There are diverse ring-like narrowings in the lower esophagus: mucosal ring, muscular ring, ring-like annular peptic stricture. Esophageal muscular ring is extremely rare and generally asymptomatic. It occurs at the proximal border of the esophageal vestibule that corresponds to the upper end of the lower esophageal sphincter. We experienced a case of lower esophageal muscular ring with a symptom of intermittent mild dysphagia for 40 years and report the findings with a review of the literature.(Korean J Gastroenterol 2003;42:539-543)
대장내시경 용종절제술 후 지연성 출혈의 위험인자: 환자-대조군 연구

Background/Aims: Colonoscopic polypectomy is a valuable procedure for preventing colorectal cancer, but is not without complications. Delayed bleeding after colonoscopic polypectomy is a rare, but serious complication. The aim of this study was to identify risk factors of delayed bleeding after colonoscopic polypectomy. Methods: A retrospective case-control study was conducted in a single university hospital. Forty cases and 120 controls were included. Data collected included comorbidity, use of antiplatelet agents, size and number of resected polyps, histology and gross morphology of resected polyps, endoscopist`s experience, resection method, use of sedation, and use of prophylactic hemostasis. Results: In univariate analysis, size, histology and number of resected polyps, endoscopist`s experience, resection method and use of prophylactic hemostasis were significant risk factors for delayed bleeding after colonoscopic polypectomy. In multivariate analysis, risk of delayed bleeding increased by 11.6% for every 1 mm increase in resected polyp diameter (OR, 1.116; 95% CI 1.041-1.198; p=0.002). Number of resected polyps (OR, 1.364; 95% CI, 1.113-1.671; p=0.003) and endoscopist`s experience (OR, 6.301; 95% CI, 2.022-19.637; p=0.002) were significant risk factors for delayed bleeding after colonoscopic polypectomy. Conclusions: Size and numbers of resected polyps, and endoscopist`s experience were independent risk factors for delayed bleeding after colonoscopic polypectomy. More caution would be necessary when removing polyps with these factors. (Korean J Gastroenterol 2012;59:423-427)

Carcinoid tumors show variety of pathological features and some of them are admixed with adenocarcinoma. The carcinoma-carcinoid spectrum is a concept of classifying tumors based on the tumor mass differentiation which is composed of tissues of both endocrine and nonendocrine functions. When two types of tissues exist within one tumor intermingled with each other in a similar proportion, it is called composite tumor. On the other hand, collision tumor is characterized by the presence of two localized tissue types adjacently together. Gastric composite tumors are relatively rare. According to the reports on the Korean literature, there are several collision tumors, but only one case of gastric composite tumor has been cited. Reports of multiple synchronous or metachronous cancers have increased steadily during the last decades. Multiple gastric carcinoids or carcinoid tumors developed in association with gastric adenocarcinoma contribute to this trend. We report one case of gastric composite tumor simultaneously occurring with a early gastric adenocarcinoma with review of the literature. (Korean J Gastroenterol 2003;42:533-538)

Lymphoepithelial cysts of the pancreas are rare but distinctive cystic lesions characterized by the presence of a mature, squamous epithelial lining and a dense lymphoid infiltration. To our knowledge, more than forty cases have been reported in English-written papers. Our patient was a 69-year-old male with a history of intermittent abdominal pain and loss of body weight for one year. The results of physical examination and laboratory studies were within normal range. Endoscopic ultrasonography of the pancreas revealed a 2.2 cm sized well-circumscribed, inhomogeneous hypoechoic mass with anechoic areas on the surface of the head of pancreas. Postoperative histopathologic diagnosis was a lymphoepithelial cyst of the pancreas. The histogenesis of lymphoepithelial cysts is not fully disclosed. However, they are benign and can be cured by local excision. Thus, they should be differentiated from mucinous cystic neoplasm of the pancreas. This case is reported herein with a review of the relevant literature. (Korean J Gastroenterol 2002;40:410-413)
