대퇴골 경부골절은 불유합 및 대퇴골두의 무혈성괴사가 속발되는 예가 많아 아직도 해결 되지 않은 골절로 인정되고 있다. 저자들은 대퇴골 경부골절 치료중 속발된 고정기구의 붕괴, 불유합, 대퇴골두의 무혈성괴사등 대표적인 속발증을 차례로 일으켜 여러차례의 수술가료를 요하였던 교훈적인 예를 경험하였기에 여기 보고하는 바이다. Fracture of the femoral neck still presents many problems to orthopedic surgeons due to many complications, such as, nonunion and avascular necrosis of the femoral head. Authors report a case of femoral neck fracture for which multiple operative procedures, e.g. 1. closed reduction and internal fixation with sliding compression hip screw 2. musle pedicle bone graft using quadratus femoris 3. total hip joint replacement arthroplasty was necessary as he developed failure of the internal fixation apparatus, nonunion & finally aseptic necrosis.
정복되지 않는 Hohl 과 Moore의 제 2형 경골 고평부 골절-탈구의 수술적 치료의 3년 추시 결과 : 1예 보고 1 Case Report

Hohl 과 Moore 에 의해 정의된 경골 고평부 골절 탈구는 슬관절에 고 에너지의 외력이 작용하여 발생되며, 많은 경우에 슬와 동맥, 비골 신경, 십자 인대 손상이 동반된다. 본 저자들은 골절편이 정복되지 않는 골절성 탈수 제 2형을 경험하였다. 즉, 외측 경골과의 전체 골절이 있으며 후방 탈구 된 후, 내측 전위 되어 있고 비골 골두가 골절의 정복을 막으며 슬와 동맥이 골절 부위에 끼었던 Hohl 과 Moore의 제 2형을 경험하였다. 이에 후방 도달법과 비골두 절골술을 통한 관혈적 정복 및 내고정을 하여 3년간 추시 관찰 하였기에 보고 하는 바이다. Intraarticular proximal tibia fracture and dislocation patterns, that were proposed by Hohl and Moore, are generally caused by high energy mechanism. These fracture and dislocation patterns are known to be combined with injuries of popliteal artery, peroneal nerve cruciate ligament in many cases. We experienced a case irreducible Hohl and Moore fracture-dislocation patterns type 2, in which posterior dislocation with medially displced entire lateral condyle that could not be reduced by closed mean for fibular head blocking the proper reduction. Additionally, popliteal artery was entrapted into the fracture site. We carried out open reduction and screw fixation via posterior approach and with fibular osteomy. We followed the patient for 3 years.
남아있는 후방 십자 인대 다발을 보존하고 슬괵건을 이용한 후방 십자 인대 재건술

목적: 잔존하는 후방 십자 인대의 절단단을 유지하면서 슬괵건을 이용하여 후방 십자 인대 재건술을 시행하고 덜 공격적인 재활치료을 했을 때의 임상적 결과를 알아보고자 한다. 대상 및 방법: 후방 십자 인대 완전 파열 후 슬괵건을 이용하여 일차 재건술을 시행한 20례를 대상으로 하였다. 추시 기간은 평균 31개월이었고 동반 손상이 있는 경우가 9례, 단독 손상이 11례였다. 임상적인 결과로 술전 및 추시 중 후방 전위 측면 방사선 검사, Lysholm 점수, 합병증을 평가하였다. 결과: 후방 전위 검사는 술 전에 평균 14 mm이었으며 술 후에 평균 3.2 mm이었다. 술후 단독 손상의 경우 후방 전위가 평균 2.6 mm, Lysholm 점수는 평균 93.2점였으며 동반 인대 손상이 있는 경우 후방 전위가 평균 3.7 mm, Lysholm 점수는 평균 88점이었다. Lysholm 점수는 술 전에 평균 60점에서 술후 마지막 추시시 평균 92점으로 향상되었다. 손상후 3주 이내에 재건술을 실시한 경우 후방 전위가 평균 2.6 Lysholm이었고, 3주 이후에 재건술을 실시한 경우 3.7 mm의 후방 전위가 보였다. 결론: 슬괵건을 이용한 재건술을 시행함에 있어서 잔존하는 후방 십자 인대의 절단단을 가능한 보존하고 재건술 후 덜 공격적인 재활치료을 함으로써 양호한 슬관절의 안정성과 기능 회복을 얻을 수 있었다. Purpose: The purpose of study is to evaluate the clinical results of posterior cruciate ligament (PCL) reconstruction using the quadruple hamstring tendon with preservation of the PCL remnant. Subjects and Methods: Twenty PCL reconstructions have been performed by one surgeon. The average follow-up period were at 31 months. There were 11 isolated injuries and 9 associated injuries. The clinical outcome was assessed by preoperative and postoperative posterior displacement at stress radiograph, Lysholm knee score and complications. Results: At last follow up Lysholm knee scores improved from 60 preoperatively to 92 postoperatively. The average preoperative posterior displacement at stress radiograph was 14 mm and postoperatively it was 3.2 mm. The average postoperative posterior displacement at stress radiograph was 2.6 mm in isolate injury and 3.7 mm in associated injury. The average postoperative posterior displacement at stress radiograph was 2.6 mm in case of reconstruction within three weeks and 3.7 mm in case of reconstruction after three weeks. Conclusion: Good stability and function of knee could be obtained by PCL reconstruction using hamstring tendon with preserving the PCL remnant and non-aggressive rehabilitation.
