      • Poster Session : PS 0039 ; Cardiology : Association of Proteinuria and Depressed Systolic Fuction in Patients with Acute Myocardial Infarction

        Background: Previous studies found that treatment of cardiovascular diseases and decreases in urinary protein excretion is associated with reduction in cardiovascular risks.It was also postulated in previous trials that there was an increase in urinary protein excretion preceding ischemic cardiac events, although the pathophysiology is uncertain.To date, there have been no clinical trials directly associating proteinuria with depressed systolic function in patients with Acute Myocardial Infarction (AMI). Methods: Consecutive adult patients (=19years) admitted at the UP-PGH with the diagnosis of AMI from January, 2012 to June,2012 and with written consent were included. Patients with renal failure,urinary tract infection and diabetes mellitus were excluded.Using the standardized data collection form, clinical profi le, levels of troponin I,electrocardiogram (ECG), ejection fraction (EF) based on transthoracic echocardiogram and presence of proteinuria were obtained. Chi-square, Students T-test and Fischer`s Exact test were utilized to determine statistical differences between groups. Odds Ratio was used to test for strength of association between proteinuria and depressed systolic function. Results: A total of 88 patients were included,with the mean age of 60 years. Twenty- nine patients(33%) have normal EF and 59(67%) of them have depressed systolic function. Proteinuria is present in 55% of patients.Chest pain(81%) and dyspnea(22%) were most common presenting symptom with dyslipidemia(86%) and hypertension(64%) as the most common co-morbidities. Comparing patients with normal systolic function(EF=55%) and those with depressed systolic function(EF<55%), the baseline characteristics were essentially the same except for higher incidence of ST elevation among patients with depressed systolic function(63%vs.28%, p-value=0.004). The computed odds ratio of having proteinuria for depressed systolic function is 2.75 times the estimated odds of having proteinuria for normal systolic function(95%- CI=1.10-6.89). Conclusions: There is a strong association between proteinuria and depressed systolic function, with more than twice the likelihood of having depressed systolic function in the presence of proteinuria on urinalysis.

      • Genetic Factors Contributing to Gifted and Talented Students in the Philippines

        This investigation was undertaken to determine the factors that contribute to the IQ of the students. The study revealed that uncles with high intelligence numbered 0-5, 3-5 with average level of intelligence, while 0-2 had low level of intelligence. Majority of the aunts with high, average and low levels of intelligence numbered 0-2. The biggest proportion of the students had low IQ Test scores. The intelligence level of the grandfathers and grandmothers were not significantly related to the students’ IQ. The higher the level of intelligence of the fathers and the mothers, the higher was the IQ of the students. The more the uncles with high, average and low level of intelligence the students’ IQ were consequently high, average, and low, respectively. The same is true with aunts except for those with low level of intelligence where they were not related to the students’ IQ. The author concluded that the factors to students’ IQ are father and mother’s levels of intelligence where the higher the father and the mother’s level of intelligence, the higher is the student’s IQ. Another factor is the fact that the more the uncles with high, average and low intelligence, consequently the IQ of the students is, likewise, high, average and low. The more the aunts with high and average level of intelligence, the consequence is high and average IQ of the students, respectively.

      • Poster Session : PS 0018 ; Bioethics : “Double Trouble”: Wolff Parkinson White Syndrome and Mitral Stenosis Presenting as a Stroke in the Young

        Synopsis: The Wolff Parkinson White syndrome and mitral stenosis (MS) is a unfortunate combination, with less than 15 patients reported in literature. They are individually associated with supraventricular arrhythmias and their combination may have profound hemodynamic consequences. Our case is a young male presenting with neurologic deficits who was newly diagnosed with the WPW syndrome and mitral stenosis. Case: A 25 year old male sought consult syncope, palpitations, and dizziness. On the day of admission, he experienced persistent palpitations then subsequently lost consciousness. He had a diastolic rumble, right sided hemiparesis, and dysarthria. A cranial CT scan revealed a left basal ganglia infarction. Electrocardiogram showed sinus rhythm and left atrial enlargement, a short PR interval, wide QRS complexes, and delta waves. Holter monitoring showed episodes of atrial fl utter with rapid ventricular rates. QRS complexes were narrow during other episodes of tachycardia. Echocardiogram revealed moderate mitral stenosis and a dilated LA with no thrombus. Diagnosis was WPW syndrome and rheumatic MS manifesting as a stroke in the young. Percutaneous transvenous mitral commissurotomy (PTMC) was done. He is now on regular follow up on chronic anticoagulation and medical therapy for the WPW syndrome. Conclusion: To our knowledge, this is the fi rst case of a WPW syndrome with rheumatic MS presenting as a stroke in the young who was successfully managed with PTMC for MS and anti-arhrythmics for the WPW syndrome. This unfortunate combination may have potentiated the risk for cardioembolic events which emphasizes that detection of these entities is of paramount importance. Though either of the two entities may have caused the stroke, treatment is warranted for both entities as to avoid future episodes of cardioembolic phenomena.

        The value of PET/CT for cytoreductive surgery selection in recurrent ovarian carcinoma

        Objective: To evaluate the value of positron emission tomography/computed tomography (PET/CT) in predicting no residual disease (NRD) after secondary cytoreductive surgery (SCS) compared with MSK criteria, the iMODEL, and the AGO score. Methods: We analyzed 112 patients with platinum-sensitive ovarian carcinoma who underwent SCS. We excluded patients for whom PET/CT was not performed, those without sufficient data, and who received chemotherapy before SCS. Ultimately, 69 patients were included. Results: Variables that correlated with NRD were peritoneal carcinomatosis index (odds ratio [OR]=0.91; 95% confidence interval [CI]=0.83–0.99; p=0.044), European Cooperative Oncology Group Performance Status (ECOG) 0 (OR=8.0; 95% CI=1.34–47.5; p=0.022), and ≤2 lesions by PET/CT (OR=4.36; 95% CI=1.07–17.7; p=0.039). Of the patients with ≤2 lesions by PET/CT, 48 (92.3%) underwent complete SCS. The sensitivity, positive predictive value, negative predictive value, and accuracy of PET/CT for NRD were 85.7%, 92.3%, 33.3%, and 81.2%, respectively. NRD was achieved after fulfilling the MSK criteria, iMODEL and AGO Score in 89.1%, 88.1% and 85.9%, respectively. The accuracy of the MSK criteria, iMODEL, and AGO score in predicting NRD was 87%, 83.3%, and 77.3%, respectively. The PET/CT findings agreed well with the AGO score and iMODEL. The addition of PET/CT to these models increased the NRD rates (92.2%, 91.8%, and 89.4% for MSK+PET/CT, iMODEL+PET/CT, and AGO+PET/CT, respectively), but lowered their accuracy. Conclusion: We observed NRD in 92.3% of patients with ≤2 lesions by PET/CT, with an accuracy of 81.2%. PET/CT did not increase the accuracy of the MSK criteria, iMODEL, or AGO score models.

        Effect of Rheedia longifolia Leaf Extract and Fractions on the P2X_7 Receptor In Vitro: Novel Antagonists?

        Recently, the P2X_7 receptor has been reported to be associated with chronic inflammatory and neuropathic pain. Because Rheedia longifolia extract has analgesic and anti-inflammatory activity, we evaluated the in vitro inhibitory potential of methanol extract and fractions from its leaves on the P2X_7 purinergic receptor. The activity of P2X_7 was studied with a dye uptake assay and with the whole-cell patch clamp technique in mouse peritoneal macrophages treated with methanol extract of R. longifolia leaves and fractions. The dye uptake was evaluated by flow cytometry and fluorescence microscopy. The R. longifolia extract and some fractions showed an inhibitory effect on the P2X_7 purinergic receptor in a dose-dependent manner. The ethyl acetate fraction exhibited the most potent inhibitory effects. The methanol extract and the butanol fraction showed the same inhibitory effects, despite their lower potency compared with the other fractions. The R. longifolia extract and some of its fractions may be anti-inflammatory because of their inhibitory effect on the P2X_7 receptor. Further investigation is needed to determine the pattern of inhibition and selectivity. Chromatographic analysis indicated the presence of bisflavonoids in the methanol extract fractions. A member of this chemical family is the most probable active compound responsible for the P2X_7 inhibitory effects present in the R. Longifolia extract and fractions.
