        Combination of Vitamin C and Rutin on Neuropathy and Lung Damage of Diabetes Mellitus Rats

        We investigated the role of vitamin C or rutin on neuropathy and lung damage of diabetic mellitus(DM) rats. Norepinephrine content was significantly decreased in sciatic nerves of DM rats compared with non-DM controls but vitamin C had no effect on decreases of norepinephrine. 2,4-dinitrophenylhydrazine (DNPH) incorporation, which is biomarker of protein oxidation, was increased in sciatic nerve of DM rats as compared with normal control. However, vitamin C had no effects on increases of DNPH incorporation . We measured the content of conjugated dienes (CD) as a biomarker of lipid oxidation in sciatic nerve. CD was increased in DM as compared with normal control, Vitamin C or rutin had no effects on increases of CD. However, Rutin plus vitamin C significantly decreased the content of CD as compared with CIM rats. In lung of DM rats, DNPH incorporation or CD was increased as compared with normal control. Vitamin C or Rutin had no effects on increases of CD However, Rutin plus vitamin C significantly decreased the content of DNPH incorporation or CD in lung tissue. Vitamin C caused marked pathological changes such as the increases of parenchyma and the thickening of alveolar septa in the lung of DM. Rutin had protective effects on the pathological changes in the lung of DM rats. In conclusion, Vitamin C had no effects on oxidative parameter, such as DNPH incorporation or CD, and on the decreases of norepinephrine content in DM rats. Vitamin C caused the marked pathological changes in the lung of DM rats but rutin had protective efforts against the pathological changes.

        腦脊髓液內의 Norepinephrine, Epinephrine 및 Dopamine의 定量法

        Radioenzymatic assay法과 high pressure liquid chromatography를 병행하여 Picogram(2x10?∼5x10?gram) 정도의 norepinephrine, epinephrine 및 dopamine을 동시에 測定할 수 있는 方法을 고안하였다. 腦脊髓液의 catecholamines를 alumina등에 흡착시켜 농축하는 過程없이 직접 測定할 수 있었다. Internal standard는 3 catecholamines 모두 5 picogram에서 50 picogram까지 直線關係를 유지하였으며 norepinephrine, epinephrine 및 dopamine에 導入된 放射能은 picogram당 각각 26.5 ±2.7, 19.1 ±3, 20.1 ±1.4였고 duplication tube의 平均差(mean difference)도 각각 13.5 ±10.25, 29.2 ±26.1, 20.8 ±6으로서 비교적 正確性을 갖고 있었으며 그 感度는 각각 2 picogram, 5 picogram, 3 picogram으로 계산되었다. 精神分裂症 患者의 腦脊髓液內에서 catecholamines를 測定하였는데 norepineprine의 경우는 rostralcaudal gradient나 haloperidol투여 後의 치료효과도 正確히 판단할 수 있었다. Tritiated methyl誘導體의 分離를 HPLC를 이용함으로서 전 分離過程을 10分으로 단축했기 때문에 catecholamines의 測定時間을 대폭 단축할 수 있어 norepinephrine만 測定하면 하루 30試料(60 tubes), 그리고 3 catecholamines를 모두 測定한다고 하더라도 하루 10개의 試料(20 tubes)의 測定이 可能하였다. A direct rapid radioenzymatic assay for measuring simultaneously picogram quantities(2-5pg) of norepinephrine, epinephrine and dopamine by a high pressure liquid chromatography has been developed. Fifty microliters of CSF was incubated with catechol-O-methyltransferase and S-adenosyl-L[methyl-³H] methionine to convert the three catecholamines to their O-metnylated analogues. These derivatives were purified by the solvent extraction and isolated by high pressure liquid chromatography using μ-Bondapak-C? column and 0.17 M acetic acid-5% methanol-0.02 M Na₂So₄as eluant. When the integration mark was automatically placed on the chart of data module, manual collection was started two seconds later, and each peak was collected. The norepinephrine and epinephrine derivatives fractions were oxidized to vanillin, and then the solvent extraction of the vanillin and the dopamine derivative fractions were performed and counted by liquid scintillation. A rapid high pressure liquid chromatographic separation of the three catecholamine derivatives, together with the above oxidation and the solvent extraction contributes to high sensitivity and specificity of the method and makes us to complete the differential analysis of 10 CSf samples (20 tubes) within one day. In case of norepinephrine assay alone, more than 30 samples can be completed in a day.

      • 진행성 비소세포폐암에서 Vinorelbine Ifosfamide, Cisplatin (VIP) 복합항암화학요법의 효과

        Objectives: This study designed to evaluate the efficacy and toxicity of vinorelbine, ifosfamide, and cisplatin (VIP) combination chemotherapy in patients with advanced non-small-cell lung cancer(NSCLC), retrospectively. Methods and Materials: From March 1997 through March 2000, ninety-one untreated patients with inoperable stage Ⅲ-Ⅳ were included. The median age was 60 years (range 37-76) : stage ⅢA/ⅢB/Ⅳ 12/34/45. The regimen consisted of vinorelbine (25㎎/㎡ day 1 and 8), ifosfamide (3,000 ㎎/㎡ day 1 with uroprotective mesna), cisplatin (60 ㎎/㎡ day 1) every 4 weeks. Results: Ninety-one patients and total 228 cycles of chemotherapy were evaluable for toxicity and seventy-seven for response. Toxicity was mainly hematologic: grade 3-4 leukopenia (23.7%), thrombocytopenia (1.3%), and anemia(13.2%). Overall response rate was 32.5% with 25 patial responders. The median survival was 13.6 months and the mean survival 9.3±.5 months(SE). One-year survival was 54.2%. There was significant difference in survival according to the performance status (EGOG 1/2-3 8.8±.5 months/6.9±.0 months. p=0.001), which suggested that it was significant prognostic factor in this study. Responder show significant improvements in mean survival compared with non-responder (responder/non-responder 9.5±.5 months/8.3±.7 months. p=0.008). Conclusions: The combination of vinorelbine, ifosfamide and cisplatin chemotherapy(VIP) produced acceptable efficacy and tolerance in patients with advanced NSCLC.

      • Streptozotocin으로 유발된 당뇨병쥐의 신경전도속도에 대한 Evening primrose oil (EPO), Vit.C, Vit.E의 약물효과

        The effects of evening primrose oil (EPO). Vit.C and Vit.E on nerve conduction velocity (NCV) in streptozotocin -induced diabetic rats were investigated. 5 weeks- diabetes caused 27±0.5 % reduction in sciatic motor conduction velocity as compared with normal control. Dietary supplement of 5% EPO. 125mg/kg Vit.C. and lg/kg Vit.E prevented significantly the development of the motor nerve conduction velocity deficit about 21±0.5, 21±0.5, 26±0.5% respectively. The total serum cholesterol and triglyceride were decreased by administration 5% EPO. Vit.C, and Vit.E as compared with diabetic control. However, the serum glucose was not decreased by administration 5% EPO. 125mg/kg Vit.C but the serum glucose was decreased by administration lg/kg Vit.E as compared with diabetic control.

        Splenic marginal zone lymphoma is very rare B-cell lymphoma, characterized by an indolent clinical course. Clinical features are moderate-to severe splenomegaly, absolute lymphocytosis, and bone marrow intrasinusoidal infiltration of lymphocyte. We report the case of a 37-year-old male with SMZL. He complained of LUQ pain, who had a massive splenomegaly and moderate lymphocytosis in peripheral blood. Immunophenotyping findings and morphologic findings were consistent with SMZL. He underwent splenectomy and received chemotherapy with cyclophosphamide for eight months. Now, one year later after surgery he has a nearly normal blood count with no treatment.

        톨루엔을 포함한 유기용제의 직업적 폭로로 인한 신경내분비계 영향

        Long term occupational exposure to solvent mixtures may cause adverse effects to the central nervous system with neurobehavioral symptoms. And some organic slovents have been suggested to cause impairment of tuberoinfundibular dopaminergic activity and neurochemical mechanisms controlling pituitary secretion. For the purpose of assessing neuroendocrine effects in occupational solvents exposure, hormonal study settings (shoes-manufacturing industry & fishing products industry) and compared with nonexposed controls(33 men, 85 women). Male workers exposed to solvent mixtures had significantly lower plasma level of follicle-stimulating hormone(FSH) than nonexposed male controls. While female workers exposed to solvent mixtures had significantly lower plasma levels of growth hormone(GH), and thyroid-stimulating hormone(TSH) than nonexposed female controls. The results of significant decrease in plasma concentrations of pituitary hormones(FSH, GH, and TSH) in workers exposed solvent mixtures indicate that occupational exposure to solvent mixtures may induce neuroendocrine effects through an effect on hypothalamic pituitary axis.

        Idiopathic hypereosinophilic syndrome is characterized by marked elevation of eosinophil count ( >1,500/㎣) in peripheral blood without explaining etiology. It shows tissue involvement in many organs such as liver, G-I tract, myocar dium, lung, skin, kidney and nerve. Some case of hypereosinophilic syndrome with hepatic involvement have been reported, but not much. We have recently experienced a case with idiopathic hypereosinophilic syndrome of a 40 year old man who presented with RUQ pain. By abdominal CT, ill- defined subcapsular hematoma was detected. Liver biopsy showed eosinophilic infiltration within the portal tract and sinusoid. Drain procedure and corticosteroid adminiiatbrought marked improvement of clinical manifestation, hematologic abnormality and radiologic abnormality.

        Helicobacter pylori 의 간균형에서 구균형으로의 형태 변화에 따른 항원 발현의 변화에 관한 연구

        Background/Aims: There are several opinions whether the coccoid forms of Helicobacter pylori (H. pylori) are viable, infective or degenerative. The aims of this study were to induce coccoid conversion of H. pylori, to study antigenic evolutions during this conversion, and then, to elucidate the viability of coccoid forms. Methods: The antigenic evolution during coccoid conversion of H. pylori was studied by Western blotting using different sera from forty six patients who were H. pylori positive. These sera were used to obtain the total antigens of the strain cultured for 2 days (100% rod) and for 15 days ($gt;99% coccoid). Results: The antigenic profiles were not changed in 47.8% (22/46 cases) and were changed in 52.2% (24/46 cases) during coccoid conversion. Antigenic fractions disappeared during coccoid conversion were belonged to protein bands at 120 kDa (corresponding to CagA) and at 35 kDa (corresponding to porin or adhesin). Conclusions: The result showed that coccoid forms of H. pylori retain the same cellular structures as rod form, and some of the antigens disappeared during coccoid conversion. Therefore, coccoid form might be viable, and represent one of the stages of biological cycle of H. pylori. (Kor J Gastroenterol 1999;33:20 - 26)
