        안와골절의 외과적 접근에 대한 증례보고

        The Orbital fracture which is often combined with midface fracture can cause decreased visual acuity, limitation of eyeball movement, diplopia, enophthalmos, etc. Traumatic orbital fracture causes change of orbital volume, results in diplopia and enophthalmos thus, accurate repositioning of displaced bone and reconstruction of orbital defect with autogenous materials(cranial, rib, iliac bone, cartilage and fascia lata) or alloplastic materials(gelatin film, polyglactin mesh, methylmethacrylate, Teflon, silicone, Supermid, hydroxyapatite and metal). The key point of reconstruction of orbit is accurate repositioning of displaced orbital floor, lateral, medial orbital wall and sufficient bonegraft in anatomical defect. As this cases, we obtained good results through transconjunctival and coronal approach who were required orbital reconstruction. Also, we accurately diagnosed orbital fracture with C.T., 3D model and good result for orbital reconstruction with sufficient parietal block bone graft during average 16.8 months follow-up.

        Al-Zn-Mg-Cu-Zr 합금의 집합조직 발달과 기계적 특성에 미치는 Sc 첨가의 영향

        The texture and mechanical properties during fabrication processes - extrusion, cold rolling and aging treatment - of plate in the 7050 Al alloys without/with 0.1%Sc were investigated. In both the cross section(CS) and longitudinal section(LS) of the specimen without Sc, after cold rolling, texture of R-fiber was predominant. In the Sc added alloy after cold rolling, {112}<111>(Cu)+weak (123)<634>(S) texture components developed in the CS of the specimen while strong (3-fiber+(110)<001>(Goss) components developed in the LS of the specimen. After solution treatment and aging treatment, the alloy (CS and LS) without Sc had random texture, but by the alloys with the 0.1%Sc, the components of the rolling texture are not changed. After the aging treatment, hardness and tensile strength of the alloy with 0.1%Sc increased about 15% and 17%, respectively, than those without Sc. Tensile strength of the Sc added specimen was the highest along the rolling direction where the <111> texture was strongly developed. On the other hand, in the specimen without Sc, the mechanical properties were not dependent on the direction of the sample. (Received May 3, 2004)

      • Decrease of Both Cofilin and LIM Kinase Phosphorylation in the Skeletal Muscles of Immobilization-induced Atrophy Rats

        <P> [Purpose] Immobilization-induced atrophy is a general phenomenon caused by prolonged muscle disuse associated with orthopaedic conditions. However, changes in the phosphorylation of atrophy-related cofilin and LIM kinases are still poorly understood. In this study, we examined whether or not phosphorylation of cofilin and LIM kinases is altered in the skeletal muscles of rats after 3, 7, 14, and 21 days of cast immobilization. [Methods] We used two-dimensional gel electrophoresis, mass spectrometry, and western blotting to examine protein expression and phosphorylation in atrophied rat gastrocnemius muscles. [Results] The expression of the cofilin was detected in gastrocnemius muscle strips using proteomic analysis. Cast immobilization after 3, 7, 14, and 21 days significantly diminished the phosphorylation of cofilin and LIM kinases. [Conclusion] The present results suggest that cast immobilization-induced atrophy may be in part related to changes in the phosphorylation of cofilin and LIM kinases in rat skeletal muscles.</P>

      • Association Between Alterations in the Serum 25-Hydroxyvitamin D Status During Follow-Up and Breast Cancer Patient Prognosis

        Background: Serum vitamin D status can affect the prognosis of breast cancer patients. Our aim was to determine the association between alterations in the 25-hydroxyvitamin D [25(OH)D] status during follow-up and the prognosis of breast cancer patients. Additionally, we evaluated the association between the 25(OH)D status at the time of diagnosis and the prognosis using a detailed age and stage categorization. Materials and Methods: Four hundred and sixty-nine Korean breast cancer patients were included. We collected patient clinicopathological data, including their serum 25(OH)D concentration at diagnosis and at the annual follow-up until 4 years after diagnosis. The patients were divided according to their 25(OH)D status at diagnosis into a deficient (<20 ng/ml) and a non-deficient (${\geq}20ng/ml$) group. At follow-up, patients were categorized into the four following groups according to 25(OH)D status alterations: persistently deficient, improved, deteriorated and persistently non-deficient. Results: At diagnosis, 118 patients were classified into the deficient group and 351 into the non-deficient group. After a median follow-up period of $85.8{\pm}31.0$ months, the patients with advanced-stage disease or an older age in the non-deficient group showed a significantly better survival compared with the deficient group. Furthermore, at the 1-year follow-up of 25(OH)D status, the persistently non-deficient group and the improved group had better survival compared with the other two groups. Conclusions: Our results suggest that maintaining an optimal 25(OH)D status at diagnosis and during the 1-year follow-up period is important for improving breast cancer patient survival.

        The effects of the elastin polymorphisms on carotid intima-media thickness in women aged 30 - 70

        [Purpose] Elastin is one of the major determinants of arterial distensibility of large blood vessels that forms the principal component of elastic fibers from the media of arteries. However, the association between elastin(ELN) genotype and vascular function is still unclear. [Methods] 120women were recruited from the Saha-gu (Busan, Korea) Community Center. Measurements of body composition and vascular function included carotid intima-media thickness (CIMT), carotid artery luminal diameter (CLD), minimum (diastolic) artery luminal diameter (CLDmin) and maximum (systolic) artery luminal diameter (CLDmax). Genotyping for the ELN (rs 2071307) polymorphism was performed using the TaqMan approach. ELN gene distribution of subjects were in the Hardy-Weinberg equilibrium (p=0.402). [Results] The relative CIMT differed significantly among the ELN genotypes. And not significant differences in CLD and CIMT/CLD ratio, but AA genotype was tended higher than other genotypes (AG and GG). The relative CIMT and CLD min differed significantly between the ELN alleles. And not significant differences in CLD max and CIMT/CLD ratio, but A allele was tended higher than G allele. [Conclusion] These results suggest that ELN gene polymorphism might be used a one of the genetic determinants of vascular disease in both pre- and postmenopausal women.

      • Maintenance Hormone Therapy as a good therapeutic option in metastatic breast cancer patient

        Background: The maintenance hormone therapy (MHT) after cytotoxic chemotherapy is widely used by many clinicians in metastatic breast cancer (MBC) patients with limited evidences. We evaluated the efficacy of MHT and identified those who had benefited the most from it. Methods: MBC patients who had been treated with MHT from 2006 to 2010 in a single institute were retrospectively reviewed. By definition of MHT, the patients should not have progressed during prior chemotherapy. Progression free survival (PFS) was a primary endpoint and predictive factors for PFS were analyzed. Results: 90 patients were reviewed with a median age of 53 (range, 33-70). The median PFS was 13.0 months (range, 1-66). Prolonged PFS was associated with less previous palliative chemotherapy (1 vs 2 vs >2, 16.6 vs 7.4 vs 6.0months, respectively; p=0.006) and less metastatic sites (1 or 2 vs >2, 15.7 vs. 5.5; p=0.03) Multivariate analysis showed that only the number of previous palliative chemotherapy remained as significant variable. (HR 1.59, 95% CI 1.11-2.20; p=0.01) Most patients have not experienced any serious side effects, and only three patients (3.3%) discontinued MHT due to arthralgia, general weakness, and depression. Conclusion: MHT demonstrated considerable efficacy and excellent tolerability in this study. It could be considered as a good therapeutic option in the MBC patients who have not progressed on the cytotoxic chemotherapy. Key Words: Breast cancer, Hormone therapy, maintenance therapy.

        After 10 weeks of Intermittent Exercise Change Serum Lipids, Inflammatory and Physical Activity Quantity of Night Shift Workers

        [Purpose] Regular exercise or physical activity is an important for improving health and body composition. However, increasing disability is linked to decreasing levels of physical activity and lack of exercise or physical activity may contribute to an increased risk of obesity which known to negatively affect disease. Thus, most important maintain exercise or physical activity for health. The purpose of this study was to determine changes serum lipids, inflammatory and physical activity quantity of night shift workers. [Methods] 30 male participants who night shift workers; 15 male are experimental group and 15 male are control group. The experimental group underwent three time 10-min bouts (30 minutes of day), 3day of week, 10 weeks of intermittent exercise. Measurements comprised serum lipids, inflammatory and physical activity quantity. [Results] After intermittent exercise, the TC, LDL-C, HLD-C, IL-6, TNF-α and CRP are statistically significant. Physical activity quantity did not change intervention 10 weeks and follow up 4 weeks. Also did not change physical activity quantity ≥ 3 METs in both groups. [Conclusion] Therefore, these findings suggest that after intermittent exercise may represent an effective intervention strategy for improving serum lipids, inflammatory and keep the physical activity ≥ 3 METs, leading to improved quality of life in night shift workers.

        Transient Fault Current Limiting Characteristics of a Transformer Type SFCL Using an Additional Magnetically Coupled Circuit

        In this paper, a transformer type SFCL (superconducting fault current limiter) using an additional magnetically coupled circuit was suggested. Its transient fault current limiting characteristics, due to the winding direction of additional coupled circuit, were analyzed through fault current limiting tests. The suggested transformer type SFCL was composed of the primary winding, and one secondary winding wound on the same iron core together with an additional magnetically coupled circuit. That circuit consists of the other secondary winding together with the other SC (superconducting) element connected in parallel with its other secondary winding. As one of the effective design parameters to affect the transient fault current of the SFCL, the fault current limiting tests of the suggested SFCL were carried out considering the winding direction of its additional coupled circuit. It was confirmed that, through the analysis on the fault current tests of the SFCL, the quench sequence of two SC elements comprising the suggested SFCL could be adjusted by the winding direction of the additional coupled circuit.
