A recombinant human erythropoietin (rHuEPO) was administered intravenously at dosage levels of 0, 100, 500, and 2500 IU/㎏/day for a period of 13 weeks. There were no observed clinical signs and deaths related to treatment in all groups tested. Decreases in body weight gain and food consumption were observed only in males of 2,500 IU/㎏ group after 2 weeks. In hematological parameters, erythrocyte content, hematocrit values and hemoglobin concentration were dose-dependently increased in rHuEPO treated groups. The ratio between kidney weight and whole body weight was significantly increased in females of 500 and 2,500 IU/㎏ groups. The spleen weight was also increased in both sexes of 500 and 2,500 IU/㎏ groups. However, the absolute weight change of other organs was not observed. In histopathological examinations, the renal tubular basophilia was observed only in males and females of 2,500 IU/㎏ groups. From these results, it is concluded that the no-observed adverse effect level(NOAEL) of rHuEPO is 100 IU/㎏ in rats in the present study.

Eye irritation of Sangmosu was evaluated in New Zealand White rabbits. In ocular irritation test, any injury on iris, conjunctival membrane, and cornea was not observed. No injuries of the ocular mucous membrane were also recorded. These results indicate that Sangmosu was not considered to be irritant in test organs of animals.

목적: C-reactive protein (CRP) is a serum marker for diagnosis of acute inflammation and/ or infection and has been examined among patients with preterm premature rupture of the membranes (PPROM). In our study, the predictive value of maternal CRP at admission for duration of the latency period in PPROM patients has been evaluated. 방법: This was a retrospective study of 72 consecutive singleton pregnancies diagnosed with PPROM from 24 to 34weeks, delivered at Inje University College of Medicine Ilsan-Paik Hospital. Maternal CRP was assessed at admission. Duration of Latency period was categorized by days of latency (2days, 7days, 14days, 21days, 28days). Receiver operating characteristic (ROC) curves were employed to evaluate the predictive value of maternal CRP for duration of the latency period in PPROM patients. 결과: ROC curves showed that maternal CRP at admission in latency period of 14days (area under the curve(AUC) : 0.745, p=0.004), 21days (AUC : 0.752, p=0.006) and 28days (AUC : 0.716, p=0.027) are predictive of latency period (Table 1). 결론: Maternal serum CRP (≤0.3mg/dL) at admission in PPROM patients may be predictive value in latency period of above 14days.
Granulomatous mastitis in pregnancy: A case report

Introduction Granulomatous mastitis (GM) is an uncommon chronic inflammatory disease of the breast that may clinically imitator breast carcinoma. GM is strongly associated with lactation and frequently reported to occur in postpartum period. Little is known about clinical course of GM during pregnancy. We present a woman who was diagnosed as having GM in the postpartum period and continued to be treated by steroid during her subsequent pregnancy and delivered uneventfully at 38+2 weeks’ gestation without aggravation of symptoms. Case presentation A 30-year-old multiparous woman with the chief complaint of a painful mass in her right breast visited our hospital. Physical examination showed a 2 x 1 cm, oval shape, hard and movable mass in her right breast, 2 cm apart from nipple. Her medical and surgical history was unremarkable. Ultrasonography showed that there was a 4.3 x 1.1 cm-sized, irregular-shaped, heterogeneous hypoechoic lesion. Left breast and both axilla were unremarkable. Core-needle aspiration biopsy was conducted and pathologic findings presented GM. She was treated with varying dose of prednisolone and immunosuppressive drugs(PROGRAF,Azafrine,Calcort).After 7 months, she conceived and prednisolone and immunosuppressive drugs were continued throughout pregnancy. She delivered uneventfully at 38+2 weeks’ gestation without aggravation of the disease. Thereafter, she has been treated by methotrexate and prednisolone and her last ultrasonography has suggested improving state of GM. Conclusion This GM case indicates that pregnancy appears neither to aggravate nor to mitigate clinical course of the disease. Favorable results are expected to be achieved by treatment in women with GM in pregnancy.

<P><B>Objective</B></P> <P>This retrospective case control study aimed to evaluate the feasibility of using Estimation of Physiological Ability and Surgical Stress (E-PASS) and Physiological and Operative Severity Score for the enumeration of Mortality and Morbidity (POSSUM) systems in patients undergoing spinal surgical procedures. Degenerative spine disease has increased in incidence in aging societies, as has the number of older adult patients undergoing spinal surgery. Many older adults are at a high surgical risk because of comorbidity and poor general health.</P> <P><B>Methods</B></P> <P>We retrospectively reviewed 217 patients who had undergone spinal surgery at a single tertiary care. We investigated complications within 1 month after surgery. Criteria for both skin incision in E-PASS and operation magnitude in the POSSUM system were modified to fit spine surgery. We calculated the E-PASS and POSSUM scores for enrolled patients, and investigated the relationship between postoperative complications and both surgical risk scoring systems. To reinforce the predictive ability of the E-PASS system, we adjusted equations and developed modified E-PASS systems.</P> <P><B>Results</B></P> <P>The overall complication rate for spinal surgery was 22.6%. Forty-nine patients experienced 58 postoperative complications. Nineteen major complications, including hematoma, deep infection, pleural effusion, progression of weakness, pulmonary edema, esophageal injury, myocardial infarction, pneumonia, reoperation, renal failure, sepsis, and death, occurred in 17 patients. The area under the receiver operating characteristic curve (AUC) for predicted postoperative complications after spine surgery was 0.588 for E-PASS and 0.721 for POSSUM. For predicted major postoperative complications, the AUC increased to 0.619 for E-PASS and 0.842 for POSSUM. The AUC of the E-PASS system increased from 0.588 to 0.694 with the Modified E-PASS equation.</P> <P><B>Conclusions</B></P> <P>The POSSUM system may be more useful than the E-PASS system for estimating postoperative surgical risk in patients undergoing spine surgery. The preoperative risk scores of E-PASS and POSSUM can be useful for predicting postoperative major complications. To enhance the predictability of the scoring systems, using of modified equations based on spine surgery-specific factors may help ensure surgical outcomes and patient safety.</P>
Original Article : Geographic difference of epidemiological features of HCV infection in Korea

Background/Aims: The prevalence of hepatitis C virus (HCV) infection in Korea exhibits significant geographic variation, with it being higher in Busan and Jeonam than in other areas. The reason for this intranational geographic difference was investigated in this study by conducting a comparative analysis of the risk factors related to HCV infection among three geographic areas: the capital (Seoul), Busan, and the province of Jeolla. Methods: In total, 990 patients with chronic HCV infection were prospectively enrolled at 5 university hospitals located in Seoul (n=374), Busan (n=264), and Jeolla (n=352). A standardized questionnaire survey on the risk factors for HCV infection was administered to these three groups of patients, and a comparative analysis of the findings was performed. Results: The analysis revealed significant regional differences in exposure to the risk factors of HCV infection. By comparison with patients in Seoul as a control group in the multivariate analysis, patients in Busan had significantly more experience of invasive medical procedures, acupuncture, cosmetic procedures, and multiple sex partners. In contrast, patients in Jeolla were significantly older, and they had a higher prevalence of hepatocellular carcinoma, a lower prevalence of multiple sex partners, and had experienced fewer invasive procedures. Conclusions: There was a significant geographic difference in the exposure to potential risk factors of HCV infection between patients from the three studied regions. This may explain the regional variation of the prevalence of HCV infection in Korea, and should be taken into account when planning strategies for the prevention and management of HCV infection. <P>New circuit architecture composed of nonvolatile memory thin-film transistors (M-TFTs) and oxide-semiconductor TFTs (Ox-TFTs) has been proposed to realize low-power consumption and compact size for dynamic driving circuit applications. In order to compensate the slow operation speed of the nonvolatile M-TFTs in the dynamic driving circuits, we proposed the read-out modulation (ROM) scheme. The device operation characteristics of the M-TFTs were confirmed under various specified conditions to effectively apply the ROM, in which a top-gate structure was fabricated to be ferroelectric poly(vinylidene fluoride-trifluoroethylene) gate insulator/Al2O3/In-Ga-ZnO active layers. Full fabrication processes for the M-TFTs were optimized, so that they can be integrated with Ox-TFTs on the same glass substrate. The memory device characteristics, including the current ratios between ON- and OFF-programmed drain current (I-D) values (Ip-ON/Ip-OFF) before and after the ROM, drain-bias dependence of the Ip-ON/Ip-OFF, and the program I-D scalability, were confirmed for the fabricated M-TFT. These results suggest the feasibility of a low-power display driver circuit embedded with the nonvolatile M-TFTs.</P>
