        A Study on Disability and Impairment of Dental Disorders in Korea

        The definition and application of disability are becoming more important as society becomesmore complex. The precise criteria for the assessment and classification of disabilities are primarilyfocused on certain medical conditions with a lack of guidelines for dental conditions. This study provides an evaluation and comparison of the current tables available for the assessmentof disabilities in the dental field with the aim of identifying areas to be improved.

        Neurophysiological Mechanisms Underlying Functional Knee Instability Following an Anterior Cruciate Ligament Injury

        PURPOSE: Functional knee instability, which is defined as repetitive episodes of the knee “giving way” during physical activity, has received great attention to identify mechanisms due to serious pathological complications. Growing evidence suggests that insufficient neural adaptation in the central nervous system (CNS) may result in permanent functional deficits in patients with anterior cruciate ligament (ACL) injuries. The purpose of this review was to address neurophysiological mechanisms underlying functional joint instability following an anterior cruciate ligament rupture. METHODS: Previous studies conducted from PubMed with particular emphasis on mechanisms underlying joint instability and neuromuscular control deficits after an anterior cruciate ligament were reviewed. RESULTS: Inappropriate neuromuscular control, inconsistent correlations between joint laxity and functional outcomes, and altered neural activation in the brain during proprioceptive tasks may underscore the idea that persistent functional joint instability is an indication not only of the peripheral deafferentation input, but also neuromechanical decoupling between the injured ACL and CNS due to neuroplasticity. CONCLUSIONS: Persistent functional instability can develop following knee joint injury due to altered neural processing in the CNS. Therefore, it must be considered for improving patient outcomes, minimizing functional disability, and returning to one’s chosen physical activity in ACL patients.

        측두하악장애 재발환자의 임상소견과 치료에 관한 연구

        TMDs are known as the major etiology of non-odontogenic pain in orofacial area and subtype of skeletomuscular disorders. Treatment goals of TMDs which are similar to other orthopedic disorders or rheumatic disorders are pain relief, reducing stress, rehabilitation of function, and going back to normal life. Several literature had showed that these routine conservative treatments relieved TMD symptoms and made TMD patients go back to their normal life. However patients often revisited with recurrent symptoms. The conservative treatments which were given at their first visit were routinely applied to these recurrent patients. Therefore this study might be a help in clinical approach for TMD patients by comparing clinical findings and treatment of recurrent TMD patients with initial TMD patients. Subjects for the study were the patients who had visited the Department of Oral Medicine, Pusan National University Hospital from 1993 to 2000, diagnosed as TMDs and treated with conservative treatments such as behavior therapy, medication, physical therapy, and splint therapy. Patients were divided into 2 groups; non-recurrent patients(102 patients whose symptoms didn't recur at least for 3 years) and recurrent patients(65 patients whose symptoms recurred and re-treated). The obtained results were as follows; 1. Recurrent rate was higher in patients whose muscles and joints were all affected. 2. Onset duration was longer in recurrent patient. 3. The symptoms of recurrent patients were not as severe as the symptoms at the initial visit. 4. There was no difference between recurrent and non-recurrent patients in treatment modalities and primary and secondary treatments were the same in recurrent patients. 5. Treatment duration and frequency were shorter in recurrent patients and secondary treatment duration was shorter in arthrogenous patients.

        Fear of reinjury matters after ACL injury

        Purpose: Fear of re-injury has been linked to functional joint instability after an anterior cruciate ligament injury. However, it is still unknown how fear and function are linked sequentially in patients with a knee sprain. Therefore, the purpose of this narrative review article was to provide a comprehensive summary of the interrelationships between neuromuscular control, cognition, and emotion, the neural mechanisms underlying cognitive and emotional regulation processes, and potential emotional regulatory training after knee ligamentous injury. Methods: Previous research articles and more relevant research articles through a cited reference emphasizing fear of re-injury, cognition, emotion, neuromuscular control, and anterior cruciate ligament injury searched from PubMed databases were reviewed. Results: Higher fear of re-injury in patients after an anterior cruciate ligament (ACL) injury may be associated with diminished neuromuscular control. As negative emotion requires better cognitive neural processing in the several brain’s regions that are also essential for maintaining dynamic joint stability, increased neural demands as a result of higher fear of re-injury can disrupt neuromuscular control and long-term pathological sequalae. Therefore, enhanced neural efficiency in cognitive control networks through executive function training may help ACL patients to regulate high fear of re-injury and dynamic restraint systems to maintain functional joint stability during intense physical activity. Conclusions: As negative feelings can interfere with the cognitive processing for neuromuscular control, executive function training may improve affective control and muscle coordination. Understanding the interrelationships that exist between neuromuscular control, cognition and emotion help to not only develop better rehabilitation program, but also improve patient outcomes after an anterior cruciate ligament injury.

        Forensic odontology can be defined in many ways. One of definitions is simply that forensic odontology represents the overlap between the dental and the legal professions. The most important function of forensic odontology to determine of human identity from dental character. We had found 7 unidentified human bodies in Busan metro politan city, 2007 and we have identified 5 bodies among them.

        Evidence of Neuroplasticity and Neuroimaging Techniques following Anterior Cruciate Ligament Injury

        PURPOSE: Recent evidence has linked neural changes in the brain, known as neuroplasticity, to anterior cruciate injury, possibly explaining why some patients suffer repetitive functional instability. Therefore, the purpose of this article was to review the manifestation of central nervous system reorganization following anterior cruciate ligament injury and establish further the application of neuroimaging assessment tools in the evaluation of neuroplasticity after ligamentous injury. METHODS: Previous research articles emphasizing neuromuscular deficits, functional joint instability, and anterior cruciate ligament injury and neuroimaging techniques searched from PubMed databases were reviewed. Additionally, more relevant research articles through a cited reference search from the original research articles were reviewed. RESULTS: Diminished neuromuscular control such as proprioception, muscle activation, and joint stiffness regulation appears to be indirect evidence of neural adaptations that may lead to persistent functional joint instability and future pathological complications. Results of direct measurement of the brain’s function using a variety of neuroimaging tools supports altered cortical activation in the brain corresponding to diminished knee function. CONCLUSIONS: Knee functional deficits after an anterior cruciate ligament injury may be associated with neural adaptations in the brain. Neuroimaging techniques that allow for examination of neural activity in the central nervous system help to better understand the neurophysiological mechanisms underlying the link between the brain’s function and functional joint instability, to develop more effective rehabilitation programs, and to improve patient outcomes.

        Forensic odontology in individual identification has been progressing rapidly and its significance has been increasing. But not all individual identification can be done single method solely. Therefore, understanding the advantage and disadvantage of each method is necessary to achieve forensic individual identification properly in each case for it to be applied promptly, economically and precisely. Now We have identified almost 20 case in Busan and Kyeong Nam area in 2008, 2009 and we would like to report some of cases. Finally We will consider role of forensic odontology.
