수용전념치료(ACT)가 아동기 외상을 경험한 청소년의 자아존중감, 자아탄력성 및 대인관계 문제에 미치는 효과

The purpose of the study is to examine the effects of Acceptance and Commitment Therapy (ACT) on self-esteem, interpersonal problem, and ego resilience of adolescent with childhood trauma. A total of 460 adolescents in middle school and high school of Chonbuk province completed Childhood Trauma Questionnaire (CTQ), Child Report of Post-traumatic Symptoms (CROPS), Self-Esteem Scale (SES), Ego-Resiliency Scale (ER), and Inventory of Interpersonal Problems (IIP). Twenty-eight patients who selected by hose scales were randomly assigned to the ACT group(n=14) or the control group(n=14). ACT program was administered for 8 sessions. All participants completed CTQ, CROPS, SES, ER, IIP at pre-test and at one month follow-up period. Our result resealed that ER in the therapy group was increased more than that in the control group. However, post-traumatic symptom, and interpersonal problem in the therapy group were decreased. Implications, limitations of this study, and suggestions for future research were discussed. 본 연구는 수용-전념 치료(ACT) 프로그램이 아동기 외상 경험이 있는 청소년의 자아존중감, 자아탄력성, 대인관계 문제에 미치는 효과를 알아보았다. 전북지역 소재의 중·고등학교 학생 460명을 대상으로 아동기 외상경험 질문지, 외상후 증상 수준 검사, 자아존중감 척도, 자아탄력성 척도, 대인관계 문제 항목표를 실시하였으며 28명을 참가자로 선정하여 수용 전념 치료 집단에 14명, 대기 통제집단에 14명을 무선할당 하였다. 수용 전념 치료 프로그램은 주 2회씩 총 8회기가 진행되었으며, 프로그램 종료 후 사후검사를 실시하였고, 1개월 후 추적검사를 실시하였다. 그 결과, 수용 전념 치료(ACT) 프로그램에 참여한 집단이 대기 통제 집단보다 자아탄력성이 유의하게 상승하였고, 외상후 증상 수준, 대인관계 문제는 유의하게 감소하였다. 그 수준은 추적 검사까지 유지되었다. 마지막으로 본 연구의 의의, 제한점 및 후속 연구에 대안 제언이 논의되었다.

Background/Aim: Cancer-associated fibroblasts (CAFs) play an immunosuppressive role in the tumor microenvironment (TME) of human cancers; however, their characteristics and role in hepatocellular carcinoma (HCC) remain to be elucidated. Methods: Nine tumor and surrounding liver tissue samples from patients with HCC who underwent surgery were used to isolate patient-derived CAFs. Cell morphology was observed using an optical microscope after culture, and cell phenotypes were evaluated using flow cytometry and immunoblotting. Cytokines secreted by CAFs into culture medium were quantified using a multiplex cytokine assay. Results: CAFs were abundant in the TME of HCC and were adjacent to immune cells. After culture, the CAFs and non-tumor fibroblasts exhibited spindle shapes. We observed a robust expression of alpha-smooth muscle actin and fibroblast activation protein in CAFs, whereas alpha-fetoprotein, epithelial cell adhesion molecule, platelet/endothelial cell adhesion molecule-1, and E-cadherin were not expressed in CAFs. Furthermore, CAFs showed high secretion of various cytokines, namely C-X-C motif chemokine ligand 12, interleukin (IL)-6, IL-8, and C-C motif chemokine ligand 2. Conclusions: CAFs are abundant in the TME of HCC and play a crucial role in tumor progression. These fibroblasts secrete cytokines that promote tumor growth and metastasis.
