황구침법(黃丘鍼法)을 이용한 고혈압 환자의 혈압강하 효과에 대한 증례보고

Objectives: The aim of the study was to investigate the antihypertensive effect of Hwang-gu Acupuncture in hypertensive patients. Methods: We measured the blood pressure of the patients who were admitted in the Oriental Medical Clinic of Brother from 25th November 2005 to 27th March 2006. We included the patients only in case of the systolic blood pressure was over 140mmHg or diastolic blood pressure was over 90mmHg, Twenty two patients were treated by Hwang-gu Acupuncture. In order to evaluate the effect of the Hwang-gu Acupuncture, the blood pressure was measured before and after Acupuncture procedure total 4 times, 10times. Results: There were a significant decrease in the systolic blood pressure and significant decrease in the diastolic blood pressure by Hwang-gu Acupuncture 4 times. The effect of Hwang-gu Acupuncture by measurement time on blood pressure were follows: In a systolic blood pressure was gradually deceased significantly from 1st to 4th but diastolic blood pressure was significantly decreased after 2nd and 4th only. Conclusions: These results suggest that Hwang-gu Acupuncture is effective in decreasing the systolic and diastolic blood pressure.

악교정 수술시 교합평면의 차이에 따른 술후 경조직의 안정성에 관한 연구

악안면 부조화를 치료하여 적절한 기능적, 심미적 형태를 얻고자 하는 악교정 수술에 있어 서는 정확한 진단과 치료계획의 수립이 필수적이다. 특히 심한 상 하악의 복합적인 문제를 가지고 있는 경우에 시행되는 양악 수술에서 상악골과 하악골이 삼차원적인 공간에서 동시에 재배치되는데. 상악골의 재배치에 따라 하악골은 전후 좌우 위치가 결정되면서 새로운 교합평면이 만들어진다. 따라서 교합평면은 양악수술에서 중요한 기준이 되어 상악골의 새로운 위치를 결정하게되므로 진단 및 치료계획 수립시에 최적의 기능적, 심미적 결과와 수술 후 안정성을 얻을 수 있게 계획되어져야 한다. 본 연구에서는 양악 수술을 시행받은 골격성 제 III급 부정교합 환자 48명(남자25명 여자23명, 평균 연령 21.5세)을 대상으로 하여 교합평면으 설정에 사용되어지는 몇 가지 진단 기준중, 많이 사용되어지고 있는 Delaire의 구조적, 구성적 분석에 의한 각 개인의 이상적인 교합평면으로 수술되었을 때 경조직의 술후 안정성에 대해서 평가하고자 이상적인 교합평면에 부합되게 수술이 시행된 군(A군-24명)과 이 평면에서 벗어난 군(B군-24명)으로 나누어, 수술 직전(T1)과 수술 직후(T2-평균 4.3일), 수술8개월 이상 경과 후(T3-평균 1년3개월)에 두군의 측모 두부 방사선 사진을 비교하여 다음과 같은 결론을 내렸다. 1. A군과 B군 모두 수술에 의하여 ANS의 수직적 위치보다 PNS의 수직적 위치가 감소되었다. 즉 상하악 복합체가 후상방으로 회전되었다. 2. T3에 T2와 유의차를 보이는 값은 두군 모두에서 수직 계측항목에서 HRP-Me(p<0.01), 각도 계측항목에선 Articular angle(p<0.01), Gonial angle(p<0.01), Mn. plane angle(p<0.05)이 유의차를 보였다. 두 군 모두에서 HRP-Me이 감소하면서 하악평면각은 증가 하였다. 3. A군 B군사이에서 술후 경조직의 안정성에는 유의성 있는 차이가 없었다. 4. 수술에 의한 B와 Pog의 수평 이동량은 수술후 B와 Pog의 수평재발량과 그리고 PNS의 수직 재발량과 통계적으로 유의성 있는 높은 역상관관계를 보였고, 하악평면각의 수술에 의한 변화량과 gonial angle의 술후 변화량과도 통계적으로 유의성 있는 높은 역상관관계를 보였다. In orthognathic surgery to obtain proper functional and esthetic form after skeletal discrepancy treatment, precise diagnosis and treatment plane are essential. Especially in two jaw surgeries that have serious upper and lower jaw problems, maxilla and mandible are arranged in three dimensions. Based on the maxillary rearrangement, mandibular sagittal and transverse positions are determined, and thus new occlusal plane is established. The object of this study is to evaluate the stability of the individual ideal occlusal plane based on the architectural and structural craniofacial analysis of Delaires. The subjects of this study were 48 patients who underwent two jaw surgeries, and they were equally divided into two groups. A and B. A group was operated with ideal occlusal plane and B group was not.Two groups were compared at the preoperative, immediate postoperative (average 4.3days), and long-term postoperative (average 1.3years) lateral cephalometric radiographs. The following results were obtained: 1. ANS was lower than that of PNS for both A and B after the surgery. That is. maxilla and mandible are rotated in posterior and superior direction. 2. Significances were found between T2 and T3 for both A and B are HRP-Me at vertical measurements, articular angle(p<0.01), gonial angle(p<0.01), and Mn. plane angle(p<0.05) at angular measurement. Mn. plane angle is increased at HRP-Me is decreased for both A and B. 3. There is no significance in skeletal stability after the surgery between group A and B. 4. Horizontal movements of B and Pog by surgery have statistically significant inverse correlations with horizontal relapse of B and Pog. and vertical relapse of PNS. as well as Mn. plane angle, and gonial angle after the surgery.

Delftia sp. JK-2로부터 분리된 Catechol 2,3-dioxygenase의 정제와 특성 연구

The purpose of this work was to investigate the purification and characterization of catechol 2,3-dioxygenase isolated from Delftia sp. JK-2, which could utilize aniline as the sole source of carbon, nitrogen and energy. In initial experiments, several characteristics of C2,3O separated with ammonium sulfate precipitation and DEAE-sepharose were investigated. Specific activity of C2,3O was approximately 4.72 unit/㎎. C2,3O demonstrated its enzyme activity to another substrates including catechol and 4-methylcatechol. The optimum temperature of C2,3O was 30℃, and the optimal pH was approximately 8. Metal ions including Ag^(+), Hg^(+), and Cu^(+2) showed inhibitory effect on the activity of C2,3O. Molecular weight of the enzyme was determined to approximately 35 kDa by SDS-PAGE.

에니어그램 집단상담이 대학생의 대인관계능력 및 자아존중감에 미치는 효과

본 연구는 에니어그램 집단상담이 대학생의 대인관계능력과 자아존중감에 미치는 효과를 밝히는데 목적이 있다. 연구의 대상은 부산광역시에 소재한 D대학교 학생들이며 최종적으로 본 연구에 참여한 학생은 22명이다. 이들은 학생상담 센타 주최로 공고된 ‘대인관계 증진을 위한 에니어그램 집단상담’이라는 공고문을 보고 자발적으로 참여한 학생들로 실험집단과 통제집단에 각각 11명씩 무선배치되었다. 본 연구에서 설정한 가설을 검증하기 위해 사용된 측정도구는 집단상담을 실시하기 전 대인관계능력 검사와 자아존중감 검사가 이루어졌고, 집단상담은 매 회 120분, 주 1회로 10회 과정으로 이루어 있다. 집단상담을 실시 후 대인관계능력 검사와 자아존중감 검사가 이루어져 하위요인별로 채점하였다. 또한 에니어그램 집단상담이 대학생의 대인관계능력과 자아존중감에 미치는 효과를 검증하기 위하여 사전․사후 검사에서 얻어진 점수를 근거로 각 가설에 따라 평균, 표준편차를 구하고 집단 간의 평균차 유의성을 알아보기 위하여 t-검증을 실시하였다. 이러한 절차를 통해서 얻어진 결과를 통하여 얻어진 결론은 다음과 같다. 첫째, 에니어그램 집단상담에 참여한 집단은 참여하지 않은 집단에 비하여 대인관계능력에 대한 접수가 유의미하게 나타났으므로 에니어그램 집단상담은 대학생의 대인관계능력을 향상시키는데 효과적임을 알 수 있다. 둘째, 에니어그램 집단상담에 참여한 집단은 참여하지 않은 집단에 비하여 자아존중감의 하위 영역인 일반적 능력에 대한 점수가 유의미하게 나타났으므로 에니어그램 집단상담은 대학생의 자아존중감의 일반적 능력을 향상시키는데 효과적임을 알 수 있다. 셋째, 에니어그램 집단상담에 참여한 집단은 참여하지 않은 집단에 비하여 자아존중감의 하위 영역인 사회적 능력에 대한 접수가 유의미하게 나타났으므로 에니어그램 집단상담은 대학생의 자아존중감의 사회적 능력을 향상시키는데 효과적임을 알 수 있다. 넷째, 에니어그램 집단상담에 참여한 집단은 참여하지 않은 집단에 비하여 자아존중감의 하위 영역인 학교적 능력에 대한 접수가 유의미하게 나타났으므로 에니어그램 집단상담은 대학생의 자아존중감의 학교적 능력을 향상시키는데 효과적임을 알 수 있다. 다섯째, 에니어그램 집단상담에 참여한 집단은 참여하지 않은 집단에 비하여 자아존중감의 하위 영역인 가정적 능력은 통계적으로 유의미한 차이가 없는 것으로 나타났다. 이 점은 자아존중감의 가정적 능력을 향상시키기 위하여 집단상담의 효율적인 방안이 요구된다.

NaF 0.05% 양치액 사용시 고정성 교정창치 장착 환자에서의 Streptococcus mutans 변화에 관한 연구

부정교합의 치료를 위한 고정식 교정장치는 치아 이동에는 매우 효율적이지만 치면 세균막 관리는 어렵게 된다. 이에 따라 치태 및 세균이 증가하고 결과적으론, 치아우식 및 치아표면의 탈회등 부작용이 발생하게 된다. 본 연구에선 고정식 장치로 치료받는 환자에게서 탈회 및 치아우식을 낮추기 위한 방법으로, 0.05%의 NaF와 10% Xylitol이 함유된 양치액의 효과를 평가하고자 하는데 그 목적이 있다. 고정식 교정장치로 치료받는 환자 11세부터 14세 까지(평균연령 12.7세)20명을 대상으로 하여 각 10명씩 실험군과 대조군으로 구분하였다. 실험군에선 0.05%의 NaF와 10% Xylitol이 함유된 양치액을 사용하게 하였으며, 대조군에선 증류수를 주로한 placebo양치액을 사용하게 하였다. 양치액 사용전과 사용 후 3주, 6주, 9주시 치태를 채취하여 MSB배지 및 BHI배지에 배양하여 S.mutans의 수와 비율의 변화를 관찰하여 다음과 같은 결과를 얻었다. 1. 0.05%의 NaF와 10% Xylitol이 함유된 양치액을 사용한 군에서는 S. mutans C.F.U.의 수는 양치액 사용전과 3주(p<0.01), 9주 (p<0.05)에서 통계적 유의차가 있었다. 2. Total C.F.U.에 대한 S. mutans C.F.U.의 비율은 실험군에서 사용전과 3주, 6주, 9주(p<0.01)에서 통계적 유의차가 있었다. 3. 양치액 사용후 일단 감소한 S. mutans수는 그후에 3주, 6주, 9주에서 계속적인 유의성 있는 감소는 없었다. 4. 실험군과 대조군에서의 S. mutans C.F.U.의 가장 현저한 감소의 차이는 양치액 사용 3주째였다. The purpose of this study was to evaluate the effectiveness of gargling solution with 0.05% NaF and 10% Xylitol in orthodontic patients with fixed appliance. The sample consisted of 20 patients who were classified into an experimental group and a control group, 10 patients each. Experimental group was used experimental gargling solution and the control group was used with placebo solution. The change of S. mutans was analysed by culture on MSB and BHI agar plate pre and post 3,6,9 weeks. The results were as follows. 1. There were significant reduction in the number of S. mutans C.F.U. between pre and post 3 weeks(p<0.01), 9 weeks(p<0.05) in experimental group. 2. There were significant reduction in the ratio of S. mutans C.F.U. to total C.F.U. between pre and post 3, 6, 9 weeks(p<0.01) in experimental group. 3. S. mutans, which were reduced until 3 weeks, did not show significant change after 3, 6, 9 weeks. 4. S. mutans were strongly suppressed until 3 weeks after gargling solution with 0.05% NaF and 10% Xylitol.

Pancreatic cystadenoma is very rare, slow growing tumor and generally preoperative differential diagnosis may be difficult. A 43-year-old female was admitted to our hospital with the complaint of abdominal discomfort in the epigastrium for about preceeding 5 years. Ultrasonography and Computed tomography were performed. When endoscopic retrograde cholangio-pancreatogram was performed, displacement of distal pancreatic duct by huge mass was noted and injected dye was drained irregulary into the cavity of mass. Distal pancreatectomy was performed with splenectomy. The mass size of distal pancreas was 7X4cm, and the cyst contained with yellowish thick mucoid fluid. Diagnosis was confirmed the mutinous cystadenoma of the pancreas on histologic examination.
