Pseudomembranous colitis is mainly caused by antibiotics and Clostridium difficile infection. But conditions such as gastrointestinal surgery, antacid medication, anti-neoplastic agent or immunosuppressive agent which influences the normal flora of colon can induce colitis without the administration of any antibiotics. We experienced a 13 year-old male who was taking low-dose methotrexate for juvenile rheumatoid arthritis complained diarrhea and abdominal pain for 3 weeks. Sigmoidoscopic findings revealed diffuse patch yellowish pseudomembranes on the rectum. Histologic finding was compatible to pseudomembranous colitis. His symptom was improved after stop taking methotrexate and the administration of metronidazole. If a patient treated with immunosuppressive agents or antineoplastic agents complains diarrhea, fever or abdominal pain and has not improved with conservative care, pseudomembranous colitis should be taken into account as a differential diagnosis and prompt treatment is required for better prognosis. Purpose: Acute alcohol intoxication (AAI) causes various complications such as electrolyte imbalance, alcoholic ketoacidosis (AKA), rhabdomyolysis, and acute kidney injury (AKI). Although there have been some reports about AKA and rhabdomyolysis, AKI due to acute alcohol intoxication is rarely reported. Methods: We retrospectively evaluated the medical records of 371 patients with AAI between January 2004 and May 2010 in Uijeongbu St. Mary`s Hospital. We compared the clinical findings, morbidity and mortality rate between AKI and normal kidney function (NKF) groups. Results: Of the total 371 patients with AAI, AKI occurred in 107 patients (28.8%). The peak serum creatinine level in AKI patients was 2.9±1.9 mg/dL. Thirteen of the 107 patients (12.1%) received renal replacement therapy. AKI group had higher incidence of decreased mentality (29.0% vs 16.3%, p= 0.006), dyspnea (11.2% vs 4.9%, p=0.029) and hypotension (66.0% vs 41.7%, p<0.001), and lower incidence of gastrointestinal bleeding (22.4% vs 34.8%, p=0.019), compared to NKF group. The AKI group also had higher incidence of ketoacidosis (78.5% vs 28.8%, p<0.001), rhabdomyolysis (19.6% vs 4.2%, p<0.001), and pneumonia (22.4% vs 8.0%, p<0.001), compared to NKF group. The length of ICU stay was longer (7.4±10.8 vs 4.1±6.1 days, p=0.003) and the mortality rate was higher (17.8% vs 2.3%, p<0.001) in AKI group. Conclusion: This study demonstrated that incidence of AKI in patients with AAI was 28.8% and AKI was associated with high morbidity and mortality. And multivariate analysis demonstrated that independent risk factors of AKI were ketoacidosis and increased serum osmolality.

인터루킨 27(IL-27)은 인터루킨 12계열의 이성이량체 사이토카인으로 EBI3와 p28의 두 서브유니트로 구성 되어있다. 인터루킨 27은 Th1 개시의 초기 조절에 관여하는 사이토카인으로 Th2 인자인 GATA-3의 작용을 억제하는 역할을 한다. 이 사이토카인은 CD4? T 림프구와 자연살해세포에서 아주 높게 발현하는 수용체(WSX-1)에 의하여 매개된다. 우리들은 사람의 IL-27p28유전자에서 네 개의 유전자다형성 부위를 찾아서 이들 유전자다형성이 기관지천식의 감수성에 영향을 미치는 것을 보고한 바 있다. 이 연구에서는 IL-27p28 유전자의 유전자다형성이 알레르기성 비염의 감수성에 영향을 미치는 지를 알아보기 위하여 이들 다형성에 있어서 알레르기성 비염환자 군과 정상인군 사이의 유전자형 및 대립형질의 빈도를 비교분석 하였다. 비록 알레르기성 비염환자 군에서 IL-27p28 유전자의 유전자다형성의 유전자형 및 대립형질의 빈도의 차는 정상인 군의 그것과 큰 차이가 없었으나, g.2905T>G SNP에서 두 그룹간에 주목할만한 차이(P=0.037)를 발견하였다. 이 결과는 IL-27p28 유전자의 g.2905T>G 유전자 형성이 알레르기성 비염환자에서 IgE의 생산에 영향을 준다는 것을 암시한다. Interleukin 27 (IL-27) was discovered as a heterodimeric cytokine of the IL-12 family, and is composed of two subunits - Epstein-Barr virus induced gene 3 (EBI3) and p28. It acts as a versatile cytokine in the early regulation of Th1 initiation and in the negative regulation of the Th2 factor GATA binding protein 3 (GATA-3). This cytokine is mediated by the IL-27 receptor (WSX-1), which is highly expressed on CD4? T lymphocytes and NK cells. We previously identified four polymorphisms in the human IL-27p28 gene and suggested that the polymorphism of IL-27p28 is associated with susceptibility to asthma. To determine whether these IL-27p28 SNPs are associated with susceptibility to allergic rhinitis, the genotype and allele frequencies of IL-27p28 SNPs were analyzed between allergic rhinitis patients and healthy controls. Although the genotype and allele frequencies of IL-27p28 SNPs in allergic rhinitis patients were not significantly different from those of the control group, there was a suggestive difference (P=0.037) between these groups in total serum IgE levels in the g.2905T>G SNP of the IL-27p28 gene. Our result implies that the g.2905T>G SNP of the IL-27p28 gene might have an affect on IgE production in allergic rhinitis patients.
병렬설비를 가지는 열처리공정에서의 배치일정계획 수립 연구

본 연구는 금속제품 생산 프로세스의 처리능력을 결정하는 주요 공정인 열처리 공정의 생산계획을 수립하는 것이다. 열처리로 전체의 작업완료시간(makespan)을 최소화하기 위한 유전알고리즘을 제시하는 것을 본 연구의 목표로 한다. 일반적인 유전알고리즘의 각 단계에서 해의 가능성을 만족하기 위해 ‘재할당 절차’를 통해 알고리즘의 적용을 유효하게 하였고, SPT 규칙을 기초로 한 FFD 방법론을 활용하여 각 열처리로 별로 세부 일정계획을 수립하였다. 이러한 절차를 통해 효과적인 해를 찾을 수 있도록 하였으며, 유전알고리즘 해와 비교를 위해 하한값을 제시하였다. 제시된 유전알고리즘의 성능을 분석하기 위해 성능평가를 실시하였다. This paper considers a batch scheduling problem for heat-treatment process with parallel machine environments. To derive efficient and effective solutions for the targeted problem, this paper considers the genetic algorithm to minimize the makespan objective measure. To satisfy the feasibility of the solutions provided by the GA algorithm, this paper derives some solution properties and procedures, such as ‘relocation procedure’ etc. In addition, using SPT rule based FFD methodology, detailed schedules are established at each heat treatment. To show the effectiveness of the solutions by GA algorithm, some lower bounds are also derived. The computational experiments are conducted to test the performance of the derived GA algorithm.
대퇴골 골절 환자에서 발생한 급성신손상의 위험인자와 임상경과

Background/Aims: Femoral fracture occurs most often in elderly patients and is highly associated with medical problems such as acute kidney injury (AKI); however no reports of AKI in femoral fracture patients have been published. This study was performed to identify risk factors and the clinical course of AKI in patients with femoral fracture. Methods: We retrospectively evaluated the medical records of 110 patients with femoral fracture between November 2006 and December 2011 at Uijeongbu St. Mary`s Hospital. We investigated the incidence and clinical course of AKI in femoral fracture patients and compared the clinical findings between AKI and normal kidney function (NKF) groups. Results: Of the 110 femoral fracture patients, AKI was observed in 19 (17.3%). The peak serum creatinine level in patients with AKI was 2.59 ± 1.57 mg/dL. Two of 19 patients with AKI died and two progressed to chronic kidney disease. When compared to the NKF group, the AKI group had a higher incidence of elevated lactate dehydrogenase (LDH) (63.2% vs. 34.1%, p = 0.020), erythrocyte sedimentation rate (ESR) (31.6% vs. 6.6%, p = 0.008), and C-reactive protein (57.9% vs. 46.2%, p = 0.042). The AKI group also had a longer hospitalization duration, and more patients were prescribed an angiotensin-converting-enzyme (ACE) inhibitor than in the NKF group. Multivariate analysis demonstrated elevated LDH, ESR and ACE inhibitor prescriptions as independent risk factors for AKI in patients with a femoral fracture. Conclusions: The incidence of AKI in patients with a femoral fracture was 17.3%, and AKI was associated with a longer clinical course. We recommend monitoring of laboratory findings

나노 코팅 기술로써 빠른 증착 속도와 미세구조 제어가 용이하여 항공기 엔진 부품 열차폐 코팅으로 널리 이용되는 Electron Beam Physical Vapor Deposition (EB-PVD)세라믹 코팅 기술을 연료전지 전해질 제조에 적용하였다. EB-PVD 법을 이용하여 NiO-YSZ 기판에 YSZ 전해질을 약 10$\mu$m의 두께로 짧은 시간에 코팅하였으며 증착온도에 따라 나노 구조의 표면을 가진 YSZ 막을 얻을 수 있었다. 연료전지 전해질로서의 특성을 평가하기 위하여, 같은 조건의 코팅으로 $Al_{2}O_3$기판에 전해질을 동일한 조건으로 코팅하여 전해질의 전기적 특성을 평가하였다. 또한 양극물질로서 $LaSrCoO_3$ 분말을 일반적인 스크린 프린팅 기법으로 코팅하여 EB-PVD의 코팅을 이용한 고체산화물 연료전지 제조 가능성에 대하여 논의하였다 Electron Beam Physical Vapor Deposition (EB-PVD) is a typical technology for thermal barrier coating with Yttria Stabilized Zirconia (YSZ) on aero gas turbine engine. In this study EB-PVD method was used to fabricate dense YSZ film on NiO-YSZ as a electrolyte of Solid Oxide Fuel Cell (SOFC). Dense YSZ films of -10 $\mu$m thickness showed nano surface structure depending on deposition temperature. Electrical conductivities of YSZ film and electric power density of the single cell were evaluated after screen- printing $LaSrCoO_3$ as a cathode.
