푸마르산으로부터 숙신산 생물전환을 위한 Enterococcus faecalis RKY1 균체의 포괄담체에 의한 고정화

The Enterococcus faecalis RKY1 cells were immobilized within the polymer matrices such as agar, Ca-alginate, and k -carrageenan in order to investigate bioconversion characteristics of immobilized Enterococcus faecalis RKY1. Agar and k-carrageenan gels were prepared for cubic type, and Ca-alginate gels for bead type. When the seed volume of immobilized cells was adjusted to 10%(v/v), the optimum concentrations of agar, sodium alginate, and k -carrageenan for immo- bilization of Enterococcus faecalis RKY1 cells were 2%(w/v) for all. In this condition, the conversion ratios were 93.6, 96.4, 76.5%, respectively. Immobilized cells entrapped in agar, Ca-alginate, and k -carrageenan have all the optimum pH of 7.0. The optimum temperature of immobilized cells was 40℃ for agar gels, 35℃ for Ca-alginate gels, and 40℃ for k -carrageenan gels. Therefore, Ca-alginate gels were suitable for immobilization of Enterococcus faecalis RKY1 for succinate production.

Until recently, colorectal polyps were classified predominantly as hyperplastic or adenomatous. While adenomatous polyps are well-characterized precursor lesions of adenocarcinomas, hyperplastic polyps have been considered as benign lesion. However, some hyperplastic polyps with serrated morphology of the crypts have been recognized to have distinctive features and these polyps were termed ‘serrated adenomas’. Recent data show that sessile serrated adenomas (SSA) might be the precursors of serrated colonic cancers, underlining the necessity of identifying them. SSA is approximately 3% of all polyps, commonly appears as flat or sessile and yellowish due to mucus production. In the pathogenesis of SSA, progression to high grade dysplasia or early invasive carcinoma may be associated with serrated neoplasia pathway different from adenoma-carcinoma sequence. We report a case with a colon polyp diagnosed as sessile serrated adenoma with high grade dysplasia after endoscopic submucosal dissection.

Objectives:Hypothalamic-Pituitary-Adrenocortical(HPA) system dysregulation plays an important role in the pathophysiology of major depressive disorder. Medically unexplained somatic symptoms comprised the predominant complaints of korean patients with major depressive disorder. It might be related to dysregulation of HPA system and somatic symptoms. The aim of this study was to investigate the association between Hamilton Depression Rating Scale severity and the Plasma Adrenocorticotropic Hormone(ACTH) & Serum Cortisol level in Korean patients with major depressive disorder. Methods:Our study design was prospective. A total 111 hospitalized patients with major depressive disorder (DSM-IV) at Anam Hospital, Korea University Medical Center between Jan 2009 and May 2011 were selected. Hamilton Depression Rating Scale was measured by psychiatrist. Plasma ACTH, Serum Cortisol levels were analyzed at the time of admission. Results:There are significant correlations(p<0.05) between Hamilton Depression Rating Scale of somatic component(HAM-D 11-15 item) and Plasma ACTH and Serum Cortisol level. However, other HAM-D sub items were not significantly correlated with Plasma ACTH and Serum Cortisol level except somatic component. Conclusion:Our Study results suggest that hyperactivity of HPA axis might be correlation with somatic symptoms in korean patients with major depressive disorder.

<B>ABSTRACT</B><P>The physiological functions of two amylolytic enzymes, a maltogenic amylase (MAase) encoded by <I>yvdF</I> and a debranching enzyme (pullulanase) encoded by <I>amyX</I>, in the carbohydrate metabolism of <I>Bacillus subtilis</I> 168 were investigated using <I>yvdF</I>, <I>amyX</I>, and <I>yvdF amyX</I> mutant strains. An immunolocalization study revealed that YvdF was distributed on both sides of the cytoplasmic membrane and in the periplasm during vegetative growth but in the cytoplasm of prespores. Small carbohydrates such as maltoheptaose and β-cyclodextrin (β-CD) taken up by wild-type <I>B. subtilis</I> cells via two distinct transporters, the Mdx and Cyc ABC transporters, respectively, were hydrolyzed immediately to form smaller or linear maltodextrins. On the other hand, the <I>yvdF</I> mutant exhibited limited degradation of the substrates, indicating that, in the wild type, maltodextrins and β-CD were hydrolyzed by MAase while being taken up by the bacterium. With glycogen and branched β-CDs as substrates, pullulanase showed high-level specificity for the hydrolysis of the outer side chains of glycogen with three to five glucosyl residues. To investigate the roles of MAase and pullulanase in glycogen utilization, the following glycogen-overproducing strains were constructed: a <I>glg</I> mutant with a wild-type background, <I>yvdF glg</I> and <I>amyX glg</I> mutants, and a <I>glg</I> mutant with a double mutant (DM) background. The <I>amyX glg</I> and <I>glg</I> DM strains accumulated significantly larger amounts of glycogen than the <I>glg</I> mutant, while the <I>yvdF glg</I> strain accumulated an intermediate amount. Glycogen samples from the <I>amyX glg</I> and <I>glg</I> DM strains exhibited average molecular masses two and three times larger, respectively, than that of glycogen from the <I>glg</I> mutant. The results suggested that glycogen breakdown may be a sequential process that involves pullulanase and MAase, whereby pullulanase hydrolyzes the α-1,6-glycosidic linkage at the branch point to release a linear maltooligosaccharide that is then hydrolyzed into maltose and maltotriose by MAase.</P>

Gastroesophageal reflux disorder (GERD) is the most common esophageal disorder in children. Achalasia occurs less commonly but has similar symptoms to GERD. A nine-year old boy presented with vomiting, heartburn, and nocturnal cough. The esophageal impedance-pH monitor revealed nonacidic GERD (all-refluxate clearance percent time of 20.9%). His symptoms persisted despite medical treatment for GERD, and he was lost to follow up. Four years later, he presented with heartburn, solid-food dysphagia, daily post-prandial vomiting, and failure to thrive. Endoscopy showed a severely dilated esophagus with candidiasis. High-resolution manometry was performed, and he was diagnosed with classic achalasia (also known as type I). His symptoms resolved after two pneumatic dilatation procedures, and his weight and height began to catch up to his peers. Clinicians might consider using high-resolution manometry in children with atypical GERD even after evaluation with an impedance-pH monitor.
정신건강의학과 외래 환자들의 도착지연 관련요인 분석: 일개 대학병원 사례

Objectives: The purpose of this study is to examine factors related to the delayed arrival of appointment time for outpatients of a psychiatric department in an academic medical center in Korea. Methods: This study investigated relationships of delayed arrival with demographic, appointment related, and practice related factors, by analyzing data related to outpatient appointments for about two years from November 1 in 2013 to October 30 in 2014 at a university hospital in Seoul. Results: Among 95,470 final research subjects, 80,871 outpatients arrival in their appointment time, 14,559 outpatients were late, thus, a rate of delayed arrival is 15.29%. As the results of multiple logistic regression analysis, the odds ratios of delayed arrival become greater as they are younger, male , living in the residence, In-cheon and Gyeong-gi area, and their health insurance types are industrial accident compensation insurance and car insurance. Also, the odds ratios of delayed arrival become greater as the appointment time is from 8:30 to 11:00, they appoint in December, January, and February, and temperature gap is over -2 degrees Celsius between the reserved day and average temperature. Similarly, the odds ratio of delayed arrival is higher as return patients, obsessive-compulsive disorder, and practice by doctor B. Conclusions: This study found that various factor influence delayed arrival of appointment time. Medical institutions should find various methods such as focusing continuously on patients who tend to late their arrival time, developing specific appointment system regulating the factors of delayed arrival, and developing guidelines of the patients who tend to late their arrival time.

Purpose: Mycoplasma pneumoniae (MP) is a major cause of community-acquired pneumonia in children. Since 2000, emerging macrolide-resistant MP has been reported. Three epidemics of MP pneumonia have occurred in Korea during the past 10 years: 2006-2007, 2011, and 2015. We investigated the differences in MP pneumonia of each epidemic in terms of clinical, laboratory, and radiologic perspectives. Methods: We retrospectively analyzed 529 medical records of children (1-18 years of age) who were admitted and diagnosed with MP pneumonia at Kangbuk Samsung Hospital during the past 3 epidemic periods. We compared the clinical, laboratory, and radiologic characteristics of MP pneumonia among individual epidemics and between children younger and older than 6 years of age. Results: The mean age of the patients was 5.7 years old, which had increased by each epidemic and showed the highest (6.3 years old) in 2015 compared to previous epidemics. Among 3 epidemics, there were no sex differences. The duration of fever after admission and hospitalization, and the percentage of lobar pneumonia and use of systemic steroids increased significantly in 2015 epidemic. Since 2006, the mean levels of erythrocyte sedimentation rate and lactate dehydrogenase had increased and in 2015 it marked the highest. Children older than 6 years showed a higher proportion of lobar pneumonia and pleural effusion as well as longer duration of fever (before and after admission) and hospitalization days than those younger than 6 years. Conclusion: This study suggests an increasing incidence of refractory MP pneumonia which required a more frequent use of systemic steroids over the past 10 years, and children older than 6 years were found to have more severe pneumonia than those younger than 6 years.
