Pleuropulmonary blastoma (PPB) is an unusual intrathoracic blastoma presenting in childhood and charac-terized by a biphasic neoplastic population of undifferentiated, small round blastemal cells and larger spindle-shaped sarcomatous cells with entrapped benign epithelial-lined structures. We experienced the cytologic features of PPB in imprint smear from the pleural-based huge mass of the middle lobe of the right lung in a 4-year-old boy. The smears showed high cellularity composed of small ovoid blastemal elements and scattered spindle mesenchymal tumor cells. Lobectomy and pathologic investigation confirmed the diagnosis. PPB seems to be a tumor in which accurate diagnosis may be achieved by cytoiogy if appro-priate clinical information were given. Timely and accurate diagnosis of PPB by cytology paves the way for attempting preoperative treatment in future cases.

Sodium iodide symporter (NIS) plays a key role in thyroid hormone production by efficiently accumulating iodide from the circulating blood into the thyocytes, and this is done against an electrochemical gradient. Thyroid transcription factor-1 (TTF-1) is a homeodomain-containing protein expressed in embryonic diencephalons, thyroid, and lung and has been found to bind to thyroid specific promoters and to activate their transcriptional activity. TTF-1 may be one of the factors capable of activating NIS gene expression in the thyroid gland, thus it accounts for the lower levels of NIS gene expression that are seen in the extrathyroidal tissues. However, a high frequency of TTF-1 expression has been observed, especially in primary lung adenocarcinoma. The present study was undertaken in order to elucidate the relationship between the expression of NIS and TTF-1 in primary lung adenocarcinoma. Immunohistochemical studies for NIS and TTF-1 were performed in 64 primary lung adenocarcinomas. Immunoreactivities for NIS and TTF-1 were found in 49 (76.6%) and 45 (70.3%) out of 64 cases, respectively. Forty-one (83.7%) of the 49 cases with positive NIS immunoreactivity showed positive TTF-1 expression, whereas 11 (73.3%) of the 15 cases with negative NIS immunoreactivity showed negative TTF-1 expression (P<0.05). So the NIS expression was significantly associated with the TTF-1 expression. These findings suggest that TTF-1 may be one of the factors capable of activating NIS gene expression in primary lung adenocarcinoma. Further studies are needed to define the relation between NIS and TTF-1 for examining the mechanisms of tissue-specific NIS expression.
수술 절제를 시행받은 제1기 비소세포폐암 환자에서의 Fascin 발현과 예후

연구배경: Fascin은 세포 운동에 관여하는 액틴 결합 단백질로서 정상적인 상피세포에는 증가되어 있지 않으며, 일부 악성종양에서 fascin이 증가되어 있다는 보고가 있다. 본 연구는 비소세포폐암 환자의 조직에서 fascin 발현을 조사하고 fascin이 예후 인자로 역할을 하는지를 알아보고자 하였다. 방법: 제 1기 비소세포폐암으로 근치적 절제수술을 받고 추적조사가 가능했던 환자 81명의 조직에서 fascin 발현을 면역조직화학 염색 방법으로 조사하였다. 결과: Fasin 발현은 전체 81예 중 59예(73%)에서 양성이었다. Fascin 발현 정도에 따른 5년 생존율은 fascin 발현 음성군에서 68%, fascin 저발현군에서 76%, fascin고발 현군에서 79%으로 각 군간에 유의한 차이가 없었다(p=0.86). 결론: Fascin 발현이 비소세포폐암으로 근치적 수술을 받은 환자에서 예후 인자로서 역할을 하는지 알아보았으나 통계학적으로 유의한 관련성이 없었다. Background: Fascin is an actin-bundling protein that plays an important role in cellular motility. Fascin is normally expressed in the neuronal and mesenchymal cells and its expression is low or absent in the epithelia. However, an overexpression of fascin has been linked to the invasive behavior of some neoplasms such as breast, stomach and ovarian tumors. In this study, we evaluated the expression of fascin and its prognostic significance in stage I non-small cell lung cancer (NSCLC). Methods: Immunohistochemical staining for fascin was performed on the paraffin-embeded tissue sections of 81 cases of resected NSCLC. Staining of more than 5% of the tumor cells was recorded as positive immunoreactivity. Results: Fascin expression was seen in 73% (59/81) of the cases and this was more frequently seen in squamous cell carcinoma than in adenocarcinoma (93% vs 42%). There were no significant correlations of fascin immunoreactivity with tumor recurrence and overall survival. Conclusion: The expression rate of fascin was relatively high in NSCLC, but this was without prognostic significance. The exact clinical role of fascin should be defined through further investigations. (Tuberc Respir Dis 2008;65: 105-109)

Objectives : The purpose of this study is to evaluate the association of the angiotensin converting enzyme (ACE) insertion/deletion (I/D) polymorphism with cardiovascular disease risk factors. Methods : Out of a total of 608 middle-aged adults who visited local health centers, 424 subjects (104 male, 320 female) who had not been diagnosed with hypertension, diabetes mellitus, or hyperlipidemia were included in this study. ACE genotypes were determined in all subjects by polymerase chain reaction methods. Results : Statistical differences in high-density lipoprotein (HDL) cholesterol levels according to ACE genotype were observed using ANOVA (p<0.05), but no differences were found in other cardiovascular risk factors. Specifically, men with the DD and DI genotypes had significantly lower HDL cholesterol levels than those with the 11 genotype based on the LSD multi-comparison test (p<0.05). Conclusions : In men, the D-allele of the ACE I/D polymorphism was significantly associated with reduced HDL cholesterol levels. In the future, larger studies are needed to confirm this relationship between ACE I/D polymorphism and HDL cholesterol.

위상피이형성과 위암종에서 FasL의 발현 및 Apoptosis에 관한 연구

Purpose: This study was to observe whether the apoptotic function of tumor-infiltrating lymphocytes (TIL) is induced in human gastric epithelial dysplasia and gastric adenocarcinoma according to the role of FasL expression. Materials and Methods: A total of 56 gastric epithelial dysplasia and gastric adenocarcinoma patients were enrolled in this study: 9 cases of gastric epithelial dysplasia, 18 cases of early gastric carcinomas (EGC) and 29 cases of advanced gastric carcinomas (AGC). Immunohistochemical staining was performed for FasL and CD45, and the terminal deoxynucleotidyl transferase mediated dUTP nick end labelling (TUNEL) method was used to detect cell death in tumor-infiltrating lymphocytes. Results: 1) Positive reactions of FasL to neoplastic cells were $88.9\%$ (8/9) in gastric epithelial dysplasia, $83.3\%$ (15/18) in EGC, and $75.9\%$ (22/29) in AGC. 2) Expression of TIL was decreased in the FasL positive region and was increased in the FasL negative region, and significant expression of TIL was observed in the AGC group (P=0.001). 3) Expression of apoptotic TIL was very similar to the FasL expression, and $100\%$ expression was observed in gastric epithelial dysplasia group. 4) Expression of apoptotic TIL was increased in the FasL positive region and decreased in the FasL negative region, and significant apoptotic expression was observed in the gastric epithelial dysplasia and EGC groups (P=0.0420, P=0.0263, respectively). Conclusion: These results suggest that FasL is a prevalent mediator of immune privilege in epithelial dysplasia and cancer of the stomach.

Purpose: The cDNA microarray provides a powerful alternative with an unprecedented view in monitoring geneexpression levels and leads to discoveries of regulatory pathways involved in complicated biological processes. Our aim is to explore the different gene-expression patterns in gastric adenocarcinomas. Materials and Methods: By using a cDNA microarray representing 4,600 cDNA clusters, we studied the expression profiling in 10 paired gastric adenocarcinoma samples and in adjacent noncancerous gastric tissues from the same patients. Alterations in the gene-expression levels were confirmed by Vsing Northern blots and reverse-transcription PCR (RT-PCR) in all of 4 randomly selected genes. Results: Genes those were expressed differently in cancer ous and noncancerous tissues were identified. 44 (of which 26 were known) and 92 (of which 43 were known) genes or cDNA were up- and down-regulated, respectively, in more than $80\%$ of the gastric adenocarcinoma samples. In cancer ous tissues, genes related to gene/protein expression, cellcycle regulation, and metabolism were mostly up-regulated whereas genes related to the oncogene/tumor suppressor gene, cell structure/motility, and immunology were mostly down-regulated. The semi-quantitative RT-PCR results for the four genes we tested were consistent with the array findings. Conclusions: These results provide not only a new molecular basis for understanding the biological properties of gastric adenocarcinomas but also a useful resource for future development of therapeutic targets and diagnostic markers for gastric adenocarcinomas.
한국인의 비소세포폐암종에서 O⁶-methylguanine-DNA methyltransferase (MGMT)의 발현도 분석

본 연구에서는 손상된 DNA를 수복하는 중요한 효소로 알려진 O?-methylguanine-DNA methyltransferase (MGMT) 발현의 의미를 비소세포폐암종에서 면역조직화학 염색법으로 알아보고자 하였다. 동아대학교 의료원에서 2001년부터 2004년까지 외과적으로 적출한 폐암종 조직 중 비소세포암 종으로 진단된 74예를 연구대상으로 하였다. 면역염색 결과, MGMT 발현은 총 74예 중 49예(66.2%)에서 양성을 보였으며, 25예(33.8%)에서 단백 소실을 보였다. 조직학적 유형에 따른 결과를 살펴보면, 편평세포암종은 8/39예(20.5%)에서 단백 소실이 보였고, 샘암종은 17/35예(48.6%)에서 단백 소실이 관찰되어 통계적으로 유의한 차이가 관찰되었다(p=0.021). 하지만 나이, 성별, 흡연유무, 종양 크기, T 병기 및 림프절 전이에 따른 유의한 차이는 관찰되지 않았다(p>0.05). MGMT 단백 발현 소실은 특히 promoter 메틸화와 연관되어 종양에서 관찰된다고 알려져 있으므로, 향후 연구에서는 비소세포폐암종의 MGMT 단백 소실에 대한 임상적 의의를 밝히기 위하여 promoter 메틸화 연구가 추가적으로 수행되어져야 될 것으로 사료된다. O?-methylguanine-DNA methyltransferase (MGMT) is a DNA repair protein that protects cells against the carcinogenic effects of alkylating agents. The loss of MGMT expression was commonly known due to hypermethylation of CpG islands in its promoter region. We evaluated the expression of MGMT by immunohistochemistry in order to examine the relationship between loss of MGMT expression and clinicopathological characteristics in 74 Korean patients with non-small cell lung cancers. Loss of MGMT was detected in 25 (33.8%) of 74 cases. The loss of MGMT expression was frequently seen in the adenocarcinoma than in the squamous cell carcinoma (p=0.021). However, there was no significant differences between loss of MGMT expression and other clinicopathological characteristics, including age, gender, smoking status, tumor size, tumor T stage, and lymph node metastasis (p>0.05). In conclusion, loss of MGMT expression was related with the histologic type of lung cancer. Further methylation study of MGMT promoter is needed to evaluate the relationships with immunohistochemical expression of MGMT and to clarify the role of MGMT in lung cancer.
