Investigating the Prevalence of Dementia and Its Associated Risk Factors in a Chinese Nursing Home

Background and Purpose The objective of this study was to determine the prevalence of and risk factors for dementia in residents aged 65 years and older in a Chinese nursing home. Methods A cross-sectional study was carried out in a nursing home located in Hangzhou, Zhejiang Province, China. Cognitive status, including the presence of dementia and mild cognitive impairment (MCI), was measured using a combination of medical history and objective cognitive assessments. Logistic regression analysis was conducted to predict the associated risk factors. Results In total, 943 residents (334 males and 609 females) aged 84.00±6.67 years (mean± SD) were included. Dementia was diagnosed in 420 (44.5%) residents, and MCI was diagnosed in 195 (20.7%). Mild, moderate, and severe dementia were present in 20.3%, 14.0%, and 65.7% of those in the dementia group, respectively. Logistic regression analysis revealed that dementia was associated with a low education level (p=0.000), an advanced age (p=0.010), and a history of stroke (p=0.023). Conclusions Our study found a high prevalence of dementia in a Chinese nursing home, and a high prevalence of patients with severe dementia. Risk factors for dementia included a low educational level, an advanced age, and a history of stroke. Appropriate interventions need to be applied to this population.

A multifunctional polymer-drug conjugate candesartan-graft-polyethyleneimine (CD-PEI, CP) containing low molecular weight polyethyleneimine (PEI) and candesartan (CD) conjugated via an amide bond was fabricated as a co-delivery micelle of drug and siRNA for potential lung cancer therapy. Here, CD as an angiotensin II type 1 receptor blocker rich in imidazole and tetrazole rings was utilized to strengthen endosomal buffering capacity of CP and suppress tumor angiogenesis. The selfassembled CP/siRNA complexes exhibited desirable and homogenous particle size, moderate positive charges, and efficient release of drug and siRNA in vitro. In addition, CD and siRNA could readily detached from nanovectors in tumor cells via an amidase-responsive mechanism and they achieved synergistic anti-angiogenesis efficacy by effectively downregulating the expression of vascular endothelial growth factor (VEGF) mRNA and protein via different pathways in vitro. In vivo investigation on nude mice bearing A549 tumor xenografts revealed that CP/siRNA complexes possessed strong antitumor activity. These findings suggested that CP could be an ideal nanovector for simultaneous transfer of drug and siRNA, and a multifunctional CP/siRNA co-delivery system with enhanced endosomal buffering capacity, amidase-responsive drug release and synergistic anti-angiogenesis efficacy might be a new promising strategy for effective lung cancer therapy.

Background: Prediabetes leads to declines in physical function in older adults, but the impact of prediabetes progression or regression on physical function is unknown. This study assessed this longitudinal association, with physical function objectivelymeasured by grip strength, walking speed, and standing balance, based on the Health and Retirement Study enrolling United States adults aged >50 years.Methods: Participants with prediabetes were followed-up for 4-year to ascertain prediabetes status alteration (maintained, regressed, or progressed), and another 4-year to assess their impacts on physical function. Weak grip strength was defined as <26 kg for men and <16 kg for women, slow walking speed was as <0.8 m/sec, and poor standing balance was as an uncompleted fulltandem standing testing. Logistic and linear regression analyses were performed.Results: Of the included 1,511 participants with prediabetes, 700 maintained as prediabetes, 306 progressed to diabetes, and 505 regressed to normoglycemia over 4 years. Grip strength and walking speed were declined from baseline during the 4-year followup, regardless of prediabetes status alteration. Compared with prediabetes maintenance, prediabetes progression increased the odds of developing weak grip strength by 89% (95% confidence interval [CI], 0.04 to 2.44) and exhibited larger declines in grip strength by 0.85 kg (95% CI, –1.65 to –0.04). However, prediabetes progression was not related to impairments in walking speed or standing balance. Prediabetes regression also did not affect any measures of physical function.Conclusion: Prediabetes progression accelerates grip strength decline in aging population, while prediabetes regression may not prevent physical function decline due to aging.
Controllable synthesis of zone-distributed Pd over CeO2–ZrO2/Al2O3 as advanced three-way catalyst

Two optimally designed catalysts (Ce0.7Zr0.3O2/Pd/Al2O3, Pd/Ce0.7Zr0.3O2/Pd/Al2O3) were synthesized and compared with conventional Pd/Ce0.7Zr0.3O2/Al2O3. The characterization results reveal that for Pd/Ce0.7Zr0.3O2/Al2O3, Pd species undergo severe agglomeration and encapsulation during thermal aging process, leading to dramatic catalyst deactivation. While for the two optimized catalysts, some Pd species initially distributed on Al2O3 would migrate to the surface of CexZr1−xO2(CZ) grains upon aging treatment, resulting in enhanced Pd–CZ interaction, thus the two aged samples possess higher dispersion, larger amount of oxidized Pd2+ species and more oxygen vacancies compared with conventional Pd/Ce0.7Zr0.3O2/Al2O3-a. Consequently, modified reducibility and improved three-way catalytic performance are obtained, especially for Pd/Ce0.7Zr0.3O2/Pd/Al2O3-a.

In this work, an advanced Rh/CeO2–ZrO2 (Rh/CZ) three-way catalyst was synthesized with the assistanceof monoethanolamine. Compared with conventional Rh/CZ, the dispersion and oxidation state of Rhspecies, as well as the hydrothermal aging resistance are remarkably modified. That is, upon agingtreatment, conventional Rh/CZ undergoes severe agglomeration of Rh species and formation of largeamount of reduction-resistive RhO2, resulting in serious catalyst deactivation. Fortunately, after theintroduction of monoethanolamine, the sintering of Rh species and transformation into inactive RhO2upon aging are effectively suppressed. Consequently, improved reducibility and advanced three-waycatalytic activity are achieved for the modified catalyst.
