        Concomitant Neuropathy of Recurrent Motor Branch of Median Nerve in Carpal Tunnel Syndrome: A Case Report

        Selective fascicular involvement of median nerve in carpal tunnel syndrome (CTS) has been reported, such as predominant involvement of volar-radially located motor and sensory fibers to thumb. Also, recurrent motor branch (RMB) of median nerve could be affected separately. A man has suffered from right-hand weakness and mild tingling sense on I to III digits visited. Thenar muscles wasting and weakness were noted without sensory loss. Whereas the results of sensory conduction and comparison of median-second-lumbrical/ulnar-interossei studies suggested mild CTS, motor responses revealed severe neuropathy. The patient was diagnosed with right CTS, mild degree and concomitant incomplete but severe neuropathy of RMB. The patient underwent an operation. The RMB was found as the transligamentous type with a degenerative change and severely compressed by ligament and thenar muscles. Open carpal tunnel release and decompression with neurolysis were done. Through precise analysis of electrodiagnostic findings, correct diagnosis and proper management were achieved.

        GPU 연산을 활용한 유사이송 예측모형 개발

        전 세계적으로 연안침식 문제가 대두됨에 따라 많은 해안선이 지형변화를 겪고 있다. 기후변화 및 해안인구증가로 미루어 볼 때 그 현상은 가속화될 수 있으며, 이에 대응하기 위해 신속하게 지형변화를 모의할 수 있는 유사이송 예측모형 개발의 중요성이 강조된다. 본 연구에서는 GPU (Graphics Processing Unit)를 기반으로 한 유사이송 예측모형을 제안하였으며, GPU 병렬연산을 활용함으로써 기존의 CPU 기반모형 대비 더욱 개선된 속도로 지형변화를 모의할 수 있도록 모형이 개발되었다. 개발된 모형에 대해 수치모형 성능과 GPU 연산효율에 초점을 맞추어 분석을 수행하였다. 모형의 성능검증을 위해 Dam-break 수리실험에 대해 수치모의를 수행하였으며, 모의결과가 관측된 실험데이터와 잘 일치하는 것을 확인하였다. GPU 연산효율은 CPU 기반모형과 수치모의 연산시간을 비교하여 분석하였으며, 개발된 GPU 기반모형이 연산시간의 효율이 상당히 우수한 것으로 확인되었다.

        고분해능 토과전자현미경에 의한 자기터널접합 내 AIOx의 산화방법차이 및 열처리효과에 대한 연구

        Both natural oxidation and plasma oxidation methods were employed to fabricate AlO$_x$ insulating layers which were analyzed by using high-resolution transmission electron microscopy (HRTEM). The oxidation in natural oxidation primarily occurred through the grain boundary at an early stage of oxidation and then throughout the grains at a later stage. The morphology of the naturally oxidized AlO$_x$ layer was rugged and modulated. In plasma oxidation, the oxidation did not occur through the grain boundary of Al but occurred uniformly throughout the Al layer. The morphology of the plasma-oxidized AlO$_x$ layer was flat and uniform. A ferromagnetic Co layer was partially oxidized because of preferred grain boundary oxidation. The locally oxidized Co layer was reduced and the interface was improved by thermal annealing. 자기터널접합에서 절연층의 형성은 자기적 성질의 향상 및 안정화에 매우 중요한 영향을 미친다. AlO$_x$ 절연층을 제조하는 방법으로서 자연 산화법과 플라즈마 산화법을 이용하여 고분해능 투과전자현미경을 사용한 미세조직 관찰 및 비교분석을 행하였다. 자연 산화법에서의 산화는 초기에 Al 결정립계를 통하여 주로 발생하였고 이후 Al 결정립 내부에서 추가의 산화가 진행되었다. 산화층의 형상은 굴곡이 심한 표면을 이루고 있었다. 플라즈마 산화법에서 산화는 Al 층 전체를 통해서 매우 빠르게 이루어졌고 시간이 지남에 따라 산화가 포화상태에 이름을 확인하였다. Al과 AlO$_x$ 간 계면은 비교적 평평하였다. 또한 자연 산화 후 열처리를 행하면 자기저항 등 자기적 성질이 보다 향상된다는 여러 연구 결과들이 보고되고 있는데 Co를 강자성층으로 택하여 고분해능 투과전자현미경으로 관찰한 결과 Al의 결정립계를 통해 국부적으로 산화된 Co 조직이 열처리에 의해 결정 Co로 환원됨을 확인하였다.

        투과전자현미경에 의한 고 규소강에서의 규칙상 소멸조건에 대한 연구

        The effects of cooling rate, forming method and alloying on the suppression of ordering reaction have been studied in high Si steels. Four kinds of cooling were used : air cooling, oil quenching, water quenching and ice brine quenching. An order-disorder transition diagram was obtained in terms of Si content and cooling rate. It was confirmed that the cooling rate was a more important factor to suppress the ordered phase than annealing conditions. When 0.15 wt% Mn was added to 6.5 wt% Si steel and fabricated by spray casting, the ordering did not take place.

        Common Peroneal Neuropathy Caused by Intraneural Ganglion Cyst, Electrophysiological Findings Related to Fascicular Involvement and Operative Findings in Two Cases

        Foot drop can occur as a manifestation of a number of pathological and clinical conditions and is a fairly common disability encountered in electrodiagnostic clinics. This report describes two cases of atraumatic foot drop. Both cases were electrophysiologically diagnosed as common peroneal neuropathy around the fibular head. Radiographic and surgical findings confirmed intraneural ganglion cyst of common peroneal nerve and the surgical resection was performed. Although the radiographic and surgical findings were compatible with the lesion of common peroneal nerve, the precise analysis of the electrophysiologic findings could differentiate the fascicular involvement in two cases.

        The Impact of an Emergency Fee Increase on the Composition of Patients Visiting Emergency Departments

        Objectives: This study aimed to test our hypothesis that a raise in the emergency fee implemented on March 1, 2013 has increased the proportion of patients with emergent symptoms by discouraging non-urgent emergency department visits. Methods: We conducted an analysis of 728 736 patients registered in the National Emergency Department Information System who visited level 1 and level 2 emergency medical institutes in the two-month time period from February 1, 2013, one month before the raise in the emergency fee, to March 31, 2013, one month after the raise. A difference-in-difference method was used to estimate the net effects of a raise in the emergency fee on the probability that an emergency visit is for urgent conditions. Results: The percentage of emergency department visits in urgent or equivalent patients increased by 2.4% points, from 74.2% beforeto 76.6% after the policy implementation. In a group of patients transferred using public transport or ambulance, who were assumedto be least conscious of cost, the change in the proportion of urgent patients was not statistically significant. On the other hand, the probability that a group of patients directly presenting to the emergency department by private transport, assumed to be most conscious of cost, showed a 2.4% point increase in urgent conditions (p<0.001). This trend appeared to be consistent across the level 1 and level 2 emergency medical institutes.         Co / Cu / Co / FeMn 스핀밸브의 자기저항 특성 향상 연구

        It was observed that exchange bias field was increased with smooth surface and better γ-FeMn formation. Sputtering conditions were varied for the control of the surface roughness and γ-FeMn formation. From the results of Cu deposition as underlayer, it was found that γ-FeMn formation was closely related with the thickness of underlayer. After heat treatment, exchange bias field was increased over three times. This improvement was likely that the crystallites of γ-FeMn were well formed. In Co/Cu/Co/FeMn spin valve structure, magnetoresistance was increased over 1.4 times through the heat treatment. This was due to the disappearance of Co/ Cu intermixed dead layer and removal of defect, and this was examined by AES analysis.
