        Distal Femoral Medial Opening Wedge Osteotomy for Post-Traumatic, Distal Femoral Varus Deformity

        Restoration of neutral mechanical alignment of the lower limb is an important factor in the treatment of unicompartmental arthrosis. Traditionally, medial opening wedge high tibial osteotomy has been widely performed to correct varus malalignment with unicompartmental arthrosis. However, an ideal indication for the high tibial osteotomy is the knee with metaphyseal tibial varus malalignment. The basic principle of corrective osteotomy is performing an osteotomy at the center of the deformity to prevent abnormal joint line obliquity. If pathologic distal femoral varus deformity is the cause of genu varum, the osteotomy should be performed in the distal femur. Reports of medial opening wedge distal femoral osteotomy (DFO) to correct varus malalignment are rare. We present a case of this very rare and challenging condition in a 47­year­old male, which was successfully treated by medial opening wedge DFO.

        Sucrose fatty acid ester가 약과 품질에 미치는 효과

        This study was performed to know effect of Yachwa quality when sucrose fatty acid esters(S-570, S-970, S-1570) were added as an emulsifier. In mechanical characteristics, the Yackwa of sucrose fatty acid esters did greatly decrease the hardness, cohesiveness, springiness and gumminess, respectively, and increase the brittleness compared to the Yackwa fof non-emulsifier and egg yolk. This tendency showed at the above level of 1.0% S-570, and 0.5% S-970 and S-1570, respectively. In sensory evaluation, the Yackwa of sucrose fatty acid esters increased the softness compared to the Yackwa of non-emulsifiers and egg yolk. There were significantly differences in the preference at 0.5% level of S-970 would be mostly useful level of emulsifier for making Yackwa.

        The incidence of double primary cancer is about 10% in all kinds of cancers. Double primary cancer is usually accompanied by gastrointestinal cancer. Esophageal cancer is supposed to be frequently accompanied by squamous cell carcinoma of the head and neck or the upper respiratory tract, because of such common carcinogens as smoking and alcohol ingestion. Esophageal cancer and hypopharyngeal cancer are usually diagnosed by esophagogastric endoscopy with biopsy. We performed endoscopic ultrasound-guided Trucut biopsy(EUS-TCB) to diagnose a mediastinal mass in a 49 year-old patient, who visited our hospital because of voice change. During procedure, we accidently found enlarged celiac lymph node, and obtained tissue from the mediastinal lymph node and the celiac lymph node. We confirmed double primary cancer of squamous cell carcinoma in the esophagus and the hypopharynx via esophagogastric endoscopy.

      • 배선기구의 접촉불량으로 인한 전기화재 위험성에 관한 연구

        각종 화재원인 중에서 차단기 및 터미널 등의 배선기구의 접촉불량으로 인한 전기화재가 증가하고 있는 추세이다. 본 논문에서는 매년 증가 추세에 있는 접촉불량으로 인한 전기화재 위험성에 관해 발생현황을 살펴보고, 저기화재 판별의 주요한 원인인 아산화동의 증식 발열현상에 대해서 살펴본다. 아산화동의 증식발열이 전기화재의 원인임을 밝히기 위하여 출화된 차단기의 단자에서 빼낸 전선을 확대경으로 관찰하여 아산화동에서만 발견되는 특징을 제시한다. 또한 접촉불량으로 인한 전기화재를 예방하기 위한 예방대책을 제시한다. Electrical fires due to the weakness of the electrical contact is being increased. In this paper, the various electrical fires due to the weakness of the electrical contact are investigated. In addition, the characteristics of Cu20 which is main cause of electrical fire are investigated. Further, the methods to prevent the electrical fires due to the weakness of the electrical contact are proposed.

        주기적인 운동이 뒷다리 부유에 의해 유발된 쥐의 뒷다리근 위축경감에 미치는 영향

        The purpose of this study was to determine the effect of periodic low-intensity exercise during hindlimb suspension on the mass, relative weight, myofibrillar protein content in soleus, plantaris and gastrocnemius muscles. To examine the effectiveness of periodic low-intensity exercise on mass, and myofibrillar protein content of hindlimb muscles, adult female Wistar rats were suspended(HS) and half of these rats walked on a treadmill for 45min/day(15 min every 4h) at 5m/min and a 15˚ grade(HS-EX). Soleus wet weight was 33.51% significantly smaller(p<0.005) and relative soleus weight of hindlimb suspended rats was 31.96% smaller(p<0.005) compared with those of control rats following seven days of hindlimb suspension. Plantaris wet weight was 7.5% smaller(p<0.01) and relative plantaris weight was 11.83% smaller(p<0.05) compared with those of control rats following seven days of hindlimb suspension. Gastrocnemius wet weight was 11.31% significantly smaller(p<0.005) and relative gastrocnemius weight was 17.13% significantly smaller(p<0.005) compared with those of control rats following seven days of hindlimb suspension. Soleus wet weight while increased by relative soleus weight increased by 25.13%, 27.59% each through periodic low intensity exercise during hindlimb suspension(p<0.05, p<0.05). Plantaris wet weight and relative plantaris weight increased by 1.04%, 10.98%(p<0.05) each, and gastrocnemius wet weight and relative gastrocnemius weight increased by 1.98%, 12.02%(p<0.05) each through periodic low intensity exercise during hindlimb suspension. Wet weight of soleus, plantaris and gastrocnemius in HS-EX rats did not recover to control level. Myofibrillar protein content of soleus, plantaris and gastrocnemius was 48.24%, 40.85% and 37.33% significantly smaller(p<0.005) respectively compared with those of control rats following seven days of hindlimb suspension. Myofibrillar protein content of soleus, plantaris and gastrocnemius increased by 40.68%, 25.07% and 17.93%(p<0.005) each through periodic low intensity exercise during hindlimb suspension. Myofibrillar protein content of soleus, plantaris, and gastrocnemius in HS-EX rats did not recover to control level. The results suggest that periodic low intensity exercise can attenuate hindlimb muscle atrophy induced by hindlimb suspension.

        스테로이드 치료 후 회복기의 규칙적인 운동이 쥐의 스테로이드 유발성 위축 뒷다리근에 미치는 영향

        This study was conducted to determine whether low intensity regular exercise following steroid treatment could attenuate steroid-induced muscle atrophy. Thirty-eight Sprague-Dawley rats weighing 165~175g were divided into six groups;control group(C), dexamethasone administration group(D), sedentary after normal saline administration group(C+Se), exercise after normal saline administration group(C+Ex), sedentary group after dexamethasone administration(D+Se), exercise group after dexamethasone administration(D+Ex). Either dexamethasone(5mg/kg) or normal saline was injected for 7days accordingly. Exercise was started at 10m/min on the 10?grade treadmill and gradually increased up to 15m/min by the 7th day for 60minutes/day(20min×3). The data were analyzed by Kruskal-Wallis test and Mann-Whitney U test using the SPSS WIN 9.0 program. Body weight, muscle weight and myofibrillar protein content of both plantaris and gastrocnemius, Type I, II muscle fiber cross-sectional area of plantaris, and Type II muscle fiber cross-sectional area of gastrocnemis in D group were significantly lower than those of C group(p<.05) respectively. Hindlimb muscle weight, myofibrillar protein content of both plantaris and gastrocnemius, Type I muscle fiber cross-sectional area of soleus and Type I, II muscle fiber cross-sectional area of plantaris in D+Ex group tended to increase compared to those of D+Se group. Myofibrillar protein content of both plantaris and gastrocnemius, Type I muscle fiber cross-sectional area of plantaris in D+Ex group tended to increase compared to those of C+Se group. Based on these results, it is suggested that regular low-intensity exercise during recovery period after steroid treatment might facilitate the recovery from steroid-induced muscle atrophy.

        The deterioration ratio of flexural capacity in the beams reinforced with carbon fiber sheets due to the gap between concrete surface and sheet is presented through the experimental study using full-scale members. Experiment results show that bond failure occurred in all specimens. The deterioration ratios of flexural capacity in the specimens reinforced on the bottom are 6%, 8% and 11% when the gap ratios are 10%, 20%, and 30%, respectively. Also, the deterioration ratios of flexural capacity in the specimens reinforced on the bottom and the sides are 5%, 8%, 11% and 16% when the gap ratios are 10%. 20%, 30% and 40%. respectively.

      • 태권도장 수련생 관여도가 도장경영 활성화에 미치는 영향

        The result of research marketing satisfaction by element by the practice students was as following. First, in men case, as schooltime was prolonged, participation was rising. However, in women case, It was the reverse result. It meant realization about Taekwondo by men was higher than women's. Second, among the practice students, in elementary and high school's students case, schooltime was long, participation rate was long, in middle school students case. It was the reverse result. Third, the most important thing for product satisfaction by practice students was facilities environment of Taekwondo. Fourth, in price satisfaction by the practice students case, most of students did not complain about equipment purchase. Also, satisfaction about price did not display big difference, because Price of all Taekwondo's registration fee was equal. Fifth, realization means of Taekwondo exerted the biggest effect on promotion satisfaction by the practice students. sixth, access easy exerted the biggest effect on place satisfaction by the practice students. It meant students chose near place for Taekwondo. Through marketing satisfaction by element by the practice students, hereafter, I wish to propose marketing operation for Taekwondo gymnasium. First, Taekwondo need to make a new program for women because, as women students' schooltime was long, participation rates was low. Also, It needs to develope the reasonable business management and programs for high participation rates. Second, because access easy was most important thing to registrate, Taekwondo Gymnasium has to preoccupy the best position. Also, It needs to develope the facilities service like vehicles running service

        In rescent years, there have been needs of accessing spatial data from distributed and pre-existing spatial database systems interconnected through a network. In a distributed environment, spatial joins for two spatial relations residing in geographically seperated sites are expensive in terms of computation and transmission cost because of the large size and complexity of spatial data Previous distributed improvements by eliminating objects before transmission to reduce both transmission and local processing cost But with a widespread of a high bandwidth data transmission, the parallelism through data redistribution may improve the performance of spatial join processing for distributed spatial database We apply the task distribution using a graph' partitioning method In experiments, we show that the proposed method provides useful reductions in the cost of evaluating a join.
