      • Single-particle investigation of summertime and wintertime Antarctic sea spray aerosols using low-Z particle EPMA, Raman microspectrometry, and ATR-FTIR imaging techniques

        <P>Abstract. Two aerosol samples collected at King Sejong Korean scientific research station, Antarctica, on 9 December 2011 in the austral summer (sample S1) and 23 July 2012 in the austral winter (sample S2), when the oceanic chlorophyll a levels on the collection days of the samples were quite different, by ∼ 19 times (2.46 vs. 0.13 µg L−1, respectively), were investigated on a single-particle basis using quantitative energy-dispersive electron probe X-ray microanalysis (ED-EPMA), called low-Z particle EPMA, Raman microspectrometry (RMS), and attenuated total reflection Fourier transform infrared (ATR-FTIR) imaging techniques to obtain their characteristics based on the elemental chemical compositions, molecular species, and mixing state. X-ray analysis showed that the supermicron summertime and wintertime Antarctic aerosol samples have different elemental chemical compositions, even though all the individual particles analyzed were sea spray aerosols (SSAs); i.e., the contents of C, O, Ca, S, and Si were more elevated, whereas Cl was more depleted, for sample S1 than for sample S2. Based on qualitative analysis of the chemical species present in individual SSAs by the combined application of RMS and ATR-FTIR imaging, different organic species were observed in samples S1 and S2; i.e., Mg hydrate salts of alanine were predominant in samples S1 and S2, whereas Mg salts of fatty acids internally mixed with Mg hydrate salts of alanine were significant in sample S2. Although CaSO4 was observed significantly in both samples S1 and S2, other inorganic species, such as Na2SO4, NaNO3, Mg(NO3)2, SiO2, and CH3SO3Mg, were observed more significantly in sample S1, suggesting that those compounds may be related to the higher phytoplankton activity in summer. </P>

        Membranous Obstruction of the Inferior Vena Cava (MOVC) is a raredevelopmental abnormality of the inferior vena cava at the level of diaphragm. Because MOVC secondarily impedes hepatic venous drainage, it causes severe chronic venous congestion and centrolobular fibrosis of liver. MOVC has recently been documented as a risk factor for hepatocellular carcinoma in South African Blacks and Japanese. We report here our experience with a case of MOVC with hepatocellular carcinoma to give special emphasis to the differences in presentation and treatment with classic Budd-Chiari syndrome and the imperative necessity of the surgical correction of the defect with early detection.

        여성 노인 당뇨병 환자의 당뇨관리식 두유 섭취 전과 후의 영양 및 혈액 상태 변화에 관한 연구

        본 연구에서는 삼육식품이 당뇨병 조절을 위해 개발한 두유를 당뇨병 환자의 동의하에 섭취시켰으며, 삼육케어(당뇨관리식)의 효능을 입증하기 위한 임상시험을 실시하였다. 당뇨관리식 섭취 전과 후의 BMI는 18.88±1.16 kg/㎡, 20.74±1.06 kg/㎡였다. 당뇨관리식 섭취 후에 식사할 때, 음식에 소금이나 간장을 더 넣지 않는다는 비율과 식사는 매일 세끼를 규칙적으로 한다는 비율은 증가하였다. 당뇨관리식 섭취 후에 에너지 섭취량, 단백질, 지방, 비타민 B6, 비타민 B12, 엽산, 칼슘, 철, 셀레늄, 인 및 식이섬유 섭취가 증가하였다. 당뇨관리식 두유를 섭취시키기 전과 후의 혈중 콜레스테롤 수치는 144.00±8.02 mg/dL에서 142.46±7.49 mg/dL로 감소하는 경향을 보였으며, LDL-콜레스테롤 수치는 10.30±1.72 mg/dL에서 8.11±0.74 mg/dL로 유의하게 감소하였다(P<0.05). 따라서 당뇨관리식 두유 섭취는 당뇨환자의 영양상태 개선 및 LDL-콜레스테롤 수치 개선에 도움을 주는 것으로 사료되며, 앞으로 지속적인 임상 연구가 진행되어야 할 것으로 판단된다. This study evaluate the health improvement efficacy of Sahmyook Care (diabetic control diet) vegetable soymilk intake in elderly diabetic women. This randomized clinical trial was conducted by enrolling 18 elderly diabetic women aged 65 to 80 years, after obtaining their informed consent. Subjects were randomly assigned to consume a diet containing soy milk developed by Sahmyook Food for diabetes management for 4 weeks. The average BMI of the subjects before and after diabetic control diet intake were 18.88±1.16 kg/㎡ and 20.74±1.06 kg/㎡, respectively. Eating habits after consuming the diabetes control diet, showed an increase in the ratio of not adding salt or soy sauce to food and regularly eating three meals a day. Energy intake, protein, fat, vitamin B6, vitamin B12, folic acid, calcium, iron, selenium, phosphorus, and dietary fiber intakes were also increased after partaking in the diabetes control diet (P<0.05). The level of LDL-cholesterol in blood before and after intake of soymilk for diabetes management showed a significant decrease, from 210.30±1.72 mg/dL to 108.11±0.74 mg/dL (P<0.05). Therefore, the consumption of soymilk in a diabetes-controlled diet is considered helpful in improving the nutritional status and LDL-cholesterol levels in diabetic patients.

        언어재활전공 교과과정의 국내 현황 조사

        배경 및 목적: 국내 대학에 언어재활전공이 처음 개설된 1988년을 기점으로 2018년 현재 총 50개의 대학(대학교, 전문대학, 대학원)에서 총 87개의 프로그램이 운영되고 있다. 이러한 양적 팽창이 이루어진 현 시점에서 각 프로그램들의 운영 현황을 살펴보고 미래를 준비하는 노력이 필요할 것으로 판단된다. 이에 본 연구에서는 전국 대학에 개설되어 있는 언어재활전공 교과과정 현황을 분석하여 기초 자료를 제공하고 우수한 언어재활사 양성을 위한 미래의 방향 모색을 위한 초석을 마련하고자 하였다. 방법: 대학교, 전문대학, 대학원에 언어재활전공이 개설된 학교에 대학의 기본정보, 2015년에서 2017년 사이의 전공 교과목 개설 현황, 교과과정에 대한 수요 등의 내용이 포함된 설문지를 2차에 걸쳐 배포하였다. 설문에 응하지 않은 학교들을 제외한 후, 1차 설문지 내용은 총 43개 프로그램을 대상으로 분석하였고 2차 설문지 내용은 총 33개 프로그램을 대상으로 분석하였다. 결과: 세 가지 학제별 교과과정을 살펴본 결과, 대학교와 전문대학 중 언어재활사 자격 이외의 자격증 과정을 개설하고 있는 대학의 경우 개설 교과목 수가 상대적으로 많은 것으로 나타났다. 대학의 상황에 따라 분반 운영 및 전공선택과 전공필수 구분 등에 있어서 편차가 많은 것을 알 수 있었다. 대학교와 대학원 과정의 교과 과정 수요 분석에서는 학령기, 노인(노화), 보완대체의사소통, IT 등의 키워드가 공통적으로 포함된 것을 알 수 있었다. 논의 및 결론: 본 연구자료는 국내 언어재활전공의 교과과정 현황을 살펴봄과 동시에 추후 질 높은 언어재활사 양성을 위한 교과과정을 제시함에 있어 기초 자료로서 활용되기를 기대해본다. Objectives: Since 1988, when the Department of Speech-Language Pathology was first established at a university, a large number of speech-language pathologists (SLPs) have been trained. As of March 2018, there are 87 programs in 50 schools offering associate’s degree, bachelor’s degree, and/or master’s/doctoral degree. In order to support the continued growth of the SLP programs, we believe it is necessary to assess the present operation status of all SLP programs. In this study, therefore, we surveyed the current curriculum of each program, and tried to lay out future direction that the SLP programs need to pursue. Methods: We sent out the questionnaires twice to all the programs. The questionnaires included the following: information about the program (e.g., number of faculty members), the status of the existing curriculum during the period of 2015-2017 academic years, and the curriculum improvement proposals from the program faculty members. After excluding the schools that did not respond or filled the questionnaire incorrectly, the contents of the first and second questionnaires were analyzed on 43 programs and 33 programs, respectively. Results: The questionnaire responses revealed that there were relatively larger number of curriculum subjects in associate’s degree programs than bachelor’s degree programs because the associate’s degree programs tended offer other certification courses in conjunction with the SLP program. It was also found that some SLP programs limited number of students in a class to offer better student support and offered different sets of electives and mandatory courses. In the analysis of the curriculum improvement proposals, it was found that the most common keywords included ‘school age’, ‘elderly/aging’, ‘augmentative and alternative communication’, and ‘IT (information technology)’. Conclusion: In this study, we provide the picture of the current SLP programs in Korea. The results of this study could be used as the basis for the SLP programs’ continued improvement in the future.

        Carcinosarcoma of the stomach is regarded as a rare malignant neoplasm composed of both carcinomatous and sarcomatous components in a given tumor. Few cases have been reported since 1904. This is a case of carcinosarcoma of the stomach in a 61-year-old man. He suffered from indigestion, vomiting, and epigastric pain. Endoscopic finding showed a huge protruding mass with intact mucosa on the posterior wall of the antrum up to the pylorus. Surgery was performed and carcinosarcoma with pancreatic invasion was confirmed by pathology.

        Propranolol, a β-adrenergic antagonist, attenuates the decrease in trabecular bone mass in high calorie diet fed growing mice

        We investigated the effects of high calorie and low caloriediets on skeletal integrity, and whether β adrenergic blockade(BB) attenuates bone loss induced by dietary calorie alteration. Male 6-week old C57BL/6 mice were assigned to either anad-lib fed control diet (CON), a high calorie diet (HIGH), or alow calorie diet (LOW) group. In each diet group, mice weretreated with either vehicle (VEH) or propranolol, a β-adrenergicantagonist. Over 12-weeks, β-blockade mitigated bodyweight and fat mass increases induced by the high calorie diet. Femoral trabecular bone mineral density and the expressionlevels of osteogenic marker genes in bone marrow cells were reduced in HIGHVEH and LOWVEH mice, and BB significantly attenuated this decline only in HIGH mice. In summary, the magnitude of bone loss induced by low calorie diet was greater than that caused by high calorie diet in growing mice, and β-blockade mitigated high calorie diet-induced bone loss.

        Tumor Necrosis Factor α up-regulates the Expression of beta2 Adrenergic Receptor via NF-κB-dependent Pathway in Osteoblasts

        Tumor necrosis factor alpha (TNFα) is a multifunctional inflammatory cytokine that regulates various cellular and biological processes. Increased levels of TNFα have been implicated in a number of human diseases including diabetes and arthritis. Sympathetic nervous system stimulation via the beta2-adrenergic receptor (β2AR) in osteoblasts suppresses osteogenic activity. We previously reported that TNFα upregulates β2AR expression in murine osteoblastic cells and that this modulation is associated with TNFα inhibition of osteoblast differentiation. In our present study, we explored whether TNFα induces β2AR expression in human osteoblasts and then identified the downstream signaling pathway. Our results indicated that β2AR expression was increased in Saos-2 and C2C12 cells by TNFα treatment, and that this increase was blocked by the inhibition of NF-κB activation. Chromatin immunoprecipitation and luciferase reporter assay results indicated that NF-κB directly binds to its cognate elements on the β2AR promoter and thereby stimulates β2AR expression. These findings suggest that the activation of TNFα signaling in osteoblastic cells leads to an upregulation of β2AR and also that TNFα induces β2AR expression in an NF-κB-dependent manner.

      • Blocking β <sub>1</sub> /β <sub>2</sub> -Adrenergic Signaling Reduces Dietary Fat Absorption by Suppressing Expression of Pancreatic Lipase in High Fat-Fed Mice

        <P>We investigated whether β-adrenergic antagonists attenuates dietary fat absorption through the regulation of pancreatic lipase (PNLIP) expression in pancreatic acinar cells in the context of high fat diet feeding. Male six-week-old C57BL/6 mice were assigned into an ad libitum fed control diet (CON) and a high fat diet (HIGH). Within each diet group, subgroups of mice were treated with vehicle (VEH) or propranolol, a β-adrenergic antagonist (BB). Over 12 weeks, body weight gain observed in HIGHVEH was mitigated in HIGHBB (+103% vs. +72%). Increase in fecal fat amount observed in HIGHVEH was further increased in HIGHBB. Increase in PNLIP expressions observed in HIGHVEH pancreatic tissues was abolished in HIGHBB. PNLIP expression in mouse primary pancreatic acinar cells and 266-6 cell lines increased with isoproterenol treatment, which was blocked by propranolol. Isoproterenol increased PNLIP expression in a cAMP/protein kinase A/ cyclic AMP response element binding protein (CREB)-dependent manner. CREB directly bound to the CRE on the mouse PNLIP promoter and transactivated PNLIP expression. These results suggest that sympathetic activation increases dietary fat absorption through the upregulation of PNLIP expression and that a β-adrenergic antagonist attenuates obesity development partly through the downregulation of PNLIP expression and inhibition of dietary fat absorption in the context of high fat diet feeding.</P>

        대량 토혈을 초래한 급성 아니사키스증 1 예

        Acute gastric anisakiasis is caused by gastric mucosal penetration from an Anisakis larvae. It occurs with those who ingest raw or inadequately cooked saltwater fish or squid containing anisakis. The clinical symptoms are severe abdominal pain, nausea, vomiting, diarrhea, and so on. A case of acute gastric Anisakiasis with massive hematemesis was presented. Initial endoscopic examination revealed an edematous or raised erosive lesion with a small blood clot-covered vessel in the fundus which was thought to be a stigmata of recent bleeding. The lesion was treated with an epinephrine-hypertonic saline injection, electrocoagulation, and an ethanol injection. A follow-up gastroscopy revealed an artificial coagulation-induced ulcer at the previous bleeding site. At the ulcer margin, a whitish linear worm was found with half of its body penetrating the gastric mucosa. The worm was removed using biopsy forceps and the patient was subsequently placed on a soft diet. The next day however, hematemesis recurred. Gastroscopic band ligation of the vessel was performed with a cessation of bleeding. Three weeks later, a gastroscopy determined that the ulcer had completely healed.

        제 4 기 진행성 위암과 제 1 기 신세포암이 병발된 중복암 1예

        Multiple primary malignancy is called when two cancers occur independently in one individual. Multiple primary malignancy may be divided into two groups, synchronous or metachronous depending on the interval between their diagnosis. Synchronous cancer is diagnosed simultaneously or within an interval of 6 months and metachronous cancer at interval of more than 6 months. Since Billroth reported the first case of multiple primary malignancy, many cases have been reported partly owing to the advancement of diagnostic procedures and treatment modalities. The incidence of multiple primary malignancy tends to increase and it is fundamental that patients who have been treated for cancer receive a careful follow-up study. Here we report on a case of stage-IV advanced gastric cancer with stage-I renal cell carcinoma which was discovered during the staging procedure of gastric cancer. Both cancers were confirmed histologically and classified as gastric adeno-carcinoma and renal cell carcinoma.
