Purpose: Rectal prolapse is still a relatively understudied medical condition, especially in women, whereas physical symptoms, depressive symptoms, and quality of life (QOL) in women with pelvic organ prolapse have been steadily studied. This study aimed to examine the interaction and main effects of physical and depressive symptoms on physical and mental QOL of women seeking care for rectal prolapse in Korea. Methods: Ninety-two women with rectal prolapse were recruited from a colorectal surgery clinic of a tertiary teaching hospital in Gwangju, Korea. Physical symptoms related to rectal prolapse (pelvic organ prolapse distress, POPD; colorectal-anal distress, CRAD; and urinary distress, UD), depression, and QOL were measured. The data were analyzed using descriptive statistics, Pearson correlation coefficient, and two-way analysis of variance. Results: The interaction between POPD symptoms and depressive symptoms (F=4.51, p=.037) affected physical QOL. The interaction between POPD (F=9.66, p=.003) and CRAD symptoms (F=7.48, p=.008), respectively, and depressive symptoms affected mental QOL. Depressive symptoms had a significant main effect on the physical QOL in the CRAD (F=6.22, p=.014) and UD (F=6.01, p=.016) groups and on mental QOL in the UD group (F=24.54, p<.001). Conclusion: Physical and depressive symptoms should be considered together to improve the QOL in women with rectal prolapse. Nursing interventions and medical treatments to decrease rectal prolapse-related physical and depressive symptoms are required to improve QOL in women with symptomatic rectal prolapse.

Effects of quercetin inhalation on immediate (IAR), late-phase (LAR) and late late-phase (LLAR) asthmatic responses by exposure to aerosolized-ovalbumin (AOA) (2w/v% in saline, inhalation for 3 min) were studied in conscious guinea-pigs sensitized with AOA. We measured specific airway resistance (sRaw), and recruitment of leukocytes, histamine and protein contents and phospholipase A2 (PLA2) activity in bronchoalveolar lavage fluid (BALF). Effects of quercetin (10 mg/mL, inhalation for 2 min) compared with cromolyn sodium, salbutamol, and dexamethasone inhalations, respectively. Quercetin inhalation decreased sRaw by 57.15 ± 3.82% in IAR, 57.72 ± 7.28% in LAR, and 55.20 ± 5.69% in LLAR compared with AOA-inhaled control. Salbutamol inhalation (5 mg/mL) significantly inhibited sRaw in IAR, but inhalations of cromolyn sodium (10 mg/mL) and dexamethasone (5 mg/mL) significantly inhibited sRaw in IAR, LAR and LLAR, respectively. Inhibitory activity of quercetin inhalation on sRaw was similar to effect of its oral administration (10 mg/kg) in asthmatic responses. Quercetin (10 mg/mL, inhalation for 2 min) significantly decreased histamine and protein contents, PLA2 activity, and recruitments of leukocytes in BALF and also improved slightly infiltration of eosinophils and neutrophils in histopathological survey. Its anti-asthmatic activity was similar to cromolyn sodium and dexamethasone.
Deep learning framework for bovine iris segmentation

Iris segmentation is an initial step for identifying the biometrics of animals when establishing a traceability system for livestock. In this study, we propose a deep learning framework for pixel-wise segmentation of bovine iris with a minimized use of annotation labels utilizing the BovineAAEyes80 public dataset. The proposed image segmentation framework encompasses data collection, data preparation, data augmentation selection, training of 15 deep neural network (DNN) models with varying encoder backbones and segmentation decoder DNNs, and evaluation of the models using multiple metrics and graphical segmentation results. This framework aims to provide comprehensive and in-depth information on each model’s training and testing outcomes to optimize bovine iris segmentation performance. In the experiment, U-Net with a VGG16 backbone was identified as the optimal combination of encoder and decoder models for the dataset, achieving an accuracy and dice coefficient score of 99.50% and 98.35%, respectively. Notably, the selected model accurately segmented even corrupted images without proper annotation data. This study contributes to the advancement of iris segmentation and the establishment of a reliable DNN training framework.
Prevalence and impact of airway diseases on clinical outcomes in idiopathic pulmonary fibrosis

Background/Aims: The prevalence and effects of airway diseases, including asthma, eosinophilic bronchitis (EB), chronic obstructive pulmonary disease (COPD), and asthma-COPD overlap (ACO) have not been thoroughly studied in patients with idiopathic pulmonary fibrosis (IPF). This study aimed to evaluate the prevalence of airway diseases in patients with IPF and to identify the differences in symptoms based on the presence of airway diseases. Methods: This single-institution prospective cohort study was conducted from June 2017 to September 2018, at the Seoul National University Hospital. Spirometry with bronchodilator, methacholine bronchial provocation test, induced sputum with eosinophil stain, and exhaled nitric oxide were performed to confirm the presence of airway disease. The modified Medical Research Council (mMRC) dyspnea scale, COPD assessment test (CAT), St. George’s Respiratory Questionnaire (SGRQ), EuroQol-5 dimension (EQ-5D) index, and cough-specific quality of life questionnaire (CQLQ) data were collected to assess symptom severity. Results: Total 147 patients with IPF were screened, and 70 patients were analyzed. The prevalence of airway diseases in the participants was as follows: 5.0% had COPD, 1.7% had asthma, 3.3% had ACO, and 1.7% had EB. The mMRC, CAT, SGRQ, EQ-5D, and CQLQ scores did not differ regardless of combined airway disease. After 3 months, the SGRQ (p = 0.028) and CQLQ (p = 0.030) scores were significantly higher in patients with airway disease than in those without. Conclusions: The prevalence of airway diseases in patients with IPF is low, but when airway diseases are accompanied by IPF, symptom severity and quality of life may worsen rapidly.

접근 제어분야의 통합 ID관리 시스템은 도메인 내에서의 서비스 이용을 위하여 설계되었기 때문에 도메인 간에 적용하기에는 문제점이 많다. 이를 위하여 통합 접근 제어 관리 시스템이 제안되었으나 클라이언트의 권한이 도메인 내로 한정되기 때문에 권한의 이용이 유연하지 못하다. 본 논문에서는 도메인 내의 클라이언트가 자신의 권한을 도메인 밖에 있는 다른 클라이언트에게 일시적으로 부여할 수 있는 모델을 제안한다. 제안된 모델에서의 클라이언트는 도메인 내의 정책 엔진이 정한 권한 레벨 이하로 다른 클라이언트에게 특정 시간이나 횟수의 제약 조건을 붙여 해당 서비스로의 접근 권한을 부여 할 수 있다. 특히, 부여받는 권한의 종류는 최초 도메인 정책 엔진과 접근 제어 관리기에 의해 제한된다. 제한된 방법의 유효성 증명을 위하여 본 모델을 실세계 시나리오에 적용하였다.
노력선을 적용한 UN PKO 연합훈련 분석 및 발전방안 연구 - 레바논 평화유지단의 연합훈련 사례를 중심으로 -

동명부대를 비롯하여 파병부대가 성과있는 연합훈련을 진행하는데는 많은 어려움이 있다. 연합훈련 대상국에 대한 협조, 적절한 과제의 선정, 제 작전가용요소의 참가 등 쉽지 않은 문제들이 항상 기다리고 있기 때문이다. 본 논문에서는 동명부대의 최근 수년 동안의 연합훈련에 대하여 알아보고 노력선의 개념을 적용하여 부대의 작전목적 달성을 위한 연합훈련 발전방향에 대해서 알아보았다. 연합훈련 과제조정, 작전가용요소 추가, 연합작전과 연계된 훈련편성 등의 노력을 통하여 연합훈련의 동명부대 임무달성 기여도를 높일 수 있다. 이는 동명부대뿐만 아니라 현재 임무를 수행하고 있는 다른 파병부대들도 연합훈련 발전에 참고가 될 것이다. There are many difficulties in conducting a successful combined exercise for the Dong-Myung Contingent and other contingent units. This is because difficult issues await, such as cooperation with countries subject to conbined exercise, selection of appropriate tasks, and participation in the all elements for operations. In this paper, we investigated the combined exercise of the Dong-Myung contingent in recent years, and applied the concept of the Line of effort to find out the direction of the combined exercise development to achieve the unit's operational purpose. Through efforts such as coordination of combined exercise tasks, addition of operational elements, and exercise evaluation linked to conbined operations, the contribution of the conbined exercise to the mission achievement of the Dong-Myung contingent can be increased. This will serve as a reference for the development of combined exercise not only for the Dong-Myung contingent but also for other contingents currently on a mission.
