        A meta-analysis on advantages of peripheral nerve block post-total knee arthroplasty

        Background: Postoperative pain management is crucial for patients undergoing total knee arthroplasty (TKA). There have been many recent clinical trials on post-TKA peripheral nerve block; however, they have reported inconsistent findings. In this meta-analysis, we aimed to comprehensively analyze studies on post-TKA analgesia to provide evidence-based clinical suggestions. Methods: We performed a computer-based query of PubMed, Embase, the Cochrane Library, and the Web of Science to retrieve related articles using neurothe following search terms: nerve block, nerve blockade, chemodenervation, chemical neurolysis, peridural block, epidural anesthesia, extradural anesthesia, total knee arthroplasty, total knee replacement, partial knee replacement, and others. After quality evaluation and data extraction, we analyzed the complications, visual analogue scale (VAS) score, patient satisfaction, perioperative opioid dosage, and rehabilitation indices. Evidence was rated using the Grading of Recommendations Assessment, Development, and Evaluation approach. Results: We included 16 randomized controlled trials involving 981 patients (511 receiving peripheral nerve block and 470 receiving epidural block) in the final analysis. Compared with an epidural block, a peripheral nerve block significantly reduced complications. There were no significant between-group differences in the postoperative VAS score, patient satisfaction, perioperative opioid dosage, and rehabilitation indices. Conclusions: Our findings demonstrate that the peripheral nerve block is superior to the epidural block in reducing complications without compromising the analgesic effect and patient satisfaction. Therefore, a peripheral nerve block is a safe and effective postoperative analgesic method with encouraging clinical prospects.

        Three New Iridoid Glucosides from the Roots of Patrinia scabra

        To probe the chemical constituents of Patrinia scabra, we undertook the phytochemical investigation on its roots, which led to the isolation and elucidation of three new iridoid glucosides, scabroside A-C (1-3), along with three known iridoids, jatamanin J (4), isopatriscabroside I (5) and loganic acid (6) from the aqueous fraction of the ethanolic extract of the roots. The structures and relative configurations of the three new compounds were elucidated by spectroscopic methods including IR, UV, MS, 1D and 2D NMR experiments. Compound 3 was an unusual iridoid with an oxygen bridge connecting C-3 and C-8.

      • Down-regulation of Phosphoglucose Isomerase/Autocrine Motility Factor Enhances Gensenoside Rh2 Pharmacological Action on Leukemia KG1α Cells

        Aims and Background: Ginsenoside Rh2, which exerts the potent anticancer action both in vitro and in vivo, is one of the most well characterized ginsenosides extracted from ginseng. Although its effects on cancer are significant, the underlying mechanisms remain unknown. In this study, we sought to elucidate possible links between ginsenoside Rh2 and phosphoglucose isomerase/autocrine motility factor (PGI/AMF). Methods: $KG1{\alpha}$, a leukemia cell line highly expressing PGI/AMF was assessed by western blot analysis and reverse transcription- PCR (RT-PCR) assay after transfection of a small interfering (si)-RNA to silence PGI/AMF. The effect of PGI/AMF on proliferation was measured by typan blue assay and antibody array. A cell counting kit (CCK)-8 and flow cytometry (FCM) were adopted to investigate the effects of Rh2 on PGI/AMF. The relationships between PGI/AMF and Rh2 associated with Akt, mTOR, Raptor, Rag were detected by western blot analysis. Results: KG1${\alpha}$ cells expressed PGI/AMF and its down-regulation significantly inhibited proliferation. The antibody array indicated that the probable mechanism was reduced expression of PARP, State1, SAPK/JNK and Erk1/2, while those of PRAS40 and p38 were up-regulated. Silencing of PGI/AMF enhanced the sensibility of $KG1{\alpha}$ to Rh2 by suppressing the expression of mTOR, Raptor and Akt. Conclusion: These results suggested that ginsenoside Rh2 suppressed the proliferation of $KG1{\alpha}$, the same as down-regulation of PGI/AMF. Down-regulation of PGI/AMF enhanced the pharmacological effects of ginsenoside Rh2 on KG1${\alpha}$ by reducing Akt/mTOR signaling.

        정당은 국가기관과 시민사회를 잇는 가교로서 의회정치의 가장 적극적인 부분이기도 하다. 그들은 EU와 국내 정치의 연결고리가 되며, EU정치와 세계 정치 모두에 중요한 영향을 미친다. 유럽의회 내 좌파 정당과 집단의 조직구조는 비슷하고, 정당 주일의 정치활동에 의존한다. 결속력과 경쟁력은 당 그룹의 운영 특성이다. 응집력과 경쟁력은 동일하다. 의회 정당과 집단은 정책 결과에 영향을 미칠 목적으로 어떠한 입법 연합도 자유롭게 구성할 수 있다. 유럽 의회의 좌파 정당 집단의 운영 메커니즘 분석을 통해 우리는 유럽 의회의 성격, 운영 모드, 선거기능을 더욱 이해할 수 있다. 이 논문은 또 국회 안팎에서 국회 당 그룹의 기능과 그 영향력을 상세히 기술하고 향후 좌파 정당의 발전 전망을 전망하고 있다. Political parties are the bridge between state institutions and civil society, and parliamentary party groups are the most active part of parliamentary politics. They are the link between EU and domestic politics, and have an important impact on both EU politics and world politics. The organizational structure of the left-wing parties and groups in the European Parliament is similar, and they rely on the political activities of the political parties’ week. The cohesion and competitiveness are the characteristics of the operation of the party group. The cohesion and competitiveness are identical. Parliamentary parties and groups are free to form any legislative alliance with the aim of influencing policy outcomes. Through the analysis of the operation mechanism of the left-wing party group of the European Parliament, we can further understand the nature, operation mode and election functions of the European Parliament. This paper also elaborates the functions of the parliamentary party group and its influence, both inside and outside the parliament, and predicts the development prospects of the left-wing party group in the future.

        리플레이 공격 방어를 위한 무선 센서 네트워크 보안 프로토콜

        자원이 제한되어 있는 센서 노드에 보안 프로토콜 기능을 부여할 때에는 항상 어려움이 동반된다. 다양한 인증 기법들 중에서 NEKAP은 지역적 특성을 활용하는 비교적 효과적인 프로토콜이다. 반면, 이 기법은 룹홀 형성이 가능하여 리플레이 공격에 취약한 점이 있다. 본 논문에는 NEKAP의 약점을 보완하여 무선센서 네트워크가 리플레이 공격을 효과적으로 방어할 수 있도록 향상된 인증 기반 보안 프로토콜을 제안한다. 제안한 기법에서는 네 개의 보안 키를 각 노드에 부여하여 지역과 계층적 요구에 적합한 보안을 제공한다. 제안한 기법의 성능은 보안 인증, 공격 노드의 탐지, 리프레이 공격 방어 등의 측면에서 분석적 기법을 사용하여 평가하였으며 기존의 기법과 비교하였을 때 제안한 기법이 더 좋은 결과를 나타내었다. Due to the resource limitations of sensor nodes, providing a security protocol is a particular challenge in sensor networks. One popular method is the neighborhood-based key agreement protocol (NEKAP). NEKAP is an efficient and lightweight protocol, but it includes loopholes through which adversaries may launch replay attacks by successfully masquerading as legitimate nodes. In this paper, we present a modified security protocol for wireless sensor networks. We provide four types of keys for each node, which adapt to different security requirements; and an improvement is made to alleviate the replay attack. According to our qualitative performance analyses, the proposed security protocol provides effectiveness in terms of authentication security, attacking node detection, and replay attack resilience when compared to the conventional method.

        Improved snow ablation optimizer with heat transfer and condensation strategy for global optimization problem

        The snow ablation optimizer (SAO) is a new metaheuristic algorithm proposed in April 2023. It simulates the phenomenon of snow sublimation and melting in nature and has a good optimization effect. The SAO proposes a new two-population mechanism. By introducing Brownian motion to simulate the random motion of gas molecules in space. However, as the temperature factor changes, most water molecules are converted into water vapor, which breaks the balance between exploration and exploitation, and reduces the optimization ability of the algorithm in the later stage. Especially in the face of high-dimensional problems, it is easy to fall into local optimal. In order to improve the efficiency of the algorithm, this paper proposes an improved snow ablation optimizer with heat transfer and condensation strategy (SAOHTC). Firstly, this article proposes a heat transfer strategy, which utilizes gas molecules to transfer heat from high to low temperatures and move their positions from low to high temperatures, causing individuals with lower fitness in the population to move towards individuals with higher fitness, thereby improving the optimization efficiency of the original algorithm. Secondly, a condensation strategy is proposed, which can transform water vapor into water by simulating condensation in nature, improve the deficiency of the original two-population mechanism, and improve the convergence speed. Finally, to verify the performance of SAOHTC, in this paper, two benchmark experiments of IEEE CEC2014 and IEEE CEC2017 and five engineering problems are used to test the superior performance of SAOHTC.

        Mechanism of MnS Precipitation on Al2O3–SiO2 Inclusions in Non‑oriented Silicon Steel

        This study investigates the mechanism of MnS precipitation on Al2O3–SiO2 inclusions during the solidification of nonorientedsilicon steel, especially the influence of the phase structures and sizes of the oxides on the MnS precipitation, byscanning electron microscopy and transmission electron microscopy coupled with energy dispersive spectrometry. Theinvestigation results show that MnS tends to nucleate on submicron-sized Al2O3–SiO2 inclusions formed by interdendriticsegregation and that it covers the oxides completely. In addition, MnS can precipitate on micron-sized oxides and its precipitationbehavior is governed by the phase structure of the oxides. The MnS embryo formed in a MnO-containing oxidecan act as a substrate for MnS precipitation, thus permitting further growth via diffusion of solute atoms from the matrix. MnS also precipitates in a MnO-free oxide by the heterogeneous nucleation mechanism. Furthermore, MnS is less proneto precipitation in the Al2O3-rich regions of the Al2O3–SiO2 inclusions; this can be explained by the high lattice disregistrybetween MnS and Al2O3.

      • Whole-liver Radiotherapy Concurrent with Chemotherapy as a Palliative Treatment for Colorectal Patients with Massive and Multiple Liver Metastases: a Retrospective Study

        The purpose of this study was to investigate whether whole-liver radiotherapy plus a tumor-boost dose with concurrent chemotherapy is beneficial for colorectal cancer patients with massive and multiple liver metastases. From January 2007 to December 2012, 19 patients who exhibited massive (with a longest diameter > 5 cm) and invasive liver metastases and multiple metastases were treated with radiotherapy and concurrent chemotherapy. The total radiation dose was 53.4 Gy (range 38.8 Gy-66.3 Gy). All of the patients received a continuous intravenous dose of 5 fluorouracil (5-FU) 225 mg/m2 concurrently with radiation. The median survival time was 19 months. The 1- and 2- year overall survival rates were 78.3% and 14.3%, respectively. Of all of the patients who presented with abdominal pain, 100% experienced a decrease in pain. Decreases in the rates of ascites and jaundice were confirmed by ultrasound and bilirubin levels. No cases of Grade 4 or 5 acute or late toxicity were recorded. There were only two cases of Grade 3 toxicity (elevated bilirubin). These data provide evidence that whole-liver radiotherapy plus a tumor-boost dose with concurrent chemotherapy is beneficial for colorectal cancer patients with massive and multiple liver metastases.
