께 사회구조의 변화와 수득수준의 상승으로 삶의 질이 높아지면서 기존의 지식공간은 다양한 활동이 함께 이루어지는 복합문화 지식공간으로 변모하였다. 그러나 높은 교육열에 반하여 국가의 지원을 받고 있는 공공도서관은 턱없이 부족한 시설과 쾌적하지 못한 환경으로 이용률 저하를 보이고 있다. 이는 우리나라와 비슷한 조건의 낮은 인구율과 자원을 대신하여 교육열로 국력을 높이고자 노력하는 핀란드와는 대조적이다. 핀란드는 모든 시민이 매일방문하고 싶어 하는 공간을 목표로 정부의 전폭적인 지원 아래 공공도서관 이용률이 세계 1위이다. 이러한 핀란드 공공도서관의 실내 공간 구성요소의 특성을 살펴보기 위하여 구축적 요소인 벽, 천장, 창, 바닥, 계단과 장식적 요소인 색채, 가구, 재료, 조명으로 나누고, 핀란드의 공공도서관의 사례를 통하여 분석하였다. 핀란드 공공도서관에서는 낮은 기온으로 실내 활동이 발달 된 지역적 특성을 적용하여 컬러를 통한 강조나 창을 통해 자연광을 최대한 실내로 도입하려 하였다. 따라서 본 연구에서는 핀란드 공공도서관에서의 실내 공간 구성요소에서 나타난 것과 같이 국내 도서관도 지역성이나 특징적인 부분을 차용하여 보다 질 높은 교육공간으로 발전시키고자 한다. The beginning of the information age made changes in how a society is formed and, the increasing level of incomes consequently enhanced the life quality of people. Current knowledge spaces were eventually transformed into some complex cultural knowledge spaces in which all these different activities would be shared and enjoyed. However, in comparison with the severest educational fever in the country, the governmental support for public libraries has been always unbelievably less satisfactory and, both insufficient facilities and unpleasant environments are only decreasing the occupancy ratio of the public libraries. That is the one of the biggest differences between South Korea and Finland which has been working as hard as the former has been doing to improve the national competitiveness through the educational fever that would replace the low population rate and the scarcity of resources, Those are the special conditions which put both countries in the similar situations. In order to create spaces good enough to be visited every day by all its people in the country, the government of Finland has never been reluctant to support public libraries and, the occupancy ratio of the libraries in Finland is considered highest around the world. To look into characteristics of indoor space components of the public libraries in Finland, the study first categorized the components into constructive factors such as walls, ceilings, windows, floors and stairs as well as into decorative factors that would include colors, furniture, materials and lighting and there, the study investigated all of them as referring to several cases of the public libraries in Finland. As for public libraries in Finland, they were trying either to emphasize their places with colors or to attract natural light as much as possible to the indoor through windows by making good use of local characteristics of how low temperatures encourage people to stay inside. In the light of that, as observed from those indoor space designing components of the public libraries in Finland, this study applies regionality or distinguishing features of libraries in South Korea to improving the libraries as an educational space of high-quality.

        브랜드 콜라보레이션 마케팅을 통한 디자인호텔의 실내디자인에 관한 연구

        The rapid growth of economy improves not only the life quality of people in this modern society but also standards of value in connection with how those people would spend money as they engage in various leisure and cultural activities. That being the case, hotels are currently changing into places of new concepts depending on those new lifestyles of these people, and in those new places, people can entertain and experience as enjoying cultures. The marketing collaboration of the hotels with brands can be used as a more inclusive way to enhance brand images. In addition, as having infinite possibilities of the collaboration`s being able to create a new-concept space with an identity of a brand included, this collaboration makes it possible for the design hotels to decorate their interior spaces differently from those of other hotels. In the light of that the brand collaboration is basically divided into three groups which are the fashion collaboration, the technique collaboration and the designer collaboration, and the brand collaboration is also able to express the interior spaces of the design hotels as working on those six components, such as reproduction, events, alteration, messages, culture and image. After all, through the brand collaboration marketing, this thesis expects an interior design of a new-concept design hotel which would play a role as a complex cultural space.

        다래끼제거술 중 리도카인 국소마취제 주입으로 인해 생긴 데스메막분리 1예

        목적: 다래끼 수술 도중 리도카인 국소마취제 주입 시 의도치 않게 발생한 각막 천차로 인해 생긴 데스메막분리 및 각막 부종에 대해 전방 내 14% C3F8 가스 및 공기 주입술 시행으로 데스메막분리가 유착된 1예를 보고하고자 한다. 증례요약: 8세 여자 환아에서 다래끼제거술을 위한 국소마취를 위해 상안검에 리도카인 주사를 주입하던 중 강한 통증과 급격한 시력 저하를 호소하여 술기를 중단하고 본원으로 전원되었다. 세극등 소견상 각막의 찰과상 및 전반적인 각막의 부종으로 인해 각막의 세부구조가 잘 관찰되지 않았으나 각막단층촬영 검사상 데스메막분리가 관찰되었다. 초진 후 3일째 각막 상피는 완전히 재생되었으나 이후 3주까지 데스메막의 박리가 계속되어 다시 시행한 각막단층촬영 검사상 일부 데스메막 파열이 관찰되어 적극적인 치료를 위해 전방내 가스 및 공기 주입술을 시행하였다. 48시간 만에 데스메막 파열이 발생하였던 부분이 말려 있는 것을 제외하고는 전체적인 유착을 얻었으며 이후 약 3개월간 경과관찰 후 안정적인 각막소견과 정상시력을 회복하였다. 결론: 안검의 마취주사 중 예상치 못한 각막천자로 발생한 데스메막분리 및 파열은 각막부종으로 인한 시력저하를 초래하였고 일반적인 데스메막분리와 달리 시간의 경과에 따른 회복을 가져오지 못했다. 이에 적극적인 치료방법으로 선택한 전방내 14% C3F8 가스주입술 및 공기주입술을 통해 데스메막분리가 유착되었으며, 정상 시력을 회복한 증례를 경험하였기에 이를 보고하고자 한다. <대한안과학회지 2016;57(11):1790-1794> Purpose: To report a case of Descemet’s membrane detachment and corneal edema caused by an iatrogenic corneal perforation created while performing a local anesthetic (lidocaine) injection into the eyelid for a hordeolum incision and a drainage procedure. The detachment resolved after 14% C3F8 gas and air injections into the anterior chamber. Case summary: An 8-year-old female visited our clinic after the onset of severe pain and decreased visual acuity while receiving a local anesthetic injection into the upper eyelid in preparation for a hordeolum incision and drainage procedure. Corneal optical coherence tomography (OCT) showed Descemet’s membrane detachment. Three days after the first visit, the corneal epithelium had entirely healed. However, Descemet’s membrane detachment persisted even after three weeks of follow-up. A corneal OCT was repeated after three weeks and showed a partial Descemet’s membrane rupture. A more aggressive treatment method was deemed necessary, and gas and air injections into the anterior chamber were performed. After 48 hours, aside from some Descemet’s membrane rolling at the site of rupture, overall reattachment of Descemet’s membrane was noted. After three months of follow-up, the patient showed a stable corneal state and normalized vision. Conclusions: Descemet’s membrane detachment and rupture resulting from an iatrogenic corneal perforation during an injection of lidocaine to the eyelid led to decreased visual acuity from corneal edema. As a more aggressive treatment method, 14 % C3F8 gas and air injections into the anterior chamber were performed and resulted in near complete reattachment of Descemet’s membrane’s and normalization of the patient’s visual acuity.         인공눈물 점안 후 안구 표면 두께 간섭계를 이용한 눈물 지질층 두께의 변화 분석

        목적: 농도 및 보존제 유무에 따른 히알루론산 나트륨제제의 인공누액 점안 후 LipiView?? II 간섭계(TearScience Inc., Morrisville, NC, USA)를 이용하여 눈물 지질층 두께의 시간에 따른 변화를 측정, 비교 분석함으로써, 인공눈물의 사용이 LipiView?? II 검사 결과에 미치는 영향을 분석하고자 한다. 대상과 방법: 건성안 환자를 대상으로 전향적 연구를 시행하였다. 모든 대상자에게 눈물막 파괴 시간 측정, 쉬르머 검사, 각막 및 결막형광염색 검사, 마이봄샘 기능부전 평가, ocular surface disease index 설문조사를 실시하였으며, 대상자를 난수표를 이용한 무작위 추출로 인공누액 종류에 따라 세 그룹(1군: 무보존제 0.1% 히알루론산 나트륨제제 인공눈물, 2군: 무보존제 0.3% 히알루론산 나트륨 인공눈물, 3군: 0.003% 염소 벤잘코니움을 포함하는 0.1% 히알루론산 나트륨제제 인공눈물)으로 나눠 각각의 인공눈물을 점안 전, 직후, 1, 3, 6시간 후의 눈물지칠층의 두께를 LipiView?? II 간섭계로 측정 후 비교 분석하였다. 안검염의 유무에 따라 두 군으로 나누어 눈물지칠층의 두께를 추가로 분석하였다. 결과: 눈물 지질층의 두께를 측정한 결과, 인공눈물제제를 점안하고 3시간 뒤 눈물 지질층의 두께 변화가 유의하게 나타났다(p=0.03). 제1군 및 2군에서 시간에 따른 눈물 지질층 두께의 변화가 유의하게 측정되었고(p<0.01, p<0.01), 제3군에서는 눈물 지질층 두께 변화가 관찰되지 않았다(p=0.71). 환자를 안검염 그룹과 비안검염그룹으로 세분화하여 분석해 보았을 때, 비안검염 그룹에서는 인공눈물점안과 상관 없이 눈물지질층의 두께가 일정하였고(p=0.19), 안검염 그룹에서는 제1군과 2군에서 인공눈물 점안 후 시간 경과에 따라 눈물지질층 두께가 변화하였다(p<0.01, p=0.01). 안검염 그룹에서 제1군과 2군의 눈물 지질층의 두께를 비교하면, 인공눈물 점안 직후 및 1시간, 3시간 뒤까지 유의한 차이를 보였으며(p=0.04, p<0.01, p=0.02), 6시간째에는 유의한 차이가 없었다. 제1군과 3군의 경우에는 시간별 차이가 없었다(p=0.08). 결론: LipiView?? II 간섭계를 이용해 측정한 눈물 지질층 두께는 인공눈물의 농도와 보존제의 유무에 따라서도 수치의 차이를 보였으며, 안검염의 유무에 따라서도 인공눈물 점안 후 눈물 지질층의 두께 변화 양상에 차이가 있었다. 이는 LipiView?? II 간섭계로 눈물 지질층을 측정하는 경우 인공눈물 종류와 관계없이 적어도 6시간의 점안을 금해야 하며, 또한 안검염이 있는 환자군과 보존제를 포함하는 인공눈물을 사용하는 환자에서 더욱 유의해야 함을 시사한다. Purpose: The changes in tear film lipid layer thickness (LLT) after artificial tears application using LipiView??II interferometer were assessed. Methods: We performed a prospective study of patients with dry eye disease. All subjects underwent measurement of tear film break-up time, Schirmer test, ocular surface staining, meibomian gland evaluation, and subjective score assessment using the Ocular Surface Disease Index. All subjects were randomly assigned to 1 of 3 groups using table of random numbers (group 1, sodium hyaluronate [HA] 0.1% eye drops without preservatives; group 2, HA 0.3% eye drops without preservatives and group 3, HA 0.1% with benzalkonium chloride 0.003%). LLT was measured before, immediately after and 1 hr, 3 hrs, and 6 hrs after artificial tears application. Additionally, the patients were divided into 2 subgroups depending on the presence of meibomian gland dysfunction (MGD) and further evaluated. Results: Significant change in LLT was observed at 3 hrs after artificial tears instillation. LLT in groups 1 and 2 showed significant changes over time (p < 0.01 and p < 0.01, respectively). However, LLT in group 3 showed no change. LLT was unchanged in patients without MGD. Conversely, in MGD patients, a significant difference in LLT between groups 1 and 2 was observed immediately after and 1 hr and 3 hrs after instillation of artificial tears (p = 0.04, p < 0.01 and p = 0.02, respectively) but not at 6 hrs. However, no significant difference in LLT between groups 1 and 3 was observed in MGD patients. Conclusions: LLT after instillation of artificial tears measured using LipiView??II interferometer was affected by artificial tear concentration and presence of preservatives. Additionally, the presence of MGD can impact the pattern of LLT changes induced by artificial tear instillation. Therefore, LLT measurements using LipiView??II interferometer require at least a 6-hrs interval after use of eye drops, especially for patients with MGD or using artificial tears with preservatives.

        시신경 수막종의 치료 및 임상 경과

        목적: 한국인에서 시신경 수막종 환자들의 임상양상 및 치료 경과를 알아보고자 하였다. 대상과 방법: 2002년 1월부터 2015년 12월까지 세브란스병원에서 시신경 수막종으로 진단 받은 환자 11명 11안을 대상으로 의무기록을 통한 후향적 방법으로 조사하였다. 결과: 환자의 평균 연령은 47.6세, 여성이 10명이었으며 모두 단안 침범 소견을 보였다. 환자의 초기 증상으로는 시력저하, 안구돌출이 가장 흔했고, 3명은 안구운동장애, 7명은 시야결손이 있었다. 이 중 3명은 종양 크기 증가 및 두개내로의 진행, 2명은 시력 저하가 발생하여 이들 5명에서 치료를 시행하였다. 치료 없이 경과관찰한 5명은 의미 있는 시력저하나 시야결손의 악화를 보이지 않았으나, 수술을 받은 1명은 시력저하 및 시야결손의 악화가 나타났으며 3차원 입체조형 방사선치료를 시행 받은 2명 중 1명은 치료 후 시야결손이 호전되었고, 다른 1명은 치료 후 시야결손의 변화는 없었으나 종양의 크기가 감소하고 더 이상 증가하지 않는 소견을 보였다. 감마나이프 치료를 받은 2명 중 1명은 시야결손이 악화되었고, 다른 1명은 치료 후 시력 저하가 발생하였다. 결론: 시신경 수막종은 진행이 느린 경우가 많고, 적극적인 치료 후에 시력 저하 또는 시야 결손이 악화될 수 있으므로 병의 초기에 치료 여부는 신중하게 결정해야 하며, 병의 진행이 빠르거나 경과가 진행된 경우에 국한하여 시행하는 것이 좋을 것으로 판단된다. 치료가 필요한 경우 3차원 입체조영 방사선치료가 우선적으로 고려될 수 있으나 이에 대해서는 향후 더 많은 연구가 필요하다. Purpose: To evaluate the clinical course of optic nerve sheath meningioma (ONSM) in Korean patients. Methods: A retrospective study of 11 eyes of 11 patients with a diagnosis of ONSM between 2002 and 2015 at Severance Hospital. Results: The mean age at symptom onset was 47.6 years. Ten females and 1 male participated in the study and all tumors were unilateral. Patients typically presented with visual loss and proptosis. Three patients complained of limited extraocular movements and seven patients exhibited visual field defects. Three patients who had a greater growth rate with intracranial involvement and two patients who had decreased vision received treatments. Five patients maintained good vision and visual field during the follow-up period. However, one patient who underwent surgical treatment presented significant visual loss and deterioration of visual field defect. One out of two patients who received three-dimensional conformal radiotherapy (3D-CRT) experienced improvement in visual field, and the other showed no change in visual field defect but remained stable with decreased tumor size. One out of two patients who underwent gamma-knife surgery showed aggravated visual field defect and the other presented with visual loss. Conclusions: ONSM is typically a slow-growing tumor. Deterioration of visual loss and visual field defect can occur after treatment of ONSM. Therefore, management should be considered carefully and should be limited to cases in which progression of the disease is advanced or tumor growth is fast. 3D-CRT can be considered in patients in need of treatment.
