감염성 질염에 대한 질내 산도측정, Amine 검사 및 현미경 검사의 진단적 가치

1987년 5월 1일부터 9월 31일까지 5개월간 영남대학교의 의과대학 부속병원 산부인과 외래에 내원한 환자 549예를 대상으로 질내 pH 검사, 아민검사, 생리식염수 및 10% KOH습식도말현미경검사를 시행하였다. 질염과 관련된 임상증상 및 소견과 이를 기초로 한 이학적 진단과 비교분석한 결과 다음과 같은 성적을 얻었다. 1. 각 감염성 질염에서의 질산도 측정 및 아민검사와 생리식염수 습식도말검사성적은 세균성 질염 환자군에선 질 pH 5.0이상이 81.0%, 아민검사 양성이 62.8%, 유산균 감소 소견이 77.4%, 백혈구 감소가 70.1%이었으며, 트리코모나스 질염 환자군은 질 pH 5.0이상이 93.3%, 아민검사 음성이 90.0%, 유산균 감소 소견이 77.4%, 백혈구 감소가 70.1%이었으며, 트리코모나스 질염 환자군은 질 pH 5.0이상이 93.3%, 아민검사 음성이 90.0%, 유산균 감소가 90.0%, 백혈구 증가가 93.3%이었다. 진균성 질염 환자군은 질 pH 3.0 또는 4.0이 83.2%, 아민검사 음성이 100%, 정산유산균 소견이 89.7% 및 정상백혈구 소견이 72.4%이었다. 2. 생리식염수 및 10% KOH습식도말검사에 대한 이학적 진단이 정확도는 정상군이 77.6%, 세균성 질염균 26.3%, 진균성 질염군 47.5% 및 트리코모나스 질염군이 70.0%이었다. 질 Ph 측정 및 아민검사에 의한 진단의 정확도는 정상 및 진균성 질염군이 83.2%, 세균성 질염군이 60.6%, 트리코모나스 질염군이 83.3%이었다. 3. 세균성 질염군에서 질 pH 3.0 또는 4.0 및 아민검사 음성을 나타낸 23예의 질상피 세포에 대한 clue cell의 비율은 10.0%이하가 12예, 20.0% 및 30.0%가 5예, 50.0%가 4예, 그리고 50.0% 이상이 2예이었다. 이상의 성적으로 외래에서 손쉽게 시행할 수 있는 질내 산도측정과 아민검사는 각 감염성 질염의 감염여부에 대한 screening과 감별진단에 유용한 것으로 사료되며, 생리식염수 및 10% KOH습식도말검사에 이들 검사를 추가할 경우 진단의 신빙도를 높일 수 있을 뿐만 아니라 배양검사가 요구되는 환자의 선택에 있어서도 가치가 있을 것으로 추정되었다. Vaginitis is one of the most common disease in gynecologic field in recent days. About 90% of these patients suffer from the infection of the vagina caused by Gardnerella vaginalies. Candida, or Trichomonas, either alone or in combination. For the effective diagnosis and management, it is essential to get an accurate identification of the causative agent. Applying simple and easy diagnostic methods such as pH of vaginal discharge, amine test and microscopic examination of wet mount preparation of normal saline and 10% KOH to 549 cases of randomly selected patients visiting. Out-patient Department of Obstetrics and Gynecology of Yeung-nam University Hospital through May 1st to Sept 31st. 1987, the following results were obtained. 1. In the Gardnerella vaginities vaginal pH was more than 5.0 in 81.0% of total cases, positive amine test in 62.8%, decreased lactobacilli in 77.4% & decreased WBC counts in 70.1%. In the Trichomonas vaginitis vaginal pH more than 5.0 was noted in 93.3% of the cases. negative amine test in 90%, decreased lactobacilli in 90% & increased WBC counts in 93.3%, whereas Candida vaginitis and normal groups showed vaginal pH of 3.0~4.0 in 83.2%, negative amine test in 100%, normal distribution of lactobacilli in 89.7%, normal WBC counts in 72.4%. 2. The accuracy rates of physical diagnosis by wet mount preparation of normal saline and 10% KOH revealed 26.3%, 47.5%, 70.0% in Gardnerella vaginitis, Candida vaginitis, and Trichomonas vaginitis, respectively. The vagnal pH and amine test showed 83.2% of accuracy rate in Candida vagnities group. 60.6% in Gardnerella vagnitis group and 83.3% in Trichomonas vagnitis group. 3. In 23 cases of Gardnerella vagnitis showing vaginal pH of 3.0 or 4.0 and positive amine test, the clue cells were observed in 10% or less in 12 case 30% in 5 cases, 50% in 4 cases and 50% or more in 2 cases. In summary vaginal pH an amine test could be useful in screening and differentiating the different types of infectious vaginitis. Furthermore by adding microscopic examination of normal saline and 10% KOH wet mount preparation to them, they not only could obtain higher diagnostic accuracy rate but would be more valuable in selecting the patients requiring cultures to confirm the diagnoses.

The purpose of this study is to develop models and instruments for teaching and assessing scientific experiments and practices. General objectives for teaching and evaluating scientific experiments and practices in secondary schools were analyzed and categorized into three domains:they were inquiry skill, scientific thinking skill, and scientific attitude. The sub-categories for each domain were constructed, and specific objectives for each general objective were also stated in terms of behavioral objectives according to Mager's suggestion. The tables of specification (content×behavior) were developed in order to assist secondary school teachers to teach and evaluate scientific experiments in class. In addition, we introduced some useful assessment techniques which can be adopted to evaluating learning outcomes of each specific objective. The sample test items corresponding to each objective were listed for practical usage in class.

The social and the physical environment, especially residential environment as a primary living space, have great influence on children's behavior development. So it is necessary to compare the differences according to children's residential environments. The purpose of this study is intended to find the influence of physical and social dwelling environments due to the difference between a separate dwelling and an apartment of the characteristics on children's behavior development. The subject of this study was 471 children(separate dwelling=297, apartment=174). The data of this study was obtained by questionnaire that administered to the sample.

N/A After the historical discovery of a soluble vasodilating substance from the vascular endothelium (endothelium-derived relaxing factor, EDRF), impaired EDRF-mediated vasorelaxation is discussed as one of the main pathogenic mechanisms of coronary artery spasm. Recently, some clinical reports that hypomagnesemia may be implicated in the induction of coronary artery spasm were presented, although there are controversial reports on the effects of the extracellular magnesium ion on endothelium-dependent vasorelaxation. Isometric contraction-relaxation was recorded in the transverse strips of porcine coronary artery to clarify the exact effects of Mg2+ on tension-development by a vasoconstrictor agent and on endothelium-dependent vasorelaxation and to predict a possible mechanism of hypomagnesemia in the induction of coronary artery spasm. The results of our study were as follows: 1) Vasoconstrictor activity of PGF2α was significantly more attenuated in the endothelium-intact transverse strips of porcine coronary artery than in the endothelium-denuded strips. 2) Lowering the magnesium concentration enhanced the vasoconstrictor activity of PGF2α while a marked reduction of isometric tension was observed with the elevation of magnesium concentration. 3) Bradykinin-induced endothelium-dependent vasorelaxation is a dose-dependent manner which was completely abolished with the pretreatment of hemoglobin. 4) Endothelium-dependent vasorelaxation of bradykinin was markedly inhibited by lowering the extracellular calcium ion concentration. 5) Change of the extracellular magnesium ion concentration did not alter the IC50 (inhibitory concentration of bradykinin causing a 50% inhibition of the maximum contraction) values of bradykinin-induced endothelium dependent vasorelaxation. From these results it is concluded that if magnesium deficiency is implicated in the induction of coronary artery spasm, the main mechanism would be direct enhancement of responsiveness of the vascular smooth muscle to the vasoconstrictors. Further study with a bioassay system and more various stimulators of EDRF secretion is necessary for exact clarification of the effects of Mg2+ on endothelium-dependent vasorelaxation.
Neurotensin Enhances Gastric Motility in Antral Circular Muscle Strip of Guinea-pig

<P> Many reports suggest that neurotensin (NT) in the gastrointestinal tract may play a possible role as a neurotransmitter, a circulating hormone, or a modulator of motor activity. NT exerts various actions in the intestine; it produces contractile and relaxant responses in intestinal smooth muscle. This study was designed to investigate the effect of NT on motility of antral circular muscle strips in guinea-pig stomach. To assess the role of Ca<SUP>2</SUP> influx in underlying mechanism, slow waves were simultaneously recorded with spontaneous contractions using conventional intracellular microelectrode technique. At the concentration of 10<SUP>7</SUP> M, where NT showed maximum response, NT enhanced the magnitude (863⁑198%, mean⁑SEM, n=13) and the frequency (154⁑10.3%, n=11) of spontaneous contractions. NT evoked a slight hyperpolarization of membrane potential, tall and steep slow waves with abortive spikes (278⁑50%, n=4). These effects were not affected by atropine (2μM), guanethidine (2μM) and tetrodotoxin (0.2μM). NT-induced contractile responses were abolished in Ca<SUP>2</SUP>-free solution and reduced greatly to near abolition by 10μM of verapamil or 0.2 mM of CdCl<SUB>2</SUB>. Verapamil attenuated the effects of NT on frequency and amplitude of the slow waves. Taken together, these results indicate that NT enhances contractility in guinea-pig gastric antral circular muscle and Ca<SUP>2</SUP> influx through the voltage-operated Ca<SUP>2</SUP> channel appears to play an important role in the NT-induced contractile mechanism.
방사성동위원소표지 Liposome의 분포에 대한 실험적 연구

N/A Liposome was labelled with Tc-99m after negative charged liposome was formed with combination of a few lipid components. Tc-99m liposome was injected through the tail vein of C3H mice bearing fibrosarcoma and biodistribution of Tc-99m liposome was evaluated. The results were as follows: 1) We confirmed formation of liposome which was small unillamellar and multilamellar vesicles. 2) In this experiment the optimal concentration of SnCl2 was 156μg/ml to label liposome with Tc-99m SnCl2 labelling efficiency was 98%. 3) The labelled liposome was stable when it was incubated with human serum for 24 hours. Mean labelling efficiency was 94% at 24 hour. 4) The main uptake sites of Tc-99m liposome were liver and spleen. It showed significantly higher uptake than Tc-99m HSA (p〈0.001). 5) Tc-99m liposome uptake in tumor tissue was not significantly higher than Tc-99m HSA uptake. In conclusion,Tc-99m liposome disclosed high labelling efficiency and was highly stable. Liver and spleen were main uptake sites of Tc-99m liposome. The uptake mechanism of Tc-99m liposome also seemed to be different from that of Tc-99m HSA. We conclude that Tc-99m liposome would be a promising agents for the imaging of some tumor.
