        Carbonated Lidocaine 에 의한 미추마취의 임상적 평가

        Thirty-six adult patients who received caudal anesthesia for perianal surgery were randomly assigned to two grgups. Of these, 15 patients ingroup I were given 1.5 and 2% plain lidocaine in 18 and 7 ml doses, repectively. Twenty·one patients in group 2 were given lidooaine carbonated with 5% NaHCO_3 0.1 ml per 1 ml-lidocaine in the same volume and concentration as in group 1. The time of onset of analgesia for the pin prick and scratch tests was significantly more rapid in group 2(2.04±0.63 and 4,69±1.12 min for the pin prick and scratch tests, respectively) than those in group 1(5.00±1.70 and 9.48±5.40 min for the pin prick and scratch test, respectively). However, the duration of anesthesia in both groups was not significantly different (111.80 ±40.24 and 105.95±45.04 min in group 1 and 2, respectively). The mean pH of the 1.5 and 2% agents used in group 1 was 5.289±0.206 and 5.257±0.193, respectively, while the mean PH of the 1.5 and 2% carbonated lidocaine used in group 2 was 7.004±0.079 and 7 reapectively. The results iridicate that pH-adjusted lidocaine for caudal anesthesia has a more rapid onset than plain lidocaine hut that there is no difference in the length of duration of its effect.

        Analysis of Mechanical Loading after Anatomic Anterior Cruciate Ligament Reconstruction Using Combined Single-Photon Emission Computerized Tomography and Conventional Computerized Tomography

        Purpose: This study was to evaluate changes of the mechanical loading pattern after anatomic anterior cruciate ligament (ACL) reconstruction by analyzing uptake patterns using combined single­photon emission computerized tomography and conventional computerized tomography (SPECT/CT). Materials and Methods: On SPECT/CT, high signal intensity of the articular surface which shows biological activity and mean increase of mechanical loading was compared with that of the tibiofemoral shaft as a comparative signal. The proportion of positive signals was evaluated in all compartments of the operated knee. Analysis was performed according to combined injury.Results: A relatively high proportion of positive signals was detected in the posterior zone of the lateral tibial plateau (23.5%) and trochlear groove (23.5%) although increased signal intensity was detected in all compartments. There was no statistical difference depending on the presence of combined injury and between single­bundle and double­bundle ACL reconstruction. Conclusions: Following anatomic ACL reconstruction, higher signal intensity was detected, particularly in the posterior part of the lateral tibial plateau and trochlear groove. Close observation for further signal changes or osteoarthritic changes would be required even if there was no combined injury and anatomic reconstruction was performed.

        전기영동에 의한 황어의 계통발생 단계 및 성체 조질들에서 동위효소의 유형

        Developing embryos of dace, Tribolodon hakonensis, were analyzed with regard to both gross morphological changes and specific enzymatic changes from the unfertilized egg stage until some 3 week posthatching. The patterns of gene expression among the different tissues in adults were also investigated. The tissue and developmental expression of 13 loci encoding various enzymes and isozymes were visualized by histochemical staining after polyacrylamide gel electrophoresis. Distribution of enzymes and isozymes in the adult tissues usually show tissue specific patterns. The expression of LDH-C and G6-PD-1 loci was the more restricted than the other loci, and these loci were expressed only predominantly in the liver. Many of the enzyme systems investigated exhibited rich patterns (all loci except for MDH-1A locus) of ontogenetic change, while a few (MDH-1A) remained relatively unchanged throughout the period studied. The appearance of several rather tissue - specific isozymes (LDH-A, C and MDH-1B) was closely correleated with the morphological and functional differentiation of the muscle, liver and gill tissue.

        상완골 대결절 단독 골절의 스프링 금속판을 이용한 수술적 치료 결과

        목적: 상완골 대결절 단독 골절에 대한 스프링 금속판을 이용한 수술적 치료결과를 분석해 보고자 하였다. 대상 및 방법: 대결절 단독 골절에 대한 수술적 치료를 시행받은 환자 중에서 1년 이상 추시가 가능하였던 14명을 대상으로 하였다. 평균연령은 51세 (범위, 25∼73세)였다. 모든 예에서 삼각 흉근 도달법으로 접근하여 스프링 금속판을 이용하여 고정하였으며, 골편의 분쇄 정도 및 위치에 따라 회전근개를 통한 봉합, 골편간 나사나 긴장대 고정을 추가하였다. 임상적 결과는 UCLA (The University of California at Los Angeles) scoring 및 KSS (Korean Shoulder Score)를 이용하였다. 결과: UCLA 점수는 평균 29.8이었으며, KSS는 평균 89.4이었다. 금속 내고정물 실패로 재수술을 했던 1예를 포함하여, 골유합은 전례에서 술 후 평균 10.2주 (범위, 7∼14주)에 얻을 수 있었다. 관절 강직이 4예, 창상치료로 치유된 감염이 1예 있었다. 결론: 스프링 금속판을 이용한 상완골 대결절 단독 골절의 치료는 신뢰할 만한 임상결과를 제공하는 좋은 치료방법이라 생각한다. In some circumstances, sutures incorporating the rotator cuff, interfragmentary screw or tension band wire were added. We evaluated the clinical outcomes using UCLA scoring system and KSS (Korean Shoulder Score). Results: The mean UCLA score was 29.8 and the mean KSS was 89.4. The average time of bony union was 10.2 weeks (range, 7∼14 weeks) after the surgery, including 1 case that was performed the secondary operation due to metal failure. The shoulder stiffness were observed in 4 cases and one case of infection was treated well without operation. Conclusion: In the treatment of isolated greater tuberosity fractures of the proximal humerus, the spring plates fixation can be a good surgical option providing reliable functional results.

        Purpose: The aim of this study was to evaluate the outcome and the effectiveness of local flaps in lower extremity reconstruction. Materials and Methods: We have performed lower extremity reconstruction with local flap in 14 cases (10 males and 4 females) from May 2006 to February 2008. The mean age was 40.1 years (range, 16~67). The defect site was the tibia in 5 cases, the ankle in 1 case and the foot in 8 cases. The local flaps were reverse flow sural artery flap in 7 cases, gastrocnemius flap in 3 cases, lateral supramalleolar flap in 2 cases, dorsalis pedis flap in 1 case and medial hemisoleus flap in 1 case. Results: All flaps were survived. Venous congestion was developed in 1 case of the rerverse flow sural artery flap but healed with secondary rotational flap. Other flaps were good without any complications. Conclusion: If we choose precisely indicated local flap in lower extremity reconstruction, the resultant coverage of defect would be excellent.

      • HCC : PE-040 ; The optimal cutoff level of serum AFP for HCC screening in patients taking entecavir

        Background: The cut-off value of alpha-feto protein(AFP) has been used for hepatocellular carcinoma(HCC) detection, although serum AFP level is not a good indicator. In patients taking entecavir(ETV) for chronic hepatitis B, serum AFP level seems to be lower than that of the patients not taking ETV. So, it can be supposed that the cut-off value of serum AFP for detecting HCC will be lower in the patients taking ETV. Method: There were 30 HCC cases that had taken ETV for treatment of chronic hepatitis B for 6 months or more. We used 207 non-HCC patients who had been on ETV as control group. The AFP level of control group was the highest level after 1 year taking entecavir. In each group, receiver operating characteristic (ROC) curves were compared to determine the optimal cut-off value for diagnosis of HCC. Result: The average duration of taking entecavir in case group is 731.5 days and the mean level of AFP level of HCC cases was 127.9 ng/mL. But in only 5 cases, AFP level was higher than 200 ng/mL. The optimal AFP cut-off value 8.6ng/mL, at which point the sensitivity and specificity was 63.3% and 96.1% respectively. The area under curve was 0.827. Conclusion: In this study, the best discriminating AFP value for HCC detection in patients taking entecavir was 8.6 ng/mL, which is much lower than that of those not taking ETV. Therefore, lowering serum AFP cut-off value for patients on ETV should be considered.
