        가토피판에서 지연조작의 시간적 길이와 혈류량에관한 연구

        If a skin flap is raised in two or more stages, its blood supply is better than that of the flap raised in one stage. This surgical delay procedure is one of the proven method for augumentation of the skin flap survival. Clinical use of delayed skin flap has been decreased in the past few years. However, the delay phenomenon still remains as an important subject in flap research. In order to study the relationship between the length of delay and the amount of blood flow, authors made delayed flaps on the rabbit dorsal skin, and measured flap blood flow with non-invasive Laser Doppler flowmetry. The sixty white New Zealand rabbits were divided into 6 groups. Delay procedure was done in 2, 3, 4, 6 , 8 and 14 days respectively according to each group, and acute flaps(control) were made on the same rabbit at the day of the end of delay. Skin blood flow was increased 6 hours after flap raising. The results obtained were as follow: 1. The rapid increase of the blood flow (over200%) in the random skin flap occurs within the first 3 to 4 days of delay(p<0.05) 2. The effective length of delay is between 6 to 8 days of delay. 3. This study seems that the delay phenomenon does not depend on neovascularization, angiogenesis, A-V shunt closure. 4. We suggest that the locally released neurohumoral substance may play a role in skin flap viability and manifestation of the delay phenomenon.

        국내 비내진 설계 철근콘크리트 아파트에 대한 지진피해 예측 연구

        우리나라는 비교적 안정된 판 내부에 존재한다. 하지만, 1976년 안전지대로 평가된 중국 탕산지역의 대규모 지진이 발생한 것처럼 우리나라도 지진에 대해 안전한 지역이라고 단언할 수는 없다. 게다가, 지진학자들도 국내에서 중규모 이상의 지진발생 가능성이 높다고 지적하고 있다. 이러한, 지진은 자체를 예방할 수 없기 때문에 내진설계에 대한 연구와 지진재해 관리체계에 대한 연구가 전반적으로 이루어지고 있다. 그러나, 지진발생 시 초기 대응이나 수습과정에 대한 연구는 미비한 상태이며, 지진피해 평가시스템 구축을 위한 극소수의 연구가 진행되고 있는 실정이다. 따라서, 본 연구는 국내실정을 고려한 지진피해 평가시스템 구축을 위한 기초자료를 제시함을 목적으로 한다. 이를 위하여 강남구 비내진 철근콘크리트 아파트를 표준형 건축물로 선정하여 지진피해를 예측하고, 취약도 함수의 도출과 층간변위에 따른 피해평가를 행하여 HAZUS 프로그램 취약도 함수 적용결과와 비교 분석한다. Korea is located away from plate boundaries which are not safe from earthquakes. However, having witnessed the large-scale earthquake in the Tangshan region in 1976 deemed as a safe plate, it should not be assured that Korea is absolutely safe from earthquakes. In addition, many seismologists have claimed that there indeed is a high possibility of earthquakes above mid scale that would occur in Korea. Because it is impossible to prevent earthquake, studies on seismic design and earthquake disaster control system are widely being conducted. However, studies on early response to earthquakes or recovery process are still very limited, and only a few studies for establishing earthquake damage evaluation system are being conducted. Thus, this study aimed to present essential data for establishing earthquake damage evaluation system that takes into account the real situation of structures in Korea. In this study, a nonseimically reinforced concrete apartment structure in Gangnamgu was selected as an standard type of such structures and its earthquake damage was estimated. The result of damage evaluation based on the derivation of vulnerability function and realtive story displacement was compared to that abtained using HAZUS Program Vulnerability Function.

        지연조작이 가토 피부피판의 경피산소분압에 미치는 영향에 관한 실험적 연구

        Delayed flaps have been known as a good method to improve circulation and viability of skin flaps and these are frequently used in the field of reconstructive surgery. Tissue oxygen levels have been monitored in skin flaps by various means and have been shown to reflect circulation. Transcutaneous PO₂might reflect circulation in certain skin sites, assuming that a normal arterial PO₂exists. The purpose of this study is to investigate whether delayed flaps improve circulation of skin flap or not, by observing the change of transcutaneous PO₂and viability of skin flaps. In this study, with a modified Clark-type oxygen electrode, transcutaneous PO₂ were measured on the delayed flaps and undelayed flaps in rabbits. We compared the mean transcutaneous PO₂value and mean flap survival rate of delay flaps with those of undelayed flaps. The results were as follows; 1. The mean transcutaneous PO₂of preoperative normal skin of rabbits was 56.15 ± 6.56 mmHg. 2. The mean transcutaneous PO₂of laps at postoperative 1st,3rd and 5th days was 23.00 ± 4.56, 25.40 ± 5.46 mmHg and 28.65 ± 4.97 mmHg respectively in undelayed group, and 28.10 ± 3.49 mmHg, 33.95 ± 5.19 mmHg and 36.90 ± 4.61 mmHg respectively in delayed group. 3. The mean transcutaneous PO₂of flaps at postoperative 1st,3rd and 5th days in comparison to preoperative normal skin of the same animal (% of mormal) was 48.84 ± 4.78%, 45.10 ± 6.48%, and 50.93 ± 4.58% respectively in undelayed group, and 50.09 ±2.59%, 60.28 ± 3.39% and 65.85 ± 5.26% respectively in delayed group. The mean transcutaneous PO₂of delayed group was about 28.5% higher than that of undelayed group.(P<0.01) 4. The mean survival rate of flaps on postoperative 5th day was 44.40 ±3.75% in undelayed group, and 55.80 ±5.35% in delayed group.(P<0.01).

        The Seismic capacity evaluation of RC apartment buildings was worked about the maximum imagination earthquake occuring across the Metropolitan area. The buildings were designed before establishing the internal standard of seismic design. The evaluation of damage amount was showed by cheching over a story drift and maximum deformations about buildings after working about a linear equivalent static analysis and time history analysis. The result showed that RC apartment buildings without seismic design had a mostly weak structure performance about a large earthquake supposed to occur. As a result, a collapse possibility of building was supposed that a earthquake disaster accompanied with a large human disaster might occur.

      • 비내진 설계된 철근콘크리트 아파트 건축물의 지진피해에 대한 연구

        수도권 일대에서 발생할 수 있는 최대 가상 지진에 대한 국내 내진 설계 기준 설정 이전에 설계된 철근콘크리트 아파트 건축물의 내진 성능 평가를 수행하였다. 피해 정도의 평가는 표준형 건축물의 선형 등가정적해석과 시간이력해석을 하여 건축물의 층간 변위 및 최대 변위를 검토함으로써 보여진다. 그 결과 비내진 설계된 철근콘크리트 아파트 건축물들은 발생이 추정되는 대규모 지진에 대해서 극히 취약한 구조 성능을 가지고 있는 것으로 나타나고 있고, 이로 인한 구조물 붕괴 가능성이 구조형식이나 규모에 상관없이 상당히 높아 엄청난 인명 피해를 동반한 진재가 발생할 것으로 추정된다.         가금에서 정맥튜브 및 근막을 이용한 굴곡건초 재건에 관한 실험적 연구

        Tendon sheath is an important structure in nutrition, healing and gliding motion of the tendon. To know the effect of tendon sheath restoration with fasica and vein graft after tendon surgery, experimental surgery was done in chickens. Middle toe profundus tendon was severed and repaired. After tendon repair, sheath was treated in three ways. In group A, tendon sheath of 1cm length was simply excised. Then, sheath was excised and restored with femoral rectus muscle fascia in group B and vein conduit graft in group C. After casting in flexion for 14 days, the operated foot and digits were taken through a full passive range of motion every 3 days. At the 30th postoperative day, all immobilization was removed and free active range of motion began. Active flexion was measured after death by continuous traction on the proximal produndus tendon(simulated active flexion). The non-operated contralateral foot served as the control. All operated middle toes achieved significant greater functional return when the sheath was restored either with fascia patch or vein conduit graft in comparison with simple excision of the sheath. The histologic examination showed that area of healing was better organized with less adhesion when the sheath defect was grafted. The results indicated the possibility of utilizing fascia patch or vein conduit during primary tendon repairs as well as the efficacy of sheath closure.

        한국연안 해조류 생물량의 연간 변동 양상: 제주도 문섬지역

        Seaweed biomass was estimated using a nondestructive method in the rocky subtidal zones in Munseom, Jeju Island, Korea from July 2006 to April 2008. Seasonal samplings were done at the depth of 1, 5, 10 m using 50 x 50 cm quadrat. Mean biomass was comprised of 2,784 g wet wt m$^{-2}$ and the biomass values varied seasonally from 1,176 g wet wt m$^{-2}$ to 4,217 g wet wt m$^{-2}$ with the highest point in April. Biomass reached maximum at 5 m depth in spring, but was shifted to 10 m depth in summer. Common seaweeds appeared year round in Munseom were Codium minus, Ecklonia cava, Sargassum spp., articulated corallines and Plocamium telfairiae. Among them, E. cava showed the highest biomass (average of 1,288 g wet wt m$^{-2}$), comprising 4% of total biomass. Only 12 species’' biomass covered 98% of total value, which indicated the contribution of few common species to algal community. Seaweed biomass in Munseom represented one of the highest values in coastal regions in Korea.

        간세포암 환자에 있어서 AST / ALT 비의 진단적 의의

        N/A To assess the possibility that the measurement in peripheral venous blood of AST/ALT ratios might be helpful in detection of hepatocellular carcinoma (HCC), the AST/ALT ratios were evaluated in 282 cases of chronic active hepatitis (CAH), 59 cases of CAH with early cirrhotic change (CAH with LC), 213 cases of liver cirrhosis (LC), and 220 cases of hepatocellular carcinoma (HCC). The ratio of male to female was 72.9% to 27.1% in HBsAg seropositive groups and 68.8% to 3I.2% in HBsAg seronegative groups, respectively. The mean age was 43.7 years old in HBsAg seropositivity and 49.9 years old in HBsAg seonegativity, respectively. In HBsAg seropositive groups, the AST/ALT ratio was 0.68+-0.30 in CAH group, 0.78+-0.19 in CAH with LC, 1.68+-0 83 in LC, 2.17+- l.60 in HCC. In HBsAg seronegative groups, AST/ALT ratio was 0.71+-0.58 in CAH group. 0.82+-0.31 in CAH with LC, 1.72+-0.87 in LC, 1.95+-1.38 in HCC. AST/ALT ratio was gradually increased according to the progress of the chronic liver disease in both HBsAg seropositive groups as well as negative groups. AST/ALT ratio was significantly (p<0.005) elevated in patients with HCC, being above 2.0, as compared to other groups. AST/ALT ratio in the patients with serum AST>200 units was higher than in those with AST<200 units, and the mean AST/ALT ratio in the HCC patients with AST.> 200 units was significantly higher than in other chronic liver disease. AST/ALT ratio above 3.3, without evidence of circulatory disturbance, was almost entirely restricted to the patients with HCC. In conclusion, since elevated AST/ALT ratio reflects the progress of chronic liver disease from CAH to HCC, a gradual increase in AST/ALT ratio might be a useful marker assessing for detection of HCC.
