증례 : 수년에 걸쳐 진행한 폐결절을 가진 불명열 환자에서의 항암화학요법 1예

저자들은 수년에 걸쳐 진행한 폐결절을 가진 불명열 환자에서 조직학적으로 규명되지는 않았으나 EB 바이러스 관련 림프세포증식질환을 의심하여 항암화학요법을 시행한 후 관해를 이룬 예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Classic causes of fever of unknown origin (FUO) are infections, neoplasms, collagen vascular diseases and drugs. However, large studies of FUO have consistently reported that a considerable proportion of patients with FUO remain undiagnosed. We report a patient with a prolonged course of FUO and progressive pulmonary nodules in whom extensive clinical and laboratory evaluation did not reveal specific diagnosis. The patient had recurrent episodes of fever for more than three years, and the pulmonary nodules gradually progressed in size and number despite administration of antibiotics including antifungal and antituberculosis agents. Several pathologic examinations demonstrated an organizing pneumonia or non-neoplastic lung parenchyma with lymphoplasmacytic infiltration only. After a protracted course of disease, we thought that this patient might have lymphoproliferative disorder, possibly EBV-associated, and started anti-cancer chemotherapy. He was successfully treated with eight cycles of chemotherapy including cyclophosphamide, doxorubicin, vincristine and prednisolone (CHOP).(Korean J Med 71:442-448, 2006)

유아의 동기 유형: 유아의 기질 및 어머니의 상호작용과의 관련성

This research was based on achievement goal theory which categorizes young children`s motivation into three types: mastery, the performance-approach, and performance-avoidance motivation. The participants consisted of forty-eight pairs of children aged 3-4 and their mothers. The participants completed two tasks in order to indentify their motivational types. The short form of the Child Behaviour Questionnaire(CBQ-SF), modified scales from Kim and Kim(2001), and the GIPEPS were also employed. The major findings of the research are as follows: First, in terms of the motivational types of the given sample, performance-approach motivation was ranked first followed by mastery motivation. Second, it was found that the temperament of young children was not related with their motivational types. Third, there were some differences in mother-child interaction between groups based on the frequency of feedback, questions, and the percentage of commands. Fourth, the mood of the relationship appeared to be the only factor of maternal interaction quality which related to children`s mastery motivation.

온라인 방송콘텐츠 유통 중개업자의 핵심 역량과 유통구조 개선효과에 관한 사례

디지털 기술과 정보통신망의 발달로 인하여 다매체-다채널이 등장함에 따라 방송콘텐츠시장의 유통경로가 더욱 다원화 되고 있다. 본 연구에서는 온라인 방송콘텐츠 다운로드 시장을 중심으로 온라인 방송콘텐츠시장의 문제점을 해결하는데 중요한 역할을 담당하고 있는 콘텐츠 유통 중개업자(Master Contents Provider, MCP)의 핵심 역량차원과 유통구조개선 효과간의 관계를 사례연구를 통해 규명하고자 하였다. MCP의 핵심 역량은 자원기반이론(Resource_based theory)을 조사하여 물적, 재무, 인적, 기술, 무형 및 조직자원으로 도출하였으며, 6개 자원중심으로 MCP의 핵심 역량에 대한 사례조사를 수행하였다. 사례연구 결과, 온라인 방송콘텐츠 다운로드 시장의 문제점은 불법거래 및 저작권 침해, 지상파 방송사업자와 웹하드 서비스업체 간의 직접거래로 인한 거래비용증가, 방송콘텐츠 서비스 품질 하락, 온라인 방송콘텐츠 다운로드 시장의 파편화 등으로 규명되었다. MCP의 물적, 재무, 기술, 인적, 무형 및 조직자원이 결합된 MCP의 핵심 역량이 온라인 방송콘텐츠 다운로드 시장의 문제점을 해결하여 유통시장의 효율성 및 투명성을 제고하고, 유통구조를 통합하며, 나아가 지상파 방송사업자와 웹하드 서비스업체의 매출 증대에 크게 기여하고 있는 것으로 나타났다. Wired and mobile Internet has led to increase in online broadcasting content market size. In particular, the fast-growing smart devices like smart phone and tablet PC in mobile Internet market has accelerated the growth of online broadcasting content service market. Meanwhile, the illegal distribution of online broadcasting content has been widespread in this market. There are also significant transaction costs, search costs, contracting and coordination costs in online broadcasting content market. MCP(Master content provider) which is a content distributor has been playing a critical role in preventing illegal content distribution, reducing costs and removing inefficiencies of online broadcasting content market. Thus, the purpose of this study is to investigate the competency of MCP to streamline the online broadcasting contents market from the resource- based view. And this study conducted a case study to explain the status of online broadcasting content market and define what kinds of problems and issues are there in the market in a systematic way. The case study also showed how MCP competency plays an important role in reducing administrative and transaction costs and in solving illegal content distribution and other inefficiencies of the online broadcasting contents market.

지각된 대기시간 경험에 따른 고객감정반응이 서비스품질만족과 수용가능성에 미치는 영향 -미용서비스를 중심으로-

This study aims to explore the effect of customer emotion according to delayed waiting time on service quality satisfaction and acceptability. Data collection was done against customers of beauty parlors and total of 286 questionnaires were analyzed using SPSS 20.0 to conduct frequency analysis, factor analysis, reliability analysis, and multiple regression analysis. As for the demographic result, unmarried women over 30 to below 40 years old accounted for the majority. Dimensions on experience of delayed waiting time were deduced as five factors and customer emotional response as two. When average value and correlation between waiting time experience and customer emotional response were looked at, the factor ‘occurrence of loss’ from delayed waiting time dimension showed to have strongest positive effect on negative customer acceptability. Such result can be explained by the fact that most of research targets were professionals, adding to more negative emotion as prolonged delay time can cause damage to their business. ‘Delay understanding’ type, which is service provider’s guidance and consideration, showed to have highest positive effect on the positive acceptability type. Such result is probably because customers are loyal due to service provider’s skill. However, customer’s temporal loss or unconditional waiting results negative emotion no matter what. As shown, delayed waiting time acts as parameter to negative and positive emotions on acceptability. Therefore hopefully a strategy will be built to manage delay time to lower negative emotion and improving service quality evaluation.

웰니스가 미용종사자의 주관적 행복감에 미치는 영향에 관한 연구

Beauty treatment industry is a service area that is highly dependent on human factor, which can influence business performance by commercializing service quality and technique provided beauty treatment professions. Therefore, this research aimed at suggesting basic material to develop wellness program that can achieve positive business performance through enhancing service quality by having beauty treatment professions get satisfied with their life and job and provide high quality service to customers through researching the effect of wellness factors on beauty treatment professions`` subjective feeling of happiness. To achieve the research purpose, this research utilized total 1,080 questionnaires for final analysis targeting beauty treatment professions in Seoul, and conducted frequency analysis, factorial analysis, reliability analysis, and multiple regression analysis on collected data utilizing SPSS 16.0. As the result, first, wellness was drawn with 11 levels, and subjective feeling of happiness was drawn with 5 levels, second, subjective feeling of happiness, ‘the others comparative accomplishment satisfaction’ factor had highest affirmative(+) factor, while ‘self-blame’ factor had negative(-) influence when tendency of ‘positive self-development’ factor is high among wellness factors, third, subjective feeling of happiness, ‘negative anxiety’ factor, had highest affirmative(+) influence, while ‘positive thinking’ factor had negative(-) influence when tendency of ‘negative expression’ factor was high among wellness factors, fourth, subjective feeling of happiness, ‘ideal satisfaction’ factor had highest affirmative (+) influence, while ‘self-blame’ factor had negative(-) influence when tendency of ‘positive self-development’ factor was high among wellness factors, fifth, subjective feeling of happiness, ‘aging pressure’ factor had highest affirmative(+) factor, while ‘positive thinking’ factor had negative(-) factor when tendency of ‘negative expression’ factor was high among wellness factors, fifth, subject feeling of happiness, ‘aging pressure’ factor had highest affirmative(+) influence, while ‘positive thinking’ factor had negative(-) influence when tendency of ‘negative expression’ factor was high among wellness factors, lastly, subjective feeling of happiness, ‘old age satisfaction’ factor had highest affirmative(+) influence, while ‘regular food control’ factor had negative(-) influence when tendency of ‘positive value’ factor was high among wellness factors. In conclusion, the researcher wishes that companies recognize and develop necessity and importance of wellness program for beauty treatment professions to get high quality service and positive business performance for the customers, also, beauty treatment professions could promote healthy and satisfactory composure of life for themselves and the future, also, expects researches and efforts to be continued to enhance recognition and feeling of happiness of beauty treatment professions in various aspects.

계획된 행위이론에 근거한 대학생의 헌혈의도 및 헌혈행위 예측요인

본 연구는 Ajzen의 계획된 행위이론(TPB)에 근거하여 대학생의 헌혈의도와 헌혈행위에 영향을 미치는 요인을 파악 하는 데 목적이 있다. 연구대상은 G시에 위치한 일개 대학 대학생 258명으로 하였으며, 자료 수집은 2013년 6월 3일부터 14일까지의 기간에 구조화된 자기기입식 설문조사로 하였다. 자료 분석은 SPSS/WIN 19.0을 이용하였고, 대학생의 헌혈의도 에 영향을 미치는 요인은 단계별 중회귀 분석을, 헌혈행위에 영향을 미치는 요인은 로지스틱 회귀분석을 시행하였다. 대학생 의 헌혈의도에 영향을 미치는 요인을 확인한 결과 헌혈에 대한 지각된 행위통제, 헌혈에 대한 태도, 헌혈에 대한 주관적 규범 순이었으며, 65.6%의 영향력이 있었다. 또한, 헌혈행위에 영향을 미치는 요인을 확인한 결과 헌혈의도(OR=1.129, p=.010)만이 유의하였으며, 헌혈에 대한 지각된 행위통제는 헌혈행위에 유의한 영향을 미치지 않은 것으로 나타났다. 따라서 추후 대학생 의 헌혈의도를 증가시키기 위한 프로그램에서 지각된 행위통제, 태도 및 주관적 규범을 강화하는 것은 헌혈의도를 높이고 이로써 헌혈행위를 증가시킬 수 있을 것으로 본다. This study examined the factors affecting blood donation intention and behavior in college students based on the theory of planned behavior. The subjects were 258 students from one college in G city, and the data was collected by a survey using self-administered questionnaires structured from June 3 to June 14, 2013. The data was analyzed using the SPSS 19.0 program, the variables affecting the blood donation intention were analyzed by stepwise multiple regression, and the variables affecting the blood donation behavior were analyzed by Logistic regression. The factors affecting the blood donation intention were in order the perceived behavioral control, attitude toward the behavior and subjective norm, and the explanatory power was 65.6%. In addition, the factor affecting the blood donation behavior was only the blood donation intention (OR=1.129, p=.010) except for perceived behavioral control. Therefore, the perceived behavioral control, attitude and subjective norm should be considered when developing blood donation programs for college students.

자폐스펙트럼장애 아동의 사회인지 프로그램 개발을 위한 예비연구: 언어재활사의 인식을 중심으로

본 연구는 ASD 아동의 사회적 의사소통 능력을 향상시키기 위한 조기 개입 및 교육 지원에 있어서 효과적인 교수프로그램을 모색하고자 계획된 예비연구이다. 이를 위해 사회적 기능 손상으로 나타나는 문제를 사회성 발달에 관여하는 사회인지 개념으로 보고 언어재활사가 생각하는 사회인지 중요 요소와 그에 따른 치료 만족도, 사회인지 구성요소 간 중요도와의 상관관계를 살펴보았다. 먼저 사회인지 하위 구성요소를 중심으로 필수 선행능력 2개 항목이 포함된 총 12개 항목을 설문 문항으로 최종 선정하고 5점 리커트 척도로 제작했다. 설문 응답을 통해 사회인지 중요도와 치료 만족도의 평균값을 비교하고 사회인지 구성요소 간 중요도와의 상관관계를 분석하였다. 분석 결과 사회인지 하위 구성항목 간 중요도와 치료 만족도에서 ‘공동주의 능력’에서는 일치를, 나머지 11개 항목은 불일치를 보였으며 이러한 결과는 대다수 언어재활사들이 ASD 아동의 사회성 지도에 어려움을 보이고 있음을 시사한다. 사회인지 구성요소 간 중요도와의 상관관계는 모든 항목에서 유기적인 정적 상관을 보였다. 본 연구 결과는 ASD 아동의 성공적인 사회적 의사소통을 위해서는 발달 초기에 나타나는 사회적 참조와 공동주의 능력이 체계적으로 개입되어야 함을 시사한다. 또한 사회인지와 필수 선행능력 등에 대한 더 많은 연구로 ASD 아동의 사회통합을 지원할 증거기반 교수법(Evidence-based practices)들이 개발되어져야 할 것이다. This study is a preliminary study designed to seek effective teaching programs in supporting early intervention and education to improve ASD children's social communication skills. To this end, we looked at the social functional impairment problem as a society involved in social development and looked at the relationship between the important factors and the importance of treatment satisfaction and social recognition components. First, a total of 12 items, including two essential pre-executives, were selected as questionnaires based on the social awareness subcomponents and produced on a five-point recursive scale. The survey responses compared the average values of social awareness importance and treatment satisfaction and analyzed the correlation between social recognition components. The analysis showed a consensus in the 'ability of communalism' in importance and treatment satisfaction among sub-components of society and the remaining 11 items showed inconsistencies, suggesting that the majority of language rehabilitation workers were having difficulty guiding social skills of ASD children. The correlation between social and component importance showed organic static correlation in all categories. The results of this study suggest that successful social communication of ASD children requires systematic involvement in the social reference and communalism capabilities that appear in the early stages of development. In addition, evidence-based teaching methods to support social integration of ASD children will need to be developed with more research on social recognition and essential prior ability.

조기영어교육이 7세 아동의 모국어(한국어) 나레이티브에 미치는 영향: 복잡성, 정확성, 유창성 및 어휘사용양상을 중심으로

This study examined the influence of early English education on children’s narratives in L1 Korean. An oral story-telling task was conducted in Korean using a textless picture book “Frog Story” with twenty 7-year-old graduates of English-only kindergarten and another twenty regular Korean kindergarten graduates of the same age. The narrative data from the task were compared between the two groups in the aspects of complexity, accuracy, fluency and vocabulary use. The results revealed that there were no significant differences in fluency, complexity between the two groups. However, a significant difference was found in accuracy; the English-only kindergarten graduates spoke with more errors than the regular Korean kindergarten graduates. In addition, there was a significant difference in the use of foreign words between the two groups, but not in the use of Sino-Korean words. The English-only kindergarten graduates used more foreign words than regular kindergarten graduates. These resultsindicate that negative effects of early L2 English education on children’s L1 narrative is only limited to accuracy, but not to overall aspects of speaking, providing useful information based on empirical study for elementary school language teachers as well as parents of the children. 한국과 중국 어머니의 자녀양육관 및 교육가치관과 조기 사교육 이용 실태 비교
