<P>A combined axial-torsional low cycle fatigue test was carried out to predict the fatigue life under in-phase and out-out-phase loading conditions for CF8M cast stainless steels. The Fatemi-Socie (FS) parameter which is based on the critical plane approach is not only one of the many methods but also the best method that can predict the fatigue life under a biaxial loading condition. But the result showed that, a biaxial fatigue life prediction by using the FS parameter with several different parameters for the CF8M cast stainless steels is not conservative enough but at the same time it was the best result so far. So in this present research, we proposed a new fatigue life prediction parameter (Park-Kwon parameter) by considering effective the shear stress instead of the FS parameter which considers the maximum normal stress acting on the maximum shear strain and its effectiveness was verified.</P>

In this paper, the transmitted noise reduction of piezoelectric smart panels is experimentally studied. The proposed piezoelectric smart panels are comprised of plate structure on which piezoelectric sensor/actuators are bonded and sound absorbing material and/or gap are provided. It is a combination of passive and active approaches utilizing a passive effect at high frequencies and active effect at low frequencies. To prove the concept of piezoelectric smart panels, and acoustic measurement experiment is performed. An acoustic tunnel is designed and its acoustic characteristics are tested. Below 800㎐, the tunnel exhibits a plane wave guide characteristics. When an absorbing material is bonded on a single plate, a remarkable transmitted noise reduction in mid frequency range is observed except the first resonance frequency. By enabling the active control of smart single panel, much noise reduction is achieved at the resonance frequencies. The smart double panel has more potential to reduction the transmitted noise in wide range frequency. Piezoelectric smart panels incorporating passive absorbing material and active piezoelectric devices is a promising technology for noise reduction in a wide range frequency.

Female; 1,909)were measured from May 1 to May 31 in 1986 and various physical and nutritional indices related to them were calculated.

Purpose: The aim of this study was to show that bariatric surgery (BS) is more effective than medical therapy (MT) in Asian obese patients. Methods: In this prospective, multicenter, nonrandomized, controlled trial, obese patients with body mass index of ≥35 kg/㎡ or 30.0–34.9 kg/㎡ with obesity-related comorbidities were assigned to undergo BS, such as laparoscopic sleeve gastrectomy and Roux-en-Y gastric bypass, or MT. Patients who underwent BS were evaluated 4, 12, 24, and 48 weeks after surgery, whereas patients who received MT were monitored at a hospital every 6 weeks for 1 year. At each visit, weight, waist and hip circumference, and blood pressure were measured, and patients underwent physical examination and laboratory testing. Health-related quality of life (HQOL) was investigated using Euro QOL-5 Dimension, Impact of Weight on Quality of Life questionnaire-Lite and Obesity-related Problems scale. Results: The study included 264 patients from 13 institutions; of these, 64 underwent BS and 200 received MT. Of the patients who underwent BS, 6.3% experienced early complications. Relative weight changes from baseline to 48 weeks were significantly greater in the BS than in the MT group (26.9% vs. 2.1%, P < 0.001), as were the rates of remission of diabetes (47.8% vs. 16.7%, P = 0.014), hypertension (60.0% vs. 26.1%, P < 0.001), and dyslipidemia (63.2% vs. 22.0%, P < 0.001). HQOL was better in the BS than in the MT group at 48 weeks. Conclusion: BS was safe and effective in Korean obese patients, with greater weight reduction, remission of comorbidities, and quality of life improvement than MT.

<P>OBJECTIVES: Systemic chemotherapy may lead to immune suppression and possible reactivation of hepatitis B virus (HBV), suggesting prophylactic antiviral therapy in cancer patients with HBV. Transcatheter arterial chemoembolization (TACE) for hepatocellular carcinoma (HCC) is not a systemic chemotherapy, but has been partially associated with HBV reactivation. The aim of this study was to evaluate whether TACE aggravates HBV hepatitis in patients with HBV-related HCC. METHODS: Eighty-nine patients with HBV-related HCC were studied prospectively. Patients treated with TACE were enrolled in the case group (n = 69), and patients in follow-up or awaiting treatment were enrolled in the control group (n = 20). TACE was performed with doxorubicin (20-60 mg) and lipiodol (2-20 mL). RESULTS: Three (4.3%) patients in the TACE group and 2 (10%) patients in the control group showed HBV reactivation (p= 0.334). A twofold or more increase in serum HBV DNA was detected in 21 (30.4%) patients in the TACE group and 4 (20%) patients in the control group (p= 0.361). Exacerbation of viral hepatitis B was found in 4 (5.8%) patients in the TACE group and no patients in the control group, but the difference between the two rates was not statistically significant (p= 0.271). Three of the four reactivated patients showed spontaneous recovery within 1 month, and one showed tumor-progression-related exacerbation. CONCLUSIONS: One session of TACE using doxorubicin and lipiodol does not significantly aggravate HBV hepatitis in patients with HBV-related HCC.</P>
Risk Factors for Gallstone Formation after Surgery for Gastric Cancer

Purpose: The incidence of gallstones after gastrectomy for gastric cancer is higher than in the general population. However, the causes and mechanisms of post-gastrectomy gallstones are unclear. The aim of this study was to evaluate the incidence of gallstone formation and the risk factors for their development after gastrectomy for gastric cancer. Materials and Methods: Of 1,744 gastric cancer patients who underwent gastrectomy at Seoul National University Bundang Hospital between January 2010 and December 2012, 1,284 were included in this study and retrospectively reviewed. Patients' age, sex, body mass index (BMI), tumor location, stage, type of gastrectomy, type of reconstruction, and extent of node dissection were evaluated. Results: The incidence of gallstones after gastrectomy for gastric cancer was significantly higher in men than in women (P=0.019). Exclusion of the duodenum during reconstruction was associated with a significantly higher incidence of gallstones (P=0.003). Overweight and obese patients with BMI ${\geq}23kg/m^2$ had significantly higher incidence of gallstones than those with a lower BMI (P=0.006). Multivariate analysis showed that obesity (hazard ratio, HR=1.614; 95% confidence interval, CI: 1.135~2.296; P=0.008), male sex (HR=1.515, 95% CI: 1.029~2.231, P=0.033), and exclusion of the duodenum (HR=1.648, 95% CI: 1.192~2.280, P=0.003) were significant, independent risk factors for gallstones after gastrectomy. Conclusions: The cumulative incidence of gallstones for 5 years after gastrectomy was 15.3%. Male sex, obesity, and exclusion of the duodenum were risk factors for gallstone formation after gastrectomy. Careful surveillance will be required for these patient groups after gastrectomy.
