        건선 환자에서 Secukinumab 투여 중 초발한 궤양성 대장염 1예

        A 30-year-old male with an 8-year history of plaque-type psoriasis visited our dermatologic clinic. His past medical and family history were non-contributory to the chief complaint. Since 30 weeks of oral systemic therapy provided an inadequate response to symptoms, secukinumab administration was started. Four weeks after secukinumab injection, the patient complained of abdominal pain and diarrhea, followed gradually by bloody stool and severe abdominal pain. A colonoscopic biopsy was performed, which showed chronic active inflammation with cryptitis and crypt abscess. Ulcerative colitis (UC) was suspected to be associated with secukinumab; hence, we changed the drug to guselkumab. Twelve weeks after treatment with guselkumab, UC-associated symptoms markedly improved. In this report, we present a case of UC following secukinumab treatment for psoriasis. There have been several reports of the association between Interleukin-17 inhibitors and inflammatory bowel disease (IBD); however, clear mechanisms and causality remain unestablished. Thus, based on available evidence, the possibility of new-onset or exacerbation of underlying IBD should not be overlooked when selecting a biologic treatment for psoriatic patients.

        측두하악관절의 관절염 진단에 있어서 골스캔과 단광자방출 전산화 단층촬영의 유용성에 관한 연구

        Temporomandibular joint arthritides is divided into osteoarthritis, osteoarthrosis and polyarthritis. Because the signs, symptoms and radiographical features of osteoarthritis and osteoarthrosis are similar without arthralgia, diffenential diagnosis is difficult. Also non-radiographically change in early Osteoarthritis leads to misdiagnose. Planar bone scan and SPECT are useful to detect bone change early. This study was carried out in order to make diagnostic criteria of planar bone scan and SPECT. Three hundred and four temporomandibular joints were examined with clinical examination, computerized tomograph, planar bone scan, and SPECT. The obtained results were as follows. 1. If temporomandibular joint simple uptake ratio of patient in twenties is over 1.397%, it's condition may be osteoarthritis. And simple uptake ratio over in thirties-fourties may mean osteoarthritis. 2. It may mean osteoarthritis of temporomandibular joint that the number of coronal and transverse SPECT frame with hot spot is over four. 3. Destructive stage may goes on, if simple uptake ratio is over 1.370% in tweenties and over 1.104% in thirties-fourties. 4. If the number of coronal SPECT frame with hot spot is over four, temporomandibular joint may be on destructive stage in tweenties, thirties-fourties. And if the number of transverse SPECT frame with hot spot is over three, it may be on destructive stage in all ages. 5. When patient complains subjective arthralgia and palpation arthralgia, bone change may be more active than each arthralgia. 6. Osteoarthritis may progress gradually worse in 4.5 anamnesis. And then it may be stable gradually and turn to osteoarthrosis.

      • 바다방석고둥의 동결저장중의 품질의 변화

        To investigate the physical, chemical. and histological quality changes during frozen Omphalius pfeifferi capenteri, was stored under the temperatures of -18℃, -25℃, and -30℃ immediately after shelling. The results are as follows : 1. Generally. pH was increased but a little change was observed at -30℃. Amount of free drip was increased with higher frozen temperaure and longer frozen storage period. 2. Water holding capacity was almost as constant at -30% as immediately after frozen, but the longer storage period goes the lower water holding capacity was observed. 3. The extractibility and composition of muscle protein, sarcoplasmic protein and stroma protein were rather stable regardless of frozen temperature and frozen storage period however, extractibility of myofibrillar protein was decreased with higher frozen temperature and longer frozen storage period. 4. On the changes of muscle tissue structure, following points were observed. 1) In the muscle tissue structure of fresh sample, fine muscle fiber was closely distributed all over the tissue regardless of cross and longitudinal section. 2) Immediately after freezing, tiny ice crystals were in extracellular part regardless of different frozen temperature. these were restored satisfactorily into the muscle tissue after thawed. 3) In tissue structure under frozen state, it was observed that ice crystals apparently grew with the higher storage temperature. Empty spaces between muscle bundles which were formed by aggregation of muscle fiber were observed after 3 months storage at -18%. 4) Tissue structure in thawed state was restored satisfactorily after 1 month storage regardless of storage temperature. After 3 months storage at -3O℃. muscle tissue was well restored, but at -18℃, empty spaces were appeared due to imperfected restoration.

      • 절리 암반물성의 크기효과 및 그 적용에 관한 연구

        본 논문은 절리 암반물성의 크기효과가 암반의 불균질성 및 불연속성에 기인한다고 가정하였다. 이를 위해 대표체적요소의 개념이 적용되는 일반적인 등가물성이론을 탈피하여 불규칙적인 절리기하와 임의의 절리수 및 임의의 절리 방향성을 고려할 수 있는 등가물성이론에 대해서 연구하였다. 이론적인 연구를 바탕으로 이 이론을 실제 문제에 적용한 결과 암반요소의 크기에 따라 달라지는 물성 변화를 관찰하고, 크기효과에 관한 여러 가지 연구를 수행하였다. 특히, 수치해석적인 방법으로 크기효과를 증명하는 과정의 타당성을 입증하고 크기효과의 구체적인 원인을 알아보기 위해서 4개의 모델에 대해서 전산실험을 수행하였다. 이 실험으로부터 증명된 내용을 토대로 실제 3차원 구조물을 대상으로 크기효과 실험을 수행하였다. 이 실험 과정 중 절리의 여러 역학적인 성질들이 암반강도 및 탄성계수의 크기효과에 미치는 영향을 관찰해 보았다. 또 크기효과가 특정 절리구조에서 발생되는 것이 아니고 절리를 포함하는 모든 경우에서 발생됨을 증명하기 위해 절리 구조가 다른 두 모델을 대상으로 크기효과 실험을 수행하였다. This study has the assumption that scale effects in rock mass properties are atrributed to the discontinuous and inhomogeneous nature of rock masses. In order to escape the general equivalent material approach applied to the concept of representative volume element, this study presents the new method considering irregular i oink geometry and arbitrary numbers of i oink and arbitrary joint orientations. Based on the theoretical approach, this theory is applied to a real engineering project. Showing the property variations with size of rock mass element, various numerical experiments about scale effect are conducted. Particularly, to prove the adequacy of the verification process in scale effect with nomerical method, and to investigate the detailed source of scale effect, 4 models with increas ins number of joints are tested. On the basis of the experimental results, the test results of scale effects in 3-D rock mass are presented. From these experiments the effects of the mechanical properties of rock joints on the scale effects in rock mass strength and elastic constants are discussed. To verify the mechanism of scale effects in jointed rock mass, two models with different j oink geometries are studied.

      • 스포츠美學의 基礎硏究 : 藝術性을 中心으로

        In has been said the discipline of aesthetics involves the theoretical study of the fine arts and of aesthetic experience related to the arts. In the past 15years physical educators and others have been discussing aesthet ic issues in relationship to sport and human movement. It is clear that the answer to the question “In sport art?” presupposes some kind definition of both sport and art. However, in the last 25years there have been dramatic changes in aesthetic the orizing as the fundamental enterprise of defining art under attack by philosophers influenced by the ideas of Wittgenstein. This paper has two major purpose. One is to examine the question “Is sport art?,” to look at the presuppositions and implications of the question, and to cla rify some of the issues. A second major purposr is to look at some of the tradit ional assumptionsregarding a project of definition and to suggest hou one might attemt to answer the question. In is not a purpose of this paper to provide a claer answer. In conclusion, an attemt is made to present a way of “defining” art and sport so as to answering the question “Is sport art?”
