        Investigating the Prevalence of Dementia and Its Associated Risk Factors in a Chinese Nursing Home

        Background and Purpose The objective of this study was to determine the prevalence of and risk factors for dementia in residents aged 65 years and older in a Chinese nursing home. Methods A cross-sectional study was carried out in a nursing home located in Hangzhou, Zhejiang Province, China. Cognitive status, including the presence of dementia and mild cognitive impairment (MCI), was measured using a combination of medical history and objective cognitive assessments. Logistic regression analysis was conducted to predict the associated risk factors. Results In total, 943 residents (334 males and 609 females) aged 84.00±6.67 years (mean± SD) were included. Dementia was diagnosed in 420 (44.5%) residents, and MCI was diagnosed in 195 (20.7%). Mild, moderate, and severe dementia were present in 20.3%, 14.0%, and 65.7% of those in the dementia group, respectively. Logistic regression analysis revealed that dementia was associated with a low education level (p=0.000), an advanced age (p=0.010), and a history of stroke (p=0.023). Conclusions Our study found a high prevalence of dementia in a Chinese nursing home, and a high prevalence of patients with severe dementia. Risk factors for dementia included a low educational level, an advanced age, and a history of stroke. Appropriate interventions need to be applied to this population.

        Candesartan-graft-polyethyleneimine Cationic Micelles for Effective Co-delivery of Drug and Gene in Anti-angiogenic Lung Cancer Therapy

        A multifunctional polymer-drug conjugate candesartan-graft-polyethyleneimine (CD-PEI, CP) containing low molecular weight polyethyleneimine (PEI) and candesartan (CD) conjugated via an amide bond was fabricated as a co-delivery micelle of drug and siRNA for potential lung cancer therapy. Here, CD as an angiotensin II type 1 receptor blocker rich in imidazole and tetrazole rings was utilized to strengthen endosomal buffering capacity of CP and suppress tumor angiogenesis. The selfassembled CP/siRNA complexes exhibited desirable and homogenous particle size, moderate positive charges, and efficient release of drug and siRNA in vitro. In addition, CD and siRNA could readily detached from nanovectors in tumor cells via an amidase-responsive mechanism and they achieved synergistic anti-angiogenesis efficacy by effectively downregulating the expression of vascular endothelial growth factor (VEGF) mRNA and protein via different pathways in vitro. In vivo investigation on nude mice bearing A549 tumor xenografts revealed that CP/siRNA complexes possessed strong antitumor activity. These findings suggested that CP could be an ideal nanovector for simultaneous transfer of drug and siRNA, and a multifunctional CP/siRNA co-delivery system with enhanced endosomal buffering capacity, amidase-responsive drug release and synergistic anti-angiogenesis efficacy might be a new promising strategy for effective lung cancer therapy.

        71% Common‑mode voltage suppression modulation strategy for indirect matrix converters

        Common-mode voltage (CMV) with a high amplitude and frequency is generated when an indirect matrix converter (IMC) is operating, which damages the motor winding insulation and accelerates motor aging. Existing IMC modulation methods can only suppress the CMV by 42.3%. However, this paper proposes a modulation strategy with a 71% reduction in the peak CMV. The proposed method selects active vectors based on the characteristics of the CMV amplitude under each of the active vectors. The rectifier stage selects two active vectors within each modulation sector. According to the sector where the input reference current vector is located, the inverter stage chooses active-voltage vectors whose corresponding peak CMV is 1/3 the minimum peak line voltage for modulation. Moreover, the CMV spikes caused by the dead zone effect are eliminated by reasonably arranging the switching sequence of the voltage vectors in the inverter stage. The proposed approach considerably lowers the peak CMV and has a good suppression effect on the amplitude of high-frequency CMV. Finally, experimental results illustrate the CMV reduction efficiency of the modulation strategy.

        Prediabetes Progression and Regression on Objectively- Measured Physical Function: A Prospective Cohort Study

        Background: Prediabetes leads to declines in physical function in older adults, but the impact of prediabetes progression or regression on physical function is unknown. This study assessed this longitudinal association, with physical function objectivelymeasured by grip strength, walking speed, and standing balance, based on the Health and Retirement Study enrolling United States adults aged >50 years.Methods: Participants with prediabetes were followed-up for 4-year to ascertain prediabetes status alteration (maintained, regressed, or progressed), and another 4-year to assess their impacts on physical function. Weak grip strength was defined as <26 kg for men and <16 kg for women, slow walking speed was as <0.8 m/sec, and poor standing balance was as an uncompleted fulltandem standing testing. Logistic and linear regression analyses were performed.Results: Of the included 1,511 participants with prediabetes, 700 maintained as prediabetes, 306 progressed to diabetes, and 505 regressed to normoglycemia over 4 years. Grip strength and walking speed were declined from baseline during the 4-year followup, regardless of prediabetes status alteration. Compared with prediabetes maintenance, prediabetes progression increased the odds of developing weak grip strength by 89% (95% confidence interval [CI], 0.04 to 2.44) and exhibited larger declines in grip strength by 0.85 kg (95% CI, –1.65 to –0.04). However, prediabetes progression was not related to impairments in walking speed or standing balance. Prediabetes regression also did not affect any measures of physical function.Conclusion: Prediabetes progression accelerates grip strength decline in aging population, while prediabetes regression may not prevent physical function decline due to aging.

        Normalized Creatinine-to-Cystatin C Ratio and Risk of Diabetes in Middle-Aged and Older Adults: The China Health and Retirement Longitudinal Study

        Background: Creatinine-to-cystatin C ratio is recently suggested to be a surrogate marker for sarcopenia. However, little is known about its association with diabetes. This study aimed to fill in this gap based on a large-scale prospective cohort.Methods: A population-based representative sample of 5,055 participants aged ≥45 years from the China Health and Retirement Longitudinal Study was enrolled between 2011 and 2012 and followed at least once during the subsequent surveys at 2013, 2015, or 2018. Creatinine-to-cystatin C ratio was calculated and normalized by body weight. Incident diabetes was ascertained by plasma glucose, glycosylated hemoglobin, self-reported history, or use of anti-diabetic drugs. Logistic regression analysis and mediation analysis were employed.Results: During follow-up, 634 participants developed diabetes. The risk of diabetes was gradually and significantly decreased with increased normalized creatinine–cystatin C ratio. The multivariable-adjusted odds ratio for diabetes was 0.91 (95% confidence interval, 0.83 to 0.99) per 1 standard deviation higher of normalized creatinine-to-cystatin C ratio, and this relationship remained significant after controlling for muscle strength. The risk reduction in diabetes was significantly larger in participants with normal-weight and high normalized creatinine-to-cystatin C ratio compared with those with overweight/obesity and high normalized creatinine-to-cystatin C ratio (Pinteraction=0.01). Insulin resistance and inflammation appeared to be key mediators accounting for the observed relationship between normalized creatinine-to-cystatin C ratio and risk of diabetes, with their mediating effect being 93.1% and 22.0%, respectively.Conclusion: High normalized creatinine-to-cystatin C ratio is associated with reduced risk of diabetes in middle-aged and older adults.

        Controllable synthesis of zone-distributed Pd over CeO2–ZrO2/Al2O3 as advanced three-way catalyst

        Two optimally designed catalysts (Ce0.7Zr0.3O2/Pd/Al2O3, Pd/Ce0.7Zr0.3O2/Pd/Al2O3) were synthesized and compared with conventional Pd/Ce0.7Zr0.3O2/Al2O3. The characterization results reveal that for Pd/Ce0.7Zr0.3O2/Al2O3, Pd species undergo severe agglomeration and encapsulation during thermal aging process, leading to dramatic catalyst deactivation. While for the two optimized catalysts, some Pd species initially distributed on Al2O3 would migrate to the surface of CexZr1−xO2(CZ) grains upon aging treatment, resulting in enhanced Pd–CZ interaction, thus the two aged samples possess higher dispersion, larger amount of oxidized Pd2+ species and more oxygen vacancies compared with conventional Pd/Ce0.7Zr0.3O2/Al2O3-a. Consequently, modified reducibility and improved three-way catalytic performance are obtained, especially for Pd/Ce0.7Zr0.3O2/Pd/Al2O3-a.

      • Analysis and Research on Space Vector Over-modulation Strategy of Indirect Matrix Converter

        In this paper, based on the characteristics of six space vector modulation methods, a variety of Space Vector Over-modulation strategies with voltage transfer ratio of 1.05 for indirect matrix converter is proposed. Firstly, the six space vector modulation methods are combined by adjusting the phase of the input current vector and the amplitude and phase of the output voltage vector. Secondly, the limit input current and output voltage vector characteristics, the limit fundamental voltage amplitude and the main low-frequency harmonic components of the input and output under the six modulation methods are analyzed in detail. According to the limit characteristics of the six modulation methods and the principle of limit vector superposition, multiple space vector over-modulation strategies based on single-mode, doublemode and multi-mode are proposed. Finally, the characteristics of the six space vector modulation methods are verified by experiments.

        Performance Degradation Model and Reliability Evaluation of Brush DC Motor for the Intelligent on–Off Valve

        Application of the intelligent on–off valve reduces heat loss. As the critical control component of the intelligent on–off valve, the health state of the brush DC motor directly impacts whether the intelligent on–off valve can perform the regular operation. Therefore, the reliability evaluation of the brush DC motor is of great significance to the whole heat supply network. In this study, the failure mechanism of the brush DC motor is analyzed, and the absolute value of steady-state current variation is taken as the performance degradation characteristic parameter. According to the performance degradation characteristic parameter, the performance degradation model based on the Wiener process is established, and the inverse power rate acceleration model is introduced into the Wiener process to derive the reliability function of the motor. Based on the actual operating conditions that the motor in the intelligent on–off valve needs to start and stop frequently, an accelerated life test based on start-stop is designed. The error analysis result shows that the error between the expected start-stop times predicted by the proposed and the actual average failure start-stop times is only 2.14%. The proposed degradation model is more accurate than the motor's traditional performance degradation model.

        Enhanced activity and hydrothermal stability of Rh-based three-way catalyst for emission control from motorcycles with the assistance of monoethanolamine

        In this work, an advanced Rh/CeO2–ZrO2 (Rh/CZ) three-way catalyst was synthesized with the assistanceof monoethanolamine. Compared with conventional Rh/CZ, the dispersion and oxidation state of Rhspecies, as well as the hydrothermal aging resistance are remarkably modified. That is, upon agingtreatment, conventional Rh/CZ undergoes severe agglomeration of Rh species and formation of largeamount of reduction-resistive RhO2, resulting in serious catalyst deactivation. Fortunately, after theintroduction of monoethanolamine, the sintering of Rh species and transformation into inactive RhO2upon aging are effectively suppressed. Consequently, improved reducibility and advanced three-waycatalytic activity are achieved for the modified catalyst.

        Four-year changes in central fatness, risk of diabetes, and metabolic control in older adults: a cohort study with mediation analysis

        Background/Aims: Older adults are vulnerable to central obesity, while the association of changes in central fatness with risk of diabetes and metabolic control has not been investigated among this particular population. This study was aimed to address these issues. Methods: A total of 1,815 adults aged ≥ 60 years without diabetes at baseline were followed for 4 years. Incident diabetes was ascertained based on plasma glucose, hemoglobin A1c, medical history, and/or the use of anti-diabetic drugs. Central fatness was assessed by waist circumference (WC), waist-height ratio (WHtR), and body roundness index (BRI). Logistic regression analyses were used to assess the association of changes in central fatness with risk of diabetes, along with dose-response and mediation analyses. Results: During the 4-year follow-up, 177 participants developed diabetes. The risk of diabetes was increased by 42%, 41%, and 40% per 1 standard deviation increases in WC, WHtR, and BRI, respectively, in multivariable-adjusted models (all p < 0.01). Moreover, these relationships were all linearly-shaped (all p<sub>nonlinearity</sub> ≥ 0.11). Increases in WC, WHtR, and BRI correlated with increases in hemoglobin A1c, triglycerides-and-glucose index, triglycerides, white blood cell, and C-reactive protein (all p ≤ 0.04). Yet only changes in hemoglobin A1c and triglycerides-and-glucose index were identified as the possible mediators for risk of diabetes, with their mediating effect being about 35% and 21%, respectively. Conclusions: Increases in central fatness were related to elevated risk of diabetes, and this association might be partly explained by the worsening of glycemic control and insulin resistance in older adults.
