        유치 외상에 관한 연구

        경희대학교 치과대학병원 소아치과에 2003년 1월 1일부터 2007년 12월 31일까지의 최근 5년간 치아외상을 주소로 내원한 생후 6개월에서 7세 미만(평균 2.8세)의 어린이 758명의 외상 받은 유치 1533개를 대상으로 하였다. 모든 대상자들의 진료기록 및 방사선 사진으로부터 내원한 어린이의 성별, 연령별 발생빈도, 원인, 장소, 외상 후 치료까지의 경과시간, 월별발생률, 발생부위, 치아손상의 개수, 외상의 양상, 초진 시 처치 등을 분석하여 다음과 같은 결과를 얻었다. 1. 유치의 외상은 남아가 여아보다 1.77배 많은 것으로 나타났으며, 남녀 모두 1-2세, 2-3세에 많이 발생하였다(p<0.001). 2. 외상의 주요 원인은 넘어짐, 부딪힘으로 나타났고, 주로 저연령층 어린이에서 두드러지게 나타났다(p<0.05). 외상의 발생장소는 집안, 집밖, 유치원순으로 나타났으며, 특히 기후가 온화한 5월, 9월, 10월에는 집밖의 비율이 높게 나타났다(p<0.001). 3. 외상 후 주로 24시간 이내(77.6%)에 내원하였으며, 월별 치아외상은 겨울을 제외한 3-9월에 고르게 나타났다(p<0.001). 4. 가장 손상을 많이 받은 치아는 상악 유중절치로 나타났으며, 치아손상은 단일치아 또는 두 개의 치아 손상이 주로 나타났다. 5. 치주조직 손상이 치아경조직 손상에 비해 2배정도 많이 발생하였으며, 외상의 양상은 아탈구, 측방탈구, 법랑질파절, 함입, 치근파절 순으로 나타났다. The purpose of this study was to research the traumatized primary teeth and investigate following factors: sex, age, cause, place and time(of the year) of injury, elapsed time, area and type of injury, dental treatment and prognosis. The analysis includes total of 1533 traumatized primary teeth from 758 children aging from 6 months to 6 years(mean age: 2.8) from 2003 to 2007. The result follows : 1. The children of age between 1-2 and 2-3 were involved in the largest number of injuries in both sexes with boy/girl ratio of 1.77:1(p<0.001). 2. Fall and collision were the main causes of traumatic dental injury, especially in younger children(p<0.05). Places of injury occurrence varied: home, outdoors, and kindergarten. Warm climate accounts for frequent outdoor injuries in May, September and October(p<0.001). 3. Most of the children visited dental clinic within 24 hours of the injury(77.6%). From March to September, dental trauma occurrences were distributed evenly, except for Winter period(p<0.001). 4. Upper central incisors were the most commonly affected teeth, and the injuries usually involved 1 tooth or 2 teeth. 5. Periodontal tissue injuries dominated and subluxation was the most common type. Lateral luxation, enamel fracture, intrusion and root fracture followed.

        K 강선 및 긴장대 강선을 이용한 원위 쇄골 제 2형 골절의 치료

        목 적: 쇄골 윈위부 제 Ⅱ형 골절의 치료에 있어 견봉 쇄골 관절을 통과하지 않고 직접 골편을 고정시키는 K 강선 및 긴장대 강선을 이용한 고정술로 치료한 환자에서 임상 결과를 후향적으로 분석하여 보고하고자 한다. 대상 및 방법: 2000년 5월부터 2001년 5월까지 쇄골 원위부 제 Ⅱ형 골절로 관혈적 정복술 및 K 강선 및 긴장대 강선을 이용한 고정술을 실시한 12례 중 최소 1년 이상 추시가 가능한 11례를 대상으로 하였다. 임상적 평가를 위해 최종 추시에서 변형된 견관절 기능 평가법을 사용하여 결과를 분석하였고 골유합 상태, 합병증, 견관절 운동범위를 측정하였다. 결 과: 전례에서 골유합을 얻었으며, 수술 후 골 유합까지의 기간은 평균 10주 (8~12주)였다. 최종 추시 상 견관절의 운동범위는 모두 정상 소견이었고, 변형된 견관절 기능 평가상 우수 9례, 양호 2례였다. 결 론: 쇄골 원위부 제 Ⅱ형 골절에서 K 강선 및 긴장대 강선을 이용한 고정술은 좋은 치료 방법 중 하나로 사료된다. Purpose: This is retrospective study to analyze the clinical results of the usefulness of K-wires and tension band wiring that fix the fracture fragment directly without passing the acromioclavicular joint in distal clacivle type Ⅱ fractures. Materials and Methods: From May 2000 to May 2001, eleven patients with distal clavicle type Ⅱ fracture were treated by open reduction and internal fixation with K-wires and tension band wiring. The clinical results were analyzed according to modified shoulder rating scale for distal clavicle freacture. Radiological union, complication, and range of motion of the shoulder were assessed. Results: All fractures were united at 10 weeks (8~12 weeks) in average. Finally, full range of motion of the shoulder joint was achieved in all patients. No complication was found and the modified shoulder rating scale for distal clavicle fracture were as follows: excellent 9 and good 2. Conclusion: K-wires and tension band wiring can be a good treatment method for distal clavicle type Ⅱ fractures.

        Differencies of lipid composition of mnrine sarcoma virus 970-17 traiisfomed rabbit kidney cells and their plasma membranes were studied. Free cholesterol was the major neutral lipid extracted from the whole cells and plasma membranes of all these cells. The micromoles of total neutral lipids and phospholipids are elevated in plasma membranes compared to whole cells from monolayer cultures. The distribution of fatty acids in the major lipid classes of plasma membranes and whole cells generally follow the composition of calf serum used in the culture media.

        ECAP 공정을 거친 1050 Al 합금 판재의 전기화학 특성에 관한 연구

        The effect of equal channel angular pressing (ECAP) pass number on the electrochemical properties of AA 1050 (UNS A91050) in 0.1M Na₂SO₄ solution containing 100 ppm chloride were investigated by electrochemical techniques (potentiodynamic test, potentiostatic test, electrochemical impedance spectroscopy (EIS) and surface analyses (OM, SEM, EDS). The ECAPed specimens that have a various numbers of passes from N=l to N=5 were selected for electrochemical experiments in this study. The results of potentiodynamic test indicated that the breakdown potential increased with increasing the ECAP pass number. EIS measurements also showed that the corrosion resistance of the AA 1050 increased with increasing the ECAP pass number at the passivation and breakdown states. The results of surface analyses (SEM, EDS) showed that silicon-containing impurities were observed on the surface of the alloy. It was found that the size of these Si-containing impurities decreased with increasing the ECAP pass number. The Si-containing impurities induced the microgalvanic reaction(i) between the Al matrix and the Si-containing mixed oxide and (ii) between the Al matrix and the Si-containing impurities. Due to its ultrafine-grained microstructure, ECAPed AA 1050 had smaller Si-containing impurities, and reduced microgalvanic currents. Thus, in the case of ECAPed AA 1050, it has increasing corrosion resistance with increasing ECAP pass number because of its small cathodic impurities.

        이소매복된 하악 제2소구치의 자가치아이식을 이용한 치험례

        악골내 이소매복된 치아로 인해 유치의 만기잔존, 교합이상과 같은 교정적인 문제가 발생할 경우 그 해결책으로 주기적 관찰, 최소한의 개입, 교정적 견인, 발치, 그리고 환자 자신의 매복치아를 이용하는 자가이식술 등을 들 수 있다. 자가치아이식은 치아를 구강내의 한 위치에서 다른 발치와나 외과적으로 형성된 치조와로 이동시키는 술식으로, 치아가 교정력을 가할 수 없는 위치에 존재하거나 치아 이동에 제한이 있어 통상적인 치료가 불가능할 경우에 발거에 앞서 고려할 수 있는 방법이다. 자가치아이식은 치료기간을 단축시키고, 치근 형성이 완료되지 않은 어린 환자의 경우 이식된 새로운 위치에서 치근 형성이 이루어지며, 새로운 치조골의 형성을 유도할 수 있다. 본 증례는 하악 우측 제2소구치의 매복과 하악 우측 제2유구치의 만기잔존을 주소로 본원에 내원한 11세 여아로, 하악 우측 제2소구치의 매복의 깊이가 깊고, 방향이 교정적 견인에 불리하다는 점, 미성숙 치근 발육 상태와, 만기잔존한 유구치로 인해 공간 상실이 없다는 점 등을 고려하여 자가치아이식을 시행한 뒤, 1년간의 주기적 관찰 결과, 양호한 결과를 얻어 이에 보고하는 바이다. In the case of the impacted teeth, the clinician has to consider development of tooth, site of impaction, eruption path, and cooperation of patient. Treatment options for the management of impacted teeth are separated into four categories: observation, intervention, orthodontic or surgical relocation and extraction. Autotransplantation may be defined as the transplantation of embedded, impacted or erupted teeth, from one site to another in the same individual into extraction site or surgically prepared sockets. Autotransplantation ensures preservation of natural tooth, induction of alveolar bone growth and root development, offers one of the fastest and most economically feasible means in the replacement of young patients' missing teeth. This case presents a malpositioned impacted mandibular premolar of an 11-year-old girl. It was thought that orthodontic traction was difficult because of its unfavorable impacted position. Therefore the tooth was treated by autotransplantation, we can observe good healing pattern during 12 months.

      • 태권도 품새 수련이 아동의 심박수, 심근산소소비량, 에너지 대사에 미치는 영향

        The Taegeuk Poomse 1 through 8 of Taekwondo are based upon eight major branches of philosophical theories. The propose of this study was to investigate the nfluence of Taekwondo poomse trainning on heart rate, MVO2, energy metabolism in primary school students. Subjects for this study were eight male the fifth∼sixth grades students who participated in exercise program consist of Taekwondo poomse trainning in dojang which is located in Shinlim-2dong, Kwanak-Gu, Seoul(12.7± 0.5years, 150.8 ±4.6cm, 45.1 ± 9.4kg, 24.9 ± 9trainning length). The results of thet this study are as follows: 1) The average of heart rate was Taegeuk Poomse 1 through 8 of Taekwondo 90.57± 4.gbeats/min in rest and 139.7 ± 17.5beats/min in exercise. The minimum heart rate was 89.0±2.8, peak heart rate was 167.1 ±6.4 in exercise. It was ranged from 42% to 80% 2) of the max heart rate and ranged 42 to 65%HRmax. Taegeuk 1 Jang was differenced with others Jangs significantly(p<0.05), 2) The time of the Taegeuk 1 through 8 Jang of Taekwondo was 5minutes 12seconds. Heart rate was ranged from 90.6 ± 7.85beats/min to 171.8 ±8.Ibeats/min and increased linearly. 3) The average of the MVO2 was 11.1 ±0.78mmHg · bpm-3 in rest and 19.1 ± 1.2mmHg ㆍbpm-3 in exercise. MVO2 was ranged from 10.5mmHg · bpm-3 to 20.3mmHg · bpm-3. There were not difference between Taegeuk Poomses(p<0.05). 4) The energy metabolism was as follows: Mean VO2 Respiratory exchange ratio, energy consumption were 0.51(V02L/min),0.71(RQ),2.32Ckca1/min) in rest respectively. but In exercise Mean VO2 Respiratory exchange ratio, energy consumption were 0.89 ± 0.18( V 02L/min), 0.79±0.08(RQ), 4.26±0.51(kcal/min) respectively . The difference was only between Taegeuk 1 Jang and Taegeuk 3 Jang(p<0.05). This study showed that the Taekwondo Poomse trainning was proper exercise for fitness, body composition and weight control in normal, abnormal(obese) children because the Taekwondo trainning are dynamic and exciting sport.

      • 葉成分과 土壤의 理化學的性質이 林木生長에 미치는 영향

        This study was carried out to analyze physical and chemical properties of soil and leaf analysis to estimating amount of tree growth in Chamaecyparis pisifera, Jinhae city, Gyeongnam province. The results were summarize as follows; As the results from analysis of relation of physical-chemical properties of soil and leaf component, the relationship p component inside soil and n component inside leaf showed positive correlation with determination coefficients(R^2=0.9986), and also relation of nitrogen component inside soil and component of Na_2O inside leaf showed positive correlation with determination coefficients(R^2=0.9987), but the relation of component Ca^2+ inside soil and component of Na_2O inside leaf showed negative correlation with determination coefficients(R^2=-0.9984), and also relation of p component inside soil and Fe component inside leaf showed negative correlation with determination coefficients(R^2=-0.7215). The results from analysis of relation of leaf component and tree growth, showed correlation of determination coefficient(R^2=0.4256) in height growth, determination coefficient (R^2=0.6248) in DBH growth, determination coefficient(R^2=0.6248,) in volume growth, respectively. In each tree growth of recent five years, N and P components showed positive correlation of determination coefficient(R^2=0.6170, 0.5780, respectively) in height growth, and also N and Na^+ components showed positive correlation of determination coefficient(R^2=0.6016, R^2=0.8727, respectively) in DBH growth, but K= component showed negative correlation in DBH growth As the results from analysis of stepwise regression to estimating amount of DBH growth, sequence of inputted variables entered in order of N, Na+, Mg++, ,and estimated equation was Y=44.791N-56.640Mg+0.6867Na-26.47, determination coefficients of 69%.
