        기분안정기 1형 양극성장애 환자의 심상 특성 및 정서 조절

        Objectives : We investigated characteristics of mental imagery and its relation to emotion regulation in euthymic patients with bipolar I disorder using self-report measurements. Methods : Fifty euthymic bipolar I patients and 50 healthy control subjects participated. We applied Korean version of Difficulties in Emotion Regulation Scale (KDERS) to assess emotion regulation difficulties. Spontaneous Use of Imagery Scale (SUIS) and Vividness of Visual Imagery Questionnaire (VVIQ) were conducted to assess tendency towards general use and vividness of imagery, respectively. Results : Patients with bipolar I disorder showed significantly lower scores in VVIQ (t=-2.043, p=0.044), but not in SUIS, than healthy participants. Bipolar patients also showed significantly higher scores in KDERS than healthy controls (t=2.091, p=0.039). However, there was no association of the ratings of VVIQ or SUIS with K-DERS, and also no significant relations between characteristics of mental imagery and mood symptom scores. Conclusion : These findings suggest that bipolar patients maintain the high level of vividness in mental imagery despite euthymic mood. However, we couldn’t find any relationships between emotion regulation and mental imagery. More delicate measures, such as specific imagery or emotion regulation tasks, will be needed to demonstrate the relationship of two aspects.

        Clinical Validity of Neuropsychological Assessment in Dementia: A Univariate and Multivariate Methodological Comparison

        Previous studies have documented validity evidence of neuropsychological measures in the assessment of dementia. However, known cognitive test measures were mostly validated as separate measurements rather than asconstituents of a whole battery. In this study, the neuropsychological battery (CERAD-K) and neuropsychiatric measures were acquired in older adults with Mild Cognitive Impairment, dementia of Alzheimer’s Disease (AD), and Vascular dementia (VD). The assessment measures and demographic information were used to predict two validity criteria: dementia severity (CDR) and dementia type (AD or VD). A correlation between a single test measure and the target criteria indicated univariate validity, whereas relative impor- tance among multiple regression models indicated the multivariate validity of a single measure as a constituent of the battery. We identified that test measures including the Boston Naming Test, Trail Making Test, and Word List Recall were predictive of the clinical outcome criteria as univariate validity; however, this strength of association did not remain consistent when evalu- ated in terms of multivariate validity. Regarding the multivariate validity, measures including Word List recognition, and neu- ropsychiatric impairment showed robust validity. This contrasting validity indices between univariate and multivariate frame- works may be owing to shared information between other measures, which can distort the conclusions of validity evidence. The findings suggest that the validity of a neuropsychological test differs as a function of the target criteria and whether ad- ministered as a whole battery.         알츠하이머병에서 혈장 크레아틴과 염증반응, 인지기능과의 연관성

        Objective: Creatine, energy buffer in high energy demanding systems including muscle and brain, may play a beneficial role against neuroinflammation in Alzheimer’s disease (AD), and thus be a potential biomarker. This study aimed to compare the levels of plasma creatine between persons with and without AD and investigate associations of plasma creatine levels with cognitive function and blood-based inflammatory markers. Methods: We classified elderly participants by cognitive statuses: normal cognition (NC, n=17), mild cognitive impairment (MCI, n=21), and AD (n=21). To assess cognitive function and inflammatory condition, we performed neuropsychological tests and measured plasma C-reactive protein (CRP) levels, respectively. Results: Plasma creatine levels were comparable among participants with AD, MCI, and NC. In overall participants, plasma creatine levels were not associated with neuropsychological test scores, but negatively associated with plasma CRP levels. In AD group, plasma creatine levels were negatively associated with neuropsychological test scores and, although not significant, CRP levels (p=0.086). In participants without AD (NC plus MCI), these associations disappeared. Conclusion: Plasma creatine levels may not be useful as a biomarker indicating cognitive statuses. However, our results suggest that, in AD, plasma levels of creatine might reflect the extent of neuroinflammation as well as cognitive deterioration.
