        Utilization of End-of-Life Care Rooms by Patients Who Died in a Single Hospice Unit at a National University Hospital in South Korea

        Purpose: For the dignity of patients nearing the end of their lives, it is essential to provide end-of-life (EoL) care in a separate, dedicated space. This study investigated the utilization of specialized rooms for dying patients within a hospice unit. Methods: This retrospective study examined patients who died in a single hospice unit between January 1, 2017, and December 31, 2021. Utilizing medical records, we analyzed the circumstances surrounding death, the employment of specialized rooms for terminally ill patients, and the characteristics of those who received EoL care in a shared room. Results: During the 1,825-day survey period, deaths occurred on 632 days, and 799 patients died. Of these patients, 496 (62.1%) received EoL care in a dedicated room. The average duration of using this dedicated space was 1.08 days. Meanwhile, 188 patients (23.5%) died in a shared room. Logistic regression analysis revealed that a longer stay in the hospice unit was associated with a lower risk of receiving EoL care in a shared room (odds ratio [OR]=0.98, 95% confidence interval [CI] 0.97~0.99; P=0.002). Furthermore, a higher number of deaths on the day a patient died was associated with a greater risk of receiving EoL care in a shared room (OR=1.66, 95% CI 1.33~2.08; P<0.001). Conclusion: To ensure that more patients receive EoL care for an adequate duration in a private setting, additional research is necessary to increase the number of dedicated rooms and incorporate them into the hospice unit at an early stage.

        Effects of an Online Imagery-Based Treatment Program in Patients with Workplace-Related Posttraumatic Stress Disorder: A Pilot Study

        Objective We developed easily accessible imagery-based treatment program for patients with post-traumatic stress disorder (PTSD) related to workplace accidents and investigated the effects of the program on various PTSD related symptoms. Methods The program was based on an online platform and consisted of eight 15-min sessions that included script-guided imagery and supportive music. Thirty-five patients with workplace-related PTSD participated in this program 4 days per week for 4 weeks. Its effects were examined using self-report questionnaires before and after the take-home online treatment sessions. Results After completing the 4-week treatment program, patients showed significant improvements in depressed mood (t=3.642, p=0.001) based on the Patient Health Questionnaire-9 (PHQ-9), anxiety (t=3.198, p=0.003) based on the Generalized Anxiety Disorder seven-item (GAD-7) scale, and PTSD symptoms (t=5.363, p<0.001) based on the Posttraumatic Stress Disorder Check List (PCL). In particular, patients with adverse childhood experiences exhibited a greater degree of relief related to anxiety and PTSD symptoms than those without adverse childhood experiences. Conclusion The present results demonstrated that the relatively short online imagery-based treatment program developed for this study had beneficial effects for patients with workplace-related PTSD.

        경상남도 웰니스관광의 염증면역지표, 자율신경균형, 정서, 피로, 스트레스, 삶의 질 및 수면의 질 개선에 미치는 단기효과

        Purpose : Recently, there has been a growing social interest in aging well. Consequently, wellness tourism has begun to attract attention. However, no studies on whether wellness tourism has any objective health benefits have been carried out yet. In this study, we assess the health benefits as well as the degree of improvement in health of a wellness tourism program. Methods : The study involved 30 adults over the age of 19 who live in the Gyeongsangnam-do region. Participants were evaluated on health indicator before and after participating in wellness tourism program. Participants took heart rate variability (HRV) test, and LFT, RFT, CBC, FBS HbA1C, and CRP test were conducted before and after the tour. Additionally, a survey was conducted before and after the program, and participant satisfaction was evaluated. Statistical differences in the tests conducted before and after the program were analyzed using a design t-test, a Wilcoxon signed-rank test, and McNemar’s test. Results : The study showed that participants were very satisfied with and had significant health improvements after the wellness tourism program. The program was also found to be beneficial in improving participants' emotions as follows: BDI (p<.001), fatigue recovery (p=.006), stress relief (p=.003), improved quality of life (p<.05), and improved sleep quality (p<.001). Conclusion : Wellness tourism programs are specifically beneficial for improving participants' emotions (depression, anxiety), fatigue, stress levels, quality of life, and sleep. Therefore, they are beneficial to the overall health. Further research in the future by way of a follow-up study on the long-term effects on health after short-term interventions will provide more validation data.

        Association between Sleep Duration and Presbycusis in Korean Adults: Korea National Health and Nutrition Examination Survey

        Background: Sleep duration is associated with hearing loss, especially presbycusis, which is the most common type of hearing loss; however, there is limited evidence regarding this association among the Korean population. We aimed to determine the relationship between sleep duration and high-frequency hearing loss in Korean adults aged ≥40 years.Methods: We examined 5,547 Korean adults aged ≥40 years who completed audiometric tests and questionnaires regarding sleep duration during the 2010–2012 cycle of the Korea National Health and Nutrition Examination Sur-vey. Mild presbycusis was defined as >25 decibels (dB) and <40 dB, whereas moderate-to-severe presbycusis was defined as >40 dB pure tone averages at high frequencies (3,000, 4,000, and 6,000 Hz) for both ears. Additionally, the sleep duration was divided into quartiles. Odds ratios and 95% confidence intervals were estimated using mul-tivariable logistic regression after adjusting for covariates.Results: The prevalence of presbycusis in South Korean adults was 62.1%, of which 61.4% showed moderate to se-vere presbycusis. The incidence of moderate-to-severe, but not mild, presbycusis showed a significant positive cor-relation with sleep duration.Conclusion: Our findings suggest that sleep duration is associated with the prevalence of presbycusis.

        Bioactivities of the Herb Extracts Used for Gamhongroju, a Korean Liqueur

        In this study, antioxidative and anti-inflammatory activities of the herb (cinnamon, clove, glehnia root, ginger, violet-root cromwell, licorice, citrus peel and longan) extracts used for gamhongroju, one of the popular liqueurs in Korea, were investigated. Twenty grams of individual herbs were extracted in 60% purified ethanol and freeze-dried. A mixture of the individual herb extracts (HEM) was separately prepared. Cytotoxicity of the individual extracts and HEM on murine RAW264.7 macrophage cells were examined along with their recovering activity on $H_2O_2$-treated RAW264.7 cells. Antioxidant and anti-inflammatory activities of the extract-treated cells were determined by measuring superoxide dismutase (SOD) and glutathione peroxidase (GPx) activities, and Trolox equivalent antioxidant capacity (TEAC), nitric oxide (NO) and prostaglandin E2 (PGE2) levels. Violet-root cromwell extract showed the least cytotoxicity in terms of treated concentration. Most of the extracts, below levels of cytotoxicity, recovered the $H_2O_2$-treated cells. Treatment with some of the extracts increased SOD and GPx activities and TEAC levels while a majority inhibited the production of NO and PGE2 in lipopolysaccharide (LPS)-treated cells.

        Vaccine Storage Practices and the Effects of Education in Some Private Medical Institutions

        Objectives: Although vaccination rates have increased, problems still remain in the storage and handling of vaccines. This study focused on inspecting actual vaccine storage status and awareness, and comparing them before and after education was provided. Methods: In the primary inspection, a status survey checklist was completed by visual inspection. A questionnaire on the awareness of proper vaccine storage and handling was also administered to vaccine administrators in private medical institutions in 4 regions in Gyeongsangbuk-province. One-on-one education was then carried out, and our self-produced manual on safe vaccine storage and management methods was provided. In the secondary inspection, the investigators visited the same medical institutions and used the same questionnaire and checklist used during the primary inspection. The results before and after education were compared, by treating each appropriate answer as 1 point. Results: The average checklists score was 9.74 (out of 15 points), which increased significantly after education was provided (by 0.84, p<0.001). The participants demonstrated improved practices in recording storage temperatures(p=0.016), storing vaccines in the center of the refrigerator (p=0.004), storing vaccines with other medication and nonmedical items (p=0.031) after education. The average score calculated from the questionnaires was 10.48 (out of 14points), which increased after education (by 1.03, p<0.001). Conclusions: This study suggests that vaccine storage practices and awareness are inadequate, but can be partially improved by providing relevant education. Repetitive education and policy-making are required to store vaccines safely because one-off education and unenforced guidelines offer limited efficacy.

        Selective hydrodechlorination of trichloromethane to dichloromethane over bimetallic Pt-Pd/KIT-6: Catalytic activity and reaction kinetics

        <P><B>Abstract</B></P> <P>Selective hydrodechlorination (HDC) of trichloromethane (TCM, CHCl<SUB>3</SUB>) to dichloromethane (DCM, CH<SUB>2</SUB>Cl<SUB>2</SUB>) was studied on the mesoporous bimetallic Pt-Pd/KIT-6 to verify the roles of the active bimetals to catalytic activity and product distribution. The optimal bimetal ratio of 2wt%Pt-0.05wt%Pd on the highly ordered mesoporous KIT-6 calcined at 400°C showed a maximum TCM conversion with DCM yield, and the optimal calcination temperature of Pt-Pd/KIT-6 was mainly responsible for the formation of homogeneously and closely distributed smaller Pt-Pd nanoparticles with higher oxidation states of Pt crystallites supported by XPS and EXAFS analysis. Based on the proposed kinetic models on the optimal Pt-Pd/KIT-6, the competitive reaction mechanisms of the formations of CH<SUB>4</SUB> and monochloromethane (MCM, CH<SUB>3</SUB>Cl) from DCM intermediate can well explain the product distributions. Using the seven different kinetic models, the best model on the basis of the Akaike’s Information Criteria showed the activation energies of the HDC of TCM to be in the range of 18–60kJ/mol. The kinetic model also revealed the inverse relationship between the conversion of TCM and selectivity to DCM. Either an increase of space velocity or decrease of H<SUB>2</SUB>/TCM ratio increased the DCM yield, while there existed the optimal operating conditions. The DCM productivity of 1.5mol-DCM/(g<SUB>cat</SUB>·h) was achievable at the highest space velocity and lowest H<SUB>2</SUB>/TCM ratio under a specific reaction condition of 190°C and 0.1MPa.</P> <P><B>Highlights</B></P> <P> <UL> <LI> HDC of TCM to DCM was studied on the mesoporous 2wt%Pt-0.05wt%Pd/KIT-6. </LI> <LI> At an optimal calcination T=400°C, a maximum DCM yield was obtained. </LI> <LI> Close interactions of smaller Pt-Pd nanoparticles with higher oxidation states are beneficial for a higher DCM yield. </LI> <LI> The kinetic model based on LHHW was well fitted with experimental results. </LI> <LI> Higher DCM productivity was achievable at the highest SV and lowest H<SUB>2</SUB>/TCM ratio. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

      • 조선 후기 은일 공간으로서의 충주

        본 연구는 조선 후기 문인들의 한시에 표상된 충주(忠州)의 이미지를 정리하여 충주가 정치적 좌절을 겪은 문인들의 미련과 좌절, 기대 등의 심리가 문학적으로 투영된 공간으로서 정치적·문화적으로 중요한 의미를 지녔음을 밝히고자 하였다. 충주는 예로부터 한양에 접근하기가 쉬웠으며, 풍부한 물산을 자랑하고, 산과 강이 어우러진 승경과 함께 탄금대와 같은 명승지를 완상할 수 있었다. 이러한 지리적인 특징으로 인해 많은 문인들이 충주를 은거지로 삼았다. 특히 기묘명현의 많은 이들이 충주에 은거하며 자신들만의 세력을 구축하였다. 조선 후기에 노론(老論) 핵심 인사들이 호서 지역에 머물게 되면서 충주는 기호사림(畿湖士林)의 학맥을 잇는 공간으로 자리 잡았다. 누암서원의 존재는 충주가 기호사림의 중심지였음을 알려준다. 조선 시대 문인들은 정치적 혼란기에 일시적으로 혹은 오랜 기간 중앙정계에 설 자리를 잃었을 때, 그들 나름대로 영향력을 행사할 만한 공간으로 충주를 택하였다. 조선 후기 문인들 중 많은 이들이 충주에 내려왔지만 대부분 충주에 은거하는 기간은 짧은 편이었다. 대부분 출사와 은거를 반복하였기 때문이다. 그중 정호(鄭澔)와 안중관(安重觀)은 노론 문인으로 충주가 고향이며, 정치적으로 불리할 때마다 충주에 은거하였다. 두 문인은 충주에 거주하며 많은 시를 남겼는데, 충주에서 누리는 아름다운 풍경과 한적함에 대한 만족감을 표출하는 한편, 자신의 뜻을 알아주지 못하는 현실에 대한 염증, 출사에 대한 미련을 표출하였다. 정호는 <sub>영의정 ‧ 좌의정을 </sub>역임할 정도로 중앙 관료로서 활약하였으며, 당시 노소(老少) 갈등의 중심에 있던 인물이었다. 정호는 은거를 자신의 뜻을 관철시키기 위한 의사표시의 수단으로 활용하였다. 그렇기에 언제든지 도성으로 돌아갈 수 있는 충주를 은거지로 택하였다. 안중관은 주로 외직에 있었으며, 그가 스스로 사직한 이유가 뚜렷하게 드러나지 않는다. 다만 그가 은거하며 쓴 시에는 그의 불우의식과 현실에 대한 염증이 짙게 드러난다. 안중관은 현실에 대한 체념으로 인해 은거를 택하였지만 도성을 향한 미련으로 인해 최후의 배수진으로서 도성과 가까운 충주에 머물렀던 것으로 보인다. 충주는 조선 후기 문인들에게 복잡한 현실 속에서 잠시 떨어져 여유로움과 위안을 얻어갈 수 있는 곳이자 언제든 출사의 기회를 놓치지 않을 만한 공간이었음을 확인하였다. This study summarized the image of Chungju (忠州) represented in the literati’s classical Chinese poetry in the late Joseon, and confirmed that Chungju had important political and cultural significance as a literary space in which the psychology of regret, frustration, and expectations of the literati who had suffered political setbacks was projected. Since ancient times it was easy to access Seoul from Chungju, and it was possible to appreciate famous sights such as Tangeumdae along with the scenery of the mountains and rivers. It was also an area where products were abundant. Due to these geographical features, many of the literati stayed in Chungju. In particular, many figures of victims of Kimyo Year Literati Purge secluded in Chungju and established their own influence. In the late Joseon, as key Noron figures stayed in the Hoseo region, Chungju also established itself as a space connecting Kiho Sarim’s academic vessel. The existence of Nuam-seowon indicates that Chungju was the center of Kiho Sarim. The literati in the Joseon era lost their place in central politics temporarily or for a long period of time during the political turmoil, but they chose Chungju as a space where they could exert their own influence. Many of the literati in the late Joseon came to Chungju, but there was a tendency that most of them were secluded in Chungju for a short period of time. Most of these repeated going to office and being secluded. Among the literati in the late Joseon, Jeongho(鄭澔) and Ahn Jung-gwan (安重觀) were the Noron literati, and were secluded into Chungju as their hometown whenever they were politically disadvantaged. The two literati lived in Chungju and left many poems, and while expressing satisfaction with the beautiful scenery and serenity enjoyed in Chungju, they expressed their disgust about the reality of not being able to understand their intentions, and their lingering about going to office. Jeongho was active as a central bureaucrat to the extent that he served as Yeonguijeong-Jwauijeong, and he was at the center of the noron-soron conflict at the time. Jeongho’s seclusion was utilized as a means of expressing his will to carry out his own will. Thus, that’s why he chose to be secluded in Chungju, where he could return to Seoul at any time. Ahn Jung-gwan was mainly employed at a government post away from the capital, and it is not clear why he resigned himself. However, his poems that he wrote while he was secluded deeply reveal his awareness of misfortune and his disgust of reality. Although Ahn Jung-gwan chose to be secluded because of his resignation from reality, he seems to have stayed in Chungju, close to Seoul, as a position taken up with a river behind the troops because of his lingering toward Seoul. It was confirmed that Chungju was a place where the literati in the late Joseon could get relaxation and solace from the complex reality for a while, and a space where they could not miss an opportunity to go to office at any time. Through this, the literati’s awareness and literary discourse in the late Joseon was investigated in a space outside of Seoul, a symbol of central power.
