N/A Objectives: Cardiogenic shock resulting from acute myocardial infarction is a serious complication with high mortality. The early identification of patients at high risk of developing post-infarction cardiogenic shock might allow early intervention in an attempt to prevent cardiogenic shock and to reduce the mortality due to cardiogenic shock. The aim of the present study was to examine the risk factors of inhospital development of cardiogenic shock among patients with acute myocardial infarction. Methods: We studied 152patients with acute myocardial infarction who were admitted to Kang-Nam St. Mary's hospital within 24hours after the onset of chest pain and did not have cardiogenic shack on admission between March 1991 and May 1994. Clinical data of these patients were analyzed. Results: Of 152patients, 17(11.1%) developed cardiogenic shock during their hospital stay. Cardiogenic shock developed in 53% of cases more than 24hours after admission. 82.4% of patients with cardiogenic shock died whereas a 6.7% in-hospital mortality was found among patients without cardiogenic shock. Multivariate regression analysis that controlled for variables affecting incidence of postinfarction cardiogenic shock showed that independent risk factors for in-hospital cardiogenic shock were history of myocardial infarction (adjusted relative odds[RO]=5.294, 95% confidence interval[CI]=2.149 to 13.041); heart failure on admission (RO=3.344, 95% CI=1.738 to 6.432); hyperglycemia (>180mg/dl) in non-diabetic patients (RO=3.270, 95% CI=1,590 to 6.727); age over 70 year old (RO=2.912, 95% CI= 1.816 to 4.668); ST deviation over 4mm (RO=2.417, 95% CI=1.225 to 4.767); peak LDH level greater than 1600U/ml (RO=1.154, 95% CI=1.080 to 1.233). Patients with one independent risk factor had an estimated probability of 10.5% for developing inhospital cardiogenic shock; patients with two independent risk factors, 48.5%, patients with three risk factors, 65.0% patients with four risk factors, 65.7% patients with five risk factors, 67.2%. Conclusion: Of post-infarction cardiogenic shock during admission, 53% developed more than 24hours after admission. The more independent risk factors on admission for inhospital cardiogenic shock patients with acute myocardial infarction had, the more likely in-hospital cardiogenic shock developed.

The rail clamp is the device to prevent that a crane slips along rails due to the wind blast as well as to locate the crane in the set position for loading and unloading containers. The wedge typed rail clamp should be designed to consider the structural stability and the durability because it compresses both rail side with large clamping force by the wedge working as the wind speed increases. In this research, the design of experiments(DOE) and the variation technology(VT) built in ANSYS WORKBENCH are utilized to determine the optimum shape of a jaw. The optimum results obtained by two methods are compared and examined. 컨테이너 터미널에서 컨테이너의 양 하역 작업 시 컨테이너 크레인을 정위치에 고정시키고, 돌풍으로 인해 컨테이너 크레인이 레일방향으로 미끄러지는 것을 방지하는 장치가 레일클램프이다. 쐐기형 레일클램프는 초기에는 작은 압착력으로 레일을 압착하다가 풍속이 증가하면 쐐기작용에 의해 압착력이 증가하는 방식을 취함으로서 구조적으로 안정성과 내구성이 높은 장점을 가지고 있다. 본연구에서는 레일클램프의 주요부인 조에 대해 형상최적설계를 수행하였다. 본 논문에서는 솔리드 요소로 유한요소 모델링된 조(jaw)의 경량화 설계를 위하여 강도를 고려하였다. 설계변수로는 조의 측면부의 두께, 조의 중간부의 롤러지지부의 두께, 조의 하단부의 롤러지지부의 두께, 조의 곡면부의 위치로 설정하였다. 본 연구에서는 상용프래그램인 ANSYS WORKBENCH의 최적화 기능을 이용하였다.

컨테이너의 양.하역 작업 시 크레인을 정 위치에 고정시키고, 돌풍으로 인해 크레인이 레일방향으로 미끄러지는 것을 방지하는 장치가 레일클램프이다. 쐐기형 레일클램프는 풍속이 증가하면 쐐기작용에 의해 압착력이 증가하므로 구조적으로 안정성과 내구성이 있게 설계해야한다. 따라서 본 연구에서는 순차적 표본방법을 기반으로 하는 크리깅 모델을 이용하여 조 형상최적설계를 수행하였다. 크리깅은 표본점의 반응치를 기초로 내삽법에 의해 임의의 점에서의 반응치를 예측하는 근사기법의 하나이다. 이것은 최적설계 분야에서 자주 사용되는 근사기법인 반응표면법에 비해 반응치의 보다 정확한 예측이 가능하며 특히 비선형성이 강한 함수의 예측에 적합하다고 알려져 있다. 순차적 크리깅 모댈에 의하여 구해진 최적해를 상용프로그램을 이용하여 구한 최적해와 비교하고 제안된 방법의 유용성을 검토하였다. In this research, the kriging interpolation method using sequential is utilized to find the optimum shape of the jaw in the rail clamp. The suggested method predicts more accurate response value than the response surface method. The optimum results obtained by the proposal method are compared with those by the commercial software.
Terfenadine과 Ketoconazole 복용 후 발생된 Torsades de pointes 심실 빈맥증의 치험 1례

Torsades de pointes is a form of polymorphic ventricular tachycardia that is associated with prolongation of the QT interval, characterized by QRS complexes of progressively changing amplitude and contour that seem to revolve around the isoelectric line. The most common cause of Torsades de pointes induced by antiarrhythmic agents, occupying in certain drugs or toxin, electrolyte disturbance, congenital QT prolongation syndrome. Cardiotoxicity terfenadine, in overdose a widely used, nonsedating antihistamine, has been reported. Case after administration of terfenadine and ketoconazole causing syncope and torsades de pointes have been reported in recommended dose. We report a case of QT prolongation with torsades de pointes after therapeutic administration of terfenadine, a long acting histamine H1 receptor antagonist, together with ketoconazole, an antifungal preparation. The patient is a 43-year-old female, who experienced palpitation, dizziness and syncopal episoders after coadministration of terfenadine and ketoconazole, for a fungal infection of the skin. The ECG showing typical episodes of torsades de pointes with QT prolongation. Magnesium sulfate was given, torsades de pointes did not recur. The patient was discharged with improved condition.
LTCC적층형 대역통과필터의 공정 및 특성 최적화 연구

Multi-layered ceramic chip bandpass filter for 2.4 GHz ISM band was fabricated by using LTCC (Low Temperature Co-fired Ceramics) technology, and related processing parameters such as lamination condition, sintering temperature, time and electrode condition were investigated in terms of filters characteristics. The BPF consists of coupled stripline resonators and coupling capacitors. The sintering shrinkages was affected substantially by lamination pressure and sintering temperature. And insertion loss with inner conductor thickness has been discussed and realized. Insertion losses in the passband were less than 2 dB and the attenuation more than 25 dB at 2.1 GHz. The frequency response characteristics of the chip BPF is well acceptable for bluetooth application.
