        Is Reflective Practice a Useful Task for Student Nurses?

        Background Reflective practices have been advocated as a method to bridge the gap between nursing theory and practice, and as a tool to develop knowledge embedded in practice. Aim The aim of this study was to examine the perceptions of student nurses towards reflective practice in their clinical practice. Methods A cross sectional descriptive survey was carried out to examine the perception of a cohort of 108 final year, pre-registration Diploma of Nursing students towards reflective practice. A structured questionnaire was used to collect the quantitative data. The questionnaire consisted of two parts; part A: perception of students towards reflective practice; and part B: issues reflected by students. Data collected were analyzed using descriptive statistics. All (n = 98) students completed the questionnaire. Results The mean score of perception to reflective practice was 4.07. Students found reflective practice useful to them with a mean score of 3.82, despite the perceived barrier to good reflective practice with a mean of 3.60. The perception on the appropriateness of reflective practice as a tool to assess was 3.47. The validity of reflective practice as an assessment tool for practical examinations was not conclusive, as the mean score was only 3.47. Conclusion The results indicated that even though some students were skeptical with reflective practice, they found it useful. The College of Nursing should review the content of reflective practice and prepare nurse educators for their role in teaching this subject more effectively.         What Influences Malaysian Nurses to Participate in Continuing Professional Education Activities?

        Purpose A cross sectional descriptive study, which involved government hospitals and health clinics from Peninsular Malaysia sought to identify the continuing professional education (CPE) needs and their readiness for E-learning. This paper focuses on the first phase of that study that aimed to determine the factors that influence nurses’ participation in CPE. Methods Multistage cluster sampling was used to recruit 1,000 nurses randomly from 12 hospitals and 24 health clinics from four states in Peninsular Malaysia who agreed to be involved. The respondent rate was 792 (79.2%), of which 562 (80%) had participated in CPE in the last 12 months. Results Findings suggested that updating knowledge and providing quality care are the most important factors that motivate participation in CPE, with respective means of 4.34 and 4.39. All the mean scores for educational opportunity were less than 3.0. Chi-square tests were used to test the association of demographic data and CPE participation. All demographical data were significantly associated with CPE participation,except marital status. Conclusions Implementation of mandatory CPE is considered an important measure to increase nurse’s participation in CPE. However, effective planning that takes into consideration the learning needs of nurses is recommended. [Asian Nursing Research 2011;5(1):38–47]

        A Theory-Based, Technology-Assisted Intervention in a Hybrid Cardiac Rehabilitation Program for Patients with Coronary Heart Disease: A Feasibility Study

        Purpose To assess the feasibility of a technology-assisted intervention in a hybrid cardiac rehabilitation program among patients with coronary heart disease. Methods This study was a two-arm parallel randomized controlled trial. Twenty-eight patients with coronary heart disease were randomly assigned to either the intervention group, receiving a 12-week technology-assisted intervention (n = 14), or the control group (n = 14), receiving usual care. Guided by the Health Belief Model, the intervention group received three center-based, supervised exercise training sessions, a fitness watch that served as a cue to action, six educational videos, and a weekly video call. The Self-efficacy for Exercise, exercise capacity, and Health Promoting Lifestyle Profile II were assessed at baseline and immediately post-intervention (12-weeks). Results Among the 28 patients who participated in this study, 85.7% completed the program, with a relatively low attrition rate (14.3%). The number of exercise training sessions accomplished by the participants in the intervention group was 51.27 ± 19.41 out of 60 sessions (85.5%) compared to 36.46 ± 23.05 (60.8%) in the control group. No cardiac adverse events or hospitalizations were reported throughout the study. Participants in the intervention group showed greater improvement in health-promoting behaviors when compared with the control group at 12 weeks. Within-group effects demonstrated improvement in exercise self-efficacy and exercise capacity among participants in the intervention group. A participant satisfaction survey conducted immediately post-intervention revealed that participants were “very satisfied” (23.1%) and “satisfied” (76.9%) with the technology-assisted intervention. Conclusions The findings demonstrated that technology-assisted intervention in a hybrid cardiac rehabilitation program was feasible and suggested to be beneficial in improving exercise self-efficacy, exercise capacity, and health promoting behavior among patients with coronary heart disease.         This project is about using methane derived from waste to generate electricity. To be able to do this, a few processes have to be done to get the final product; electricity. Instead of just letting methane destroy the ozone layer, I believe that this project that I’m proposing can help put methane gas into good use; as a form of sustainable energy.

        A Novel Approach Based on Fluid Dynamics for On-Site Safety Assessment

        Construction safety assessment is a major component of safety management in construction projects. However, thus far, most assessment studies have focused on either cross-sectional or longitudinal performance. This study aims to develop a proactive and preliminary safety assessment method using a fluid dynamics (FD) approach by integrating longitudinal contractor performance with cross-sectional safety conditions. To this end, an FD framework was first developed using three processes: (a) identifying the connections between FD principles and safety analogs, (b) incorporating the Darcy–Weisbach equation into the framework, and (c) modifying FD equations for construction safety assessment. Subsequently, two case studies were investigated, and the results obtained were compared and verified with one existing assessment results. The results indicate that the state of safety at a construction site incorporates both the occurrence of hazards and conservation of energy. Thus, the proposed FD approach can be used to preliminarily assess and predict safety conditions by combining safety-related measures with the dynamic characteristics of construction processes. The approach considers a comprehensive range of indicators and parameters, which enables the comparison of safety performance between projects or assessment periods by independently changing parameters.

        Effects of mediator competence and mediation process on user satisfaction with child custody, finances, and property outcomes

        This study draws on survey data from 205 family mediation service users concerning their satisfaction with the outcomes of mediation to address two research questions: (a) how satisfied are service users? and (b) what factors account for users' satisfaction with the outcome of child custody arrangements and the allocation of finances and property? Overall, users' level of satisfaction with outcomes was high. While the me-diation process explained a significant variance in satisfaction with the outcomes for child custody and for finances and property, the mediator's perceived professional competence did not predict user satisfaction with the outcomes for either issue. This study contributes to our knowledge on the significance of mediators' process skill, specifically their calm and clear responses to the socioemotional needs of the medi-ated parties, on the service outcomes for both types of dispute. The implications for mediation training are discussed.

      • KLF4 suppresses the tumor activity of cutaneous squamous cell carcinoma cells via the regulation of SMAD signaling and SOX2 expression

        Background: Kruppel-like factor 4 (KLF4) is a zinc -finger transcription factor that plays a role in terminal differentiation of epidermal keratinocytes. There are conflicting reports regarding the role of KLF4 in tumor development, with both the tumor suppressive and/or oncogenic properties depending on different conditions and cell types. Objectives: In this study, we investigated the functional importance of KLF4 in cutaneous squamous cell carcinoma (SCC). Methods: To examine the effects of KFL4, we transduced SCC lines (SCC12 and SCC13 cells) with the KLF4- expressing recombinant adenovirus. Results: Immunohistochemistry showed that KLF4 expression was relatively low in SCC lesion compared to normal epidermis. Overexpression of KLF4 significantly decreased cell proliferation and colony forming activity. In addition, overexpression of KLF4 markedly reduced invasive potential, along with the down-regulation of epithelial-mesenchymal transition (EMT)-related molecules. In a mechanistic study, KLF4 inhibited SOX2, of which expression is critical for tumor initiation and growth of SCC. Further investigations indicated that SOX2 expression is induced by TGF-β /SMAD signaling, and that overexpression of KLF4 inhibited SMAD signaling via up-regulation of SMAD7, an important inhibitory SMAD molecule. Conclusion: Based on these data, KLF4 plays a tumor suppressive role in cutaneous SCC cells.

      • Clinical Study of Hepatectomy Combined with Jianpi Huayu Therapy for Hepatocellular Carcinoma

        Background: Traditional Chinese Medicine (TCM) possesses several advantages for treating patients with hepatocellular carcinoma (HCC). The theory of 'Jianpi Huayu Therapy' rooted from 'Jin Kui Yao Lue'is one of the most important therapies in this respect. This study was conducted to investigate the clinical effect and safety of hepatectomy combining with 'Jianpi Huayu Therapy' in the treatment of HCC. Materials and Methods: One hundred and twenty patients with HCC were randomized allocated into hepatectomy combined with 'Jianpi Huayu Therapy' group (treatment group, n=60) and hepatectomy alone group (control group, n=60). Disease- free survival (DFS) and overall survival (OS) were the primary end-points. Liver function at the end of one week after surgery, complications, average days of hospitalization as well as performance status (PS) at the end of one month post operation were also compared. Results: No significant differences existed between two groups on baseline analysis (p>0.05). No treatment related mortality occurred in either group. Post-operative complications were detected among 14 patients (23.3%) in the treatment group, and 12 (20.0%) in the control group (p=0.658). Alanine aminotransferase (ALT) at the end of one week after operation was lower in the treatment than control groups (p=0.042). No significant differences in other indexes of liver function were discovered between two groups. Average days of hospitalization reduced by 0.9 day in treatment group than in control (p=0.034). During follow-up, 104 patients (86.6%) developed recurrence. The rates of 1-, 3-, and 5-year DFS and median DFS for all patients were 77.4%, 26.3%, 9.0% and 25.6 months (range, 6.0~68.0), respectively (78.2%, 29.2%, 14.3% and 28.7 months for the 48 patients in the treatment group and 75.0%, 23.3%, 6.4%, and 22.6 months for the 56 patients in the control group (p=0.045)). 101 patients had died at the time of censor, with 1-, 3-, and 5-year overall survival rates and median survival for all patients of 97.5%, 76.4%, 40.5% and 51.2 months (range, 10.0~72.0), respectively (98.3%, 78.0%, 43.6% and 52.6 months, for treatment and 96.7%, 74.7%, 37.4%, and 49.8 months, for controls, respectively (p=0.048)). Conclusions: Hepatectomy combined with 'Jianpi Huayu therapy'was effective in the treatment of HCC, and reduced post-operative recurrence and metastasis and improved DFS and OS of HCC patients.
